Extremity reconstruction surgery is increasingly performed rather than amputation for patients with large-segment pathologic bone *** persists as to the optimal void filler for this“limb salvage”surgery,whether metal or allograft *** focus on optimizing important functional gains for patients,and the risk of devastating implant infection has been thought to be similar regardless of implant *** insights into infection pathophysiology are challenging this equipoise,however,with both basic science data suggesting a novel mechanism of infection of Staphylococcus aureus(the most common infecting agent)into the host lacunar–canaliculi network,and also clinical data revealing a higher rate of infection of allograft over *** current translational study was therefore developed to bridge the gap between these insights in a longitudinal murine model of infection of allograft bone and ***-time Staphylococci infection characteristics were quantified in cortical bone vs metal,and both microarchitecture of host implant and presence of host immune response were *** orders-of-magnitude higher bacterial burden was established in cortical allograft bone over both metal and cancellous *** establishment of immune-evading microabscesses was confirmed in both cortical allograft haversian canal and the submicron canaliculi network in an additional model of mouse femur bone *** study results reveal a mechanism by which Staphylococci evasion of host immunity is possible,contributing to elevated risks of infection in cortical *** presence of this local infection reservoir imparts massive clinical implications that may alter the current paradigm of osteomyelitis and bulk allograft infection treatment.

Background Although recent data in healthy humans suggestthat treatment with intranasal oxytocin(0T)may facilitate extinction recall,to date,little is known about the effects of 0T on memory *** To examine the effect of intranasal administration of OT compared with placebo on memory consolidation blockade of a de novo fear memory in a classical 2-day fear conditioning *** There were no significant differences between the 0T and the placebo groups on the first two extinction trials(mean(SD)=0.01(0.39)vs 0.15(0.31),t=-1.092,p=0.28).Similarly,during early extinction,analysis of variance for repeated measures failed to show significant main effects of extinction trials:trials(F(4,112)=1.58,p=0.18),drug(F(1,112)=0.13,p=0.72)or drug x trials interaction(F(4,112)=0.76,p=0.56).Conclusion Our results suggest that 0T administered in a double-blind fashion immediately after fear conditioning does not significantly reduce consolidation of fear learning as measured by a differential skin conductance response tested at the beginning of extinction.

Despite the many submarine telecommunications and power cables laid world-wide there are fewer than ten published studies of their environmental effects in the refereed *** paper describes an investigation into the effects of laying and operating the Basslink High Voltage Direct Current(HVDC)cable and its associated metallic return cable across Bass Strait in South East *** more than 95%of its length the cable was directly laid into a wet jetted trench given the predominantly soft sediments *** remote video investigations found that within two years all visible evidence of the cable and trench was gone at over a third of the transects at six deep water sites(32-72 m deep).At other deep water transects the residual trench trapped drift material providing habitat for the generally sparsely distributed benthic *** surveys at both of the near shore sites(ide of the Strait also found the cable route was undetectable after a *** the southern side,where the cable traversed hard basalt rock near shore,it was encased in a protective cast iron half *** studies by divers over 3.5 years demonstrated the colonization of the hard shell by similar species occupying hard substrates elsewhere on the basalt *** field strengths associated with the operating cable were found to be within 0.8%of those predicted from theory with strength dropping rapidly with distance from the *** 20 m the field was indistinguishable from background.

California Simulation of Evapotranspiration of Applied Water and Agricultural Energy Use in California

