Impact of Helicobacter pylori virulence markers on clinical outcomes in adult populations

Hepatocellular carcinoma(HCC)is the most common primary tumor of the liver and often evolves from regenerating nodules in liver *** there have been significant advances in the treatment of HCC,the overall prognosis in patients with a large tumor burden,vascular invasion,or extrahepatic spread remains poor.1 The magnitude of liver stiffness in cirrhosis has been consistently associated with an increased risk of development of HCC.

Surgical perspectives in peptic ulcer disease and gastritis

For much of the twentieth century, surgery was frequently the solution for peptic ulcer disease. Our understanding of the pathophysiology of ulcers paralleled the development of potent pharmaceutical therapy. As the surgical world developed parietal cell vagotomy which would minimize the complications of surgery, patients failing medical therapy became rare. Emergent surgery for complicated peptic ulcers has not declined however. The development of proton pump inhibitors and the full understanding of the impact of H pylori has led to a trend towards minimalism in surgical therapy for complicated peptic ulcer disease. In addition to the changes in patient care, these developments have had an impact on the training of surgeons. This article outlines these trends and developments.

Peroxisomes are highly dynamic single-membrane-bound eukaryotic organelles displaying great variability in enzymatic content （Platta and Erdmann, 2007）. Plant peroxisomes function in a plethora of crucial development and stress ameliorating processes. Among the functions of plant peroxisomes are 13-oxidation of fatty acids, phytohormone production, participation in photorespiration, the glyoxyalte cycle, detoxification processes, and signal molecule generation （Hu et al., 2012）.

The use of anesthetics is a well-known treatment for severely injured *** the present study we tested the pathophysiology of several levels of injury damage in a rat model and also tested the effect of Equithesin on brain vitality in these *** Brain Injury(TBI)was induced using thefluid percussion injury model in four levels:mild,moderate and two levels of severe *** real-time evaluation was performed by the multiparametric monitoring assembly(MPA)which enable cerebral bloodflow(CBF)monitoring by laser Dopplerflowmetry,mitochondrial NADH(Nicotinamide adenine dinucleotide)monitoring by thefluorometric technique,ionic homehostasis using special mini-electrodes,intracranial pressure(ICP)by the ICP camino device and needle electrodes for ECoG(Electrocorticogram)*** results showed high correlation between the level of impact and the extent of changes in the physiological properties of the injury as indicated by the changes in all parameters monitored using the MPA ***,Equithesin improved CBF,ionic extracellular level and mitochondrial redox state following mild and moderate TBI while in severe TBI,Equithesin did not improve the metabolic state of the cerebral cortex,although it decreased the mortality rate from 66%to 20%,and following extra-severe TBI level,Equithesin did not improve survival *** conclusion it seems that Equithesin's protective effect exists under mild to moderate levels of injury and not in case of severe injuries.

Identification of sumoylated targets in proliferating mouse spermatogonia and human testicular seminomas

Spermatogenesis is regulated by a complex network of posttranslation ***(a modification by small ubiquiti like modifiers,or SUMO proteins)was identified as an importa nt cellular event in d iff ere nt cell *** proteins are highly expressed in the testis,and their role during spermatogenesis has begun to be *** the important role of sumoylation in the regulation of mitosis and cancer progress!on in other tissues,the aim of the current study was to identify the targets of SUMO in proliferating mouse spermatog onia and huma n semi noma tissues and to in itially exami ne the level of sumoylation in relation to the proliferative activity of the *** freshly purified spermatogonia and Cl8-4 spermatogonia cell line,mass spectrometry analysis identified several SUMO targets implicated into the proliferation of spermatogonia(such as heat shock protein 60[HSP60]and prohibitin).Tissue array and western blot approaches showed that SUMO expression is a prominent feature of human seminomas and that the proliferative activity of the tumor tissues was positively correlated with the level of SUMO *** regulation of sumoylati on with si-RNA was not sufficie nt to sign ificantly affect the proliferation of C18-4 spermatogonia;however,SUMO overexpression in creased the proliferation rate of the *** data suggest that cells are more sensitive to an elevated level of SUMO,and that this situation may lead to an upregulated cellular proliferation and,possibly,*** spectrometry analysis identified around a hundred SUMO targets in seminoma ***,many of the identified proteins(such as proliferating cell nuclear antigen[PCNA],DNA topoisomerase 2-alpha[Top2A],prohibitin,14-3-3 protein,and others)were implicated in oncogenic transformation and cancer progression.

