Prevalence of respiratory syncytial virus infection among children hospitalized with acute lower respiratory tract infections in Southern India

bACKGROUND Respiratory syncytial virus(RSV) is a leading cause of lower respiratory infections among *** To investigate the proportion of RSV and non-RSV respiratory viral infections among hospitalized children ≤ 5 *** Hospitalized children aged between August 2011-August 2013, were *** were defined as laboratory-confirmed RSV and non-RSV respiratory viruses by direct fluorescence assay from the nasopharyngeal *** Of 383 1-59 mo old children hospitalized with an acute lower respiratory infection, 33.9%(130/383) had evidence of viral infection, and RSV was detected in 24.5%(94/383). Co-infections with RSV and other respiratory viruses(influenza A or b, adenovirus, para influenza 1, 2 or 3) were seen in children 5.5%(21/383). Over 90% of the RSV-positive children were under 2 years of age. RSV was detected throughout the year with peaks seen after the monsoon *** hospitalized with RSV infection were more likely to have been exposed to a shorter duration of breastfeeding of less than 3 mo. RSV positive children had a shorter hospital stay, although there were significant complications requiring intensive care. Use of antibiotics was high among those with RSV and non-RSV viral *** Our study provides evidence of a high proportion of RSV and other virusassociated ALRI among hospitalized children in India. RSV infection was associated with fewer days of hospital stay compared to other causes of lower respiratory infections. A high level of antibiotic use was seen among all respiratory virus-associated hospitalizations. These results suggest the need for implementing routine diagnostics for respiratory pathogens in order to minimize the use of unnecessary antibiotics and plan prevention strategies among pediatric populations.

Mortality and rebleeding following variceal haemorrhage in liver cirrhosis and periportal fibrosis

AIMTo investigate mortality and rebleeding rate and identify associated risk factors at 6 wk and 5 d following acute variceal haemorrhage in patients with liver cirrhosis and schistosomal periportal fibrosis. METHODSThis is a prospective study conducted during the period from March to December 2014. Patients with portal hypertension presenting with acute variceal haemorrhage secondary to either liver cirrhosis (group A) or schistosomal periportal fibroses (group b) presenting within 24 h of the onset of the bleeding were enrolled in the study and followed for a period of 6 wk. Analysis of data was done by Microsoft Excel and comparison between groups was done by Statistical Package of Social Sciences version 20 to calculate means and find the levels of statistical differences and define the mortality rates, the P value of RESULTSA total of 94 patients were enrolled in the study. Thirty-two patients (34%) had liver cirrhosis (group A) and 62 (66%) patients had periportal fibrosis (group b). Mortality: The 6-wk and 5-d mortality were 53% and 16% respectively in group A compared to 10% and 0% in group b (P value P value P value P value P value P value P value CONCLUSIONThe 6-wk and 5-d mortality and rebleeding rate were significantly higher in patients with liver cirrhosis compared to patients with schistosomal periportal fibrosis.

Study of corneal biomechanical properties in patients with childhood glaucoma

Hepatitis C virus-associated neurocognitive and neuropsychiatric disorders:Advances in 2015

Since its identification in 1989, hepatitis C virus(HCV) has emerged as a worldwide health problem with roughly 185 million chronic infections, representing individuals at high risk of developing cirrhosis and liver cancer. In addition to being a frequent cause of morbidity and mortality due to liver disease, HCV has emerged as an important trigger of lymphoproliferative disorders, owing to its lymphotropism, and of a wide spectrum of extrahepatic manifestations(HCV-EHMs) affecting different organ systems. The most frequently observed HCV-EHMs include mixed cryoglobulinemia and cryoglobulinemic vasculitis, b-cell non-Hodgkin's lymphoma, nephropathies, thyreopathies, type 2 diabetes mellitus, cardiovascular diseases, and several neurological conditions. In addition, neuropsychiatric disorders and neurocognitive dysfunction are reported in nearly 50% of patients with chronic HCV infection, which are independent of the severity of liver disease or HCV replication rates. Fatigue, sleep disturbance, depression and reduced quality of life are commonly associated with neurocognitive alterations in patients with non-cirrhotic chronic HCV infection, regardless of the stage of liver fibrosis and the infecting genotype. These manifestations, which are the topic of this review, typically occur in the absence of structural brain damage or signal abnormalities on conventional brain magnetic resonance imaging(MRI), although metabolic and microstructural changes can be detected by in vivo proton magnetic resonance spectroscopy, perfusion-weighted and diffusion tensorMRI, and neurophysiological tests of cognitive processing. Several lines of evidence, including comparative and longitudinal neuropsychological assessments in patients achieving spontaneous or treatment-induced viral clearance, support a major pathogenic role for HCV in neuropsychiatric and neurocognitive disorders.

Entomopathogenic microorganisms occupy an important place among the alternative methods of fighting against pests insect. The fungus beauveria bassiana is an agent naturally present in ecosystems. It has potential to control pest populations. In the context of biological control, the present work aims to the study of linear growth of beauveria bassiana on different natural environments from the food industry. They are the raw whey, water and pomace, and followed the development of the fungus through a trial production of biomass on deproteinized whey.

Endoscopic retrograde cholangiopancreatography with rendezvous cannulation reduces pancreatic injury

PillCam~ Sb3 capsule: Does the increased frame rate eliminate the risk of missing lesions?

Since its emergence in 2000, small bowel capsule endoscopy(SbCE) has assumed a pivotal role as an investigation method for small bowel diseases. The PillCam Sb2-ex offers 12 h of battery time, 4 more than the previous version(Sb2). Rahman et al recently found that the PillCam Sb2-ex has a significantly increased completion rate, although without higher diagnostic yield, compared with the Sb2. We would like to discuss these somewhat surprising results and the new potentialities of the PillCam Sb3 regarding the diagnostic yield of small bowel studies. PillCam Sb3 offers improved image resolution and faster adaptable frame rate over previous versions of SbCE. We recently compared the major duodenal papilla detection rate obtained with PillCam Sb3 and Sb2 as a surrogate indicator of diagnostic yield in the proximal small bowel. The PillCam Sb3 had a significantly higher major duodenal papilla detection rate than the PillCam Sb2(42.7% vs 24%, P = 0.015). Thus, the most recent version of the PillCam capsule, Sb3, may increase diagnostic yield, particularly in the proximal segments of the small bowel.
