Long-term result of endoscopic Histoacryl （N-butyl-2-cyanoacrylate）injection for treatment of gastric varices

AIM:To evaluate the long-term efficacy and safety of endoscopic obliteration with Histoacryl  for treatment of gastric variceal bleeding and ***:Between January 1994 and March 2010 at SoonChunHyang University Hospital,a total of 127 patients with gastric varices received Histoacryl  injections *** hundred patients underwent endoscopic Histoacryl  injections because of variceal bleeding,the other 27 patients received such injections as a prophylactic ***:According to Sarin classification,56 patients were GOV1,61 patients were GOV2 and 10 patients were *** of the varices were large(F2 or F3,111 patients).The average volume of Histoacryl  per each session was 1.7±1.3 cc and mean number of sessions was 1.3±0.6.(1 session-98 patients,2 sessions-25 patients,≥3 sessions-4 patients).Twenty-seven patients with high risk of bleeding(large or fundal or RCS+or Child C) received Histoacryl  injection as a primary prophylactic *** these patients,hepatitis B virus was the major etiology of cirrhosis,25 patients showed GOV1 or 2(92.6%)and F2 or F3 accounted for 88.9%(n=24).The rate of initial hemostasis was 98.4%and recurrent bleeding within one year occurred in 18.1%of *** hemostasis during episodes of rebleeding was achieved in 73.9%of *** survival was 50 mo (95%CI 30.5-69.5).Major complications occurred in 4 patients(3.1%).The rebleeding rate in patients with hepatocellular carcinoma or GOV2 was higher than in those with other *** of the 27 subjects who were treated prophylactically experienced treatment-related *** survival rates of the 127 patients at 6 mo,1,3,and 5 years were 92.1%,84.2%,64.2%,and 45.3%,*** 6 mo cumulative survival rate of the 27 patients treated prophylactically was 75%.CONCLUSION:Histoacryl  injection therapy is an effective treatment for gastric varices and also an effective prophylactic treatment of gastric varices which

Clinicopathological features and prognosis of combined hepatocellular carcinoma and cholangiocarcinoma after surgery

BACKGROUND： Combined hepatocellular carcinoma and cholangiocarcinoma （cHCC-CC） is a rare subtype of primary liver cancer consisting of both hepatocellular carcinoma （HCC） and cholangiocarcinoma （CC）. Because of the rarity of this tumor, its feature is poorly understood. The present study aimed to evaluate the clinicopathological features and long-term prognosis of patients with cHCC-CC after surgery and to compare with those of the patients with stage-matched HCC and CC. METHODS： The dinicopathological features of the patients who underwent surgery for cHCC-CC at our center during the period of 2001-2010 were retrospectively analyzed and compared with those of stage-matched HCC and CC patients. Cancer staging was performed according to the AJCC Cancer Staging Manual （6th ed.）. Overall survival and disease-free survival were compared among the groups and prognostic factors of cHCC-CC were evaluated. RESULTS： Significant differences were observed in clinico- pathological features among 42 patients with cHCC-CC, 90 patients with HCC and 45 patients with CC. Similar to HCC patients, cHCC-CC patients had frequent hepatitis B virus antigen positivity, microscopic vessel invasion, cirrhosis and high level of serum alpha-fetoprotein. Similar to CC patients, cHCC-CC patients showed increased bile duct invasion and decreased capsule. The 1-, 3-, and 5-year overall survival and disease- free survival of patients with cHCC-CC were not significantly different from those with stage-matched patients with CC;but significantly poorer than those with HCC. In subanalysis of patients with stage Ⅱ, the overall survival in patients with cHCC-CC or CC was significantly poorer than that in patients with HCC. We did not find the difference in patients with other stages. Univariate analysis of overall and disease-free survival of patients with cHCC-CC showed that the vascular invasion and intrahepatic metastasis were the significant predictive factors. Optimal proximal resection margin distance for gastrectomy in advanced gastric cancer

