Unicuspid Aortic Valve, an Extremely Rare Congenital Anomaly in Adults: A Systemic Review

The most common congenital heart defect is the bicuspid aortic valve (BAV) occurring with fusion of one of the three-valve commissures. A rarer valvular phenomenon is the unicuspid aortic valve (UAV) which has an estimated prevalence of 0.02% in the adult population. Two morphologic subtypes of UAV exist: the earlier presenting, acommissural UAV and the later appearing unicommissural. To better characterize and manage patients with UAVs, a systematic review of this rare phenomenon was performed. The objective of our study was to conduct a systematic review of adequate case studies involving UAV patients to describe patient characteristics, clinical presentation, management, and common sequela featured there within. The mean age of presentation in this review was 35.6 years. When diagnosing UAV, TEE was the most utilized diagnostic modality (65%) followed by intraoperative diagnosis (17%) discovered with valve repair or incidentally during another procedure. TTE was utilized to diagnose only 7% of the cases reviewed likely owing to the difficulty and inaccuracies in identifying UAV with this method that were previously established by multiple series. Interventional options for UAV are balloon versus surgical valvotomy, aortic valve replacement, Ross procedure and aortic root replacement. They are performed on an individual basis and all cases ultimately require aortic valve replacement (AVR) or Ross procedure (to avoid anticoagulation). Aortic root replacement is additionally required if aortic root/ascending aorta diameter exceeds 4.5 cm. In this review, Aortic aneurysm (16%) cases resulted in surgical interventions. There may be a need to implant permanent pacemaker (in 3% - 8%) during AVR when calcified UAV has calcification extending into interventricular septum.

Background: Adverse pregnancy outcomes continue to contribute substantially to maternal mortality as well as poor maternal and fetal health outcomes whose burden can be reduced by the initiation of preventive behaviors like the uptake of maternal health services. This continuous unacceptably high maternal mortality justifies a need for progressive research to better understand the predictors of the preventive behaviors of women towards adverse pregnancy outcomes. This study was conducted to assess levels of knowledge, attitudes about adverse pregnancy outcomes and preventive practices among women of reproductive age in Nassarawa local government area, Kano State, Nigeria. Methodology: The study was a cross-sectional descriptive study that employed a four-sectioned pretested researcher administered questionnaire to collect data on knowledge, attitude and practice of adverse pregnancy outcomes prevention among 164 consenting respondents in Kano State, Nigeria between November 2020 and December 2020. Responses were transformed and computed using SPSS version 26 to generate descriptive statistics. Regression analysis was done to test the degree of association between the predictors and practice of adverse pregnancy outcomes prevention with the level of significance set at a cut-off of p ≤ 0.05. Results: The findings showed that the majority of the respondents were between 20 - 24 years of age and had secondary educational attainment. Results further revealed that knowledge and attitude about adverse pregnancy outcomes which were both measured on 18-point scales, reported for the respondents’ mean scores of 9.46 (9.07 ± 9.85) and 10.72 (9.79 ± 10.62) respectively. The practice of adverse pregnancy outcomes prevention when assessed on a 15-point scale shows that respondents scored a mean of 7.42 ± 8.30 which denotes 53.4% of the level of prevention practice anticipated from the respondents. We describe a female Nigerian infant of otherwise healthy parents with no prior history of congenital anomalies who was born with left-sided cleft lip and palate, left anophthalmia, malformed left auricle, an atrial septal defect, and abnormal fusion of the medial ends of her ribs prior to their insertion into the sternum. She presented on account of respiratory difficulty following bouts of feed aspiration. Cautious feeding and the need for respiratory support are important aspects of care in patients with Goldenhar syndrome.

Drug Prescribing Trends among Consultants and General Practitioners in Sharjah-UAE

Background: Inappropriate prescribing can lead to errors in dispensing medications and serious problems for patients. Objectives: Prescription analysis can identify such drawbacks of prescribing, increase awareness of prescribers of rational prescribing and consequently lead to proper delivery of pharmaceutical care and enhance therapeutic outcomes. Methods: In the present study, prescriptions issued by consultants from a hospital and by general practitioners from private practice in Sharjah-United Arab Emirates were analyzed using indicators suggested by World Health Organizations. These include information with regard to prescriber, patient and the medication prescribed. We also determined the average number of drugs/encounter and % of prescriptions with antibiotics and those with injections. Data were collected and analyzed using Microsoft Excel and expressed in terms of both counts and percentages. Results: Almost all prescriptions were handwritten with easily readable ones being 65% for consultants and 46% for general practitioners. Average number of drugs/encounter was 2.1 and 2.8 for consultants and general practitioners, respectively. Antibiotics were prescribed in 27% and 44%;generic prescribing was 5% and 10% by consultants and general practitioners respectively and 8% of prescriptions by consultants contained injections. Variable results were obtained on information regarding the patient but consultants seem to be better in documenting patient’s age and gender. Consultants and general practitioners tend to prescribe 3 drugs and more in 35% and 25% respectively. The most commonly prescribed therapeutic classes for both groups of prescribers were NSAIDs and antibiotics with ibuprofen and amoxicillin-clavulanic acid combination being the most commonly prescribed drugs of each class. Water safety plan as conceived by WHO can lead to prevention of pollution in each component of water supply chain which leads to ensuring safe drinking water. Risk assessment is one of the key components during the development of water safety plan, achieved by identifying hazardous events and estimating their risk towards implementing control measures. This study reports the risk assessment from catchment to consumers in Maiduguri water treatment plant in Northeast Nigeria. Tools such as the field visits, key informant interviews, questionnaire and water quality monitoring were used to identify the hazards and estimate their risk using semi-quantitative matrix. With the existing control measures, the study showed a total of 33 hazardous events;5 in catchment, 16 in treatment plant, 6 in distribution system and 6 at consumers’ points. The risk score indicated 6 are of medium risk and 9 of high risk. Catchment activities, upgrade of treatment facilities and lack of routine maintenance in the treatment plant, pipeline damages in distribution lines, and consumers’ lack of hygiene knowledge and awareness were found to be the major contributory factors which affect the desired quality. Therefore participation and commitment by all relevant stakeholders are fundamental requisite to manage the identified health risks.

