Does playing a sports active video game improve object control skills of children with autism spectrum disorder?

Background: Active video games(AVGs) encourage whole body movements to interact or control the gaming system, allowing the opportunity for skill development. Children with autism spectrum disorder(ASD) show decreased fundamental movement skills in comparison with their typically developing(TD) peers and might benefit from this approach. This pilot study investigates whether playing sports AVGs can increase the actual and perceived object control(OC) skills of 11 children with ASD aged 6–10 years in comparison to 19 TD children of a similar *** was a secondary ***: Actual(Test of Gross Motor Development) and perceived OC skills(Pictorial Scale of Perceived Movement Skill Competence for Young Children) were assessed before and after the intervention(6 × 45 min).Results: Actual skill scores were not improved in either group. The ASD group improved in perceived skill. All children completed the required dose and parents reported the intervention was ***: The use of AVGs as a play-based intervention may not provide enough opportunity for children to perform the correct movement patterns to influence skill. However, play of such games may influence perceptions of skill ability in children with ASD, which could improve motivation to participate in physical activities.

Liver biopsy in a district general hospital:Changes over two decades

AIM： To study liver biopsy practice over two decades in a district general hospital in the United Kingdom.METHODS： We identified all patients who had at least one liver biopsy between 1986 and 2006 from the databases of the radiology and gastroenterology departments. Subjects with incomplete clinical data were excluded from the study.RESULTS： A total of 103 liver biopsies were performed. Clinical data was available for 88 patients, with 95 biopsies. Between 1986 and 1996, 18 （95%） out of the 19 liver biopsies performed were blind and 6 （33%） were for primary biliary cirrhosis. Between 1996 and 2006, 14 （18%） out of 76 biopsies were blind; and the indications were abnormal liver tests （33%）, hepatitis C （12%） and targeted-biopsies （11%）. Liver biopsies were unhelpful in 5 （50） subjects. Pain was the most common complication of liver biopsy （5%）. No biopsy-related mortality was reported. There was a trend towards more technical failures and complications with the blind biopsy technique.CONCLUSION： Liver biopsies performed in small district hospitals are safe and useful for diagnostic and staging purposes. Abnormal liver tests, non-alcoholic fatty liver disease and targeted biopsies are increasingly common indications. Ultrasound-guided liver biopsies are now the preferred method and are associated with fewer complications.

Randomized controlled trials in frontotemporal dementia: cognitive and behavioral outcomes

Simultaneous expander and deep inferior epigastric perforator reconstruction:indications and alloderm sling technique for protecting the anastomosis

Diagnosis and Treatment of Tolosa-Hunt Syndrome in the Emergency Department

We describe herein a patient who presented with painful ophthalmoplegia and was ultimately diagnosed via magnetic imaging resonance studies and successfully treated for Tolosa-Hunt syndrome. Tolosa-Hunt syndrome is a rare, reversible and painful ophthalmoplegia characterized by recurrent unilateral orbital pain, ipsilateral oculomotor paralysis and prompt response to steroids. Specific criteria for its diagnosis exist and are discussed herein. Individuals affected may display signs of select cranial nerve palsies, ptosis, facial numbness, diplopia, midrosis, and proptosis. Appropriate recognition of the disease can allow for immediate intervention and thus decrease the length and severity of symptoms especially as symptoms may not spontaneously resolve without treatment which leads to unnecessary suffering through pain, anxiety, and decreased vision. We describe the case presentation and keys for diagnosis emergency medicine that physicians should know for this potentially devastating condition.

Ca signaling is reported to play a key role in the regulatory networks that control differentiation in many cell types, including vertebrate skeletal muscle and neurons. We are using transgenic lines of zebrafish that express: 1) the bioluminescent Ca reporter, apoaequorin, specifically in the skeletal muscle;and 2) the fluorescent Ca reporter, GCa MP7, specifically in the primary motor neurons. Using the aequorin line of fish, we recently showed that two pore channel 2(TPC2),(which is a novel member of the voltage-gated channel superfamily that is located in the membrane of lysosomes and proposed to be activated by nicotinic acid adenine dinucleotide phosphate), plays a crucial role in generating Ca signals during slow muscle cell(SMC) myogenesis in zebrafish. Morpholino(MO)-based knock-down of TPC2 resulted in a dramatic attenuation of the Ca signals, whereas co-injection of TPCN2 m RNA or treatment with IP/BM partially rescued the Ca signaling signature in the SMCs. Dual-color STED super-resolution microscopy demonstrated the presence of Ry R-TPC2 "nano"-domains in the sarcomeric Z-lines of mature myotubes. Injection of the TPCN2-MO into embryos of the GCa MP7 line of fish, resulted in an inhibition of the alternate firing of the contralateral caudal primary motor neurons(Ca Ps). This suggests a possible regulatory role for TPC2-mediated Ca signaling during Ca P activation. To validate the MO-generated data, we are in the process of generating a TPC2 zebrafish mutant line using CRISPR/Cas9 gene editing. Our preliminary results indicate that there are similarities between the MO-generated TPC2 knock-down and CRISPR/Cas9-generated TPC2 knock-out embryos obtained. Together, our data provide evidence to suggest that spontaneous TPC2-mediated Ca release might be essential for regulating the development and function of SMCs in zebrafish via modulating the upstream neurogenic signals.

Introduction The International Family Medicine Clinic(IFMC)was established in 2002 by the University of Virginia Department of Family Medicine to provide comprehensive,timely,culturally sensitive and high-quality healthcare to the growing refugee and special immigrant population in Central Virginia,*** The purpose of this paper is to describe the IFMC,with a specific focus on interprofessional roles,interprofessional collaboration,community partnerships and the services and resources available to IFMC *** The clinic has served over 3100 refugees from 60 countries in its 16-year *** 2019,the clinic staff now includes 4 attending physicians,2 nurse practitioners and 14 residents who have dedicated clinic time to see refugees;a registered nurse care coordinator and a social worker dedicated to the IFMC refugee population;2 clinical psychologists and doctoral students in clinical psychology;and a clinical *** IFMC also provides onsite psychiatric care.A process flow map depicts the interconnectivity of interprofessional team members working together with other specialty care providers within the medical centre and with community partners on behalf of refugee patients through the resettlement *** Individuals who arrive in the USA as refugees are a particularly vulnerable patient group and often require an interprofessional team *** IFMC may serve as a model for other institutions interested in starting a similar interprofessional,refugee-centred medical home.
