Vitamin E supplementation may negatively affect preimplantation development and mitochondrial ultrastructure of vitrified murine embryos

Objective:To observe the effects of vitamin E on post-vitrification preimplantation development,gross morphology as well as mitochondrial distribution and ***:Twenty-four female c57BL/6NTac mice,aged 12-16 weeks,were randomly divided into four *** A did not receive any treatment and served as the control *** B was treated with corn oil stripped of tocopherols and served as the vehicle *** c was treated with 60 mg/kg body weight of tocotrienol-rich-fraction with corn oil stripped of *** D was treated with 60 mg/kg body weight of alpha-tocopherol with corn oil stripped of *** treatments were administered orally for 7 consecutive *** superovulation and mating with fertile males,2-cell stage embryos were harvested for *** vitrification development in vitro,gross morphology and ultrastructure were compared between ***:The number of 2 and 8-cell embryo,and blastocysts in the treatment and control groups were not significantly different(P>0.05).Following vitrification,all 2-cell embryos had equal-sized blastomeres and intact zona *** aggregation toward the perinuclear region was seen in all of the treatment *** groups c and D had vacuolated mitochondria,which was reflected in the trend of preimplantation development ***:Vitamin E supplementation of 60 mg/kg body weight does not improve the viability of healthy embryos according to this *** a result,the most effective dose of vitamin E supplementation may be determined by the initial quality of the embryos.

Diabetes mellitus(DM)is known to cause reproductive *** men,it has been linked to altered sperm quality and testicular *** stress(OS)plays a pivotal role in the development of DM ***(GSH)is a part of a nonenzymatic antioxidant defense system that protects lipid,protein,and nucleic acids from oxidative ***,the protective effects of exogenous GSH on the male reproductive system have not been comprehensively *** study determined the impact of GSH supplementation in ameliorating the adverse effect of type 1 DM on sperm quality and the seminiferous tubules of diabetic c57BL/6NTac *** at the doses of 15 mg kg^(-1)and 30 mg kg^(-1)was given intraperitoneally to mice weekly for 6 consecutive *** mice were then weighed,euthanized,and had their reproductive organs *** diabetic(D Group)showed significant impairment of sperm quality and testicular histology compared with the nondiabetic(ND Group).Diameters of the seminiferous lumen in diabetic mice treated with 15 mg kg^(-1) GSH(DGSH15)were decreased compared with the D *** motility was also significantly increased in the DGSH15 *** in testicular morphology might be an early indication of the protective roles played by the exogenous GSH in protecting sperm quality from effects of untreated type 1 DM or diabetic *** investigation using different doses and different routes of GSH is necessary to confirm this suggestion.

Fecal samples or cloacal swabs are preferred over lethal dissections to study vertebrate gut microbiota for ethical reasons,but it remains unclear which nonlethal methods provide more accurate information about gut *** compared the bacterial communities of three gastrointestinal tract(GIT)segments,that is,stomach,small intestine(midgut),and rectum(hindgut)with the bacterial communities of the cloaca and feces in the mesquite lizard Sceloporus *** hindgut had the highest taxonomic and functional alpha diversity,followed by midgut and feces,whereas the stomach and cloaca showed the lowest *** taxonomic assemblages of the GIT segments at the phylum level were strongly correlated with those retrieved from feces and cloacal swabs(rs>0.84 in all cases).The turnover ratio of Amplicon Sequence Variants(ASVs)between midgut and hindgut and the feces was lower than the ratio between these segments and the *** than half of the core-ASVs in the midgut(24 of 32)and hindgut(58 of 97)were also found in feces,while less than 5 were found in the *** the ASVs level,however,the structure of the bacterial communities of the midgut and hindgut were similar to those detected in feces and *** findings suggest that fecal samples and cloacal swabs of spiny lizards provide a good approximation of the taxonomic assemblages and beta diversity of midgut and hindgut microbiota,while feces better represent the bacterial communities of the intestinal segments at a single nucleotide variation level than cloacal swabs.

