A new impurity was detected during high performance liquid chromatographic （HPLC） analysis of eslicarbazepine acetate active pharmaceutical ingredient. The structure of unknown impurity was postulated based on liquid chromatography mass spectrometry using electrospray ionization and ion trap analyzer （LC/ESI-IT/MS） analysis. Proposed structure of impurity was unambiguously confirmed by synthesis followed by characterization using 1H, 13C nuclear magnetic resonance spectrometry （NMR）, 1H-1H correlation spectro-scopy （COSY） and infrared spectroscopy （IR）. Based on the spectroscopic and spectrometric data, unknown impurity was characterized as 5-carbamoyl-10,11-dihydro-5H-dibenzo[b,f]azepin-10-yl propionate.

Diabetes Treatment-Related Myths and the Impact of COVID-19 on Diabetes Knowledge—An Indian Survey-Based Study

Objective: The main aim of the survey was to assess diabetes treatment-related myths prevalent in the Indian population and if COVID-19 pandemic improved their knowledge about diabetes. Results: The survey was completed by 309 participants;66% did not have diabetes. The responses of people with diabetes and those without diabetes were similar. Survey results of the total population showed that the majority believed that diabetes treatment should start early (92.6%);87.4% believed that the treatment should start within three months of diagnosis with modern medicines;67.3% of the participants felt that allopathic medicines for diabetes were safe, 69.6% believed that if started these medications continue lifelong, and 40.5% thought they damaged all major organs. Insulin was thought to be safe by 65% of the surveyed population;60.8% believed that if they started insulin, they would need it life-long;51.5% thought that insulin was started at the last stage of diabetes;and 58.6% believed that insulin caused kidney damage. A total of 58.6% believed that herbal medicines for diabetes were safer than allopathic;76.4% did not believe that the “diabetic cure” shown through television/newspapers was safe and effective;67.3% did not believe that ayurvedic medicines cured diabetes. Of the surveyed population, 67% felt that their knowledge about diabetes improved during the pandemic and 89.3% knew that PWDs have more serious problems with covid infection. Conclusions: Our survey shows that many diabetes treatment-related myths are prevalent in the Indian population even though the COVID-19 pandemic improved their knowledge about diabetes.

Naringin ameliorates acetic acid induced colitis through modulation of endogenous oxido-nitrosative balance and DNA damage in rats

The aim of this study was to evaluate the effect of naringin on experimentally induced inflammatory bowel dis- ease in rats. Naringin （20, 40 and 80 mg/kg） was given orally for 7 days to Wistar rats before induction of colitis by intrarectal instillation of 2 mL of 4% （v/v） acetic acid solution. The degree of colonic mucosal damage was analyzed by examining mucosal damage, ulcer area, ulcer index and stool consistency. Intrarectal administration of 4% acetic acid resulted in significant modulation of serum alkaline phosphatase, lactate dehydrogenase, superoxide dismutase （SOD）, glutathione （GSH）, malondialdehyde （MDA） and myeloperoxidase （MPO） content along with colonic nitric oxide （NO）, xanthine oxidase （XO） level and protein carbonyl content in the colonic tissue as well as in blood. Naringin （40 and 80 mg/kg） exerted a dose dependent （P 〈 0.05） ameliorative effect, as it significantly increased hematological parameter as well as colonic SOD and GSH. There was a significant （P 〈 0.05） and dose dependant inhibition of macroscopical score, ulcer area along with colonic MDA, MPO activity by the 7 days of pretreatment of naringin （40 and 80 mg/kg）. Biochemical studies revealed a significant （P 〈 0.05） dose dependant inhibition in serum alkaline phosphatase （ALP） and lactate dehydrogenase （LDH） levels by pretreatment of naringin. Increased levels of colonic NO, XO, protein carbonyl content and DNA damage were also sig- nificantly decreased by naringin pretreatment. The findings of the present investigation propose that naringin has an anti-inflammatory, anti-oxidant and anti-apoptotic potential effect at colorectal sites as it modulates the production and expression of oxidative mediators such as MDA, MPO, NO and XO, thus reducing DNA damage.

In this study,the role of twin-twin interactions on the distributions of local defects(e.g.,dislocations)and stress fields in a magnesium alloy is investigated.A co-zone(1012)-(1012)tensile twin junction in a deformed Mg-3wt.%Y alloy is analyzed using transmission electron microscopy(TEM).The results show that the morphology of the impinging(1012)twin is asymmetric,and the non-interacting boundary of the recipient(1012)twin is *** analysis of TEM images reveals that type-II pyramidal[1213](1212)dislocations concentrate in the vicinity of the twin-twin junction *** samedislocations are also observed inside the interacting twin domains along with a few ***dislocations emanating from the impinging(1012)twin boundary have edge character and are extended with faults parallel to the basal *** contrast,thedislocations connected to the recipient(1012)twin are predominantly screw orientation and ***-viscoplastic fast Fourier transform based crystal plasticity calculations are performed to rationalize the observed twin morphology and local dislocation *** model calculations suggest that the local stress fields generated at the junction site where the two twins meet are responsible for the experimentally observed concentration of*** calculated stress fields are asymmetric with respect to the junction site,explaining the observed asymmetric morphology of the impinging ***,these findings show strong effects of twin-twin interactions on the distribution of dislocations as well as the evolution of the twinned microstructure and as such,can help advance understanding of twinning in Mg alloys and their effect on mechanical behavior.

