Role of Urokinase-Type Plasminogen Activator Receptors as an Early Detector for Treatment Outcome in Adult Acute myeloid Leukemia in Egyptian Patients

Background: The urokinase-type plasminogen activator system which consists of a proteinase (the urokinase-type plasminogen activator, uPA), a receptor (the urokinase-type plasminogen activator receptor uPAR or CD87) and inhibitors (PAI1) is enriched in several types of tumors and involved in proteolysis, cell migration and invasion. High expression of uPA and uPAR is associated with an increased relapse rate and shorter survival in breast cancer and colorectal carcinomas. Aims: This study shed light on the expression of uPAR in adult acute myeloid leukemia (AmL) and its prognostic relevance. methods: Peripheral blood and bone marrow samples are obtained from 54 newly diagnosed AmL adult patients, 20 healthy controls stained with anti CD87 and estimated on flow cytometry. Results: CD87 expression was heterogeneous in different FAB subtypes of AmL with high expression in monocytic leukemia (m4/m5) (P value 0.001). High expression of CD87 was associated with shorter survival and poor response to therapy (P = 0.028, 0.002 respectively). The most discriminating cut off value of CD87 expression was 20%, patients with less than 20% expression had 4.6 times better treatment outcome than those with more than or equal to 20%. On the multivariate analysis, CD87 was the most significant single variable that affect treatment outcome compared to total leucocytic count, hemoglobin level, platelets count, cytogenetic and FLT expression. Summary/Conclusion: High CD87 expression is an independent prognostic parameter associated with poor response and shorter overall survival in adult AmL patients.

Estimation of Curie Point Depth (CPD) across the Pan African Belt in Northern Cameroon from Aeromagnetic Data

The depth to the Curie isothermal point (CPD) was estimated using aeromagnetic data from northern Cameroon. The CPD was estimated using a two-dimensional power density spectral analysis of nineteen 30 × 30 km blocks over a series of Neoproterozoic lithologies associated with the Pan African orogeny. The results show that there are two main regions characterized by the CPD analysis: 1) Shallow depths (~6.37 - 10.09 km) which are located in several regions including the northern portions of the study area (toward the regions of Hina marbak, Gawel and northern moutouroua), the southeast (Kaele region), the south (Guider) and the southwest (mayo Oulo), and 2) deeper depths (~10.68 - 13.72 km) are located in the northeast (mindif) and southwest (southern moutouroua, Guider and Bossoum). The shallow depths can be related to two tectonic regimes: 1) the West-Central African Rift System with northeast-trending strike-slip faults emanating from the Gulf of Guinea and 2) the Cameroon Volcanic Line. However, the ultimate source of these shallow regions is interpreted to be related to the Cameroon Volcanic Line based on low seismic velocities imaged by recent broadband seismic studies which are concentrated along the northeast-trending strike-slip faults. An additional finding using the CPD depths, a Curie isothermal temperature of 580°C and a one-dimensional heat flow model, was heat-flow values ranging from 105.68 to 227.63 mW/m2, which are above average global heat flow values and are therefore indicative of potential geothermal resources.

This paper proposes a single integrated traffic enforcement system that is able to recognize and report various traffic violations. It consists of a Wi-Fi infrastructure that enables communication between moving vehicles and a central node. Unlike existing solutions, which address single violations, the proposed model encompasses several issues like exceeding speed limits, entering a no entry street, car theft, congestion and tolling. OPNET simulations were run to test the Wi-Fi model and define its different characteristics and limitations. A proof-of-concept case was modeled, and the proposed architecture succeeded in meeting all design requirements.

measuring Radiotherapy Setup Errors in ImRT Treated Head and Neck Cancer Patients Requiring Bilateral Neck Irradiation, NCI-Egypt Experience

Objective: We aim to quantify the magnitude of setup errors in intensity-modulated radiotherapy (ImRT) treated Head and Neck cancer patients and recommend appropriate PTV margin. methods: 60 patients with head and neck cancer required bilateral neck irradiation were planned and treated by simultaneous integrated boost ImRT technique either treated radically or postoperative. Patients undergoing image-guided radiotherapy (IGRT) each with once weekly scheduled cone beam computed tomography (CBCT). The 3D displacements, systematic and random errors were calculated. The appropriate PTV expansion was determined using Van Herk’s formula. Results: mean 3D displacement was 0.16 cm in the vertical direction, 0.14 cm in the horizontal direction and 0.16 cm in the longitudinal direction. Conclusion: Use of weekly CBCT allows the planning target volume (PTV) expansion to be reduced according to our setup. The appropriate clinical target volume (CTV)-PTV margin for our institute is 0.30 cm, 0.38 cm, and 0.33 cm in the horizontal, vertical, and longitudinal directions, respectively.

