PillCamESO食管胶囊内镜和传统上消化道内镜在诊断慢性胃食管反流病患者准确性的前瞻性研究

Introduction: Endoscopy is commonly performed to evaluate for suspected or established esophageal diseases including gastroesophageal reflux disease (GERD) and its complications. The newly developed PillCam ESO Esophageal Capsule offers an alternative approach to visualize the esophagus and to evaluate patients with suspected esophageal disease. Aim: Compare the accuracy (specifi-city, sensitivity, positive predictive value [PPv], and negative predictive value [NPv]) of esophageal capsule endoscopy (ECE) compared with esophagogastroduodenoscopy (EGD) in evaluating patients with GERD. Methods: A multicenter pivotal trial was conducted at seven sites. The PillCam ESO esophageal capsule is similar to the standard capsule endoscope used for the small bowel but acquires video images from both ends of the device at 2 frames/second/end. A total of 106 patients (93 GERD; 13 Barrett) underwent ECE followed by EGD. ECE videos were evaluated by an investigator blinded to EGD findings. A blinded adjudication committee reviewed all discrepant findings between ECE and EGD. Results: Sixty-six of 106 patients had positive esophageal findings, ECE identified esophageal abnormalities in 61 (sensitivity, 92% ; specificity, 95% ). The per-protocol sensitivity, specificity, PPv, and NPv of ECE for Barrett esophagus were 97% , 99% , 97% , and 99% , respectively, and for esophagitis 89% , 99% , 97% , and 94% , respectively. ECE was preferred over EGD by all patients. There were no adverse events related to ECE. Conclusions: ECE is a convenient and sensitive method for visualization of esophageal mucosal pathology and may provide an effective method to evaluate patients for esophageal disease.

Objectives: To review randomised controlled trials of treat- ment with a proton pump inhibitor in patients with ulcer bleeding and to deter mine the impact on mortality,rebleedi- ng, and surgical intervention. Design: Systematic review and meta-analysis. D ata sources: Cochrane Collaboration’s trials register, Medline, and Embase,hand searched abstracts, and pharmaceutical *** methods: Included random ised controlled trials that compared proton pump inhibitor with placebo or H2 re ceptor antagonist in endoscopically proved bleeding ulcer and reported at least one of mortality, rebleeding, or surgical intervention. Trials were graded for m ethodological quality. Two assessors independently reviewed each trial, and disa greements were resolved by ***:We included 21 randomised controll ed trials comprising 2915 patients. Proton pump inhibitor treatment had no signi ficant effect on mortality (odds ratio 1.11, 95%confidence interval 0.79 to 1.5 7; number needed to treat(NNT) incalculable) but reduced rebleeding (0.46, 0.33 to 0.64; NNT 12) and surgery (0.59,0.46 to 0.76; NNT 20). Results were similar w hen the meta-analysis was restricted to the 10 trials with the highest methodol ogical quality: 0.96, 0.46 to 2.01, for mortality; 0.41, 0.25 to 0.68, NNT 10, f or rebleeding; 0.62, 0.46 to 0.83, NNT 25,for surgery. Conclusions: Treatment wi th a proton pump inhibitor reduces the risk of rebleeding and the requirement fo r surgery after ulcer bleeding but has no benefit on overall mortality.

Objective: To evaluate the role of the enzyme-linked immunosorbent assay (ELISA) test for the detection of antibodies to desmoglein 1 (dsg1) and desmoglein 3 (dsg3) in the diagnosis of pemphigus vulgaris (Pv), and its correlation with disease severity and clinical presentation (mucosal Pv, cutaneous Pv, mucocutaneous Pv). Methods: Twenty-seven active Pv patients and 26 controls with other dermatologic disorders were included in the study. The severity of oral and cutaneous involvement was assessed and recorded. ELISA test for the measurement of anti-dsg1 and anti-dsg3 antibodies was performed (Medical and Biological Laboratories Co. Ltd., Nagoya, Japan). The cut-off ELISA value for both anti-dsg1 and anti-dsg3 was taken as 20. Results: Of the 27 patients, 26 were ELISA positive for anti-dsg1 antibodies and 23 for anti-dsg3 antibodies. Of the controls, two were positive for anti-dsg1 and none for anti-dsg3 antibodies. The sensitivity and specificity of ELISA for anti-dsg1 in the diagnosis of Pv were 96.3% and 92.3% , respectively. For anti-dsg3, they were 85.2% and 100% , respectively. The different morphologic types of Pv could not be differentiated on the basis of antibody profile; however, a direct correlation between anti-dsg3 titers and the severity of oral disease was noted, and also between antidsg1 titers and the severity of cutaneous disease. Conclusions: ELISA (dsg1 and dsg3) is an efficient tool for confirming the diagnosis of Pv. Specific antibody titers correlate with disease severity; however, desmoglein testing cannot differentiate between the various morphologic subtypes of Pv.

