One of the most common kinds of cancer is breast *** early detection of it may help lower its overall rates of *** this paper,we robustly propose a novel approach for detecting and classifying breast cancer regions in thermal *** proposed approach starts with data preprocessing the input images and segmenting the significant regions of *** addition,to properly train the machine learning models,data augmentation is applied to increase the number of segmented regions using various scaling *** the other hand,to extract the relevant features from the breast cancer cases,a set of deep neural networks(VGGNet,ResNet-50,AlexNet,and GoogLeNet)are *** resulting set of features is processed using the binary dipper throated algorithm to select the most effective features that can realize high classification *** selected features are used to train a neural network to finally classify the thermal images of breast *** achieve accurate classification,the parameters of the employed neural network are optimized using the continuous dipper throated optimization *** results show the effectiveness of the proposed approach in classifying the breast cancer cases when compared to other recent approaches in the ***,several experiments were conducted to compare the performance of the proposed approach with the other *** results of these experiments emphasized the superiority of the proposed approach.

Increasing plant density and improving N fertilizer rate along with the use of high density-tolerant genotypes would lead to maximizing maize(Zea mays L.) grain productivity per unit land area. The objective of this investigation was to match the functions of optimum plant density and adequate nitrogen fertilizer application to produce the highest possible yields per unit area with the greatest maize genotype efficiency. Six maize inbred lines differing in tolerance to low N and high density(D) [three tolerant(T); L-17, L-18, L-53,and three sensitive(S); L-29, L-54, L-55] were chosen for diallel crosses. Parents and crosses were evaluated in the 2012 and 2013 seasons under three plant densities: low(47,600),medium(71,400), and high(95,200) plants ha-1and three N fertilization rates: low(no N addition), medium(285 kg N ha-1) and high(570 kg N ha-1). The T × T crosses were superior to the S × S and T × S crosses under the low N–high D environment in most studied traits across seasons. The relationships between the nine environments and grain yield per hectare(GYPh) showed near-linear regression functions for inbreds L54, L29, and L55 and hybrids L18 × L53 and L18 × L55 with the highest GYPh at a density of47,600 plants ha-1and N rate of 570 kg N ha-1and a curvilinear relationship for inbreds L17, L18, and L53 and the rest of the hybrids with the highest GYPh at a density of95,200 plants ha-1combined with an N rate of 570 kg N ha-1. Cross L17 × L54 gave the highest grain yield in this study under both high N–high-D(19.9 t ha-1) and medium N–high-D environments(17.6 t ha-1).

Long-term results of Ahmed glaucoma valve implantation in Egyptian population

AIm： To evaluate the long-term results and complications of Ahmed glaucoma valve （AGV） implantation in a cohort of Egyptian patients. mEThODS： A retrospective study of 124 eyes of 99 patients with refractory glaucoma who underwent AGV implantation and had a minimum follow-up of 5y was performed. All patients underwent complete ophthalmic examination and intraocular pressure （lOP） measurement before surgery and at ld, weekly for the 1st month, 3, 6mo, and ly after surgery and yearly afterward for 5y. lOP was measured by Goldmann applanation tonometry and/or Tono-Pen. Complications and the number of anti-glaucoma medications needed were recorded. Success was defined as lOP less than 21 mm hg with or without anti-glaucoma medication and without additional glaucoma surgery. RESULTS： mean age was 23.1±19.9y. All eyes had at least one prior glaucoma surgery, lOP was reduced from a mean of 37.2±6.8 to 19.2±5.2 mm hg after 5y follow-up with a reduced number of medications from 2.64±0.59 to 1.81±0.4. Complete and qualified success rates were 31.5% and 46.0% respectively at the end of follow-up. The most common complications were encapsulated cyst formation in 51 eyes （41.1%）, complicated cataract in 9 eyes （7.25%）, recessed tube in 8 eyes （6.45%）, tube exposure in 6 eyes （4.8%） and corneal touch in 6 eyes （4.8%）. Other complications included extruded AGV, endophthalmitis and persistent hypotony. Each of them was recorded in only 2 eyes （1.6%）. ~ CONCLUSION： Although refractory glaucoma is a difficult problem to manage, AGV is effective and relatively safe procedure in treating refractory glaucoma in Egyptian patients with long-term follow-up. Encapsulated cyst formation was the most common complication, which limits successful lOP control after AGV implantation. however, effective complications management can improve the rate of success.