AIm:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived mansoura University,Egypt between January 2002 and April *** data of the patients were collected from the medical *** were divided into two groups according to their survival:patients surviving less than 5 years and those who survived > 5 ***:There were 34(14%) long term survivors(5 year survivors) among the 243 *** survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two *** level of CA 19-9 was significantly higher in patients surviving monstrated nine variables to be significantly associated with survival > 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.

Background: Acute myeloid leukemia is a heterogeneous hematological malignancy with diversity in molecular, phenotypic and genetic alteration. Accurate assessment of prognosis is mandatory to reduce relapse, changing treatment related mortality, decision making for appropriate therapy protocols. Aim: A case control observational analytical study to assess the prognostic value of poliovirus (CD155) in acute myeloid leukemia and its significant in-patient stratification. Subject & methods: 100 newly diagnosed de novo AmL selected from oncology center, mansoura University. measurement of PVR (CD155) by multiparameter flow cytometry in addition to cytogenetic and molecular stratification. Results: Statistical stratification according to median cut off CD155 > 5.35. Higher expression was associated with an increased risk of death and relapse, LDH. While lower expression is associated with longer OS & DFS. Cox regression with univariate analysis revealed that LDH, molecular stratification, CD155 were significant risk factors for shorter OS, while positive expression of CD16, CD56 AmL m3 were significantly associated with longer survival. multivariate analysis revealed that CD155 was a significant risk factor for shorter OS. Conclusion: Variability in expression of CD155 in AmL patients due to dual costimulatory and inhibitory function, high expression associated with tumor bulk and progression, short OS, increased relapse rate. Cox regression analysis shows its independent risk factor. It could be implemented in routine laboratory for risk stratification into low, high risk that needs more therapeutic modalities with new immunotherapy. In future could be targeted therapy such as PDL, CTLA-4 and open more therapeutic hope for those patients.

This paper proposes a methodology for calculating the energy consumed by a Wireless Sensor Network as well as its throughput under the effect of a jamming node modelling interference to account for collisions and retransmissions. Accordingly, the proposed methodology takes into consideration retransmissions and data dropped due to interference and collisions simultaneously. Simulations are conducted using OPNET to model various scenarios utilizing off-the-shelf wireless communication standards, namely ZigBee, Wi-Fi and Low Power Wi-Fi. A figure of merit is developed to offer more representative results for applications with different requirements. In achieving different requirements for a given application, there is a clear trade-off between energy consumption and throughput.

Both stereo-microscope systems and moderate magnification camera systems are used to make accurate,quantitative measurements of deformation to obtain surface strain measurements on mouse carotid *** results demonstrate that the microscope system with three dimensional digital image correlation(DIC-3D) successfully measures the full 3D displacement and surface strain fields at the micro-scale during pressure cycling of 0.40mm diameter mouse *** also confirm that moderate magnification optical measurements provide reasonable estimates for the average circumferential and axial normal strains during mechanical loading,providing sufficient information for estimation of soft tissue material properties.

Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIm To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

Surgical management of primary hyperparathyroidism guided by double-phase Tc-99m-mIBI scintigraphy

Objective:The aim of this work was to study the clinicopathological features of cases with primary hyperparathyroidism (PHPT) referred to National Cancer Institute (NCI), Cairo University in the last six years and to study the role of nuclear medicine techniques in the initial diagnosis and preoperative localization directing surgical management of these cases. methods: This study included 27 patients with PHPT properly diagnosed and treated in the NCI, Cairo University from January 2005 to December 2010. Preoperative neck U/S and Tc-99m-mIBI scintigraphy were done in all cases. If preoperative localization detected a single lesion, unilateral exploration was done. Bilateral exploration was done if multiple foci of active parathyroid glands or no lesions were detected. Results: This study included 27 patients (8 males and 19 females). The median age was 43 years (range from 19 to 68 years). All cases presented with bone disease in the form of bony pain in 23 patients, bony swellings in 15 patients and pathological fractures in 6 cases. Twenty one patients (77.8%) had single adenoma, 5 cases (18.5%) had parathyroid hyperplasia, and only one case had double adenomas. The sensitivity of neck U/S in detecting single adenoma was 61.9% (13/21), with 81% positive predictive value (PPV) while the sensitivity of Tc-99m-mIBI scintigraphy was 90.5% (19/21) with 100% PPV. Based on preoperative localization tests, unilateral exploration was done in 19 patients with solitary adenomas. All cases in this group were cured with no reported case of persistent or recurrent hypercalcemia. Conclusion: Presentation of PHPT may mimic malignant bone tumors but fortunately these patients were correctly diagnosed with bone scan which confirm the presence of metabolic bone disease rather than bone metastases.
