Background:Immune checkpoint inhibitors(ICIs)are effective in a subset of patients with metastatic solid ***,the patients who would benefit most from ICIs in biliary tract cancer(BTC)are still *** and methods:We molecularly characterized tissues and blood from 32 patients with metastatic BTC treated with the ICI pembrolizumab as second-line ***:All patients had microsatellite stable(MSS)type *** of the 32 patients achieved partial response(PR),with an objective response rate(ORR)of 9.4%(95%confidence interval[CI],2.0–25.2)and nine showed stable disease(SD),exhibiting a disease control rate(DCR)of 37.5%(95%CI,21.1–56.3).For the 31 patients who had access to PD-1 ligand 1(PD-L1)combined positive score(CPS)testing(cut-off value≥1%),the ORR was not different between those who had PD-L1-positive(PD-L1+;1/11,9.1%)and PDL1-(2/20,10.0%)tumors(p=1.000).The tumor mutational burden(TMB)of PD-L1+BTC was comparable to that of PD-L1-BTC(p=0.630).TMB and any exonic somatic mutations were also not predictive of pembrolizumab *** analysis of blood and tumor samples demonstrated a relatively high natural killer(NK)cell proportion in the peripheral blood before pembrolizumab treatment in patients who achieved tumor ***,the tumors of these patients presented high enrichment scores for NK cells,antitumor cytokines,and Th1 signatures,and a low enrichment score for cancer-associated ***:This study shows the molecular characteristics associated with the efficacy of pembrolizumab in BTC of the MSS type.

BACKGROUND The classification of uterine sarcomas is based on distinctive morphological and immunophenotypic characteristics,increasingly supported by molecular genetic *** on neurotrophic tyrosine receptor kinase(NTRK)gene fusionpositive uterine sarcoma,potentially aggressive and morphologically similar to fibrosarcoma,are limited due to its recent ***-TRK immunohistochemistry(IHC)analysis serves as an effective screening tool with high sensitivity and specificity for NTRK-fusion *** SUMMARY We report a case of a malignant mesenchymal tumor originating from the uterine cervix,which was pan-TRK IHC-positive but lacked NTRK gene fusions,accompanied by a brief literature review.A 55-year-old woman presented to the emergency department with abdominal pain and distension,exhibiting significant ascites and multiple solid pelvic *** examination revealed a tumor encompassing the uterine cervix,extending to the vagina and uterine corpus.A punch biopsy of the cervix indicated NTRK sarcoma with positive immunochemical pan-TRK ***,subsequent next generation sequencing revealed no NTRK gene fusion,leading to a diagnosis of poorly differentiated,advanced-stage *** The clinical significance of NTRK gene fusion lies in potential treatment with TRK inhibitors for positive *** such rare tumors is crucial due to the potential applicability of tropomyosin receptor kinase inhibitor treatment.

Gas wiping is a decisive operation in hot-dip galvanizing process. In special, it has a crucial influence on the thickness and uniformity in coating film, but may be subsequently responsible for the problem of splashing. The progress of industry demands continuously the reduction of production costs which may relate directly with the increase of coating speed, and the speed up of coating results in the increase of stagnation pressure in gas wiping system in final. It is known that the increase of stagnation pressure may accompany a harmful problem of splashing in general. Together with these, also, from the view point of energy consumption, it is necessary to design a nozzle optimally. And there is known that the downward tilting of nozzle using in air knife system is effective to prevent in somewhat the harmful problem of splashing. In these connections, first, we design a nozzle with constant expansion rate. Next, for the case of actual coating conditions in field, the effects of tilting of the constant expansion rate nozzle are investigated by numerical analysis. Under the present numerical conditions, it was turned out that the nozzle of constant expansion rate of p = having a downward jet angle of 5^0 is the most effective to diminish the onset of splashing, while the influence of small tilting of the nozzle on impinging wall pres- sure itself is not so large.

Effect of asymmetrical channel chirping on field intensity patterns of supermodes and modal discrimination in index guided laser arrays

The effect of channel-width chirping on near- and far-field intensity patterns of the six supermodes was investigated. The supermode discrimination was evaluated in various channel-chirped index guided laser arrays. The results show that the linearly channel-chirped laser array has very good supermode discrimination which is better than that of a uniform laser array, the V channel-chirped laser array has the smallest radiation angle of the fundamental supermode among the calculated arrays, and the asymmetrically V channel-chirped array has a very small radiation angle of the fundamental supermode, which is smaller than that of the uniform array and also allows for very good fundamental supermode discrimination against the higher-order supermodes, which is better than that of the V channel-chirped laser array.

