Impact of infection in hospitalized cirrhotic patients in a university hospital

Background: Infection increases the morbidity and mortality in liver cirrhosis patients. The aim of this study was to investigate the impact of infection related to survival and risk factors for death in adult patients with liver cirrhosis in a university hospital. Methods: In a retrospective cohort study of Brazilian hospitalized cirrhotic patients, medical records data were analysed, and all patients who have had one or more confirmed bacterial infection during admission were se-ected for the study. Also, some data as biochemical investigation, Child score, MELD estimation, and evolution and death event were included. Statistical analysis: chi-square, Fisher and Mann-Whitney tests were used. Uni and multivariate analysis were performed, according to Cox regression model. The significant statistical level was p 2.5 mg/dl had increased the risk of death of 4.1, 3.2 and 3.2, respectively. Conclusion: Bacterial infections in hospitalized cirrhotic patients deserve special care, mainly spontaneous bacterial peritonitis, and also patients whose hiponatremia, upper gastrointestinal bleeding, high levels of cre-atinine and MELD high score are found.

AIM: To study the therapeutic effect of three tubeguides with electrical conductivity associated to mesenchymal stem cells(MSCs) on neuro-muscular regeneration after ***: Rats with 10-mm gap nerve injury were tested using polyvinyl alcohol(PVA), PVA-carbon nanotubes(CNTs) and MSCs, and PVA-polypyrrole(PPy). The regenerated nerves and tibialis anterior muscles were processed for stereological studies after 20 wk. The functional recovery was assessed serially for gait biomechanical analysis, by extensor postural thrust, sciatic functional index and static sciatic functionalindex(SSI), and by withdrawal reflex latency(WRL). In vitro studies included cytocompatibility, flow cytometry, reverse transcriptase polymerase chain reaction and karyotype analysis of the MSCs. Histopathology of lung, liver, kidneys, and regional lymph nodes ensured the biomaterials biocompatibility. RESULTS: SSI remained negative throughout and independently from treatment. Differences between treted groups in the severity of changes in WRL existed, showing a faster regeneration for PVA-CNTs-MSCs(P < 0.05). At toe-off, less acute ankle joint angles were seen for PVA-CNTs-MSCs group(P = 0.051) suggesting improved ankle muscles function during the push off phase of the gait cycle. In PVA-PPy and PVA-CNTs groups, there was a 25% and 42% increase of average fiber area and a 13% and 21% increase of the "minimal Feret's diameter" respectively. Stereological analysis disclosed a significantly(P < 0.05) increased myelin thickness(M), ratio myelin thickness/axon diameter(M/d) and ratio axon diameter/fiber diameter(d/D; g-ratio) in PVA-CNT-MSCs group(P < 0.05). CONCLUSION: Results revealed that treatment with MSCs and PVA-CNTs tube-guides induced better nerve fiber regeneration. Functional and kinematics analysis revealed positive synergistic effects brought by MSCs and PVA-CNTs. A method for adsorption involving banana peel (BP) was studied to remove the pollutant acetylsalicylic acid (ASA) in aqueous medium. The results show that bioadsorbent has satisfactory maximum adsorption capacity (2.29 mg/g) for removing this analgesic and anti-inflammatory drug in aqueous solution (pH 7.0) using the Langmuir mathematical model. The tested concentrations of this pollutant were higher than the levels commonly found in the aquatic environment. This and other results suggest the BP as an alternative to ASA removal in water contaminated with pharmaceuticals pollutants.

The objective of this study was to evaluate the effects of the addition of souari nut peel on the fermentative characteristics and chemical composition of the Mombasa grass silage.A completely randomized design with five levels of addition(0%,5%,10%,15%and 20%in natural matter basis)of souari nut peel with three replicates was applied in the *** silos were opened at 55 d after starting the ensiling *** data were analyzed by means of linear/quadratic regression analysis,according to the best fit of the data and the probability values to 5%,using the Statistical Analysis System(SAS,2002).There were a linear decreasing effect on the neutral detergent fiber(NDF),ash and increasing values for the effluent in the *** effect was observed for crude protein(CP),lignin and *** the other factors dry matter(DM),acid detergent fiber(ADF)and gases there was no influence on the addition of souari nut *** 15%addition level of souari nut peel increased CP content,decreased NDF and pH,which was recommended to improve the fermentation profile of Mombasa grass silage.

