Changes over time in milk test results following pancreatectomy

AIM: To investigate changes over time in, and effects of sealing technology on, milk test results following pancreatectomy. METHODS: From April 2008 to October 2013, 66 pancreatic resections were performed at the Iwakuni Clinical Center. The milk test has been routinely conducted at the institute whenever possible during pancreatectomy. The milk test comprises the following procedure: A nasogastric tube is inserted until the third portion of the duodenum, followed by injection of 100 mL of milk through the tube. If a chyle leak is present, the patient tests positive in this milk test based on the observation of a white milky discharge. Positive milk test rates, leakage sites, and chylous ascites incidence were examined. Liga Sure?(LS; Covidien, Dublin, Ireland), a vessel-sealing device, is routinely used in pancreatectomy. Positive milk test rates before and after use of LS, as well as drain discharge volume at the 2^(nd) and 3^(rd) postoperative days, were compared retrospectively. Finally, positive milk test rates and chylous ascites incidence were compared with the results of a previous ***: Fifty-nine milk tests were conducted during pancreatectomy. The positive milk test rate for all pancreatectomy cases was 13.6%(8 of 59 cases). One case developed postoperative chylous ascites(2.1% among the pancreatoduedenectomy cases and 1.7% among all pancreatectomies). Positive rates by procedure were 12.8% for pancreatoduodenectomy and 22.2% for distal pancreatectomy. Positive rates by disease were 17.9% for pancreatic and 5.9% for biliary diseases. When comparing results from before and after use of LS, positive milk test rates in pancreatoduodenectomy were 13.0% before and 12.5% after, while those in distal pancreatectomy were 33.3% and 0%. Drainage volume tended to decrease when LS was used on the 3^(rd) postoperative day(volumes were 424 ± 303 mL before LS and 285 ± 185 mL after, P = 0.056). Hox Gene Clusters of Early Vertebrates:Do They Serve as Reliable Markers for Genome Evolution?

Hox genes, responsible for regional specification along the anteroposterior axis in embryogenesis, are found as clusters in most eumetazoan genomes sequenced to date. Invertebrates possess a single Hox gene cluster with some exceptions of secondary cluster breakages, while osteichthyans （bony vertebrates） have multiple Hox clusters. In tetrapods, four Hox clusters, derived from the so-called two-round whole genome duplications （2R-WGDs）, are observed. Overall, the number of Hox gene clusters has been regarded as a reliable marker of ploidy levels in animal genomes. In fact, this scheme also fits the situations in teleost fishes that experienced an additional WGD. In this review, I focus on cyclostomes and cartilaginous fishes as lineages that would fill the gap between invertebrates and osteichthyans. A recent study highlighted a possible loss of the HoxC cluster in the galeomorph shark lineage, while other aspects of cartilaginous fish Hox clusters usually mark their conserved nature. In contrast, existing resources suggest that the cyclostomes exhibit a different mode of Hox cluster organization. For this group of species, whose genomes could have differently responded to the 2R-WGDs from jawed vertebrates, therefore the number of Hox clusters may not serve as a good indicator of their ploidy level.

A variant form of autoimmune pancreatitis successfully treated by steroid therapy, accompanied by von Meyenburg complex

Diagnostic criteria for autoimmune pancreatitis （AIP） have been proposed and used clinically because, despite its unique clinicopathological features, AIP does not have disease-specific serological tests for confirmation. However, diagnosis of a patient with pancreatic lesions mimicking cancer who deviates from these diagnostic criteria is still difficult. We present herein a patient with a variant form of AIP successfully diagnosed by fine-needle biopsy, whose response to steroid therapy was excellent. A 55-year-old Japanese man was admitted to hospital because of jaundice and pancreatic head mass. AIP was considered as one of the differential diagnoses; however, as the patient showed neither pancreatic duct narrowing nor immunological abnormalities, he did not meet the Japanese diagnostic criteria for AIP. Histopathology of the pancreatic mass demonstrated abundant infiltration by lymphocytes and interstitial fibrosis, which suggested AIP. Immunoreaction to IgG4, which is supposed to be specific to AIP, was not observed; however, response to subsequent prednisolone therapy was good, with dramatic pancreatic head mass regression. Aside from the pancreatic head mass, diffusely spreading small lesions were observed throughout the liver. The likelihood of a potential association with extrapancreatic lesions of AIP was considered and led us to carry out a liver biopsy, which revealed biliary hamartoma, also called yon Meyenburg complex （VMC）. As IgG4-positive plasma cell infiltration was not demonstrated in the hamartomatous regions, the hepatic condition was thought to have occurred incidentally; however, to the best of our knowledge, this is the first report in which the association between AIP and VMC was investigated and discussed.

