Is hepatic steatosis associated with left ventricular mass index increase in the general population?

AIM To investigate the association between hepatic steatosis and change in left ventricular mass index(LVMI) over five years, and examine whether systolic and diastolic blood pressures are mediators of the association between hepatic steatosis and LVMI using a general population *** We analyzed data from the Study of Health in Pomerania. The study population comprised 1298individuals aged 45 to 81 years. Hepatic steatosis was defined as the presence of a hyperechogenic pattern of the liver together with elevated serum alanine transferase levels. Left ventricular mass was determined echocardiographically and indexed to height2.7. Path analyses were conducted to differentiate direct and indirect paths from hepatic steatosis to LVMI encompassing systolic and diastolic blood pressure as potential mediating *** Hepatic steatosis was a significant predictor for all measured echocardiographic characteristics at baseline. Path analyses revealed that the association of hepatic steatosis with LVMI change after five years was negligibly small(β =-0.12, s.e. = 0.21, P = 0.55). Systolic blood pressure at baseline was inversely associated with LVMI change(β =-0.09, s.e. = 0.03, P between diastolic blood pressure at baseline and LVMI change was evident(β = 0.03, s.e. = 0.05, P = 0.56). The effect of the indirect path from hepatic steatosis to LVMI via systolic baseline blood pressure was small(β =-0.20, s.e. = 0.10, P = 0.07). No indirect effect was observed for the path via diastolic baseline blood pressure(β = 0.03, s.e. = 0.06, P = 0.60). Similar associations were observed in the subgroup of individuals not receiving beta-blockers, calcium channel blockers, or drugs acting on the reninangiotensin *** baseline associations between hepatic steatosis and LVMI do not extend to associations with LVMI change after five years. More studies are needed to study the longitudinal effects of hepatic steatosis on LVMI.

For a finitely triangulated closed surface M2, let αx be the sum of angles at a vertex x. by the well-known combinatorial version of the 2- dimensional Gauss-bonnet Theorem, it holds ∑x（2π- αx） =2αχ（M^2）, where X denotes the Euler characteristic of M^2, αx denotes the sum of angles at the vertex x, and the sum is over all vertices of the triangulation. We give here an elementary proof of a straightforward higher-dimensional generalization to Euclidean simplicial complexes K without assuming any combinatorial manifold condition. First, we recall some facts on simplicial complexes, the Euler characteristics and its local version at a vertex. Then we define δ（τ） as the normed dihedral angle defect around a simplex τ. Our main result is ∑τ（-1）^dim（τ）δ（τ） = χ（K）, where the sum is over all simplices τ of the triangulation. Then we give a definition of curvature k（x） at a vertex and we prove the vertex-version ∑x∈K0 k（x） = χ（K） of this result. It also possible to prove Morse-type inequalities. Moreover, we can apply this result to combinatorial （n ＋ 1）-manifolds W with boundary b, where we prove that the difference of Euler characteristics is given by the sum of curvatures over the interior of W plus a contribution from the normal curvature along the boundary b： χ（W）- 1/2 χ（b） = ∑τ∈W-b（-1）^dim（τ）δ（τ）＋ ∑τb（-1）^dim（τ）ρ（τ）.

AIM To evaluate the long-term outcome of catheter ablation of atrial fibrillation(AF) facilitated by preprocedural threedimensional(3-D) transesophageal *** In 50 patients, 3D transesophageal echocardiography(3D TEE) was performed immediately prior to an ablation procedure(paroxysmal AF: 30 patients, persistent AF: 20 patients). The images were available throughout the ablation procedure. Two different ablation strategies were used. In most of the patients with paroxysmal AF, the cryoablation technique was used(Arctic Front balloon, Cryo Cath Technologies/Medtronic; group A2). In the other patients, a circumferential pulmonary vein ablation was performed using the CARTO system [biosense Webster; group A1(paroxysmal AF), group b(persistent AF)]. Success rates and complication rates were analysed at 4-year *** A 3D TEE could be performed successfully in all patients prior to the ablation procedure and all four pulmonaryvein ostia could be evaluated in 84% of patients. The image quality was excellent in the majority of patients and several variations of the pulmonary vein anatomy could be visualized precisely(e.g., common pulmonary vein ostia, accessory pulmonary veins, varying diameter of the left atrial appendage and its distance to the left superior pulmonary vein). All ablation procedures could be performed as planned and almost all pulmonary veins could be isolated successfully. At 48-mo followup, 68.0% of all patients were free from an arrhythmia recurrence(group A1: 72.7%, group A2: 73.7%, group b: 60.0%). There were no major ***3 D TEE provides an excellent overview over the left atrial anatomy prior to AF ablation procedures and these procedures are associated with a favourable long-term outcome.