Influence of Accompanying Anions on Potassium Retention and Leaching in Potato Growing Alluvial Soils

A column study was carried out to assess the influence of accompanying anions on potassium （k） leaching at potato growing sites with different soil textures （sandy loam and clay loam） in northwestern India. Potassium was applied in the top 15 cm layer of soil column at 30 and 60 mg k kg-1 through different sources having different accompanying anions （CI-, SO^-, NO3 and H2PO4-）. Maximum k was retained in the top 0-15 cm layer with a sharp decrease in k content occurring in 15 30 cm layer of the soil column. The trend was similar for both levels of applied k as well as frequency of leaching and soil type. The decrease of k content in soil column after four leaching events was maximum in case of khanaura sandy loam, while only minor decrease was observed in Hundowal clay loam when k was applied at 60 mg k kg-1, indicating higher potential of clay rich soil to adsorb k. In general, the k leaching in presence of the accompanying anions followed the order of SO4^2-≤ H2PO42- 〈 NO3 = Cl-. Highest 1 mol·L^-1 CH3COONH4-extractable k was retained when k was applied along with SO4^2- and H2PO4 anions, and the least was retained when accompanying anion was Cl-. The influence of anions was more pronounced in the light textured soil and at high amounts of k application. Higher levels of k application resulted in higher losses of k, especially in sandy loam soil as observed from the leachate concentration. Among the different k sources, the maximum amount of k leaching was noticed in the soil column amended with kC1. After four leachings, the maximum amount of k leached out was 6.40 mg L-1 in Hundowal clay loam and 9.29 mg L-1 in khanaura sandy loam at 60 mg k kg-1 of soil application through kC1. These concentrations were lower than the recommended guideline of the World Health Organisation （12.00 mg L-1）.

This paper investigates the effects of pH on stability and thermal properties of copper oxide(CuO),graphene oxide(GO),and their hybrid nanofluid(HNF)at different mixing ***,sol-gel and Hummer’s method was employed for the synthesis of CuO and GO nanoparticles(NPs),and they are characterized with various *** effects of two different surfactants were analyzed on nanofluid’s(NF’s)stability at different pH *** properties like thermal conductivity(TC)and viscosity(vST)of NFs were measured at different volume concentration(0.1 vol%to 1.0 vol%)and temperature range of 30-60℃,*** TC and vST of GO/CuO(50:50)HNF are higher than that of GO/CuO(20:80).The figure of merit(FOM)is determined for the studied *** correlations were presented to calculate the TC as well as vST of *** modern novel machine learning-based ensemble approaches were employed for predictive model development for TC and vST of considered *** comparison of prognostic models with Taylor’s diagram revealed that Bayesian optimized support vector machine(BoASvM)was superior to Bayesian optimized boosted regression trees(BoA-BRT)for both TC and vST models.

Numerical Investigation of Convective Heat Transfer and Friction in Solar Air Heater with Thin Ribs

The present work is focusing on the synthesization and physico-chemical properties of Jatropha curcas biodiesel with diesel and *** densities of binary diesel(2)+1-alkanols(C_(3) or C_(4))(3)and ternary Jatropha curcas biodiesel(1)+diesel(2)+1-alkanols(C_(3)or C_(4))(3)blends have been reported over full range of composition at temperatures within range 288.15 to 313.15 *** densities of Jatropha curcas biodiesel(1)+diesel or 1-alkanols(C_(3) or C_(4))(2)blends have been measured at 313.15 *** molar volumes,v^E,v^E_(123)of binary and ternary blends were calculated from the measured data and the derived properties were correlated to composition using Redlich-kister equation.A reasonable agreement was found between the measured and estimated ***,densities and excess molar volumes data were reasoned to discuss molecular interactions taking into consideration effect of composition and temperature.

vibration analysis of axisymmetric functionally graded viscothermoelastic spheres

The present study focuses on the analysis of free vibrations of axisymmetric functionally graded hollow spheres. The material is assumed to be graded in radial di- rection with a simple power law. Matrix Frrbenious method of extended power series is employed to derive the analytical solutions for displacement, temperature, and stresses. The dispersion relations for the existence of various types of pos- sible modes of vibrations in the considered hollow sphere are derived in a compact form. In order to explore the character- istics of vibrations, the secular equations are further solved by using fixed point iteration numerical technique with the help of MATLAB software. The numerical results have been presented graphically for polymethyl methecrylate materials in respect of natural frequencies, frequency shift, inverse quality factor, displacement, temperature change, and radial stress.
