Cultural competence in nursing provides an ideal avenue to meet the various needs of our patients while providing them with safe, competent care. “Racial and ethnic differences in family expectations or preferences for care are not yet well identified and cataloged, but they may uniquely affect individual ratings or experience with care” [1]. The purpose of this article is to enlighten nurses as health care providers about cultural care in an effort to provide that safe, competent care. Many times patients present for care at hospitals and clinics with language barriers and preconceived ideas based on their own cultural beliefs and practices. It is ideal when we, as nurses have the ability to meet those needs in an effort to meet patient’s expectations and healthcare needs. In order to meet the diverse needs of our growing multi-cultural population, it is becoming apparent that we must not only provide care based on the physical needs, but the entire person’s needs relevant to their cultural beliefs regarding healthcare practices and the healing process.

AIM:To report 4 cases of Cryptococcus gattii(***)species complex infection with diverse ophthalmic manifestations,and to review the literature to examine pathobiology of disease,classical ophthalmic presentations and outcomes,and treatment modalities for this emerging ***:Cases of *** meningoencephalitis with ophthalmic manifestations were identified via chart review at two institutions in Australia and one institution in the mid-west region of the United States and are reported as a case ***,a MEDLINE literature review was conducted to identify all reported cases of *** with ophthalmic manifestations from *** were reviewed and tabulated,together with our series of patients,in this ***:four cases of *** with ophthalmic manifestations are presented;three from Australia and one from the USA.A literature review identified a total of 331 cases of *** with visual *** majority of cases occurred in immunocompetent *** vision and diplopia were the most common presenting symptoms,with papilloedema the most common sign,reported in 10%-50%of *** loss was reported in 10%-53%of cases,as compared to rates of visual loss of 1%-9%in *** *** intracranial pressure,cerebrospinal fluid(CSf)fungal burden,and abnormal neurological exam at presentation correlated with poor visual *** mainstays of treatment are anti-fungal agents and aggressive management of intracranial hypertension with serial lumbar *** diversion procedures should be considered for refractory *** and mannitol are associated with high complication rates,and adjuvant corticosteroids have demonstrated higher mortality rates;these treatments should be ***:Permanent visual loss represents a devastating yet potentially preventable sequelae of *** *** hypertension needs to be recognised early and aggressively ***

Halitosis, bad breath or oral malodour are all synonyms for the same pathology. Halitosis has a large social and economic impact. for the majority of patients suffering from bad breath, it causes embarrassment and affects their social communication and life. Moreover, halitosis can be indicative of underlying diseases. Only a limited number of scientific publications were presented in this field until 1995. Ever since, a large amount of research is published, often with lack of evidence. In general, intraoral conditions, like insufficient dental hygiene, periodontitis or tongue coating are considered to be the most important cause （85%） for halitosis. Therefore, dentists and periodontologists are the first-line professionals to be confronted with this problem. They should be well aware of the origin, the detection and especially of the treatment of this pathology. In addition, ear-nose-throat-associated （10%） or gastrointestinal/ endocrinological （5%） disorders may contribute to the problem. In the case of halitophobia, psychiatrical or psychological problems may be present. Bad breath needs a multidisciplinary team approach： dentists, periodontologists, specialists in family medicine, ear- nose-throat surgeons, internal medicine and psychiatry need to be updated in this field, which still is surrounded by a large taboo. Multidisciplinary bad breath clinics offer the best environment to examine and treat this pathology that affects around 25% of the whole population. This article describes the origin, detection and treatment of halitosis, regarded from the different etiological origins.

AIM:To investigate the relationship between sense of coherence,psychological distress and health related quality of life in inflammatory bowel disease(IBD).METHODS:This cross-sectional study enrolled a consecutive sample of 147 IBD(aged 45.1 ± 14.1 years; 57.1% female) patients recruited from a tertiary gastroenterology ***-four participants met diagnostic criteria for Crohn's disease,while eightythree patients had ulcerative ***-demographic data(education,age,race,gender,gross monthly income and marital status),disease-related variables(illness activity,relapse rate in past 2 years,history of surgery and time since diagnosis),sense of coherence(Antonovsky's SOC scale),psychological distress symptoms(Hospital Anxiety and Depression Scale) and health-related quality of life(HRQo L; WHOQOLBref) were *** multiple regression analyses were performed to identify factors that are independently associated with psychological distress and HRQo L in patients with IBD and to provide indications for possible moderating or mediating *** addition,formal moderation and mediation analyses(Sobel tests) were performed to confirm potential moderators/mediators of the relationship between SOC,psychological distress symptoms and ***:Lower SOC scores(std beta=-0.504; P female gender(std beta = 0.176; P = 0.021) and White race(std beta = 0.164; P = 0.033) were independently associated with higher levels of depressive symptoms,while lower levels of SOC(std beta =-0.438; P ficant interaction between time since diagnosis and SOC was found with regard to the severity of depressive or anxiety symptoms,as the interaction term(time since diagnosis X SOC) had beta coefficients of-0.191(P = 0.009) and-0.172(P = 0.026),*** levels of anxiety symptoms(std beta =-0.369; P < 0.001),higher levels

Aim： To determine the androgenic effects of Basella alba and Hibiscus macranthus extracts in the rat and the bull, and to develop a novel in vitro test system using Leydig cells from bull testes. Methods： The effect of methanol extracts from both plants on testosterone production in isolated Leydig cells from the rat and the bull was analyzed using ^125I-radioimmunoassay （^125I-RIA）. Rat Leydig cells were obtained by common methods, whereas a novel technique was used to purify Leydig cells from bull testes. Results： Bull testes from the slaughter house were a cheap source of pure Leydig cells. In culture, these cells produced testosterone for 5-6 days, which can be stimulated by human chorionic gonadotrophin （hCG）. Basella alba extracts significantly enhanced testosterone production in bull and rat Leydig cells in a concentration-dependent manner. Hibiscus macranthus showed no androgenic effect but was shown to inhibit testosterone production at higher concentrations. Conclusion： Leydig cells purified from bull testes can be used as an alternative tool in experimental animal research. Certain fractions of Basella alba extract demonstrated androgenic potential whereas Hibiscus macranthus extracts did not.

A family of symmetric (hybrid) two step sixth P-stable methods for the accurate numerical integration of second order periodic initial value problems have been considered in this paper. These methods, which require only three (new) function evaluation per iteration and per step integration. These methods have minimal local truncation error (LTE) and smaller phase-lag of sixth order than some sixth orders P-stable methods in [1-3,10-11]. The theoretical and numerical results show that these methods in this paper are more accurate and efficient than some methods proposed in [1-3,10].

We complete the determination of the maximum sizes of （k,n）-arcs, n ≤ 12, in the projective gjelmslev planes over the two （proper） chain rings Z9 = Z/9Z and S3 = f3[X]/（X2） of order 9 by resolving the hitherto open cases n = 6 and n = 7. Parts of our proofs rely on decidedly geometric properties of the planes such as Desargues＇ theorem and the existence of certain subplanes.