Pseudocirrhosis in a pancreatic cancer patient with liver metastases: A case report of complete resolution of pseudocirrhosis with an early recognition and management

We report a case of pseudocirrhosis arising in the setting of regression of liver metastases from pancreatic cancer. A 55-year-old asymptomatic woman presented to our clinic with newly diagnosed metastatic pancreatic cancer with extensive liver metastases. She underwent systemic chemotherapy with gemcitabine and oxaliplatin (GEMOX). After 8 cycles of therapy, she had a remarkable response to the therapy evidenced by decline of carcinoembryonic antigen (CEA) and CA19 by > 50% and nearly complete resolution of hepatic metastases in computed tomography (CT) scan. Shortly after, she developed increasing bilateral ankle edema and ascites, associated with dyspnea, progressive weight gain, and declining performance status. Gemcitabine and oxaliplatin were discontinued as other causes of her symptoms such as congestive heart disease or venous thrombosis were ruled out. CT scan 6 mo after the initiation of GEMOX revealed worsening ascites with a stable pancreatic mass. However, it also revealed a lobular hepatic contour, segmental atrophy, and capsular retraction mimicking the appearance of cirrhosis. She was managed with aggressive diuresis and albumin infusions which eventually resulted in a resolution of the above- mentioned symptoms as well as complete resolution of pseudocirrhotic appearance of the liver and ascites in CT scan. This case demonstrates that pancreatic cancer patients can develop pseudocirrhosis. Clinicians and radiologist should be well aware of this entity asearly recognition and management can lead to a near complete recovery of liver function and much improved quality of life as illustrated in this case.

Epidemiological studies suggest a direct link between melanoma and Parkinson＇s disease（PD）; however, the underlying molecular basis is unknown. Since mutations in Parkin are the major driver of early-onset PD and Parkin was recently reported to play a role in cancer development, we hypothesized that Parkin links melanoma and PD. By analyzing whole exome/genome sequencing of Parkin from 246 melanoma patients, we identified five non-synonymous mutations, three synonymous mutations, and one splice region variant in Parkin in3.6% of the samples. In vitro analysis showed that wild-type Parkin plays a tumor suppressive role in melanoma development resulting in cell-cycle arrest, reduction of metabolic activity, and apoptosis. Using a mass spectrometry-based analysis, we identified potential Parkin substrates in melanoma and generated a functional protein association network. The activity of mutated Parkin was assessed by protein structure modeling and examination of Parkin E3 ligase activity. The Parkin-E28 K mutation impairs Parkin ubiquitination activity and abolishes its tumor suppressive effect. Taken together, our analysis of genomic sequence and in vitro data indicate that Parkin is a potential link between melanoma and Parkinson＇s disease. Our findings suggest new approaches for early diagnosis and treatment against both diseases.

Hepatitis E in Israel: A nation-wide retrospective study

AIM: To investigate the epidemiology, risk factors and clinical course of acute hepatitis E virus (HEV) infection in Israel, an industrialized ***: A retrospective analysis of acute HEV cases diagnosed in Israel from 1993 to 2013. Acute HEV was defined by ALT/AST elevation and a positive HEV PCR test or positive anti-HEV-IgM serology. HEV RNA was tested by quantitative reverse transcription PCR. Antibodies to HEV were tested retrospectively using an ELISA assay. HEV-RNA was sequenced using RT-PCR of ORF1 and ORF2 regions to diagnose genotype of the virus. Epidemiologic and clinical data were collected by reviewing the clinical files and through a telephone interview according to a structured ***: Acute HEV was diagnosed in 68 patients. Among the 59 patients who gave an informed consent and were interviewed, 41% of infections were autochthonous (acquired in Israel), 44% travel-related and 15% imported by foreign workers. Autochthonous patients were mainly females (62.5%), more than half of them pregnant, 26% recalled consuming food or water in areas with poor sanitation, 44% ate non-kosher meat. Fulminant hepatitis developed in 3 patients (5%), all of them were females, two of them with post-partum infection, all acquired the disease in Israel (autochthonous). Israeli travelers with imported infection were predominantly males (73%), acquired the disease in the Indian subcontinent (81%), with 100% reporting having consumed fresh vegetables and drinks with ice cubes abroad. Six patients’ sera were tested for genotype and revealed HEV genotype 1 (all cases acquired in the Indian subcontinent).CONCLUSION: This is the first report which highlights the existence of hepatitis E as an autochthonous infection in Israel. Imported HEV originates mostly from the Indian subcontinent.

Geomorphological Zoning of Racha Region for Geopark Planning

Geomorphological zoning and complex relief research were conducted in the Racha region, which is located In the Caucasus, in the northwestern part of Georgia, south of the Greater Caucasus, in the central part of which there are Precambrian and Paleozoic crystalline substrates (gneisses, migmatites, crystalline shales) and granitoid intrusions of different ages. The Caucasus axis and higher massifs are built of granitoid, gneiss, and metamorphic shales. The oldest rocks (600 million years old) are found at the highest points and the age of the rocks decreases in the vicinity of the hypsometry steps (70 - 30 million rocks). The geological structure of the region has made a significant contribution to the formation of relief landscapes. Karst, erosion-denudation, volcanic, periglacial relief, and various interesting forms are formed here: caves, valleys, canyons, glacial and rocky lakes, waterfalls, deep valleys, mineral waters, and more. The peculiarity of the relief also influenced the mining geological history (5000 AD) and the creation of ancient settlements. Here, still in ancient Racha, at the head of the main river, Rioni, ancient settlements were formed due to iron, gold, and copper metal, which is confirmed by archeological monuments.
