Allogeneic and autologous stem cell transplantation with busulfan, cyclophosphamide, and etoposide conditioning therapy for relapsed/refractory non-Hodgkin lymphoma

The optimal stem cell transplantation (SCT) conditioning therapy for relapsed/refractory non-Hodgkin lymphoma (NHL) is not clearly defined. In a retrospective analysis, we examined 25 patients with “high risk” relapsed/refractory NHL who received busulfan, cyclophosphamide, and etoposide (Bu/Cy/VP16) conditioning with autologous or allogeneic SCT. The majority of patients had aggressive histology and 52% had primary refractory NHL. Furthermore, 48% of patients had chemotherapy-resistant disease at the time of SCT. Fifty-six percent of patients underwent allogeneic SCT, while 44% had autologous SCT. The median engraftment time for neutrophils and platelets was 13.5 and 14 days, respectively. The 100-day treatment-related mortality (TRM) was 16%, while the 2-year non-relapse mortality (NRM) rate was also 16%. At a median follow-up of 15 months, the estimated 2-year disease-free survival (DFS) rate was 64% (95% confidence interval (CI): 36%-82%) and the estimated 2-year overall survival (OS) was 69% (95% CI: 40%-86%). Furthermore, the 2-year disease-specific survival (DSS) rate was 73% (95% CI: 40%-90%). Using Cox proportional hazard modeling, the International Prognostic Index at time of relapse predicted DFS and OS. Altogether, Bu/Cy/VP16 was associated with early TRM;however, late toxicities (including NRM) were uncommon resulting in relatively good survival rates in a high-risk relapsed/refractory NHL population.

Different rare earth substituted perovskites LaRExFe1-xO3(where RE=Eu3+,Gd3+,Dy3+,Nd3+and x=0.02,0.04,0.06,0.08,0.1)with orthorhombic structure and narrow band gaps were successfully fabricated via sol-gel autocombustion *** the substituted perovskites are found to exhibit excellent photocatalytic activity towards the oxidative degradation of dye *** excellent increase in the rate constant values of pure perovskite(LaFeO3)photocatalytic reactions is observed with the substitution of rare earth metal *** results are obtained for LaNd0.1Fe0.9O3 which exhibits around 7 times increase in the rate constant values for degradation reaction of SO(1.76×10-1 min-1)and RBY(1.69×10-1 min-1)dyes.

Awareness,practice and views about integrating AYUSH in allopathic curriculum of allopathic doctors and interns in a tertiary care teaching hospital in New Delhi,India

Objective： To assess the knowledge, attitude, perception and practice of Ayurveda, yoga, Unani, Siddha, homeopathy and naturopathy （AYUSH） among allopathic doctors and interns and determine their opin- ion regarding integration of AYUSH education within the allopathic curriculum. Methods： This prospective, cross sectional study was conducted in a tertiary care teaching Hospital, New Delhi, India, from March 2016 to March 2017. A survey of 500 aUopathic doctors and 150 interns was performed using a structured questionnaire. Results： A majority of doctors （95.2%） were aware of the term AYUSH and had knowledge of yoga （70.6%）, homeopathy （55.6%）, Ayurveda （50.3%）, Siddha （47.8%）, naturopathy （32.9%） and Unani （28.9%）. Most of the participants （63.0%） felt that therapies under AYUSH are effective. A majority （84.0%） of the doctors were of the view that more research is required in the field of AYUSH. While homeopathy （44.0%） was the most common system used by doctors for themselves, yoga was the therapy most commonly （60.0%） recommended to patients. Nearly half of the doctors （46.7%） recommended AYUSH to their patients, mainly for chronic illness （74.0%）. A majority （77.5%） of the respondents agreed that students of allopathic med- icine should be familiarized with AYUSH. The modes of incorporation of AYUSH into the medical curriculum most commonly suggested by respondents were optional courses （54.5%） and introductory lectures （42.3%）. Conclusion： Doctors and interns ofallopathic medicine are aware of AYUSH systems. They use it for them- selves and recommend it to patients. They believe that familiarization of Bachelor of Medicine and Bachelor of Surgery students with AYUSH therapies and their integration with allopathic medicine may help in improving patient care.

In recent years,deep learning gained proliferating popularity in the cybersecurity application domain,since when being compared to traditional machine learning methods,it usually involves less human efforts,produces better results,and provides better ***,the imbalanced data issue is very common in cybersecurity,which can substantially deteriorate the performance of the deep learning *** paper introduces a transfer learning based method to tackle the imbalanced data issue in cybersecurity using return-oriented programming payload detection as a case *** achieved 0.0290 average false positive rate,0.9705 average F1 score and 0.9521 average detection rate on 3 different target domain programs using 2 different source domain programs,with 0 benign training data sample in the target *** performance improvement compared to the baseline is a trade-off between false positive rate and detection *** our approach,the total number of false positives is reduced by 23.16%,and as a trade-off,the number of detected malicious samples decreases by 0.68%.

Evaluation of anti-nociceptive activity of methanolic extract of Tecomaria capensis Thunb.(Bignoniaceae) leaves in rats

Objective:To evaluate the ann-nociceptive activity of Tecomaria capensls(***) leaves extract(TCLE) using different models in rats by acetic acid induced writhing test,(b) tail-flick test(c) tail-clip ***:TCLE(100,300,1000 and 2000 mg/kg body wt.) was given to rats orally to observe acute toxicity for 14 *** test drug TCLE were given at dose of 100,200 and 500 mg/kg *** standard drug aspirin were given at a dose of 100 mg/kg ***:No mortality was reported even after 14 *** indicates that the methanol extract is safe up to a single dose of 2 000 mg/kg body weight TCLE(100,200 and 500 mg/kg p.o.) significandy inhibited abdominal constrictions(writhing) induced by acetic acid and increased the latency period in the tail flick and tail clip *** at the dose of 500 mg/kg showed significant anti-nociceptive activity compared to standard ***:The results of this study show that methanol extract of *** possesses anti-nociceptive activity which may be mediated by the central and peripheral mechanisms.