Plantar Fascia and Foot Structure and Function in Living Individual： A Literature Review

The purpose of this literature review is better comprehension about role of the plantar fascia on the structure （arch height） and function （adaptation） of the foot and ankle in living individual. It has long been accepted that plantar fascia supports medial longitudinal arch both in weight bearing and locomotion. Literatures were reviewed through a systematic method. First, current knowledge about plantar fascia properties were established and analyzed; second, previous studies and methods to study plantar fascia were analyzed; finally, the problems in previous researches to study plantar fascia were argued. It was found that plantar fascia affects foot arch and foot adaptation under vertical load in living individual. In accordance with results of this literature review, the modification of plantar fascia thickness under vertical load is specific property of the plantar fascia in living individual. Additionally, the specific property of plantar fascia can help clinicians to diagnose the necessity of plantar fascia assessment along its length.

Impact of pain on health-related quality of life in patients with inflammatory bowel disease

AIM: To evaluate intensity, localization and cofactors of pain in crohn’s disease and ulcerative colitis patients in connection with health-related quality of life (HRQOL) and disease activity. METHODS: We reviewed and analyzed the responses of 334 patients to a specifically designed questionnairebased on the short inflammatory bowel disease questionnaire (SIBDQ) and the German pain questionnaire. Pain intensity, HRQOL, crohn’s disease activity index (cDAI) and colitis activity index (cAI) were correlated and verified on a visual analog scale (VAS). RESULTS: 87.9% of patients reported pain. Females and males reported comparable pain intensities and HRQOL. Surgery reduced pain in both genders (P = 0.023), whereas HRQOL only improved in females. Interestingly, patients on analgesics reported more pain (P = 0.003) and lower HRQOL (P = 0.039) than patients not on analgesics. A significant correlation was found in Uc patients between pain intensity and HRQOL (P = 0.023) and cAI (P = 0.027), and in cD patients between HRQOL and cDAI (P = 0.0001), but not between pain intensity and cDAI (P = 0.35). No correlation was found between patients with low cDAI scores and pain intensity. cONcLUSION: Most IBD patients suffer from pain and have decreased HRQOL. Our study reinforces the need for effective individualized pain therapy in IBD patients.

A Rare case of Superior Mesenteric Vein Aneurysm Secondary to Portal Hypertension

Venous aneurysms are less common than arterial aneurysms in clinical practice, and the occurrence of such cases is a topic for publication. Aneurysms of the superior mesenteric vein (SMV) are rare, and their origin is unknown. Many aneurysms are asymptomatic, and the diagnosis is established from radiologic findings. Others are diagnosed after complications such as gastrointestinal bleeding or thrombosis with associated abdominal pain. Because of the rarity of this disease, therapy must be adapted to fit each case. A 55-year-old woman presented with pain in abdomen since 2 months. The diagnosis of this anomaly was made after computed tomography (cT) scans, that demonstrated a mass. Apart from various etiological theories, portal hypertension was more likely cause of her SMV aneurysm. case was treated by medical treatment like Sorbitrate, Propranolol, Omeprazole and Lasix. No aneurysm growth or complication was observed even after 2 years of follow-up.

Action-oriented obesity counseling attains weight stabilization and improves liver enzymes among overweight and obese children and adolescents

Introduction: Pediatricians are encouraged to promote behavior modification to reduce childhood obesity and its co-morbidities, yet the effectiveness of office counseling is unclear. We aimed to evaluate if a low-intensity intervention (action-oriented counseling) in a clinic setting results in weight stabilization, and if the effect is modified by a diagnosis of non-alcoholic fatty liver disease (NAFLD). We hypothesized that patients with NAFLD would be more motivated to adhere to the lifestyle goals set in clinic, due to the diagnosis of an obesity-related condition;and, would therefore achieve greater weight reduction compared to similarly overweight and obese patients without a diagnosis of NAFLD. Methods: A retrospective chart review was conducted on 73 (35 male, 38 female) overweight and obese patients (BMI ≥ 85th percentile) attending a pediatric GI clinic between January 2006 and October 2011. Analysis was conducted to determine if lifestyle goals discussed with the patient at each clinic visit were associated with improved BMI, BMI z-score, and liver enzymes. Treatment outcomes among NAFLD patients and similarly obese patients without NAFLD were compared using t-tests and chi-square tests. Results: Of the children evaluated, 74.0% achieved a reduction or stabilization in BMI z-score after 3 months of follow-up. Among NAFLD patients, liver enzymes improved in 72% of those who were able to stabilize or reduce their BMI and among 43% of those who gained weight. Treatment outcome did not significantly differ based on having a diagnosis of NAFLD, although there was a trend towards greater improvements. conclusion: Our study suggests that action oriented counseling including goal-setting in a low intensity, clinic based approach is effective in improving patient BMI, in the presence or absence of an obesity-related co-morbidity, such as NAFLD. Lung tumors represent a major health impact globally. Pulmonary cryoablation treatment as a palliative measure for patients with non-operable pulmonary lesions has gained popularity over the last decade. With increasing case load and patients medical status becoming more complex, preparation for pulmonary cryoablation requires the implementation of an enhanced perioperative anesthetic plan. current literature as well as our institutional experience shows that this patient population presents with multiple comorbidities raising the challenge of providing anesthetic care. These procedures are done under challenging conditions with limited resources and in remote locations in the hospital. A team approach by the anesthesiologist, thoracic surgeon, and interventional radiologist is critical to the success of this treatment. The present review examines our institution’s anesthetic management of percutaneous cryoablation treatment (PcT) in the treatment of non-operable lung cancer and metastases. The objective of this article is to review the current literature guidelines and to discuss our retrospective institutional experience in anesthesia management of PcT procedures.

