Epigastric hernias in children and the use of ultrasound in its diagnosis

Introduction there is a paucity of clinical data on pediatric epigastric hernias despite them accounting for up to 6%of all hernia repairs in *** aimed to provide additional data to supplement those 117 cases of a recent systematic review and to further clarify the role of ultrasound in diagnosing pediatric epigastric *** We retrospectively included all 60 patients treated for epigastric hernias in children in two tertiary pediatric surgical departments within 12 *** were tested via point-biserial correlation *** Epigastric hernias primarily affected preschool children with a median age of 39 *** vast majority of patients(88%)presented with swelling that was occasionally(30%)accompanied by *** defects could be found during clinical examination in 45%of patients with a median size of 5 mm(95%CI 3 to 10).Smaller defects were less likely to be palpable(r=-0.44,95%CI-0.08 to-0.7,p=0.021).Likewise,ultrasound was used more frequently with smaller fascial defect sizes(r=-0.51,95%Cl-0.16 to-0.74,p=0.007).Laparoscopic repair was used in 11 patients(19%)and more often(4/11)in combination with another simultaneous procedure than open repair(11/48).Conclusions Epigastric hernias are primarily a condition of the preschool *** can be beneficial if the diagnosis cannot be made clinically;otherwise,it is abdicable if it does not change the management of the patient's epigastric *** repairs might be beneficial for children with multiple defects or simultaneous procedures.

Better understanding of c-reactive protein and leukocytes in psychiatric inpatients with affective disorders:A biopsychosocial approach

BACkGROUND Affective disorders(AD)have been linked to inflammatory processes,although the underlying mechanisms of this relationship are still not fully *** is hypothesized that demographic,somatic,lifestyle,and personality variables predict inflammatory parameters in *** to identify biopsychosocial factors contributing to inflammation in AD measured with two parameters,C-reactive protein(CRP)and *** this observational study investigated 186 hospital inpatients diagnosed with AD using demographic parameters,serum inflammatory markers,somatic variables,psychological questionnaires,and lifestyle *** regression analyses were used to predict inflammatory markers from demographic,somatic,lifestyle,and personality *** Analyses showed that 33.8%of the variance of CRP was explained by body mass index and other somatic medication(***-diabetics),age and education,and age of affective disorder *** leukocytes,20.1%of the variance was explained by smoking,diet,metabolic syndrome(MetS),and anti-inflammatory medication(***-steroidal anti-inflammatory drugs).Other psychiatric or behavioural variables did not reach *** Metabolic components seem important,with mounting evidence for a metabolic affective disorder *** modifications and psychoeducation should be employed to prevent or treat MetS in AD.

BACkGROUND Lymphocytic choriomeningitis virus(LCMV)is a neglected rodent-borne arenavirus associated with transplacental transmission and fetal *** to summarize the epidemiological,clinical,and diagnostic features of reported patients with congenital LCMV *** A literature search was conducted in PubMed,Medline,Google Scholar,and *** keywords used were‘congenital lymphocytic choriomeningitis virus,’and 48 studies were *** addition,we conducted a relevant search by Reference Citation Analysis(RCA)(https://*** ***).RESULtS the results have shown 27 reports of congenital LCMV infection in 86 patients,with 52.73%of them being *** presented with chorioretinitis(83.53%),hydrocephalus(54.12%),and psychomotor retardation or developmental delay(54.12%).Computed tomography and/or magnetic resonance imaging most often demonstrated ventriculomegaly(74.07%),periventricular calcifications(66.67%),and microcephaly(40%).Most mothers of congenitally infected infants were exposed to rodents during pregnancy,predominantly mice,with flu-like symptoms mainly occurring during the first two trimesters of *** in congenitally infected children was 16.47%.the diagnosis of congenital LCMV infection was confirmed serologically in most patients(86.67%).CONCLUSION LCMV is still an insufficiently recognized fetal teratogen that often leads to long-term neurologic *** need to be familiar with LCMV and its potential teratogenic effect and as well as to effectively differentiate LCMV from other tORCH(t:toxoplasma gondii,O:Other pathogens,R:Rubella virus,C:Cytomegalovirus,H:Herpes simplex virus)pathogens.

In plants,genetic information is stored in three different locations:the nucleus,the plastids,and the *** genomes of the latter organelles code for only a tiny fraction of their respective proteomes,but they nevertheless require fully functional gene-expression machineries,which are far more complex than those of their prokaryotic *** is because the organellar gene expression(OGE)apparatus is a hybrid system,which combines features of the prokaryotic gene-expression apparatus with eukaryotic inventions(Pfannschmidt et al.,2015).In the plastids of higher plants,transcription is performed by three different RNA polymerases:two monomeric,nucleus-encoded RNA polymerases and a plastid-encoded(PEP)Escherichia coli-like enzyme(Pfannschmidt et al.,2015).

