AIM： To investigate the effect of Garden/a fasm/noides （GJ） on cerulein-induced acute pancreatitis （AP） in mice. METHODS： C57BL/6 mice weighing 18-20 g were divided into three groups. （1） Normal salinetreated group, （2） treatment with GJ at a dose of 0.1 g/kg, （3） treatment with GJ at a dose of 1 g/kg. GJ was administered orally （n = 6 per group） for 1 wk. Three hours later, the mice were given anintraperitoneal injection of cerulein （50 μg/kg）, a stable cholecystokinin （CCK） analogue, every hour for a total of 6h as described previously. The mice were sacrificed at 6 h after completion of cerulein injections. Blood samples were obtained to determine serum amylase, lipase and cytokine levels. The pancreas was rapidly removed for morphologic examination and scoring. A portion of pancreas was stored at -70℃ and prepared for the measurement of tissue myeloperoxidase （MPO） activity, an indicator of neutrophil sequestration, and for reverse-transcriptase PCR （RT-PCR） and real-time PCR measurements. RESULTS： Treatment with G.1 decreased significantly the severity of pancreatitis and pancreatitis-associated lung injury. Treatment with G.1 attenuated the severity of AP compared with saline-treated mice, as shown by reduction in pancreatic edema, neutrophil infiltration, serum amylase and lipase levels, serum cytokine levels, and mRNA expression of multiple inflammatory mediators. CONCLUSION： These results suggest that GJ attenuated the severity of AP as well as pancreatitisassociated lung injury.

AIM： To determine if the fraction of Nardostachysjata- mansi （N J） has the potential to ameliorate the severity of acute pancreatitis （AP）. METHODS： Mice were administered the biologically active fraction of N J, i.e., the 4th fraction （N J4）, intra- peritoneally, and then injected with the stable chole- cystokinin analogue cerulein hourly for 6 h. Six hours after the last cerulein injection, the pancreas, lung, and blood were harvested for morphological examination,measurement of cytokine expression, and examination of neutrophil infiltration. RESULTS： N J4 administration attenuated the sever- ity of AP and lung injury associated with AP. It also reduced cytokine production and neutrophil infiltration and resulted in the in vivo up-regulation of heine oxy- genase-1 （HO-1）. Furthermore, NJ4 and its biologically active fraction, N J4-2 inhibited the cerulein-induced death of acinar cells by inducing HO-1 in isolated pan- creatic acinar cells. CONCLUSION： These results suggest that N J4 may be a candidate fraction offering protection in AP and N J4 might ameliorate the severity of pancreatitis by induc- ing HO-1 expression.

BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a complex immune-mediated condition that causes fibrotic inflammation in several organs.A significant clinical feature of IgG4-RD is hypertrophic pachymeningitis,which manifests as inflammation of the dura mater in intracranial or spinal *** IgG4-RD can affect multiple areas,the spine is a relatively rare site compared to the more frequent involvement of intracranial *** SUMMARY A 70-year-old male presented to our hospital with a two-day history of fever,altered mental status,and generalized *** initial brain magnetic resonance imaging(MRI)revealed multiple small infarcts across various cerebral *** the second day after admission,a physical examination revealed motor weakness in both lower extremities and diminished sensation in the right lower *** evaluation revealed findings consistent with acute motor sensory *** initial management with intravenous immunoglobulin for presumed Guillain-Barrésyndrome,the patient exhibited progressive worsening of motor *** the 45th day of hospitalization,an enhanced MRI of the entire spine,focusing specifically on the thoracic 9 to lumbar 1 vertebral level,raised the suspicion of IgG4-related spinal ***,the patient was administered oral prednisolone and participated in a comprehensive rehabilitation program that included gait training and lower extremity strengthening *** IgG4-related spinal pachymeningitis,diagnosed on MRI,was treated with corticosteroids and a structured rehabilitation regimen,leading to significant improvement.

Intrauterine midgut volvulus without malrotation:Diagnosis from the 'coffee bean sign'

Fetal midgut volvulus is quite rare, and most cases are associated with abnormalities of intestinal rotation or fixation. We report a case of midgut volvulus without malrotation, associated with a meconium pellet, during the gestation period. This 2.79 kg, 33-wk infant was born via a spontaneous vaginal delivery caused by preterm labor. Prenatal ultrasound showed dilated bowel loops with the appearance of a 'coffee bean sign'. This patient had an unusual presentation with a distended abdomen showing skin discoloration. An emergency laparotomy revealed a midgut volvulus and a twisted small bowel, caused by complicated meconium ileus. Such nonspecific prenatal radiological signs and a low index of suspicion of a volvulus during gestation might delay appropriate surgical management and result in ischemic necrosis of the bowel. Preterm labor, specific prenatal sonographic findings (for example, the coffee bean sign) and bluish discoloration of the abdominal wall could suggest intrauterine midgut volvulus requiring prompt surgical intervention.

