Carcinosarcoma(CS),also known as metaplastic breast carcinoma with mesenchymal differentiation,is one of the five distinct subtypes of metaplastic breast *** is considered as a mixed,biphasic neoplasm consisting of a carcinomatous component combined with a malignant nonepithelial element of mesenchymal origin without an intermediate transition *** cellular origin of this neoplasm remains controversial,most researchers declare that neoplastic cells derive from a cellular structure with potential biphasic *** recent research on the therapeutic strategies against CS neoplastic disorders,surgical resection appears the only potentially curative *** CS metastasize by the lymphatic route,axillary assessment with sentinel lymph node biopsy and/or axillary lymph node dissection is always ***,the tumor also presents a hematogenous metastatic pattern including pleural,pulmonary,liver,brain and less commonly bone ***,surgical removal of breast CS does not necessarily ensure patient’s long-term ***,alternative therapies,such as radio-and chemotherapy proved insufficient and 5-year survival rate is ***,there is evidence that following surgery,the combination of radio and chemotherapy is associated with a better prognosis than either treatment *** aim of this review is to evaluate the results of surgical treatment for breast CS with special reference to the extent of its histological *** features,histogenesis,morphological and immunochemical findings are discussed,while the role of current diagnostic and therapeutic management of this aggressive neoplasm is emphasized.

Alternative Transcription Initiation and the AUG Context Configuration Control Dual-Organellar Targeting and Functional Competence of Arabidopsis Lon 1 Protease

Cellular homeostasis relies on components of protein quality control including chaperones and proteases. In bacteria and eukaryotic organelles, Lon proteases play a critical role in removing irreparably damaged proteins and thereby preventing the accumulation of deleterious degradation-resistant aggregates. Gene expression, live-cell imaging, immunobiochemical, and functional complementation approaches provide conclusive evidence for Lonl dual-targeting to chloroplasts and mitochondria. Dual-organellar deposition of Lonl isoforms depends on both transcriptional regula- tion and alternative translation initiation via leaky ribosome scanning from the first AUG sequence context that deviates extensively from the optimum Kozak consensus. Organelle-specific Lonl targeting results in partial complementation of Arabidopsis Ion1-1 mutants, whereas full complementation is solely accomplished by dual-organellar targeting. Both the optimal and non-optimal AUG sequence contexts are functional in yeast and facilitate leaky ribosome scanning com- plementing the piml phenotype when the mitochondrial presequence is used. Bioinformatic search identified a limited number of Arabidopsis genes with Lonl-type dual-targeting sequence organization. Lon4, the paralog of Lonl, has an ambiguous presequence likely evolved from the twin presequences of an ancestral Lonl-like gene, generating a single dual-targeted protein isoform. We postulate that Lonl and its subfunctional paralog Lon4 evolved complementary sub- sets of transcriptional and posttranscriptional regulatory components responsive to environmental cues for dual-organel- lar targeting.

USE OF CONSTRUCTION AND DEMOLITION WASTESAS RAW MATERIALS IN CEMENT CLINKER PRODUCTION

The aim of the present paper was to investigate the possibility of utilizing Construction and Demolition （C＆D） wastes as substitutes of Portland cement raw meal. The C＆D wastes that were so used, were the Recycled Concrete Aggregates （RCA） and the Recycled Masonry Aggregates （RMA） derived from demolished buildings in Attica region, Greece. RCA and RMA samples were selected because of their calcareous and siliceous origin respectively, which conformed the composition of the ordinary Portland cement raw meal. For that reason, six samples of cement raw meals were prepared： one with ordinary raw materials, as a reference sample, and five by mixing the reference sample with RCA and RMA in appropriate proportions. The effect on the reactivity of the generated mixtures, was evaluated on the basis of the free lime content （fCaO） in the mixtures sintered at 1350℃, 1400℃ and 1450℃. Test showed that the added recycled aggregates improved the burnability of the cement raw meal without affecting negatively the cement clinker properties. Moreover, the formation of the major components （C3S1 C2S1 C3A and C4AF） of the produced clinkers （sintered at 1450℃） was corroborated by X-Ray Diffraction （XRD）.

BACKGROUND Sodium-glucose co-transporter-2 inhibitors(SGLT2i)significantly reduce the risk of cardiovascular(CV)and renal adverse events in patients with diabetes mellitus,heart failure(HF)and/or chronic kidney *** performed a meta-analysis to explore the impact of several different SGLT2i on all-cause mortality,CV mortality,HF hospitalizations and the combined outcome CV death/HF hospitalization in HF patients across the spectrum of left ventricular ejection fraction(LVEF)*** A systematic search in MEDLINE database and Cochrane library through March 2021 was performed without *** clinical trials that provided data about the impact of SGLT2i on all-cause mortality,CV mortality,HF hospitalizations or the combined outcome of CV death/HF hospitalization in HF patients were included.A random effects model was used for calculating the effect *** Nine studies(n=16,723 patients,mean age:65.9 years,males:70.7%)were included in the quantitative *** to placebo,SGLT2i use was associated with 14%lower risk of all-cause mortality[hazard ratio(HR)=0.86,95%CI:0.78−0.94,I^(2)=0,P=0.0008],32%lower risk of HF hospitalizations(HR=0.68,95%CI:0.62−0.74,I^(2)=0,P<0.001),14%lower risk of CV mortality(HR=0.86,95%CI:0.77−0.95,I^(2)=0,P=0.003)and 26%lower risk of CV death/HF hospitalization(HR=0.74,95%CI:0.68−0.80,I^(2)=0,P<0.001).Regarding the safety outcomes,our data revealed no significant differences between SGLT2i and placebo groups in drug related discontinuations,amputations,severe hypoglycemia,hypotension,volume depletion,ketoacidosis and genital *** contrast,a protective role of SGLT2i against placebo was found for serious adverse events and acute kidney *** In patients with HF,regardless of LVEF phenotype,all SGLT2i had an excellent safety profile and significantly reduced the risk of all-cause mortality,CV mortality,HF hospitalizations and CV deaths/HF hospitalizations compared to placeb