Here, we present a case of a mother and daughter who were both diagnosed with GD.

Identification of target Genes and transcription Factors Implicated in translation-Dependent Retrograde Sig- naling in Arabidopsis

Changes in organellar gene expression （OGE） trigger retrograde signaling. the molecular dissection of OGE-dependent retrograde signaling based on analyses of mutants with altered OGE is complicated by compensatory responses that mask the primary signaling defect and by secondary effects that influence other retrograde signaling pathways. therefore, to identify the earliest effects of altered OGE on nuclear transcript accumulation, we have induced OGE defects in adult plants by ethanol-dependent repression of PRORS1, which encodes a prolyl-tRNA synthetase located in chloroplasts and mitochondria. After 32 h of PRORS1 repression, the translational capacity of chloroplasts was reduced, and this effect subsequently intensified, while basic photosynthetic parameters were still unchanged at 51 h. Analysis of changes in whole-genome transcriptomes during exposure to ethanol revealed that induced PRORS1 silencing affects the expression of 1020 genes in all. Some of these encode photosynthesis-related proteins, including several down-regulated light-harvesting chlorophyll a/b binding （LHC） proteins. Interestingly, genes for presumptive endoplasmic reticulum pro- teins are transiently up-regulated. Furthermore, several NAC-domain-containing proteins are among the transcription factors regulated. Candidate cis-acting elements which may coordinate the transcriptional co-regulation of genes sets include both G-box variants and sequence motifs with no similarity to known plant c/s-elements.

GStM1-null and GStt1-active genotypes as risk determinants of primary open angle glaucoma among smokers

AIM： to evaluate glutathione transferase theta 1 and mu 1（GStt1 and GStM1） polymorphisms as determinants of primary open angle glaucoma（POAG） risk, independently or in combination with cigarette smoking, hypertension and diabetes ***： A case-control study with 102 POAG patients and 202 age and gender-matched controls was carried out. Multiplex-polymerase chain reaction method was used for the analysis of GStM1 and GStt1 polymorphisms. the differences between two groups were tested by the t-test or χ~2 test. Logistic regression analysis was used for assessing the risk for disease ***： the presence of GStM1-null genotype did not contribute independently towards the risk of POAG. However, individuals with GStt1-active genotype were at almost two-fold increased risk to develop glaucoma（P=0.044） which increased up to 4.36 when combined with GStM1-null carriers（P=0.024）. When glutathione transferase（GSt） genotypes were analyzed in association with cigarette smoking, hypertension and diabetes, only carriers of GStt1-active genotype had significantly increased risk of POAG development in comparison with GStt1-null genotype individuals with no history of smoking, hypertension and diabetes, respectively （OR=3.52, P=0.003; OR=10.02, P〈0.001; OR=4.53, P=0.002）. CONCLUSION： the results obtained indicate that both GStM1-null and GStt1-active genotypes are associated with increased POAG risk among smokers, suggesting potential gene-environment interaction in glaucoma development.

Plastids communicate their developmental and physiological status to the nucleus via retrograde signaling,allowing nuclear gene expression to be adjusted *** during plastid biogenesis and responses of mature chloroplasts to environmental changes are designated“biogenic”and“operational”controls,respectively.A prominent example of the investigation of biogenic signaling is the screen for gun(genomes uncoupled)*** the first five gun mutants were identified 30 years ago,the functions of GUN proteins in retrograde signaling remain controversial,and that of GUN1 is hotly ***,we provide background information and critically discuss recently proposed concepts that address GUN-related signaling and some novel gun ***,considering heme as a candidate in retrograde signaling,we revisit the spatial organization of heme biosynthesis and export from *** this review focuses on GUN pathways,we also highlight recent progress in the identification and elucidation of chloroplast-derived signals that regulate the acclimation response in green algae and ***,stress-induced accumulation of unfolded/misassembled chloroplast proteins evokes a chloroplast-specific unfolded protein response,which leads to changes in the expression levels of nucleus-encoded chaperones and proteases to restore plastid protein *** also address the importance of chloroplast-derived signals for activation of flavonoid biosynthesis leading to production of anthocyanins during stress acclimation through sucrose non-fermenting 1-related protein kinase ***,a framework for identification and quantification of intercompartmental signaling cascades at the proteomic and metabolomic levels is provided,and we discuss future directions of dissection of organelle-nucleus communication.
