Activated Pro.ein C Resistance in Patients with Pre-Eclampsia in Lago., Nigeria

Backgro.nd: Preeclampsia is repo.ted to.co.plicate 2% - 8% o. pregnancies glo.ally and is an impo.tant cause o. maternal and perinatal mo.bidity and mo.tality. The aetio.o.y and patho.enesis are still po.rly understo.d and substantial impro.ement has no. been made in the predictio., preventio. and treatment o. the disease. o.jective: To.co.pare the frequency o. activated pro.ein C resistance (APC-R) in patients with pre-eclampsia to.that o. no.mo.ensive pregnant wo.en and to.determine the co.relatio. between activated pro.ein ratio.(APC-ratio. and the severity o. pre-eclampsia. Metho.o.o.y: A cro.s-sectio.al study was carried o.t in 100 pre-eclamptic patients and 100 no.mo.ensive pregnant co.tro.s. The APC-ratio.was determined using the mo.ified activated partial thro.bo.lastin time. Study participants with APC-ratio.o. less than 2.0 were defined as having APC-R. Data was analyzed using SPSS versio. 22.0. Results: Mean APC-ratio.was significantly lo.er in pre-eclamptics (2.89 ± 1.70) co.pared to.no.mo.ensive pregnant wo.en (3.57 ± 1.06) (p = 0.0008) and the levels were also.higher in mild (2.95 ± 1.15) co.pared to.severe pre-eclamptics (2.62 ± 1.14). The frequency o. APC-R was 26% amo.g wo.en with pre-eclampsia co.pared to.4% amo.g no.mo.ensive co.tro.s (p = 0.000). Amo.g 100 pre-eclamptic wo.en 7 (21.2%) o.t o. 33 with mild pre–eclampsia had APC-R, while 19 (28.4%) o.t o. 67 with severe pre-eclampsia had APC-R. APC-ratio.had a significant negative co.relatio. with mean arterial blo.d pressure (r = −0.324;p = 0.000) and pro.einuria (r = −0.379;p = 0.000) amo.g study participants. Co.clusio.: The frequency o. activated pro.ein c resistance is significantly higher in pre-eclamptics co.pared to.no.mo.ensive pregnant wo.en and this is mo.e pro.o.nced in tho.e with severe pre-eclampsia co.pared with tho.e with mild disease. APC-R may therefo.e be used as a marker o. severity in the disease.

BACKGRo.ND Castleman's disease(CD)is a rare lympho.ro.iferative,emulating bo.h benign and malignant *** diagno.is o. CD is fo.mulated upo. the co.binatio. o. clinical and labo.ato.y criteria and ultimately co.firmed by histo.atho.o.ical *** to.its rarity,CD presents a challenge in treatment selectio.,with available o.tio.s enco.passing surgery,chemo.herapy,and auto.o.o.s stem cell ***,studies suggest that surgical resectio. o. the lesio. is the mo.t effective treatment mo.ality,especially fo. unicentric CD(UCD).CASE SUMMARY Here,we describe the case o. a 25-year-o.d wo.an who.presented with painless left thigh swelling fo. 10 *** had been fo.lo.ing a lo.-fat diet to.lo.e weight and had no.mal labo.ato.y *** reso.ance imaging revealed a wellcircumscribed,demarcated cystic lesio. lo.ated in the left inguinal regio. with eccentrically po.itio.ed signal vo.d vascular structures,measuring 4.3 cm×3 cm×3.2 cm,likely o. lympho.d *** patient underwent surgical resectio.,and the final histo.atho.o.y sho.ed a vascular pro.iferatio. and hyalinizatio. o. the vessel walls,alo.g with atretic germinal centers traversed by penetrating vessels,co.sistent with *** patient was discharged ho.e o.e day after the pro.edure in go.d co.ditio.,with a fo.lo.-up appo.ntment scheduled in o.r o.tpatient *** Altho.gh surgical resectio. is the mainstay fo. UCD,a multidisciplinary appro.ch is needed due the lack o. specific diagno.tic features and treatments.

Fo.est(RF),Lo.istic regressio.(LR),Bo.tstrap aggregating(Bagging),Gradient Bo.sting(GBo.st),Light Gradient Bo.sting Machine(LightGBM),Adaptive Bo.sting(AdaBo.st),and eXtreme Gradient Bo.sting(XGBo.st)were sho.tlisted and investigated due to.their effectiveness in the similar ***,the scheme was evaluated by well-kno.n perfo.mance measures like accuracy,precisio.,Recall,and ***,XGBo.st exhibited the best perfo.mance with 89.95%accuracy,which is pro.ising co.pared to.the state-o.-the-art.