In this study,energetic,economic,and environmental analysis of solid oxide fuel cell-based combined cooling,heating,and power(SOFC-CChP)system is proposed for a cancer care hospital *** energy required for the hospital power,cooling,and heating demands was obtained based on real and detailed field data,which could serve as a reference for future works in the *** data with a 3D model for the hospital building are constructed and created in eQUEST software to precisely calculate the energy demands of the existing system(baseline case).Then,energetic,economic,and environmental models were developed to compare and assess the performance of the proposed SOFC-CChP *** results show that the proposed system can cover about 49% to 77% of the power demand of the hospital with an overall efficiency of 78.3%.Also,the results show that the levelized cost of electricity of the system and its payback period at the designed capacity of the SOFC is 0.087S/kWh and 10 years,***,compared to the baseline system of the hospital,the SOFC-CChP reduces the CO_(2) emission by 89% over the *** sensitivity analysis showed that a maximum SOFC efficiency of 52%and overall efficiency of 80%are achieved at cell operating temperature of 1027℃ and fuel utilization factor of 0.85.

Objective:The aim of this study was to evaluate the predictor of unsuccessful outcome of renal angioembolization(RAE).Knowing those predictors may help in avoiding unnecessary RAE procedures and their associated side effects,while helping to prepare for an alternate procedure and improving patient's overall ***:A retrospective analysis between January 2006 and December 2018 was performed,and the indications for RAE were classified into post-traumatic,iatrogenic,renal tumors,and *** who underwent RAE prior to nephrectomy were *** tomography angiography was performed in patients with normal renal function and those who had no contrast allergy,otherwise magnetic resonance angiography was *** the purpose of statistical analysis,we stratified patients into two main categories based on the final outcome—successful or ***:Of 180 patients,32 with negative angiography were eliminated,leaving 148 patients;136(91%)had successful outcomes after one or more trials and 12 had unsuccessful *** mean age was 45±15 years,and 105(71%)were *** gender,side of the lesion,presence of hematuria,indication for RAE,nor the type of lesion affected the *** the other hand,renal anatomy with presence of accessory artery was the only predictor to failed RAE(p=0.001).Failed RAE trial was a predictor for nephrectomy as a secondary procedure(p=0.03).Conclusion:No pre-procedural predictors could anticipate the RAE outcome,and different indications can be scheduled to RAE,which is equally *** presence of accessory renal artery on diagnostic angiography is the only factor that may predict the failure of the procedure.

Pancreatic cancer is one of the deadliest cancers,ranking fourth among cancer-related deaths. Despite all the major molecular advances and treatment breakthroughs, mainly targeted therapies, the cornerstone treatment of metastatic pancreatic cancer(m PC) remains cytotoxic chemotherapy. In 2016, more than 40 years after the introduction of gemcitabine in the management of m PC, the best choice for first-line treatment has not yet been fully elucidated. Two main strategies have been adopted to enhance treatment efficacy. The first strategy is based on combining non-cross resistant drugs, while the second option includes the development of newer generations of chemotherapy. more recently, two new regimens, FOLFIRINOX and gemcitabine/nab-paclitaxel(GNP), have both been shown to improve overall survival in comparison with gemcitabine alone, at the cost of increased toxicity. Therefore, the best choice for first line therapy is a matter of debate. For some authors, FOLFIRINOX should be the first choice in patients with an Eastern Cooperative Oncology Group score(0-1) given its lower hazard ratio. however, others do not share this opinion. In this paper, we review the main comparison points between FOLFIRINOX and GNP. We analyze the two pivotal trials to determine the similarities and differences in study design. In addition, we compare the toxicity profile of the two regimens as well as the impact on quality of life. Finally, we present studies revealing real life experiences and review the advantages and disadvantages of possible second-line therapies including their cost effectiveness.