Impact of homeobox genes in gastrointestinal cancer

Homeobox genes, including HOX and non-HOX genes, have been identified to be expressed aberrantly in solid tumors. In gastrointestinal(gi) cancers, most studies have focused on the function of non-HOX genes including caudal-related homeobox transcription factor 1(CDX1) and CDX2. CDX2 is a crucial factor in the development of pre-cancerous lesions such as Barrett's esophagus or intestinal metaplasia in the stomach, and its tumor suppressive role has been investigated in colorectal cancers. Recently, several HOX genes were reported to have specific roles in gi cancers; for example, HOXA13 in esophageal squamous cell cancer and HOXB7 in stomach and colorectal cancers. HOXD10 is upregulated in colorectal cancer while it is silenced epigenetically in gastric cancer. Thus, it is essential to examine the differential expression pattern of various homeobox genes in specific tumor types or cell lineages, and understand their underlying mechanisms. In this review, we summarize the available research on homeobox genes and present their potential value for the prediction of prognosis in gi cancers.

AIM: To investigate the performance of Gadofluorine P-enhanced magnetic resonance imaging(MRI) on the diagnosis of diabetes in a streptozotocin(STZ)-induced diabetic rat ***: Fischer 344 rats were treated with *** not treated with STZ served as controls. T1-weighted MRI was performed using a 3T scanner before and after the injection of Gd-DOTA or Gadofluorine P(6 diabetic rats, 5 controls). The normalized signal intensity(SI) and the enhancement ratio(ER) of the pancreas were measured at each time point, and the values were compared between the normal and diabetic rats using the Mann-Whitney test. In addition,the values were correlated with the mean islet *** cut-off values were calculated using a positive test based on receiver operating *** Gd concentration after the injection of each contrast media was measured using laser ablation-inductively coupled plasma-mass spectrometry in a separate set of rats(4 diabetic rats, 4 controls for Gadofluorine P; 2, 2 for Gd-DOTA).RESULTS: The normalized SI and ER of the pancreas using Gd-DOTA were not significantly different between diabetic rats and controls. With Gadofluorine P, the values were significantly higher in the diabetic rats than in the control rats 30 min after injection(P < 0.05). The area under the receiver operating characteristic curve that differentiated diabetic rats from the control group was greater for Gadofluorine P than for Gd-DOTA(0.967 vs 0.667, P = 0.085). An increase in normalized SI 30 min after Gadofluorine P was correlated with a decrease in the mean number of islets(r 2 = 0.510, P = 0.014). Intra-pancreatic Gd was higher in rats with Gadofluorine P injection than Gd-DOTA injection(Gadofluorine P vs Gd-DOTA, 7.37 vs 0.00, P < 0.01). ＜正＞PURPOSE: High-dose preoperative radiotherapy has shown promising results in the treatment of low rectal cancer（LRC）. Laparoscopic procedure provides an enhanced view of the pelvic cavity that is not available by conventional method. We developed a sphincter-saving procedure combining the advantages of preoperative radiotherapy and laparoscopy for preserving anatomic and physiologic anal function. Our purpose is to show detailed information regarding procedure and early results. METHODS: From February 1996 to August 1997, eleven patients with LRC underwent the operation. Mean age was 56.1 years. Tumor located 4～8 cm from anal verge. All patients were treated with high-dose preoperative irradiation （4500～5040 cGy） over a period of 5 weeks. All were given our procedure four and a half to eight weeks after completion of radiation. Left colon and rectum were mobilized by laparoscopic oncologic dissection down to puborectalis muscle, after which specimen retrieval and coloanal anastomosis were performed transanally. We concluded with temporary transverse colostomy for fecal diversion. Careful follow-up of 4 to 21 months has so far been given. RESULTS: Every case was successful without any significant complication, and all revealed advantages of laparoscopy. There was one case of complete remission following radiation, and "five were stage Ⅰ, four stage Ⅱ, and one stage Ⅲ. Margin from the tumor was 1～5 cm distally, 19～32 cm proximally, 0.3～3.5 cm laterally. We had one local recurrence 11 months after operating. CONCLUSION: We have termed our procedure ’Laparoscopic Abdominal Transanal （LATA） Proctosigmoidectomy with coloanal anastomosis （LATA resection）’, and propose it as a functional and curative alternative for LRC while continuing to evaluate its efficacy.
