Safety of digoxin in nonagenarian patients with atrial fibrillation:lessons from the Spanish Multicenter registry

BACKGrOUND The association between digoxin and mortality is an unclear *** older patients with atrial fibrillation(AF),where use of digoxin is frequent,the evidence of its safety is *** aim is to assess the safety of digoxin in nonagenarian patients with *** We evaluated data from 795 nonagenarian patients with non-valvular AF from the Spanish Multicenter *** analyzed the relationship between digoxin and all-cause mortality with the Cox proportional-hazards *** Follow-up was 27.7±18.3 *** age was 92.5±3.8 years,and 71%of nonagenarian patients were *** was not associated with increased risk of mortality[adjusted hazard ratio(aHr)=1.16,95%CI:0.96−1.41,P=0.130].However,we found a significant increase in mortality in the subgroup with estimated glomerular filtration rate(eGFr)r 1.73 m^(2)(aHr=2.01,95%CI:1.13−3.57,P=0.018),but not in the other subgroups of eGFr(30−59 mL/min per 1.73 m^(2) and≥60 mL/min per 1.73 m^(2)).When exploring the risk of mortality according to sex,male subgroup was associated with an in-crease in mortality(aHr=1.48,95%CI:1.02−2.14,P=0.041).This was not observed in females subgroup(aHr=1.03,95%CI:0.81−1.29,P=0.829).Based on the presence or absence of heart failure,we did not find significant differences(aHr=1.20,95%CI:0.87−1.65,P=0.268 ***=1.15,95%CI:0.90−1.47,P=0.273,respectively).CONCLUSIONS In our large registry of nonagenarian patients with AF,we did not find an association between digoxin and mortality in the total ***,in the subgroup analyses,we found an increase in mortality with the use of digoxin in men and in patients with an eGFrr 1.73 m^(2).

Forecasting Short Time Series with Missing Data by Means of Energy Associated to Series

In this work an algorithm to predict short times series with missing data by means energy associated of series using artificial neural networks (ANN) is presented. In order to give the prediction one step ahead, a comparison between this and previous work that involves a similar approach to test short time series with uncertainties on their data, indicates that a linear smoothing is a well approximation in order to employ a method for uncompleted datasets. Moreover, in function of the long- or short-term stochastic dependence of the short time series considered, the training process modifies the number of patterns and iterations in the topology according to a heuristic law, where the Hurst parameter H is related with the short times series, of which they are considered as a path of the fractional Brownian motion. The results are evaluated on high roughness time series from solutions of the Mackey-Glass Equation (MG) and cumulative monthly historical rainfall data from San Agustin, Cordoba. A comparison with ANN nonlinear filters is shown in order to see a better performance of the outcomes when the information is taken from geographical point observation.

Stability Analysis of a Neurocontroller with Kalman Estimator for the Inverted Pendulum Case

In this paper, a practical analysis of stability by simulation for the effect of incorporating a Kalman estimator in the control loop of the inverted pendulum with a neurocontroller is presented. The neurocontroller is calculated by approximate optimal control, without considering the Kalman estimator in the loop following the Theorem of the separation. The results are compared with a time-varying linear controller, which in noiseless conditions in the state or in the measurement has an acceptable performance, but when it is under noise conditions its operation closes into a state space range more limited than the one proposed here.

Strengthening of Alumina-Based Ceramics with Titanium Nanoparticles

In this study ceramic materials with a matrix of Al2O3 strengthened with different amounts of Ti nanoparticles (0.0 wt%, 0.5 wt%, 1.0 wt%, 2.0 wt% and 3.0 wt%) were generated. High energy milling was used to mix the materials in a planetary mill type, in which powder particles were obtained with sizes of ~300 nm. These powders were uniaxially compacted in cylindrical samples using 350 MPa pressure. These samples were sintered at 1500°C for 1, 2 and 3 h and at 1400°C, 1500°C and 1600°C during 2 h. Microstructure observations were made with optical microscopy and scanning electron microscopy. Dense composites were identified with a homogeneous distribution of fine particles. Concerning the measurement results of fracture toughness, which were estimated by the indentation fracture method, it was shown that the composites made by mean procedure present higher values than the average of the monolithic alumina, up to 200%. Photographic evidence of arrest of crack growth by titanium particles was obtained, demonstrating that the reinforcement mechanism of these materials is due to the deflection of cracks owing to metallic bridges formed by the titanium used as alumina strengthener.