Seismic response of precast reinforced concrete wall subjected to cyclic in-plane and constant out-of-plane loading

This paper provides insight into the seismic behavior of a full-scale precast reinforced concrete wall under in-plane cyclic loading combined with out-of-plane loading replicated by sand backfill to simulate the actual condition of basement *** tested wall exhibited flexural cracks,owing to the high aspect ratio and considerable out-of-plane movement due to lateral pressure from the *** wall performed satisfactorily by exhibiting competent seismic parameters and deformation characteristics governed by its ductile response in the nonlinear phase during the test with smaller residual *** analysis was conducted to validate experimental findings,which complied with each *** numerical model was used to conduct parametric studies to study the effect of backfill density and aspect ratio on seismic response of the proposed precast wall *** in-plane capacity of walls reduced,while deformation characteristics were unaffected by the increase in backfill *** increase in aspect ratio leads to a reduction in in-plane capacity and an increase in *** between the ratio of in-plane yield capacity and design shear load of walls are proposed for the backfill density,which may be adopted to determine the in-plane yield capacity of the basement walls based on their design shear.

Syncytes during male meiosis resulting in 2n pollen grain formationin Lindelofia longiflora ***

Lindelofia longiflora(Royle ex Benth.) ***(Cl.) Brand(Family:Boraginaceae) is investigated cytologically(n = 12) for the first time from the cold deserts of Pangi Valley,Chamba District(Himachal Pradesh) in *** report the formation of syncytes and 2n pollen grains in the *** meiosis,the majority of the pollen mother cells(PMCs) exhibited 12 bivalents,equal segregation of chromosomes during anaphases,regular tetrads,and normal-sized pollen grain ***,two proximate PMCs fused during the early stages of prophase-I and resulted in the formation of *** frequency of syncytes in the accession is rather low,at 25 out of 1866(1.33%).Such syncyte PMCs are detectable during meiosis due to their larger size compared to typical *** syncytes or polyploid cells showed normal 24 bivalents and depicted perfectly regular meiotic *** the products of such PMCs yield 2n or larger sized pollen grains that are almost double the size of typical normal or n pollen *** origin of syncytes as a consequence of the fusion of meiocytes during the early stages of meiosis-I could be attributed to low temperature stress conditions prevailing in the Pangi Valley,where temperature during May and June dip to below freezing,the time the plants enters the reproductive/flowering bud *** is possible that such apparently fertile 2n pollen grains originating from syncytes might play a role in the origin of intraspecific polyploids in the species.

(Bignoniaceae) methanolic leaves extract(TCLE) using blood glucose level in normal fasted rats, glucose tolerance test and alloxan induced hyperglycemia ***:TCLE(100,300, 1 000 and 2 000 mg/kg body wt.) was given to rats orally to observe acute toxicity,and observed for 14 *** 200 and 400 mg/kg,and glibenclamide 0.6 mg/kg were given orally in all models. Results:Results demonstrated that the no mortality was reported even after 14 *** indicates that the methanol extract is safe up to a single dose of 2 000 mg/kg body ***(200 and 400 mg/kg p.o.) exhibited remarkable blood glucose lowering effect in blood glucose level in normal fasted rats,glucose tolerance and alloxan induced hyperglycemia *** and triglyceride also decreased in alloxan induced hyperglycemia ***:The results of this study exhibites that methanol extract of *** possesses antihypergycemic activity and it may prove to be effective for the treatment of hyperglycemia.

Laparoscopic liver resection for benign and malignant liver tumors

BACKGROUND:Laparoscopic liver resection is one of the most complex procedures in hepatobiliary *** the last two decades,laparoscopic liver surgery has emerged as an option at major academic *** purpose of this study is to describe the initial experience of minimally invasive liver resections at a non-academic institution. METHODS:We retrospectively reviewed medical records of patients undergoing laparoscopic liver resections between June 2006 and December 2009 at our ***, technical aspects,and outcomes of these patients are described. RESULTS:Laparoscopic liver resection was attempted in 28 *** these,27 patients underwent laparoscopic liver resection(22 total laparoscopic and 5 hand assisted)and one needed conversion to open *** patients had a benign lesion and 8 had malignant *** patients had multiple lesions in different segments requiring separate *** lesions were located in segments Ⅱ-Ⅲ(n=18), Ⅳ(n=3),Ⅴ-Ⅵ(n=9),and VII(n=1).Tumor size ranged from 1.5 cm to 8.5 *** surgical procedures included left lateral sectionectomy(n=17),left hepatectomy(n=2),sectionectomy (n=8),and local resections(n=4).Median operative time was 110 minutes(range 55-210 minutes),and the median length of hospital stay was 2.5 days(range 1-7 days).There was no perioperative *** patient developed hernia at the site of tumor extraction requiring repair at 3 months. CONCLUSIONS:Laparoscopic liver resections can be safely performed in selected patients with benign and malignant liver *** increasing experience,laparoscopic liver resections are likely to become a favorable alternative to open resection.