Prevalence of type 2 diabetes in Algerian patients with hepatitis c virus infection

AIM:To investigate the prevalence of,and risk factors for,diabetes mellitus(DM) in Algerian patients with chronic hepatitis c virus(HcV) infection and in a control ***:A cross-sectional study was undertaken.A total of 416 consecutive patients with viral chronic hepatitis attending the Internal Medicine Department of the University Hospital center Touhami Benflis in Batna [290 HcV-infected and 126 hepatitis B virus(HBV)-infected patients] were prospectively ***:The prevalence of DM was higher in HcV-infected patients in comparison with HBV-infected patients(39.1% vs 5%,P cirrhosis,diabetes was more prevalent in HcV-infected patients than in HBV-infected patients(33.5% vs 4.3%,P cirrhosis,diabetes was more prevalent in HcV-infected patients,but the difference was not significant(67.4% vs 20%,P = 0.058).The logistic regression analysis showed that HcV infection [odds ratio(OR) 4.73,95% cI:1.7-13.2],metabolic syndrome(OR 12.35,95% cI:6.18-24.67),family history of diabetes(OR 3.2,95% cI:1.67-6.13) and increased hepatic enzymes(OR 2.22,95% cI:1.1-4.5) were independently related to DM in these ***:The high prevalence of diabetes in HcV-infected patients,and its occurrence at early stages of hepatic disease,suggest that screening for glucose abnormalities should be indicated in these patients.

Association of cardiovascular system medications with cognitive function and dementia in older adults living in nursing homes in Australia

Objective To examine associations between cardiovascular system medication use with cognition function and diagnosis of dementia in older adults living in nursing homes in Australia. Methods As part of a cross-sectional study of 17 Australian nursing homes examining quality of life and resource use, we examined the association between cognitive impairment and cardiovascular medication use （identified using the Anatomical Therapeutic classification System） using general linear regression and logistic regression models. People who were receiving end of life care were excluded. Results Participants included 541 residents with a mean age of 85.5 years （± 8.5）, a mean Psychogeriatric Assessment Scale–cognitive Impairment （PAS-cog） score of 13.3 （± 7.7）, a prevalence of cardiovascular diseases of 44% and of hypertension of 47%. Sixty-four percent of participants had been diagnosed with dementia and 72% had received cardiovascular system medications within the previous 12 months. Regression models demonstrated the use of cardiovascular medications was associated with lower （better） PAS-cog scores [coefficient （β） = -3.7; 95% cI： -5.2 to -2.2; P 〈 0.0001] and a lower probability of a dementia diagnosis （OR = 0.44; 95% cI： 0.26 to 0.75, P = 0.0022）. Analysis by subgroups of medications showed cardiac therapy medications （c01）, beta blocking agents （c07）, and renin-angiotensin system agents （c09） were associated with lower PAS-cog scores （better cognition） and lower dementia diagnosis probability. conclusions This analysis has demonstrated an association between greater cardiovascular system medication use and better cognitive status among older adults living in nursing homes. In this population, there may be differential access to health care and treatment of cardiovascular risk factors. This association warrants further investigation in large cohort studies.