BACKGROUND: Although perioperative portal vein (PV) stent implantation is an effective treatment for steno-occlusive disease in adult living donor liver transplantation (LDLT) re-cipients, we experienced high incidence of biliary anastomotic strictures (BAS) after PV stenting. In this study, we sought to clarify the relation between BAS and PV stenting and to suggest the possible mechanism of BAS and measures to reduce its in-cidence. METHODS: We retrospectively analyzed 44 LDLT recipients who underwent PV stent implantation across the line of PV anastomosis regardless of the location of steno-occlusion (stent group) and their matched controls (non-stented LDLT recipients, n=131). RESULTS: The incidence of BAS was higher in patients in the stent group than that in the control group (43.2% vs 17.6%, P=0.001). Cumulative 6-month and 1-, 2- and 5-year BAS rates were 31.8%, 34.1%, 41.4% and 43.2%, respectively, in the stent group and 13.0%, 13.8%, 16.1% and 17.8%, respectively, in the control group (P=0.001). Multivariate analysis revealed that PV stenting was an independent risk factor for BAS. CONCLUSIONS: Although PV stent implantation is a reliable treatment modality for steno-occlusive PV in adult LDLT recipients, innovative methods to prevent the PV stent from crossing the line of PV anastomosis may be necessary to reduce the incidence of postoperative BAS.

Texture development and grain boundary phase formation in Ce-and Ce-La-substituted Nd-Fe-B magnets during hot-deformation process

In this study,the influence of Ce-and Ce-La-substitution for Nd in Nd-Fe-B magnets on their magnetic and microstructural changes in the course of the hot-deformation process was investigated to gain insight into further reducing the Nd content in fine-grained Nd-Fe-B hot-deformed *** found that the[001]texture and microstructure of the RE-rich grain boundary phases,which are the factors that de-termine the remanence and coercivity of hot-deformed magnets,were deteriorated by the Ce-and *** is because the volume fraction of the RE-rich liquid formed in the grain boundaries during the hot-press and hot-deformation decreased due to the Ce-and Ce-La-substitution,thereby in-creasing the friction between the 2:14:1 grains while the c-axes of grains were aligned by grain rotation and decreasing the RE concentration of grain boundary *** particular,the Ce-La-co-substitution further prevented the liquid formation within the grain boundaries of the magnets during hot-press and hot-deformation because the wettability of the RE-rich liquid on the 2:14:1 grains became poor when La was substituted for Nd in the 2:14:1 *** results indicate that there is room for further im-provement in both the remanence and coercivity of the Ce-and Ce-La-substituted magnets by intro-ducing additional liquid into the grain boundaries prior to hot-press and *** applying the Nd-Cu infiltration process to the melt-spun ribbons(intermediate infiltration,I-infiltration),both the remanence and coercivity of the Nd-saving(Nd 0.7 Ce 0.3)-Fe-B and(Nd 0.7 Ce 0.225 La 0.075)-Fe-B hot-deformed magnets were successfully improved.

Comparative Analysis of Four Stem Taper Models for Quercus glauca in Mount Halla,Jeju Island,South Korea

This study was conducted to fit the diameter-height data of Quercusglaucain Jeju Island, South Korea to the four commonly used stem taper equations andto evaluate the performance of the four stem taper models using four statistical criteria： Fit index （FI）, root mean square error （RMSE）, bias （）,and absolute mean difference （AMD）. Results showed that the Kozak02stem taper equation provided the best FI（0.9847）, RMSE（1.5745）,（-0.0030 cm） and AMD （1.0990 cm） whileMax and Burkhart model had the poorest performance among the four stem taper models based on the four evaluation statistics （FI ： 0.9793,RMSE ： 1.8272, ： 0.3040 cm and AMD ： 1.3060 cm）. These stem taper equations can serve as a useful tool for forest managers in estimating the diameter outside bark at any given height, merchantable stem volumes and total stem volumesof the standing trees of Quercusglaucain theGotjawal forests located in Mount Halla, Jeju Island, South Korea.

The 5-year functional outcomes after radical prostatectomy： a real-life experience in Korea