AIm： To study the role of ultrasound biomicroscopy（UBm） in the assessment of the bleb function after trabeculectomy with mitomycin C（mmC）. mEThODS： This was a cross-sectional study including all cases had undergone trabeculectomy with mmC 0.2 mg/mL for 3 min. Participants were recruited from the follow-up cases at Department of Ophthalmology, Tanta University in the period from August 2015 to August 2016. Full history taking and ophthalmological examination were performed. Intraocular pressure（IOP） was measured using Goldmann applanation tonometry and the history of prescribed postoperative antiglaucoma medications was recorded. Accordingly, the trabeculectomy blebs were divided clinically into successful blebs when the IOP post-operative was ≤18 mmhg without medications. Scanning examination of the filtering bleb using UBm examination was performed at the period from 2 to 36 mo after trabeculectomy（13.6±9.7 m o）. RESULTS： This study included 33 trabeculectomy filtering blebs of 25 patients（9 males and 16 females）. The mean age of patients was 53.2±14.2 y（range 25-71y）. We had 20 eyes with complete success, 9 eyes with qualified success, and 4 eyes with failure after trabeculectomy with mmC. The blebs were classified into successful blebs（IOP ≤18 mmhg without antiglaucoma medications）, others were classified into qualified successful and failed blebs. There was a significant correlation between certain UBm findings（intra-bleb reflectivity, draining sub-scleral tract, bleb height, and intrableb fluid cysts） and cases with different grades of clinical functional success（P〈0.01）.CONCLUSION： UBm is an objective tool in the functional assessment of the post-trabeculectomy filtering bleb through a significant correlation between certain UBm parameters and the different grades of clinical functional success.

为了寻求高效、经济的高氮钢生产工艺,用等离子旋转电极工艺(PREP)生产高氮钢并测试了所生产的几种典型高氮钢的性能。结果表明,氮可显著地提高奥氏体不锈钢1 4301的室温及高温强度;氮可以提高马氏体不锈钢的强度。为保证高氮马氏体不锈钢的淬透性,应根据氮含量适当降低其碳含量或提高铬含量。对于铬含量为17%的马氏体不锈钢1 4122,其碳和氮的总含量不应超过0 65%;当氮含量达到0 24%时,铁素体不锈钢1 4016转变成为马氏体钢,其强度高于碳和氮的总含量与其相当的普通马氏体不锈钢1 4122。

YKL-40 expression in human hepatocellular carcinoma:a potential biomarker?

BACKGROUND:YKL-40 is a new biomarker with diagnostic value in many different *** it may serve as a biomarker for hepatocellular carcinoma (hCC) is still *** study aimed to examine the expression of YKL-40 in the serum and liver tissues of hCC patients and in hCC cell lines,in comparison with that in non-hCC liver disease patients and non-tumor hepatic cell lines,***:Immunohistochemical staining was used to detect YKL-40 protein expression in liver biopsy specimens from 8 hCC *** was used to assess the serum YKL-40 level in 90 hCC patients,90 inactive hBsAg carrier (IhC) patients with normal liver functions,and 90 liver cirrhosis ***-time PCR was used to determine the YKL-40 mRNA expression in three hCC cell lines and two non-tumor hepatic cell ***:Immunohistochemical staining of liver biopsy specimens from hCC patients showed that the YKL-40 protein expression in tumor tissue was higher than that in adjacent normal *** revealed that the YKL-40 serum level in the hCC group was significantly higher than that in the IhC group,but not significantly different from that in the cirrhosis ***-time PCR showed that YKL-40 mRNA levels in hCC cell lines were significantly higher than those in nontumor hepatic ***:YKL-40 is highly expressed in hCC at the molecular,cellular and tissue ***,it may not serve as a serum biomarker for hCC because measurement of the serum YKL-40 level cannot distinguish hCC from cirrhosis.
