Hybrid IPOM: A Novel Technique for the Management of Incisional Hernia

Background and Aim: The incidence of incisional hernias has been reported to be around 15%. In the present scenario, a wide array of surgical procedures are available for their better management. In this study, we intend to share our experience with one novel technique, “Hybrid IPOM (Intraperitoneal onlay meshplasty)” as a management option for a selected cohort of patients. Methods: This prospective study was undertaken during January 2019 to July 2023 at King Abdullah medical city, Makkah. A total of 51 cases were selected for Hybrid IPOM repair as per inclusion criteria;medium sized (4 - 10 cm) hernia defects;uncomplicated hernias;age more than 18 years. The follow-up period of the patients varied from 6 months to 4 years. The operation commenced with open hernia dissection, mesh deployment into abdomen, defect closure and then conversion to laparoscopy for the posterior mesh placement. Results: A total of 51 cases were repaired successfully with this technique. 48 out of 51 cases were incisional hernias secondary to some primary procedure done either for hernias itself or some other intra-abdominal pathology. The three cases were primary hernias falling in medium to large category with unaesthetic overlying skin. The age range was 19 to 72 years. The mean (range) operative time was 135 (90 - 240) min, and the average blood loss was 70 ml. The mean (range) hospital stay was 3 (2 - 11) days. All patients returned to routine work within 2 - 3 weeks of surgery. The median follow-up was 15 (6 - 48) months. Of the 51 cases, 3 patients developed seroma (managed conservatively), 1 patient developed a large hematoma (needed evacuation), and 1 patient developed superficial wound infection (managed with antibiotics). Two patients had recurrences;one patient had previously failed multiple repairs, and the other developed a postoperative hematoma. None of our patients had an iatrogenic bowel injury. 中国东南部地区的地壳变形机制和深部成矿机制一直存在争议。本研究基于中国地震台网207个地震站观测到的地震波形数据,采用接收函数方法计算中国东南部的地壳厚度和地壳内岩石的泊松比。结果表明,研究区东部地壳厚度约为27 km,而西部地壳厚度约为43 km,平均地壳厚度为31 km,地壳呈现西厚东薄趋势。地壳内岩石泊松比的结果显示:华夏块体的南部泊松比较高,平均为0.295,而秦岭大别造山带的地壳岩石泊松比相对较低,平均为0.257。在扬子克拉通的中部以及华夏块体的中东部区域,地壳岩石泊松比为0.257~0.286。通过对比侵入体沉积岩和火成岩的泊松比,初步推断金属矿物系统的形成可能与造山和热液沉积矿床有关,这表明研究区内多期岩浆作用和成矿作用是由构造-岩浆-热事件引起的。而华夏块体东南边缘和上场子克拉通的东北区域地壳岩石的高泊松比可能与中生代中下地壳镁铁质部分熔融有关,同时部分熔融也为岩浆入侵和金属矿床的形成提供了重要环境。

Aging is linked to the deterioration of many physical and cognitive abilities and is the leading risk factor for Alzheimer’s disease. The growing aging population is a significant healthcare problem globally that researchers must investigate to better understand the underlying aging processes. Advances in microarrays and sequencing techniques have resulted in deeper analyses of diverse essential genomes(e.g., mouse, human, and rat) and their corresponding cell types, their organ-specific transcriptomes, and the tissue involved in aging. Traditional gene controllers such as DNA-and RNA-binding proteins significantly influence such programs, causing the need to sort out long non-coding RNAs, a new class of powerful gene regulatory elements. However, their functional significance in the aging process and senescence has yet to be investigated and identified. Several recent researchers have associated the initiation and development of senescence and aging in mammals with several well-reported and novel long non-coding RNAs. In this review article, we identified and analyzed the evolving functions of long non-coding RNAs in cellular processes, including cellular senescence, aging, and age-related pathogenesis, which are the major hallmarks of long non-coding RNAs in aging.

Vehicular Ad hoc Networks(VANETs) are special kind of Mobile Ad hoc Networks(MANETs),where vehicles communicate with each other in ad hoc formation. VANETs consist of Vehicles and Road Side Units(RSUs) that assist in the network management. Vehicles communicate with each other and RSUs, with the aim to provide infotainment and safety services on road. Security is an important consideration in VANETs as safety of humans(passengers) is an important issue. Vehicular Public Key Infrastructure(VPKI) is an adapted form of Public Key Infrastructure(PKI) used to achieve the key management and security services in *** generation and revocation is one of the primary functions of VPKI. Certificate revocation is used for revoking the malicious nodes and terminate their access rights to the network. In this paper we classify revocation schemes in a novel way into centralized and decentralized manners. This paper covers a survey of different certificate revocation schemes, and provides an overview of the research in the area of certificate revocation in VANETs.

