Effects of long-term ethanol consumption on jejunal lipase and disaccharidase activities in male and female rats

AIM: To study the effect of long-term ethanol consumption on jejunal lipase and disaccharidase (sucrase, maltase,and lactase) activities in rats and its gender difference. METhoDS: Age-matched male and female Wistar rats were fed control or ethanol-containing liquid diets for 12 wk following the Lieber-DeCarli model. According to both theplasma aspartate aminotransferase (AST) and alanine aminotransferase (ALT) activities, 40 rats were divided into four groups as follows: male control group (MC), male ethanol group (ME), female control group (FC), and female ethanol group (FE).RESULTS: After ethanol feeding for 12 wk, the results revealed that plasma AST and ALT activities of group MEwere significantly increased by 58% and 92%, respectively,than those of group MC (P＜0.05). Similarly, plasma AST and ALT activities of group FE were also significantly increased by 61% and 188%, respectively, than those of group FC (P＜0.05). Fat accumulation was observed in both ethanol treated groups, while fatty changes were more severe in group FE than those in group ME. The induction of hepatic microsomal cytochrome P450 2E1 (CYP2E1) was obviously seen in group ME and group FE, but was not detected in group MC and group FC. Jejunal lipase activity of group ME was significantly increased by 1.25-fold than that of group MC (P＜0.05). In contrast to, sucrase, maltase, and lactase activities of group ME were significantly decreased by 63%, 62% and 67%, respectively, than those of group MC (P＜0.05). Similarly, activities of these three enzymes of group FE were also significantly decreased by 43%, 46% and 52%, respectively, than those of group FC (P＜0.05).There were no significant epithelial changes of the duodenal mucosa in any ***: Long-term ethanol consumption significantly can increase jejunal lipase and decrease jejunal disaccharidase activities in both male and female rats.

Dissemination of gastric cancer may usually occur by direct spread through the perigastric tissues to adjacent organ, lymphatic spread, and hematogenous spread. We report a rare case of gastric cancer with mucosal metastastic lesion on the upper esophagus that was diagnosed by endoscopy and endosonography. A biopsy of the esophageal mass was performed and the pathologic findings with immunohistochemical stain for Mucin-5AC are proved to be identical to that of gastric adenocarcinoma, suggesting metastasis from main lesion of the gastric cancer. The lesion could not be explained by lymphatic or hematogenous spread,and its metastasis mechanism is considered to be different from previous studies. We suggest that the gastroesophageal reflux of cancer cells could be one of the possible metastatic pathways for metastasis of esophagus from an adenocarcinoma of the stomach.

Predictors of loss of hepatitis B surface antigen in HIV-infected patients

AIM:To study factors associated with loss of hepatitis B surface antigen (HBsAg) in patients co-infected with human immunodeficiency virus (HIV) and hepatitis B virus (HBV).METhoDS:We retrospectively reviewed the medical records of 5681 patients followed up at two New York City HIV clinics from January 1999 to May *** and laboratory parameters including baseline and follow-up HIV viral loads,CD4 cell counts,alanine transaminase levels,demographics,presence of hepatitis C infection,and treatment with highly active antiretroviral therapy dually active against both HIV and HBV infection,were analyzed to determine factors associated with loss of ***:Three hundred and fifty five patients (355/5681,6.84%) were co-infected with HIV and HBV and were *** these,226 patients with more than 12 mo follow-up were included in further analysis to determine factors associated with loss of HBsAg in the long-term *** the univariate analysis,baseline CD4 cell count was associated with loss of HBsAg (P=0.052).Cox regression analysis revealed that loss of HBsAg was associated with baseline CD4 cell count > 500 cells/mm3 (P=0.016,odds ratio:76.174,95% confidence interval:2.233-2598.481).CONCLUSION:Our study showed an interesting association of loss of HBsAg in HIV-HBV co-infected patients with higher CD4 cell count,suggesting that T-cell cytolytic activity against HBV may still be effective in clearing HBV infection.

Improving signature quality for network application identification

Network application identification is one of the core elements in network operations and management to provide enhanced network service and security. For accurate identification, an approach using common patterns called "signatures" is widely used to compensate the limitations of the traditional transport-layer port-based classification. however, our simulation results indicate that using the signatures generated from a set of well known algorithms may lead to very poor identification performance, with less than 60% of true positives even in an optimal case. To improve the quality of signatures, we present a technique in this paper, which consists of two steps:(i) pairwise merging to consider every possible combination of the initially collected signatures to reduce their specificity that causes the signatures to be less common;and (ii) signature reduction to identify effective signatures with greater importance from a large set of signatures produced in the merging step, so as to manage the space/time complexity in the identification process for greater scalability. Our experimental results show that the proposed technique can dramatically improve the performance, even with a small number of signatures (e.g., 95% true positives rate with 30 signatures per application) which is more compact than the initial signature set.

