Postpartum Family Planning at the Timbuktu Hospital in Mali

Introduction: Postpartum family planning is the prevention of pregnancies during the 12 months following childbirth. Few studies have been devoted to postpartum family planning in Mali. Our work will contribute to reducing unmet need for family planning. Objective: To study the use of contraceptive methods in the postpartum period in the obstetrics and gynecology unit of Timbuktu hospital. Materials and Methods: This was a descriptive and analytical cross-sectional study with prospective collection of data from January 1, 2022 to December 31, 2023. All women who gave birth having chosen and benefited from a contraceptive method were included. The statistical test used was the Fisher test with a significance threshold fixed at 5%. Results: The frequency of contraception in the postpartum period was 17.03%. The average age of clients was 26.14% with extremes of 14 and 45 years. They were paupiparous at 56.4% with an inter-birth interval of less than 12 months at 12.3%. More than half of the counseling (58.5%) was done during postnatal visits. The methods chosen were implants at 48.1%, injectable progestins at 21.3%, intrauterine device at 14.7%, miro-progestin pills at 13.5%, tubal ligation at 1 .4% and condoms at 1%. The regular follow-up rate was 51.1% of cases and 25.6% had no follow-up. Conclusion: The overall rate of postpartum family planning of 17.08% remains low. Improving FP staff skills will reduce unmet needs and contribute to increasing contraceptive prevalence in Timbuktu.

Introduction: Late pregnancies are considered a public health problem linked to maternal mortality due to their complications. These risks, which worsen with maternal age, should be better known and taken into account in monitoring these pregnancies. Objective: To evaluate the impact of age on the prevalence of obstetric and neonatal complications among parturients of 40 years old and over. Methodology: This was an 11-year retrospective analytical case control study from January 1, 2007 to December 31, 2017. The study population was female patients of 40 years old or older for cases and female patients younger than 40 years of control case. Results: In total, we collected 640 cases of parturients of 40 years old and over out of a total of 84,234 deliveries during the study period;i.e. a frequency of 0.76%. Spontaneous abortion increased in patients of 40 and over with 8.28%. Hypertension and diabetes were higher of women of 40 and over, 52.65%. Concerning the method of admission: 55% of parturients of 40 and over were referrals. Incidents occurring among women of 40 and over were more pronounced than in control cases. Complications during vaginal delivery, cervical tear, perineal tear, uterine dehiscence and uterine rupture are high in our cases, i.e. 11.88%;3.59%;2.19%;2.66. During our study, depending on the type of delivery, the cesarean rate was much higher among women of 40 and over than among control cases, i.e. 54.22% versus 12.24%. Hemorrhages in the 3rd trimester were the main indications for cesarean section. Hemorrhages in the 3rd trimester were more frequent with women of 40 and over, i.e. 10.47%;5.94% versus 1.74% and 0.42%. As for the different types of malformations in parturients, women under 40 years old fetal malformations were absent in 92.29%, on the other hand they were more frequent in patients with 40 years and over, i.e. a normal frequency of 36.72%. Introduction: Maternal mortality constitutes a public health problem and its rate is an indicator of a country’s development. Among the causes of maternal and perinatal death, high blood pressure associated with pregnancy occupies a significant part. It represents 5% of direct maternal deaths. Objective: to study the maternal and perinatal prognosis of high blood pressure during pregnancy in the Tenenkou reference health center in Mali. Methodology: This was a descriptive, analytical and retrospective cross-sectional study over a period of twelve months from January 1, 2021 to December 31, 2021 and involving 144 cases of high blood pressure associated with pregnancy. Results: We obtained a frequency of 11.75%. The majority of patients 70.9% were aged between 20 - 35 years. The important risk factor found was young age. During our study, 46.5% of patients had performed at least one CPN and only 13.9% performed 04 CPN. Pre-eclampsia was the most common type of high blood pressure during pregnancy, i.e. 61.1%. Eclampsia and retroplacental hematoma were the most common maternal complications, respectively 27.8% and 11.1%. The most common fetal complications were premature births and fetal distress with 20.9% and 17.4% respectively. Conclusion: Hypertension associated with pregnancy still remains a major cause of maternal-fetal morbidity and mortality in our context where diagnosis is often late. The main clinical form was preeclampsia. Eclampsia and retroplacental hematoma were the most frequent maternal complications. Fetal complications were mainly prematurity and fetal distress.