Mollusks of the rocky Intertidal Zone at Three Sites in Oaxaca,Mexico

The community of mollusks at three rocky intertidal zone in Oaxaca, Mexico was analyzed. Two samplings per site were conducted. At each sampling the inspected area was 10 m2. The sampling unit was 1 m2. 5862 specimens were examined. 68 mollusks species were identified. The families better represented in species richness were: FISSUrELLIDAE, MUrICIDAE, LOTIIDAE, MYTILIDAE, CHAMIDAE, ISCHNOCHITONIDAE y CHITONIDAE. The species that had the greatest density were Lottia discors, Siphonaria palmata, Lottia acutapex, Chormoytilus palliopunctatus, Brachidontes adamsianus, Chton articulatus e Ischnochiton muscarius. The species with the widest distribution represent 11.76%. With irregular distribution was found in 58.82% of the species. The gastropod Crucibulummonticulus, the bivalve C. palliopunctatus and the polyplacophoran C. articulatus had the greatest size. The value of diversity index was H’ = 4.29 bits/ind. and J’ = 0.72. The mollusks community in the rocky intertidal zone study sites is characterized by small size, because they rarely exceed seventy millimeters. The values of H’ and J’ indicate that the mollusk community in the study area has a high diversity and high uniformity, corresponding to mature and stable communities in a tropical region.

Plicopurpura pansa is a cultural resource with economic importance since ink can be extracted from it to dye clothes. This snail inhabits the rocky intertidal zone of Mexican Pacific, is found attached to rocks in crevices and holes. The objective was to analyze the P. pansa population structure in Costa Chica of the State of Guerrero based in density, sex ratio, size and weight, besides to realize observations on mating and recruitment, for which quarterly sampling was made during an annual cycle in three beaches. The density was 4.83 snails/m2. Sex ratio ranged from 0.76:1.00 to 1.00: 0.96 (F:M). Average size was 20.91 mm. The best represented sizes in females were from 10.5 to 28.5 mm and in males 13.5 to 28.5 mm. Averaged weight was 1.99 g, females showed a coefficient of variation in weight of 96.53%. recruitment occurred for the entire year. In December 2006 and September 2007 a larger frequency of recruits was shown. Sex ratio showed a 1:1 tendency. Snail population was characterized by being small sized and in high densities. Weight variations in females were found related to their reproductive cycle.

Protective and regenerative Effect of the Extract of Kombucha and the Fungus Ganoderma sichuanense on the Islets of Langerhans of Diabetic rats

Objective: The present study consisted of challenging the extract of kombucha and the fungus Ganoderma reported as hypoglycemic and used as alternative treatments against diabetes on the number and morphology of islets of Langerhans. Material and Methods: 64 Wistar rats were used in 4 groups: one control, three experimental, streptozotocin, Kombucha y Ganoderma induced diabetes with streptozotocin. Divided into four post-induction stages at 2, 15, 30 and 45 days of treatment, sacrificing 4 rats at each stage, to perform the morphological analysis of the pancreas. results: A decrease in the islets of Langerhans in size, volume and the number of cells within them was identified for the streptozotocin group from the second stage until almost disappearing due to diabetes, in the groups of Kombucha y Ganoderma the same was observed but they were recovered with the extract treatments and the average number of islets was similar in these groups, the group of Ganoderma. Conclusion: Under the conditions of this work, a protective and regenerative effect of both extracts is identified.

Intraoperative laparoscopic complications for urological cancer procedures

AIM: To structure the rate of intraoperative complications that requires an intraoperative or perioperative resolution. METHODS: We perform a literature review of Medline database. The research was focused on intraoperative laparoscopic procedures inside the field of urological oncology. General rate of perioperative complications in laparoscopic urologic surgery is described to be around 12.4%. Most of the manuscripts published do not make differences between pure intraoperative, intraoperative with postoperative consequences and postoperative complications. rESULTS: We expose a narrative statement of complications, possible solutions and possible preventions for most frequent retroperitoneal and pelvic laparoscopic surgery. We expose the results with the following order: retroperitoneal laparoscopic surgery(radical nephrectomy, partial nephrectomy, nephroureterectomy and adrenalectomy) and pelvic laparoscopic surgery(radical prostatectomy and radical cystectomy).CONCLUSION: Intraoperative complications vary from different series. More scheduled reports should be done in order to better understand the real rates of complications.

Low-Dose Gamma radiation Fields Decrease Cell Viability, Damage DNA, and Increase the Expression of Hsp70 and p53 Proteins in Human Leukocytes

Ionizing radiations are tools in diagnosis and treatment of diseases. Leukopenia from exposure to ionizing radiation has been reported. Due to their radiosensitivity, leukocytes are a biological model to analyze cell damage. Therefore, cell viability, DNA damage, and Hsp70 and p53 expression in human leukocytes exposed to low-dose gamma radiation fields from a 137Cs source were evaluated. A decrease in cell viability, DNA damage and an increase in the expression of Hsp70 and p53 proportional to the radiation dose received was found, which was 0.2, 0.4, 0.6, 0.8 and 1.0 mGy.