Monoclonal antibody pharmacogenomics in cancer treatment

Conventionally, in the pharmacokinetic/pharmacodynamic analysis of small molecule compounds such as cytotoxic anticancer drugs, polymorphism analysis of genes related to absorption, distribution, metabolism, and excretion has been performed in addition to the analyses of blood concentrations of drugs. Such pharmacogenetic factors play an important role in predicting therapeutic effects and adverse events and in the proper use of drugs. With the recent launch of immune checkpoint inhibitors (ICIs) and the rapid development of antibody-drug conjugates (ADCs) currently underway, there is no doubt that antibody drugs, which are large molecule compounds, will become key drugs in anticancer drug treatment. However, the pharmacokinetic and pharmacodynamic analysis of antibody drugs is still not sufficient, and further elucidation of factors and mechanisms affecting their dynamics in the human body is necessary. Moreover, the pharmacogenomic factors of antibody drugs have not yet been fully studied. There are many factors that should be clarified, such as factors that regulate the host immune response in ICI therapy and the effects of ATP-binding cassette transporter and cytochrome P450 on the payload of ADCs. This review provides an outline of antibody drugs in cancer treatment and summarizes the pharmacogenomic factors of antibody drugs known to date.

Origins of short gamma-ray bursts deduced from offsets in their host galaxies revisited

The spatial distribution of short Gamma-ray bursts (GRBs) in their host galaxies provides us with an opportunity to investigate their origins. Based on the currently observed distribution of short GRBs relative to their host galaxies, we obtain the fraction of the component that traces the mergers of binary compact objects and the one that traces star formation rate (such as massive stars) in earlyand late-type host galaxies. From the analysis of projected offset distribution and only based on population synthesis and massive star models, we find that the fraction of massive stars is 0.37+-00..4327 with an error at the 1σ level for a sample with 22 short GRBs in the literature. From these results, it is hard to accept that the origin of short GRBs with observed statistics is well described by current models using only the offset distribution. The uncertainties in observational localizations of short GRBs also strongly affect the resulting fraction.

A voltammetric determination of possible organic pollutants such as diol and phenolic compounds in water was studied using ferroceneboronic acid（FBA） as a redox-active marker.A cyclic voltammogram of FBA exhibited a pair of oxidation and reduction peaks at 230 and 170 mV at pH 7.0,respectively,while another pair of redox peaks was observed in the presence of diol or phenolic compounds *** results were rationalized based on the formation of boronate esters of FBA with the added *** changes in the redox peak currents were dependent on the concentration of the additives,suggesting a usefulness of FBA in the electrochemical determination of these compounds in water.

BACKGROUND Intraductal papillary neoplasm of the bile duct(IPNB)rarely recurs in a multicentric *** encountered a patient with multiple recurrences of the gastric subtype of IPNB one year after spontaneous detachment of the primary tumor during peroral cholangioscopy(POCS).CASE SUMMARY A 68-year-old woman on maintenance hemodialysis because of lupus nephritis had several cardiovascular diseases and a pancreatic intraductal papillary mucinous neoplasm(IPMN).She was referred to our department for dilation of the common bile duct(CBD)and a tumor in the lumen,detected using *** had no complaints,and blood tests of hepatobiliary enzymes were *** resonance cholangiopancreatography(MRCP)showed a papillary tumor in the CBD with a filling defect detected using endoscopic retrograde cholangiography(ERC).Intraductal ultrasonography revealed a papillary tumor and stalk at the *** POCS,the tumor spontaneously detached with its stalk into the *** showed low-intermediate nuclear atypia of the gastric subtype of *** 1 year,follow-up MRCP showed multiple tumors distributed from the left hepatic duct to the *** and POCS showed multicentric *** was alive without hepatobiliary symptoms at least two years after initial diagnosis of *** The patient experienced gastric subtype of IPNB without curative *** may be reasonable for patients with this subtype.

Validation of user-friendly models predicting extracapsular extension in prostate cancer patients

Objective:There are many models to predict extracapsular extension(ECE)in patients with prostate *** aimed to externally validate several models in a Japanese ***:We included patients treated with robotic-assisted radical prostatectomy for prostate *** risk of ECE was calculated for each patient in several models(prostate side-specific and non-side-specific).Model performance was assessed by calculating the receiver operating curve and the area under the curve(AUC),calibration plots,and decision curve ***:We identified ECE in 117(32.9%)of the 356 prostate lobes *** with ECE had a statistically significant higher prostate-specific antigen level,percentage of positive digital rectal examination,percentage of hypoechoic nodes,percentage of magnetic resonance imaging nodes or ECE suggestion,percentage of biopsy positive cores,International Society of Urological Pathology grade group,and percentage of core *** the sidespecific models,the Soeterik,Patel,Sayyid,Martini,and Steuber models presented AUC of 0.81,0.78,0.77,0.75,and 0.73,*** the non-side-specific models,the memorial Sloan Kettering Cancer Center web calculator,the Roach formula,the Partin tables of 2016,2013,and 2007 presented AUC of 0.74,0.72,0.64,0.61,and 0.60,***,the 95%confidence interval for most of these models *** side-specific models presented adequate *** the decision curve analyses,most models showed net benefit,but it overlapped among ***:Models predicting ECE were externally validated in Japanese *** side-specific models predicted better than the non-side-specific *** Soeterik and Patel models were the most accurate performing models.