Introduction: Uterine fibroid is a mixed mesenchymal tumor, developing from smooth muscle cells of the myometrium, separated by connective tissue. The majority of fibroids are asymptomatic and do not require any intervention or other exploratory measures. However, in some cases fibroids are symptomatic, their treatment should aim to improve symptoms and quality of life. Objectives: Describe the epidemiological-clinical aspects of uterine fibroids in the gynecology and obstetrics department of the reference health center of commune VI of the Bamako district. Methodology: This was a retrospective descriptive study carried out over a period of one year. It concerns all patients seen in consultation in the gynecology and obstetrics department of the reference health center of commune VI. Results: during the study period, the uterine fibroid frequency was 1.80%. During this same period, fibroids represented 5.59% of gyneco-obstetric pathologies operated on in the department. The 30 - 45 year old age group was the most represented with a frequency of 75.63%. The average age was 36.87 years ± 6.2 years with extremes of 25 and 63 years. Married women were the most represented 97.48%. The vast majority of our patients 95.8% were not postmenopausal. The multigravidas were the most represented, i.e. 37.50% with a large part of the pauciparous 41.29%. In our patients, 96.64% had a clinical symptom on their fibroid with the main reason for consultation being the sensation of a pelvic mass in 97.48%. We recorded three cases of infertility as associated factors. Ultrasound was performed in all patients. The location of the myxomatous nuclei was subserosal in the majority in 42.86% and with multiple nodules in 84.85% of cases. The main indication for myomectomy was the failure of medical treatment in 86.49% of cases. Surgical treatment was mainly a myomectomy 93.30%, a hysterectomy was indicated in 6.70%. Introduction: The severity of Sars-Cov-2 infection is associated with the development of acute respiratory distress syndrome (ARDS). The progression to ARDS appears to be driven by a major inflammatory mechanism potentially sensitive to corticosteroids. Observation: This article describes the case of an elderly patient was admitted to emergency departments for intense asthenia, accompanied by motor diarrhoea, dyspnoea with desaturation in ambient air, in a context of strong suspicion of infection linked to COVID-19. The article also reviews the existing literature on the diagnosis and treatment of this severe form of the disease. Conclusion: Corticosteroids, and in particular dexamethasone, have been shown to be effective in the management of patients with COVID-19, an oxygen-releasing disease.

Metavolcanic rocks are well-exposed in the Kribi area within the Nyong Group, Congo craton, but their origin, age, and tectonic significance are poorly *** we report integrated field mapping and petrography,geochemistry, and LA-ICP-MS zircon U-Pb ages of these metavolcanic rocks to constrain their petrogenesis and geodynamic implications. The studied rocks consist of mafic granulite, garnet-amphibole gneiss, and garnet-biotite gneiss, and occur interbanded with sharp contact and intruded by syenite dyke. These metavolcanic rocks are classified as MORB-like tholeiitic to calc-alkaline basalts,basaltic andesite, and rhyodacite rocks with within-plate setting geochemical signatures. The metabasite rocks(basalt to basaltic andesite protolith) are likely the equivalent of a spinel peridotite product representing ~ 2–5 %partial melting of metasomatized mantle source, while the metarhyodacite rocks are derived from the fractional crystallization of the same parental magma. Zircon U-Pb data revealed that the rhyodacite rocks initially formed at2671 ± 51 Ma and underwent later metamorphism at2065 ± 55 Ma. The Neoarchean protolith age is comparable to the ca. 2628 Ma tholeiitic magmatism and ca.2666 Ma high-K granites, suggesting bimodal Neoarchean magmatic event within the Ntem Complex, while the metamorphic ages fall within the ca. 2100–2000 Ma highgrade tectono-metamorphic event attributed to Eburnean/Trans-Amazonian orogeny. At the regional scale,metavolcanic rocks with similar origins and ages are documented in the Sa o Francisco Craton in Brazil, suggesting comparable geodynamic evolution on both sides of the south Atlantic during the Paleoproterozoic.

Statural and Weight Growth of Low Birth Weight at 9 Months

Introduction: Low birth weight (LBW) is defined as a birth weight 2500 g at birth, regardless of the term of pregnancy. The objective of this study is to evaluate their stature and weight growth from 0 to 9 months of corrected age (CA) in Senegal. Methodology: This is a prospective, descriptive, and analytical cohort follow-up up to 9 months of CA including all live newborns of LBW hospitalized and followed up from 01 August 2019 to 31 May 2020. World Health Organization growth charts were used to assess stature and weight growth. Results: During the study 136 LBW newborns were included. The mean gestational age was 32 weeks of amenorrhea. At discharge, 46 children (33.82%) were exclusively breastfed. At birth, the mean weight was 1487 g (3rd-10th percentile) and the mean height was 41.52 cm (10th-25th percentile). At 9 months of CA, the mean weight was 8119 g (median) and the mean height was 74 cm (median). The children had achieved satisfactory growth in weight (84%) and height (89%). At 9 months of CA, 27% of the children were behind in one of the four areas of psychomotor development. Conclusion: At the end of 9 months of CA, stature and weighted growth was normal.

Li-ion batteries (Libs) are a mature technology widely used for energy storage in various electronic devices. Nowadays, this technology has become a leading candidate for the portable electronics market and for electric vehicles due to its good performance. As a result, the demand for Libs containing critical metals, rare earth elements and precious metals is increasing day by day with the accelerated upgrades of consumer electronics, which promotes the supply risk of many mining resources. In addition, the problems associated with the production of end-of-life Lib are increasing on a global scale. Used Libs are e-waste containing significant levels of critical raw materials (such as Co, Li, Mn and Ni) along with harmful substances. Without proper management of Lib waste, these precious metals and toxic substances may end up in nature and cause environmental and public health problems. In order to preserve nature, ensure sustainable resource management and stimulate the circular economy, it has become crucial to properly manage and recycle end-of-life Li-ion batteries. By the way, conventional methods focusing on pyrometallurgical treatments combined with hydrometallurgical treatment are widely studied to recover design metals from Libs waste. It is in this context that we have conducted this bibliographic synthesis, focusing on the efficiency of the solvents employed and their competitiveness for a more environmentally friendly economic management. In this manuscript, recent leaching, solvent extraction, electrodeposition and precipitation strategies to recover precious metals from end-of-life Li-ion battery designs are reviewed and the evolution of these processes is discussed.