Efficacy and safety of novel anticoagulants in the elderly

Atrial fibrillation and venous thromboembolism （VTE） are common disorders associated with maleficent thrombotic events, particularly in the elderly patients. Polypharmacy, co-morbidities, and altered pharmacokinetics, often present in these patients, render the use of antico-agulants quite challenging. Novel oral anticoagulants （NOACs） have recently emerged as alternatives to Vitamin K Antagonists （VKAs） and are gradually increasing their popularity mainly because of their fewer drug and food interactions and ease of use. Their effectiveness and safety has been weU-established in the general population but the balance between benefit and harm in the elderly is still unclear. Routine use in these patients is uncommon. Accumulating data have shown that the benefit of NOACs is consistent among all age groups, featuring equal or greater efficacy in preventing thrombotic events. Excess bleedings were lower with NOACs in comparison to VKAs, but bleeding patterns were disparate among them and head to head comparison is not available. The present review highlights on the efficacy and safety of novel anticoagulants in the elderly population.

There are several therapeutic approaches in type 2 diabetes mellitus(T2DM).When diet and exercise fail to control hyperglycemia,patients are forced to start therapy with antidiabetic ***,these drugs present several drawbacks that can affect the course of *** major disadvantages of current oral modalities for the treatment of T2DM are mainly depicted in the low bioavailability and the immediate release of the drug,generating the need for an increase in frequency of *** conjugation with the manifestation of adverse side effects,patient compliance to therapy is *** the past few years nanotechnology has found fertile ground in the development of novel delivery modalities that can potentially enhance anti-diabetic regimes *** efforts have been targeted towards two main vital steps:(a)to protect the drug by encapsulating it into a nano-carrier system and(b)efficiently release the drug in a gradual as well as controllable ***,only a limited number of studies published in the literature used in vivo techniques in order to support *** we discuss the current disadvantages of modern T2DMmarketed drugs,and the nanotechnology advances supported by in vivo in mouse/rat models of glucose *** generation of drug nanocarriers may increase bioavailability,prolong release and therefore reduce dosing and thus,improve patient *** novel approach might substantially improve quality of life for *** ofmetal nanoformulations as indirect hypoglycemic agents is also discussed.

Is It Possible to Reduce Even More Need for Axillary Dissection?

It has always been disappointing to receive a histology report after an axillary dissection (ALND) saying that the only positive lymph node was the sentinel lymph node (SLN). It is for this reason that there have been many efforts to create the best predictive model in order to avoid non sentinel node dissection and in fact there are in use many of them with a reasonable success rate. The publication of the multicenter study by Giuliano et al. showed a disease free survival and overall survival rate equal between patients with positive SLN with or without axillary dissection in a large group of patients. Breast surgeons around the world have long before been interested in reducing even more the need for axillary lymph node dissection, so they easily grasped the chance of those results and applied them in their practice. Objections have been expressed regarding the integrity of the study methods and the results which make the need for a second study to confirm those results absolutely necessary.

Effects of albumin/glutaraldehyde glue on healing of colonic anastomosis in rats

To evaluate the effect of local surgical adhesive glue (albumin/glutaraldehyde-Bioglue) on the healing of colonic anastomoses in rats. METHODSForty Albino-Wistar male rats were randomly divided into two groups, with two subgroups of ten animals each. In the control group, an end-to-end colonic anastomosis was performed after segmental resection. In the Bioglue group, the anastomosis was protected with extraluminar application of adhesive glue containing albumin and glutaraldehyde. Half of the rats were sacrificed on the fourth and the rest on the eighth postoperative day. Anastomoses were resected and macroscopically examined. Bursting pressures were calculated and histological features were graded. Other parameters of healing, such as hydroxyproline and collagenase concentrations, were evaluated. The experimental data were summarized and computed from the results of a one-way ANOVA. Fisher’s exact test was applied to compare percentages. RESULTSBursting pressures, adhesion formation, inflammatory cell infiltration, and collagen deposition were significantly higher on the fourth postoperative day in the albumin/glutaraldehyde group than in the control group. Furthermore, albumin/glutaraldehyde significantly increased adhesion formation, inflammatory cell infiltration, neoangiogenesis, and collagen deposition on the eighth postoperative day. There was no difference in fibroblast activity or hydroxyproline and collagenase concentrations. CONCLUSIONAlbumin/glutaraldehyde, when applied on colonic anastomoses, promotes their healing in rats. Therefore, the application of protective local agents in colonic anastomoses leads to better outcomes.