We investigated the functional outcomes regarding erectile function and urinary continence up to 5 years following radical prostatectomy （RP） in a cohort of Korean men. We retrospectively analyzed the clinicopathologic data of 85 Korean men who received open uni- or bilateral nerve-sparing RP for clinically localized prostate cancer and were followed up for at least 5 years postoperatively. From medical records, patients＇ status regarding urinary and erectile function at baseline and postoperative followups after RP was assessed. At 24 and 60 months after RP, proportions of subjects continent （no pads used） were 89.4% and 97.6%, respectively （P = 0.007）. Excluding subjects （n = 24） who preoperatively reported having severe erectile dysfunction or lacked relevant informations, proportions of subjects capable of having vaginal intercourse regardless of erectile aid usage were 47.5% and 37.7% at 24 and 60 months from RP, respectively （P = 0.022）. Patient＇s age at surgery （P = 0.047） and salvage radiation therapy （P = 0.026） were observed to be significant predictors of having erections sufficient for intercourse at 60 months from RP in multivariate analysis. Our results showed that while patients＇ postoperative status regarding urinary continence at 2 years from RP is generally maintained or improved at 5 year point, erectile function was observed to significantly declined from 2 years to 5 years following RP. Such decline in erectile function following RP may be more significant among men who were relatively older at surgery or those who received salvage therapy during postoperative follow-ups.

Imidazole linker‐induced covalent triazine framework-ZIF hybrids for confined hollow carbon super‐heterostructures toward a long‐life supercapacitor

Carbon super-heterostructures with high nitrogen contents from the covalent hybrid precursors of covalent triazine frameworks(CTFs)and zeolitic imidazolic frameworks(ZIFs)are scarcely explored because of CTF's ordered structure and toxic superacid that dissolves or destabilizes the metal *** solve this problem,herein,we report a straightforward two-step pathway for the covalent hybridization of disordered CTF(d–CTF)–ZIF composites via preincorporation of an imidazole(IM)linker into ordered CTFs,followed by the imidazole-site-specific covalent growth of *** carbonization of these synthesized d–CTF−IM−ZIF hybrids results in unique hollow carbon super-heterostructures with ultrahigh nitrogen content(>18.6%),high specific surface area(1663m^(2)g^(−1)),and beneficial trace metal(Co/Zn NPs)contents for promoting the redox *** proof of concept,the obtained carbon super-heterostructure(Co–Zn–NC_(SNH)–800)is used as a positive electrode in an asymmetric supercapacitor,demonstrating a remarkable energy density of 61Wh kg^(−1)and extraordinary cyclic stability of 97%retention after 30,000 cycles at the cell *** presynthetic modifications of CTF and their covalent hybridization with ZIF crystals pave the way toward new design strategies for synthesizing functional porous carbon materials for promising energy applications.

Characteristics of radiation and convection heat transfer in indirect near-infrared-ray heating chamber

Numerical study was performed to evaluate the characteristics of combined heat transfer of radiation, conduction and convection in indirect near infrared ray （N/R） heating chamber. The effects of important design parameters such as the shape of heat absorbing cylinder and heat releasing fin on the pressure drop and heat transfer coefficient were analyzed with different Reynolds numbers. The Reynolds numbers were varied from 103 to 3x106, which was defined based on the hydraulic diameter of the heat absorbing cylinder. Analyses were performed to obtain the inner and outer flow and the temperature distributions in the heat absorbing cylinder and the rates of radiation heat transfer and convection heat transfer. As the Reynolds number increases, the convection heat transfer rate is increased while the radiation heat transfer rate is decreased. The average convection heat transfer rate follows a power rule of the Reynolds number. Addition of three-dimensional heat releasing fin to the outside of the heat absorbing cylinder enhances the convection heat transfer.

Surface-floating gold nanorod super-aggregates with macroscopic uniformity

We present a simple method for obtaining high-density two- and three- dimensional assemblies of gold nanorods （AuNRs） on polymer brush, referred to as ＂surface-floating super-aggregates＂, with uniform distribution spanning macroscopic distances. This was achieved via the single-step immersion of a poly（oligo ethylene glycol methacrylate） brush-containing substrate in a AuNR solution without any form of functionalization. Owing to extensive macroscale plasmonic coupling, we observed for the first time the gradual evolution of a unique sharp peak in addition to the transverse and longitudinal peaks, in this case, in the near-infrared （NIR） region. We also highlight the dynamic nature of these surface-floating super-aggregates, in which the AuNRs spread out when immersed in solution and collapse when dried to facilitate the access of probe molecules for biosensing applications. As a proof of concept, the surface-floating super-aggregates were used for surface-enhanced Raman spectroscopy, with which we detected rhodamine 6G at as low as sub-femtomolar concentrations. Owing to the excellent large-area uniform coverage and extreme simplicity of the fabrication method, such AuNR assemblies can easily be mass-produced and incorporated into cheap biosensors suitable for consumer use in the near future.