AIM To investigate the clinicopathological features and prognostic implications of combined MYC and fibroblast growth factor receptor 1(FGFR1) status in esophageal squamous cell carcinomas(ESCCs). METHODS All patients with ESCC(n = 180) underwent surgical resection at Seoul National University Hospital sometime between 2000 and 2013. A tissue microarray was constructed using cores obtained from representative tumor areas of formalin-fixed, paraffin-embedded tissue blocks. FGFR1 and MYC copy numbers were quantified using fluorescence in situ hybridization. The level of MYC expression was determined using immunohistochemistry. FGFR1 and MYC amplification status was compared between primary and metastatic lymph nodes. Univariate and multivariate survival analyses were performed according to adjuvant therapy *** FGFR1 and MYC amplifications were observed in 21.4%(37/173) and 54.2%(91/168) of patients, respectively, while MYC expression was observed in 58.9%(106/180) of patients. There was a positive correlation between MYC amplification and overexpression(P = 0.002). Although FGFR1 amplification was not associated with MYC amplification or expression, 12.3%(20/163) of patients exhibited both FGFR1 amplification and MYC expression. There was also a correlation in FGFR1 amplification status between matched primary tumors and metastatic lymph nodes(P < 0.001). MYC expression was higher in ESCCs with p T1(P < 0.001) and in those with no lymph node metastasis(P = 0.023). MYC expression was associated with prolonged diseasefree survival(P = 0.036) and overall survival(OS)(P = 0.017) but was not an independent prognostic factor. FGFR1 amplification was an independent predictor for prolonged OS in all patients(P = 0.029) and in those who did not receive adjuvant therapy(P = 0.013). Combined FGFR1 amplification and MYC expression predicted better OS in patients who did not receive adjuvant therapy(P = 0.034) but not in those who did receive adjuvant ***

Acupuncture for Bell's Palsy:A Systematic Review and Meta-analysis

Objective： To assess the clinical evidence for and against acupuncture as a treatment for Bell＇s palsy. Methods： We conducted a literature search of 15 databases from their inception to December 2010 without language restrictions. We included all randomized clinical trials （RCTs） regardless of their controls. Methodological quality was evaluated using the Cochrane risk of bias assessment tool. Results： Of the 3 474 articles, only eight RCTs met our inclusion criteria. Four RCTs tested the effects of acupuncture against drug therapy on disease response rate. The meta-analysis of these data showed significant improvements in the acupuncture group [n=463, risk ratio （RR）=1.07, 95% CI： 1.02 to 1.13; P=0.006, I^2=0%]. Six RCTs tested the effects of acupuncture plus drug therapy versus drug therapy alone. The meta-analysis of this set of RCTs also showed the favorable effects of acupuncture on disease response rate （n=512, RR=1.11, 95% CI： 1.05 to 1.17; P=0.001, I^2=13%）. Conclusions： The evidence supporting the effectiveness of acupuncture for treating Bell＇s palsy is limited. The number and quality of trials are too low to form firm conclusions. Further rigorous RCTs are warranted but need to overcome the many limitations of the current evidence.

Experimental and Computational Studies on Coanda Nozzle Flow for the Air Knife Application

To control the film thickness of zinc in the process of continuous hot-dip galvanizing, it is known from the early days that the gas wiping through an air knife is the most effective one. The gas wiping using in galvanizing process brings about a problem of splashing from the strip edge for a certain high speed of coating. So, in the present study, the effects of the deflection angle of Coanda nozzle on jet structure and the distribution of impinging pres- sure at the plate surface are investigated numerically and experimentally. In numerical analysis, the governing equations consisted of three-dimensional time dependent full Navier-Stokes equations, standard k-ε turbulence model to solve turbulent stTess and so on are employed. In experiment, 16 channel pressure scanning valve and 3-axis auto traversing unit are used to measure the impinging pressure at the strip surface. As a result, it is found that the smaller the deflection angle for the same nozzle slit of air knife is, the larger the impinging pressure is. To reduce the size of separation bubble and to enhance the cutting ability, it is recommendable to use an air knife with the Coanda nozzle.