Origin of Boron in the Gas Hure Salt Lake of Northwestern Qaidam Basin, China: Evidence from Hydrochemistry and Boron Isotopes

The Gas Hure Salt Lake(GHSL) in the northwestern Qaidam Basin, western China, is rich in boron(B) resources, but its B-resource origin is hardly known. Hydrochemical compositions and B isotope characteristics of different waters were collected around the GHSL, including the river water, stream water, spring water, salt-lake brine, intercrystalline brine, well water, drilling brine, and solar pond brine. The hydrochemical signatures suggest that silicates, carbonates and evaporates are the main B-bearing rocks during the water dynamic. The reservoir estimation of B resources shows that the Kulamulekesay River(KLMR) and the Atekan River(ATKR) contribute annually 18.3 tons and 22.84 tons of B, respectively, with a total amount of 11.72 × 10^(4) tons of B during the past 5.7 ka. In comparison with the known B reservoir(32.96 × 10^(4) tons) in the GHSL, a significant amount of B in the GHSL was probably recharged from deep fluids and sediments around the GHSL. The B concentration and B-enrichment degree are shaped by the evaporation process, which are highly elevated at the carnallite and bischofite stages.

Sweet and sour are the most important taste of blueberries,and they are produced by sugar and acid,*** contributions to the taste depend not only on the levels of sugar and acid,but also on the types and relative proportions of sugar and ***,it is very important to evaluate the composition and levels of sugar and acid in *** differences and variety diversity also affect the sugar and acid characteristics of ***,this study selected two main producing regions in northern China(Weihai and Yingkou)to examine the sugar and acid characteristics of 11 common blueberry *** indexes measured included soluble sugars,organic acids,soluble solid content and titratable *** results showed that glucose and fructose were the major sugars,and citric acid and quinic acid were the major organic *** analysis showed that glucose,fructose,and sucrose were positively correlated with total sugar content;the citric acid content was positively correlated with the titratable acidity and total organic *** acidity,glucose,fructose,sucrose,total sugar content,citric acid,shikimic acid and total acid content of the blueberries varied significantly between regions(P<0.05).In general,compared with Weihai blueberries,Yingkou blueberries had higher sugar content and lower acid *** results of this study may provide useful references for the evaluation of sweet and sour flavors and cultivar selection of blueberries.

昆特依干盐湖位于柴达木盆地西北部,为特大型综合盐类矿床.大盐滩是昆特依干盐湖内最大的盐滩,地下赋存有一定量的卤水矿床,但该矿床的水文地质条件差,主要卤水矿层含水性弱,开采难度大.核磁共振找水方法作为当今世界上唯一的直接找水地球物理新方法,具有高分辨力、高效率、信息量丰富和解的唯一性等优点,本文运用该方法对昆特依干盐滩地区地下卤水空间分布特征进行研究,通过对核磁共振数据进行处理与反演,结合已有的地质与钻井资料,对测点进行综合地质-地球物理解释,获得以下认识:1)大盐滩0~130 m深度范围内,共存在3个卤水含水层,主要呈扁平状或漏斗状、近似层状展布,W1为晶间潜卤水层,渗透系数较大,颗粒较粗,单位体积含水量为0.4%~2.7%,W2和W3为晶间承压卤水层,渗透系数较小,颗粒较粗,单位体积含水量分别为0.2%~1.1%和0.1%~0.8%;2)大盐滩地区存在两个卤水富集区,分别为研究区西南部沉积盆地中心的Ⅰ号富卤区和盆地东北部的Ⅱ号富卤区;3)根据区域内卤水富集分布以及构造情况,划定大盐滩向斜沉积中心、大盐滩北侧F1~F8及遥F6断裂发育区和冷湖构造带为区域内主要的找矿找水远景区;4)GMR核磁共振系统在干盐滩地区理论探测深度为130 m,该系统不仅可以有效地探测自由水,而且可以依据束缚水的分布解译地下各类含水盐类矿物和含水黏土矿物的存在与分布.