The California Simulation of Evapotranspiration of Applied Water （Cai-SiMETAW） model is a new tool developed by the California Department of Water Resources and the University of California, Davis to perform daily soil water balance and determine crop evapotranspiration （ETo）, evapotranspiration of applied water （ETaw）, and applied water （AW） for use in California water resources planning. ETaw is a seasonal estimate of the water needed to irrigate a crop assuming 100% irrigation efficiency. The model accounts for soils, crop coefficients, rooting depths, seepage, etc. that influence crop water balance. it provides spatial soil and climate information and it uses historical crop and land-use category information to provide seasonal water balance estimates by combinations of detailed analysis unit and county （DAU/County） over Califomia. The result is a large data base of ETc and ETaw that will be used to update information in the new California Water Plan （CWP）. The application uses the daily climate data, i.e., maximum （Tx） and minimum （Tn） temperature and precipitation （Pcp）, which were derived from monthly USDA-NRCS PRiSM data （PRiSM Group 2011） and daily US National Climate Data Center （NCDC） climate station data to cover California on a 4 kmx4 km change grid spacing. The application uses daily weather data to determine reference evapotranspiration （ETo）, using the Hargreaves-Samani （HS） equation （Hargreaves and Samani 1982, 1985）. Because the HS equation is based on temperature only, ETo from the HS equation were compared with CiMiS ETo at the same locations using available CiMiS data to determine correction factors to estimate CiMiS ETo from the HS ETo to account for spatial climate differences. Cai-SiMETAW also employs near real-time reference evapotranspiration （ETo） information from Spatial CiMiS, which is a model that combines weather station data and remote sensing to provide a grid of ETo information. The service life of refractories in ladle walls is limited by corrosion,abrasion,thermal shock and structural spalling mechanisms. When the ladle is lined with bricks they typically need to be completely removed after a certain number of heats to be replaced by new bricks of the same size as the original bricks. Not so for monolithic ladle linings. At the end of their service life the remains of the castable can almost completely be recycled as they can in fact stay in place. Only the worn-out part of the lining has to be replaced by a new castable. 20%-50% of the initial ladle lining can be recycled ＂insitu＂. The installation can efficiently be done by shotcreting technics. But it requires a castable that resists slag penetration very well. Castables based on a calcium magnesium aluminate bond provide the required penetration resistance. Pumping and shotcreting is very well adapted for repairs of blast furnace shafts,torpedo cars,hot metal and steel ladle linings[1]. However,very little is published about how a good pumping and shotcreting performance can be achieved when the installation has to be done under extreme weather conditions. At high ambient temperature the challenge is to ensure a good pumping result without early castable stiffening. Blockage of the pump would be the consequence. At low ambient temperature the difficulty is to de-activate the highly efficient deflocculant fast enough with a gelling agent added intothe wet mix at the nozzle to prevent the gunned material slipping off the wall. This paper studies strategies how to achieve good installed properties even at extreme ambient temperatures. The interactions between deflocculants,retarders,gelling agents,and calcium magnesium aluminate binder as a function of temperature are studied for an alumina- spinel ladle castable. Tendo Achilles ruptures are generally traumatic in origin and while bilateral tendo Achilles ruptures are a rare occurrence, most of them are associated with risk factors or pre-existing disease and generally involve trauma or sporting activities. Most of the cases of bilateral rupture are generally treated operatively. A spontaneous onset case of bilateral tendo achilles rupture is reported in a healthy man and its conservative (non operative) management discussed with a review of the literature.

Use of scoring systems for assessing and reporting the outcome results from shoulder surgery and arthroplasty

To investigate shoulder scoring systems used in Europe and North America and how outcomes might be classified after shoulder joint replacement. All research papers published in four major journals in 2012 and 2013 were reviewed for the shoulder scoring systems used in their published papers. A method of identifying how outcomes after shoulder arthroplasty might be used to categorize patients into fair, good, very good and excellent outcomes was explored using the outcome evaluations from patients treated in our own unit. A total of 174 research articles that were published in the four journals used some form of shoulder scoring system. The outcome from shoulder arthroplasty in our unit has been evaluated using the constant score(CS) and the oxford shoulder score and these scores have been used to evaluate individual patient outcomes. CSs of isfactory; 30-39 = fair; 40-59 = good; 60-69 = very good; and 70 and over = excellent. The most popular shoulder scoring systems in North America were Simple Shoulder Test and American shoulder and elbow surgeons standard shoulder assessment form score and in Europe CS, Oxford Shoulder Score and DASH score.

Dear Editor,Takotsubo syndrome(TS)is a stress-induced non-ischaemic cardiomyopathy that is more common in women,but is associated with higher morbidity and mortality in *** known as broken-heart syndrome,TS is characterised by transient left ventricular(LV)dysfunction independent of obstructive coronary artery *** is a polygenic condition and nowhere is this more evident than the use of positive inotropes,such as isoprenaline(iSO)in pre-clinical models.1 There is no standard therapy for broken-heart syndrome because the mechanisms underlying the condition remain ***,there is no consensus on predisposition for Takotsubo2 and our goal was to better understand the regulatory mechanism as a first step towards improved treatment *** acid or SAHA,a drug approved for cancer treatment by the US Food and Drug Administration has previously been shown to improve cardiopulmonary function.3 We tested the hypothesis that the cardioprotective benefit of SAHA in a pre-clinical model of Takotsubo is conferred by an epigenetic acetylation/deacetylation(Ac/Dc)axis.