中华文明是世界上最古老的文明之一。在语言中,有许多以实例出现的对人有益和值得学习的丰富经验。中国古代文明以人类对其生存和生活的哲学兴趣的观点出发,并涉及到生活的各个方面,尤其强调了道德和精神价值。中国民间文学史具有丰富的治理经验和文化实力,使世人多年来对其抱有极大的兴趣。汉语成语表达了一种道德社会和经济价值观念,似乎成为了一个不可置疑的真理。我们许多人关注成语和谚语,因为它们经常与现实相匹配,即使句子和词汇很少,但它们充满了实用性和总结了古人们深刻的经验,地球上的所有国家都有自己的遗产,这反映了历史上发生的许多事件,人们通过成语来表达发生在他们的日常生活中的事件,一代又一代地传下来了。通过本研究已发现苏丹留学生对于汉语成语的意思理解及使用等方面的偏误,同时也发现苏丹留学生对于成语学习的兴趣以及学生们对于老师的成语教学态度。值得一提的是,在开始学习成语阶段发现困难的基础,特别是在阅读,写作和听说这几个方面,苏丹学生学习成语的困难是从中级阶段开始出现,若老师不好重视这个问题,就会一直陪他们到高级阶段,还会进入他们的日常教育与社会生活。综上所述,苏丹学生在学习成语的过程中免不了遇到这些困难,但是在老师的帮助下学生会轻而易举地解决这个困难,本研究已经为老师提供了一些可以借用的解决方法,比如激发学生的、创造学生之间的竞争精神、激发学生的优势欲望、创造新的激发方式等等。

Electromagnetic Radiation Health Effects in Exposed and Non-Exposed Residents in Penang

A comparative study was conducted between residents exposed and not exposed to electromagnetic radiation (EMR) from TELCO towers in Penang Island with the objective of determining the possible health effects using 14 non-specific health symptoms (NSHS). Interviews on 201 respondents were conducted using structured questionnaire for demographic details, health related problems and the public concern. Comparison of symptoms frequencies and its significance (Chi-square test) between the exposed and not exposed residents from the TELCO tower showed statistical significance (p 1 for all that gave a conclusion that respondents who were exposed were more likely to suffer symptoms as compared to the respondents who were not exposed to EMR. This outcome showed that the existence of TELCO tower in these communities has detrimental health effects towards the residents who were exposed to the electromagnetic fields radiation that was emitted. Measures to be taken to minimize adverse health effects on residents should include imposing more stringent guidelines in terms of safety distance and radiation intensity, practicing of WHO precautionary approach, encouraging electromagnetic fields radiation related conference, researches and public awareness, sharing of transceivers by TELCO companies and using protective barriers. These steps will ultimately promote a healthier, harmonious and sustainable living environment.

The efficiency and performance of Distributed Database Management Systems (DDBMS) is mainly measured by its proper design and by network communication cost between sites. Fragmentation and distribution of data are the major design issues of the DDBMS. In this paper, we propose new approach that integrates both fragmentation and data allocation in one strategy based on high performance clustering technique and transaction processing cost functions. This new approach achieves efficiently and effectively the objectives of data fragmentation, data allocation and network sites clustering. The approach splits the data relations into pair-wise disjoint fragments and determine whether each fragment has to be allocated or not in the network sites, where allocation benefit outweighs the cost depending on high performance clustering technique. To show the performance of the proposed approach, we performed experimental studies on real database application at different networks connectivity. The obtained results proved to achieve minimum total data transaction costs between different sites, reduced the amount of redundant data to be accessed between these sites and improved the overall DDBMS performance.

Background: Reverse Transcriptase-Polymerase Chain Reaction (RT-PCR) SARS-CoV-2 assay remains the common test used to detect the coronavirus infection worldwide. Complete blood count (CBC) is a rapid and cost-effective test compared to RT-PCR, thus, it would be worthy to find a hematological indicator for SARS-CoV-2 infection. Objectives: This study aims to assess the efficacy of CBC indices as an initial diagnostic test for coronavirus 2019 (COVID-19) infection, which might be helpful during the outbreak of the disease. Materials and Methods: A retrospective, hospital based and case control study was carried out at Gezira isolation center for COVID-19, Gezira State, Sudan in the period from January to May 2021. A total of 178 COVID-19 patients with positive RT-PCR SARS-CoV-2 assay result and another 178 volunteers of apparently healthy people were included in this study. CBC as well as D-dimer and C-reactive protein (CRP) were recorded from patient’s file and neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR) and lymphocyte-to-monocyte ratio (LMR) were calculated. Data were analyzed using Statistical Package for Social Science (SPSS) version 20. Results: Neutrophil percentage, total white blood cells (TWBCs) and platelet (PLT) were higher in the COVID-19 patients compared to their control, whereas, lymphocyte and monocyte showed lower percentages among the COVID-19 patients. Although the differences that observed in the means of PLT, neutrophils and monocytes between the case and control group were significant, all these mean values of both groups were lying within the normal range. On the other hand, there was a highly significant elevation in NLR and PLR and quite good increasing in LMR level among COVID-19 patients compared to their control. The results of receiver operating characteristic curve (ROC) analysis showed that the area under the curve of NLR, PLR and LMR were 0.948 (P. value = 0.00, 95% CI: 0.925 - 0.972), 0.925 (P. value = 0.00