Many studies have been dedicated to the development of scaffolds for improving post-traumatic nerve regeneration. The goal of this study was to assess the effect on nerve regeneration, associating a hybrid chitosan membrane with non-differentiated human mesenchymal stem cells isolated from Wharton＇s jelly of umbilical cord, in peripheral nerve reconstruction after crush injury. Chromosome analysis on human mesenchymal stem cell line from Wharton＇s jelly was carried out and no structural alterations were found in metaphase. Chitosan membranes were previously tested in vitro, to assess their ability in supporting human mesenchymal stem cell survival, expansion, and differentiation. For the in vivo testing, Sasco Sprague adult rats were divided in 4 groups of 6 or 7 animals each： Group 1, sciatic axonotmesis injury without any other intervention （Group 1-Crush）; Group 2, the axonotmesis lesion of 3 mm was infiltrated with a suspension of 1 250 -1 500 human mesenchymal stem cells （total volume of 50 pL）（Group 2-CrushCell）; Group 3, axonotmesis lesion of 3 mm was enwrapped with a chitosan type Ill membrane covered with a monolayer of non-differentiated human mesenchymal stem cells （Group 3-CrushChitlllCell） and Group 4, axonotmesis lesion of 3 mm was enwrapped with a chitosan type III membrane （Group 4-CrushChiUll）. Motor and sensory functional recovery was evaluated throughout a healing period of 12 weeks using sciatic functional index, static sciatic index, extensor postural thrust, and withdrawal reflex latency. Stereological analysis was carded out on regenerated nerve fibers. Results showed that infiltration of human mesenchymal stem cells, or the combination of chitosan membrane enwrapment and human mesenchymal stem cell enrichment after nerve crush injury provide a slight advantage to post-traumatic nerve regeneration. Measures such as residual feed intake(RFI)have been increasingly considered to select cattle with superior feed efficiency,as it is a characteristic of feed efficiency calculated as the difference between observed intake and estimated intake considering metabolic weight and the weight *** feed intake is obtained through the multiple regression equation of observed consumption on metabolic mean live weight and weight *** standard was shared among animals into one herd selected for low RFI(high efficiency)and compared to the one selected for high RFI(low efficiency).There was no correlated response in weight per year and in weight gain,and the effectiveness of low RFI selection in reducing feed costs was proven without affecting ***,selection of animals with negative RFI is not done by selecting animals directly according to the size of the viscera,rather it is done indirectly,that is,when selecting the most efficient animals and normally these animals already have the smallest viscera *** RFI is a trait that presents genetic variability,selectable to improve and has moderate heritability,between 0.30 and *** RFI is important because it reduces the direct economic impacts of reduced feed intake,as well as the effects of environmental impact on reducing methane emissions from enteric fermentation of ruminants.

Vertical stratification of bat assemblages in flooded and unflooded Amazonian forests

Tropical rainforests usually have multiple strata that results in a vertical stratification of ecological opportunities foranimals. We investigated if this stratification influences the way bats use the vertical space in flooded and unflooded forests of theCentral Amazon. Using mist-nets set in the canopy (17 to 35 m high) and in the understorey (0 to 3 m high) we sampled four sitesin upland unflooded forests (terra firme), three in forests seasonally flooded by nutrient-rich water (varzea), and three in forestsseasonally flooded by nutrient-poor water (igapo). Using rarefaction curves we found that species richness in the understorey andcanopy were very similar. An ordination analysis clearly separated the bat assemblages of the canopy from those of the understorey inboth flooded and unflooded habitats. Gleaning carnivores were clearly associated with the understorey, whereas frugivores wereabundant in both strata. Of the frugivores, Carollinae and some Stenodermatinae were understorey specialists, but several Stenodermatinaemostly used the canopy. The first group mainly includes species that, in general, feed on fruits of understorey shrubs,whereas the second group feed on figs and other canopy fruits. We conclude that vertical stratification in bat communities occurseven within forests with lower canopy heights, such as Amazonian seasonally flooded forests, and that the vertical distribution ofbat species is closely related to their diet and foraging behaviour.
