BACKGROUND Bedside needle arthroscopy of the ankle under local anesthesia has been proposed for intra-articular delivery of injectable *** and tolerability of this approach in the clinical setting–including patients with end-stage ankle pathology and/or a history of prior surgery–is not *** To assess clinical accuracy and tolerability of bedside needle arthroscopy as a delivery system for injectable agents into the tibiotalar *** This was a prospective study that included adult patients who were scheduled for an injection with hyaluronic acid to the tibiotalar *** our center,these injections are used as a last resort prior to extensive *** primary outcome was injection accuracy,which was defined as injecting through the arthroscopic cannula with intra-articular positioning confirmed by a clear arthroscopic view of the joint *** outcome measures included a patientreported numeric rating scale(NRs,0-10)of pain during the procedure and willingness of patients to return for the same *** of ankle pain at rest and during walking was collected at baseline and at 2-wk *** were monitored from inclusion up to a 2-wk control *** We performed 24 ***(46%)participants were male,and mean age was 46.8±14.5 *** was the indication for injection in 20(83%)cases,of which 8(33%)patients suffered from osteoarthritis Kellgren-Lawrence grade IV,and 10(42%)patients from Kellgren-Lawrence grade *** osteochondral defect was the indication for injection in 4(17%)cases.A history of ankle surgery was present in 14(58%)participants and a history of multiple ankle surgeries in 11(46%)*** was possible to confirm accuracy in 21(88%)*** 3(12%)participants where needle arthroscopy did not reach a clear view of the joint space all suffered from Kellgren-Lawrence grade IV *** during the procedure was reported with a median of 1[i

Nonlinear roll damping of a barge with and without liquid cargo in spherical tanks

Damping plays a significant role on the maximum amplitude of a vessel’s roll motion,in particular near the resonant *** is a common practice to predict roll damping using a linear radiation-diffraction code and add that to a linearized viscous damping component,which can be obtained through empirical,semi-empirical equations or free decay tests in calm ***,it is evident that the viscous roll damping is nonlinear with roll velocity and *** liquid cargo motions inside cargo tanks also contribute to roll damping,which when ignored impedes the accurate prediction of maximum roll *** this study,a series of free decay model tests is conducted on a barge-like vessel with two spherical tanks,which allows a better understanding of the nonlinear roll damping components considering the effects of the liquid cargo *** examine the effects of the cargo motion on the damping levels,a nonlinear model is adopted to calculate the damping *** liquid cargo motion is observed to affect both the linear and the quadratic components of the roll *** flow memory effect on the roll damping is also *** nonlinear damping coefficients of the vessel with liquid cargo motions in spherical tanks are obtained,which are expected to contribute in configurations involving spherical tanks.

BACKGROUND In adults,bowel intussusception is a rare diagnosis and is mostly due to an organic bowel *** rare cases,this is a complication of a percutaneously placed endoscopic gastro(jejunostomy)*** sUMMARY We describe a case of a 73-year-old patient with a history of myocardial infarction,chronic idiopathic constipation and Parkinson’s *** the admission of his Parkinson’s medication,a percutaneous endoscopic gastrostomy with jejunal extension(PEG-J)was *** patient presented three times at the emergency department of the hospital with intermittent abdominal pain with nausea and *** were no distinctive abnormalities from the physical and laboratory *** abdominal computed tomography scan showed a small bowel *** push endoscopy,a jejunal bezoar at the tip of the PEG-J catheter was found to be the cause of small bowel *** intussusception was resolved after removing the bezoar during push *** Endoscopic treatment of bowel intussusception caused by PEG-J catheter bezoar.

Immunogenic antigens for vaccination are often created through the production of recombinant proteins using Escherichia coli.1 As a side product,aggregates called inclusion bodies(IBs)are formed,containing largely misfolded forms of the overexpressed recombinant protein.2 The effect of IBs on the immune system and whether they can be used as effective vaccine products remain *** non-native conformation of proteins upon accumulation in IBs abrogates their use as vaccines aimed at generating high-affinity antibodies.3 However,IBs exhibit unique properties,including mechanical and thermal stability due to their intrinsic particulate nature,biocompatibility,high antigenic content,low toxicity and relative resistance to *** characteristics make IBs attractive as an antigenic vaccine formulation for T cell responses to linear *** IBs can trigger adaptive cellular responses initiated via uptake by dendritic cells for presentation to T cells is unknown.

Dear Editor,Burn scar contracture is a common problem in healing burn wounds of the neck. It can cause both pain and dysfunction if not treated adequately (1)The treatment of such wounds often involves a combination of surgery and splinting therapy (2)A variety of splints, including the thermoplastic static neck splint [3], the Watusi collar [4], manually fabricated splints, and pre-fabricated splints such as the Philadelphia collar have been used for the management of scar contractures. However, each type of splint has its own advantages and disadvantages, and none of these splints seem to reduce the need for skin reconstruction nor delays the time until surgical re-construction [5].

CD4^(+)T cells can"help"or"license" conventional type 1 dendritic cells(cDC1s)to induce CD8^(+)cytotoxic T lymphocyte(CTL)anticancer responses,as proven in mouse *** recently identified cDC1s with a transcriptomic imprint of CD4^(+)T-cell help,specifically in T-cell-infiltrated human cancers,and these cells were associated with a good prognosis and response to PD-1-targeting ***,we delineate the mechanism of cDC1 licensing by CD4^(+)T cells in *** CD4^(+)T cells produce IFNβvia the sTING pathway,which promotes MHC-I antigen(cross-)presentation by cDC1s and thereby improves their ability to induce CTL anticancer *** cooperation with CD40 ligand(L),IFNβalso optimizes the costimulatory and other functions of cDC1s required for CTL response ***-I-producing CD4^(+)T cells are present in diverse T-cell-infiltrated cancers and likely deliver“help”signals to CTLs locally,according to their transcriptomic profile and colocalization with“helped/licensed”cDCs and tumor-reactive CD8^(+)T *** agreement with this scenario,the presence of IFN-I-producing CD4^(+)T cells in the TME is associated with overall survival and the response to PD-1 checkpoint blockade in cancer patients.

This study explores how dust from the Ekati Diamond Mine potentially affects the availability and quality of forage on the seasonal range of the Bathurst caribou herd. Understanding the effects of dust as a source of disturbance is important because the Bathurst caribou population has declined by 93% since the middle 1980s and there are reports that caribou in general may avoid mining projects. There are several challenges for quantifying dust impacts: 1) Natural variations (e.g., topography, natural disturbance, and soil pH) may also impact forage availability and quality for caribou. To minimize their masking effect, we stratified survey sites into seven land cover classes and selected the most populous class (i.e., the dwarf shrub) for assessing the impact. 2) Within class variation (e.g., the proportion of area covered by rocks where vascular plants and lichen do not grow) can further skew the analysis. We eliminated this problem by examining only the area not covered by rocks. 3) Coarse and fine suspended particulates have different spatial coverages, chemical compositions, and pH values. Consequently, their impacts on caribou forage can be different. To distinguish their impacts, we sampled two areas: transects from the Misery Haul Road that has been in active use vs. those from a rarely used spur road outside the Misery Camp. We sampled percent vegetation cover, soil pH, and dust on leaves along these transects during the summers of 2015 and 2016. Our results indicated that the amount of dust on leaves in a zone of ~1000 m from the Misery Haul Road was 3 - 9 times than that of background sites. The zone of reduced lichen percent cover was also about 1000 m. In contrast, these road dust-induced changes in caribou forage were not observed for the dust-free transect from the spur road.