Mg and its alloys are the lightest structural metals available and are extremely attractive for applications as lightweight components, particularly in the automobile, electronic, and aerospace industries. The global market for wrought Mg alloys has steadily expanded over the past decade. And numerous studies have been carried out to meet this increasing demand of high-performance Mg alloys. however, Mg extrusion alloys have had a very limited usage so far. To overcome existing industrial challenges, one desirable approach is the development of low-cost rare earth(RE) free Mg extrusion alloys with superior mechanical properties. This review will introduce the recent research highlights in the extrusion of Mg alloys, specifi cally focusing on low-cost RE-free Mg alloy. The results from both the literature and our previous study are summarized and critically reviewed. Several aspects of RE-free Mg extrusion alloys are described in detail:(1) novel alloying designs including Mg–Al-, Mg–Zn-, Mg–Ca-, Mg–Sn-, and Mg–Bi-based alloys,(2) advanced extrusion techniques, and(3) extrusion-related severe plastic deformation(SPD) processing. Accordingly, considering the large gap in mechanical properties between the current RE-free Mg alloys and high-performance aluminum alloys, new alloy design, processing route control, and recommendations for future research on RE-free Mg extrusion alloys are also proposed. We hope this review will not only off er insightful information regarding the extrusion of RE-free Mg alloys but also inspire the development of new Mg extrusion technologies.

Impact of statin intensity on adverse cardiac and cerebrovascular events in older adult patients with myocardial infarction

BACKGROUND There is insufficient evidence regarding the effect of high-intensity statin therapy in older *** study aimed to investigate the effects of high-intensity statin treatment on the clinical outcomes in older adults with myocardial infarc-tion(MI).METhoDS Consecutive patients with MI aged at least 75 years were analyzed *** primary endpoint was major adverse cardiac and cerebrovascular events(MACCE),defined as a composite of all-cause death,MI,rehospitalization due to un-stable angina,repeat revascularization,and ischemic *** high-intensity group was compared to the low-to-moderate in-tensity group in the propensity score-matched *** Average age of total 546 patients was 81 *** them,84%of patients underwent percutaneous coronary *** unadjusted seven-year MACCE rate differed by statin intensity(high-intensity statin group:38%,moderate-intensity statin group:42%,low-intensity statin group:56%,and no-statin group:61%,P=0.004).however,among these groups,many baseline characteristics were significantly *** the 74 propensity score-matched pairs,which lacked any significant differences in all baseline characteristics,the high-intensity group had a significantly lower rate of MACCE than the low-to-moderate intensity group(37%vs.53%,P=0.047).Follow-up low-density lipoprotein cholesterol levels were significantly lower in the high-intensity group than that in the low-to-moderate intensity group(69.4±16.0 mg/dL vs.77.9±25.9 mg/dL,P=0.026).CONCLUSIONS In older adult patients with MI,the use of high-intensity statin caused significantly less occurrence of MACCE in comparison to that in low-to-moderate intensity for up to seven years of follow-up.

Apoptosis induced by nucleosides in the human hepatoma HepG2

AIM： To investigate the apoptotic effects of nucleosides on the human hepatoma HepG2. METhoDS： The nucleosides included inosine （I）, cytidine （C）, uridine （U）, thymidine （T）, adenosine （A）, and guanosine （G）. Cells were incubated by the mediums with or without nudeosides at 37 ℃ in a 50 mL/L CO2 humidified atmosphere. RESULTS： It was found that the cell viabilities were significantly decreased, when cells were treated with 30 mmol/L I, 30 mmol/L C, 30 mmol/L U, 30 mmol/L T, 0.5 mmol/L A, and 0.5 mmol/L G after 12 h incubation （P〈0.05）. About the apoptotic phenomenon, the cell percentages of sub-G1 cells were significantly increased in the mediums containing nucleosides such as C, U, T, A, and G （P〈0.05）. Furthermore, the caspase-3 activity was increased, when the cells were incubated with T （P〈0.05）. The protein expressions of p53 and p21 showed no difference in each group. To investigate the mechanism of apoptosis induced by nucleosides, it was found that the contents of soluble Fas ligand contents were increased in HepG2 cells following I, U, T, and A treatment （P〈0.05）. But, TNF-α and cytochrome cwere undetectable. CONCLUSION： Thymidine may induce the apoptosis in HepG2, but the effective dosages and reactive time must be investigated in the future study. however, the apoptosis-inducing abilities of other nucleosides were still unclear in this study.

Low grade gastric mucosa associated lymphoid tissue lymphoma:Treatment strategies based on 10 year follow-up

AIM:To deduce strategic guidelines of gastric mucosa associated lymphoid tissue lymphoma (MALTOMA) by evaluating the long-term outcome of patients in respect to various treatment modalities. METhoDS:A total of 55 patients with MALTOMA from May 1992 to August 2002 were retrospectively reviewed. RESULTS:Complete remission was obtained in 24 (82.8%) of 29 patients treated with anti Helicobacter pylori (Hpylori) regimen *** duration to reach complete remission was 12 months (85 percentile,2-33 months).Five patients showed complete remission with radiation therapy (26-86 months).Two of them were Hpyloritreatment failure cases. CONCLUSION:Hpylorieradication is an effective primary treatment option for low grade MALTOMA and radiation therapy could be considered in patients with no evidence of Hpyloriinfection or who do not respond to Hpylorieradication therapy 12 months after successful eradication.

Iatrogenic subclavian artery pseudoaneurysm close to the origin of the vertebral artery: an endovascular strategy

Subclavian artery pseudoaneurysm that induced from central venous catheterization through the internal jugular vein is relatively uncommon. however, the management of subclavian artery pseudoaneurysm remains a challenge because of their non-compressibility of deep locality and relationship to important surrounding anatomy, such as the origin of vertebral artery. In this paper, the authors report a patient with larger iatrogenic subclavian arterial pseudoaneurysm near the origin of vertebral artery, that was treated successfully by endovascular covered stent and coils.