Hepadnaviridae is a family of hepatotropic DNA viruses that is divided into the genera orthohepadnavirus of mammals and avihepadnavirus of birds. All members of this family can cause acute and chronic hepatic infection, which in the case of human hepatitis b virus （HbV） constitutes a major global health problem. Although our knowledge about the molecular biology of these highly liver-specific viruses has profoundly increased in the last two decades, the mechanisms of attachment and productive entrance into the differentiated host hepatocytes are still enigmatic. The difficulties in studying hepadnaviral entry were primarily caused by the lack of easily accessible in vitro infection systems. Thus, for more than twenty years, differentiated primary hepatocytes from the respective species were the only in vitro models for both orthohepadnaviruses （e.g. HbV） and avihepadnaviruses （e.g. duck hepatitis b virus [DHbV]）. Two important discoveries have been made recently regarding HbV：（1） primary hepatoo/tes from tree-shrews; i.e., Tupaia belangeri, can be substituted for primary human hepatocytes, and （2） a human hepatoma cell line （HepaRG） was established that gains susceptibility for HbV infection upon induction of differentiation in vitro. A number of potential HbV receptor candidates have been described in the past, but none of them have been confirmed to function as a receptor. For DHbV and probably all other avian hepadnaviruses, carboxypeptidase D （CPD） has been shown to be indispensable for infection, although the exact role of this molecule is still under debate. While still restricted to the use of primary duck hepatocytes （PDH）, investigations performed with DHbV provided important general concepts on the first steps of hepadnaviral infection. However, with emerging data obtained from the new HbV infection systems, the hope that DHbV utilizes the same mechanism as HbV only partially held true. In furtherance to improving agreement between calculated and experimental nuclear data, the nuclear reaction code GAMME was used to calculate the multistep compound(MSC) nucleus double differential cross sections(DDCs) for proton-induced neutron emission reactions using the Feshbach-Kerman-Koonin(FKK) formalism. The cross sections were obtained for reactor structural materials involving ^(52)Cr(p, n)^(52)Mn,^(56)Fe(p,n)^(56)Co, and ^(60)Ni(p, n)^(60)Cu reactions at 22.2 MeV incident energy using the zero-range reaction mechanism. Effective residual interaction strength was 28 MeV, and different optical potential parameters were used for the entrance and exit channels of the proton-neutron interactions. The calculated DDCs were fitted to experimental data at the same backward angle of 150°, where the MSC processes dominate. The calculated and experimental data agree well in the region of pre-equilibrium(MSC) reaction dominance against a weaker fit at the lower emission energies. We attribute underestimations to contributions from the other reaction channels and disagreement at higher outgoing energies to reactions to collectively excited states. Contrary to the FKK multi-step direct calculations, contributions from the higher stages to the DDCs are significant. Different sets of parameters resulted in varying levels of agreement of calculated and experimental data for the considered nuclei.

This review describes the woodchuck and the woodchuck hepatitis virus (WHV) as an animal model for pathogenesis and therapy of chronic hepatitis b virus (HbV) infection and disease in humans. The establishment of woodchuck breeding colonies, and use of laboratory-reared woodchucks infected with defined WHV inocula, have enhanced our understanding of the virology and immunology of HbV infection and disease pathogenesis, including major sequelae like chronic hepatitis and hepatocellular carcinoma. The role of persistent WHV infection and of viral load on the natural history of infection and disease progression has been firmly established along the way. More recently, the model has shed new light on the role of host immune responses in these natural processes, and on how the immune system of the chronic carrier can be manipulated therapeutically to reduce or delay serious disease sequelae through induction of the recovery phenotype. The woodchuck is an outbred species and is not well defined immunologically due to a limitation of available host markers. However, the recent development of several key host response assays for woodchucks provides experimental opportunities for further mechanistic studies of outcome predictors in neonatal- and adult-acquired infections. Understanding the virological and immunological mechanisms responsible for resolution of self-limited infection, andfor the onset and maintenance of chronic infection, will greatly facilitate the development of successful strategies for the therapeutic eradication of established chronic HbV infection. Likewise, the results of drug efficacy and toxicity studies in the chronic carrier woodchucks are predictive for responses of patients chronically infected with HbV. Therefore, chronic WHV carrier woodchucks provide a well-characterized mammalian model for preclinical evaluation of the safety and efficacy of drug candidates, experimental therapeutic vaccines, and immunomodulators for the treatment and preventi