Objective: Non-hodgkin’s lymphoma (NHL) refers to a group of malignant neoplasms that arise from cellular elements of lymphoid or extranodal tissue. Lymphomas are a frequent cause of malignant lymphadenopathy in the head &neck. The head &neck is the second must frequent anatomical site of extranodal lymphomas after gastrointestinal tract. That’s why dental & maxillofacial surgeon should be aware of malignant lymphoma to diagnose properly & efficiently. This study was performed to evaluate oral & maxillofacial region manifestations, staging, grading and prognosis of NHL.Materials & Method: A retrospective analysis or review of 25patients with oral &maxillofacial region lymphoma was made in our hospital (Dalian Medical University, Stomatology Department of First Affiliated hospital) from 2002jan to 2006 august .Specifically, clinical presentation, staging, grading & therapy for NHL with oral &maxillofacial region involvement were sought. Staging was done according to the Ann Arbor staging system and diagnosis made by immunohistochemistry.Result: 25 patients of NHL were included in our study, among them 20(80%) patients with extra nodal (eNHL) &5(20%) patients with nodal (NHL) involvement. The incidence rate increases exponentially with age between 24 and 79 years. The median age at the diagnosis of the patients was *** above 50 years were 72%.Male (60%) is slightly more than female (40%). Among 20 patients, NHL of Waldeyer’s ring was the most frequent site occurring in 10 patients (50%) followed by NHL of oral cavity (20%), of salivary glands (30%). Constitutional B symptomatology was uncommon, occurring in 25% patients. The typical clinical appearance is a painless local mass lateral or bilateral (90%) and confused with squamous cell carcinoma. BACKGROUND There is evolving role of computed tomography coronary angiography(CTCA)in non-invasive evaluation of coronary artery abnormalities in children with Kawasaki disease(KD).Despite this,there is lack of data on radiation dose in this group of children undergoing *** To audit the radiation dose of CTCA in children with *** Study(December 2013-February 2018)was performed on dual source CT scanner using adaptive prospective *** dose length product(DLP in ***)was *** radiation dose(millisieverts-mSv)was calculated by applying appropriate age adjusted conversion factors as per recommendations of International Commission on Radiological *** dose was compared across the groups(0-1,1-5,5-10,and>10 years).RESULTS Eighty-five children(71 boys,14 girls)with KD underwent *** median age was 5 years(range,2 mo-11 years).Median DLP and effective dose was 21 ***,interquartile ranges(IQR)=15(13,28)and 0.83 mSv,IQR=0.33(0.68,1.01)*** DLP increased significantly across the age *** effective dose in infants(0.63 mSv)was significantly lower than the other age groups(1-5 years 0.85 mSv,5-10 years 1.04 mSv,and>10 years 1.38 mSv)(P<0.05).There was no significant difference in the effective dose between the other groups of *** the CTCA studies were of diagnostic *** child required a repeat *** CTCA is feasible with submillisievert radiation dose in most children with ***,CTCA has the potential to be an important adjunctive imaging modality in children with KD.

Comparison of Sensitivity and Specificity of ZN and Fluorescent Stain Microscopy with Culture as Gold Standard

Introduction: Reports indicate that fluorescent staining of smears increases sensitivity of direct microscopy;so ZN staining is being replaced with fluorescent microscopy in RNTCP in India. Chemical processing and sputum concentration may also improve sensitivity of microscopy. Objective: To compare the sensitivity and specificity of microscopy for AFB using ZN and fluorescent stains in direct and concentrated specimen with culture as gold standard. Methods: Morning sputum specimen of patients, suspected of having pulmonary tuberculosis, over a period of 6 months was subjected to direct microscopy using fluorescent stain;the same slide was over-stained with ZN stain. Same sputum sample was concentrated by Petroff’s method and subjected to fluorescent microscopy followed by ZN microscopy and finally to culture for AFB. Results: Sensitivity of fluorescent stained concentrated sputum samples was maximum and of ZN stained unprocessed sputum samples was minimum. Specificity of three of the methods was equal at 0.96 but of ZN stained concentrated sputum smears was 0.97. Sensitivity of total fluorescent stains was 0.85 (Specificity 0.96) and sensitivity of total ZN stained smears was 0.80 (Specificity 0.96). Discussion: We used same smear for fluorescent and ZN stains, so smear related variability is decreased. Blinding for microscopy was practically complete. Conclusion: The sensitivity of sputum microscopy for AFB can be increased by concentrating the sputum and using fluorescent microscopy. The specificity remains high in all the methods.

Primary pediatric mid-brain lymphoma:Report of a rare pediatric tumor in a rare location

Primary central nervous system lymphoma(PCNSL) is a rare disease in pediatric age group. A thirteen-yearold male child presented with complaints of headache for six months, vomiting and diplopia for three days. Magnetic resonance imaging of the brain showed a single lesion of 1.7 cm × 1.6 cm × 1.6 cm in the mid brain and tectum. He underwent a gross total resection of the tumor. The histopathological evaluation revealed B cell high grade non Hodgkin lymphoma. The patient was treated with High dose methotrexate and cranio spinal radiation. The patient was alive without disease 12 mo after completion of treatment. This case highlights importance of keeping PCNSL as differential in brain stem lesions of pediatric patients also. Radiation and chemotherapy remains the most important treatment for such patients.

Earth has been documented as a natural territory for fungi which cover individual kingdom with evolution. In subsequently vertebrates developed keratin which was a part of life as a structural aspect. Few moulds have skilled to digest keratin and crop up from soil and wastewater habitats. They take part as a keratinolytic agent in the purification of α-keratins with an incidence of disulphide and hydrogen bonds which are improperly biodegradable. The best moulds genera to decay of keratin are Microsporum, Trichophyton and Epidermophyton. The presences of these genera are open health issues in developing countries where they cause the mortal mycotic contagion. The reason behind this is perceived to be the poor hygienic environment and socioeconomic behaviour among people. The present review is a compilation of updated information concerning the nature of these keratinolytic moulds and abundances of most contributed developing countries including India.
