Little is known about the long-term survivors of acute arsenic intoxication. We present here a clinical case report of a man with chronic hepatitis B virus （HBV） infection who developed hepatocellular carcinoma four years after acute arsenic poisoning. HBsAg was detected in serum in 1990 when he voluntarily donated blood. In 1991, the patient suffered from severe psychological depression that led him to attempt suicide by massive ingestion of an arsenic-containing rodenticide. He survived with polyneuropathy and paralysis of the lower limbs, and has been wheelchair-bound since then. During participation in a follow-up study conducted among HBV carders, abdominal ultrasound detected a two-centimeter liver mass consistent with hepatocellular carcinoma. The tumor was confirmed by computed tomography （CT） and magnetic resonance image （MRI）. Because of his significant comorbidity, the patient received palliative treatment with transarterial lipiodol chemoembolization （TACE） on three occasions （1996, 1997 and 1999）. At his most recent visit in lay 2005, the patient was asymptomatic, liver enzymes were normal and the tumor was in remission on ultrasound.

相似的产品安全标准是否会带来相似的消费者安全水平?2007年,超过1900万件玩具因违反安全标准被召回;此后,欧盟和美国制定了监管儿童产品的新政策:2008年的美国消费品安全改进法案(CPSIA)和2009年的欧盟玩具安全指令。上述立法行为推动了相关玩具安全标准的建立,如ISO (ISO 8124)、美国(ASTM F963)和欧盟(EN 71)。尽管这些标准在文字上具有相似性,但是其实施结果并不一致。本文通过数据分析,展示欧盟和美国合格评定系统的执行情况,分析决定中国制造的玩具安全性的影响因素。研究发现,欧盟召回的违规产品加权总量是美国的10-20倍。这表明,国际标准的执行差异,将导致某些地区的消费者接触到更多不安全的产品。

Background: Confusion often arises in caring for diabetic foot infections and ulcers, especially with antimicrobials;we aim to shed light on this entity and alert healthcare workers to its stewardship. Methods: Records were reviewed between February 2016 and September 2023. Data for patients diagnosed with diabetes and foot ulcers, infected or not, were examined following ICD 9 search terms. Records for patients were included if they were prediabetic/diabetic adults with foot ulcers, more than 18 years old, and on antidiabetic treatment. Patients were excluded if they insulin resistant, with normal HgbA1c levels, wheel-chair dependent, bed-bound, non-diabetic patients, diabetic patients who had vascular lower limb surgery earlier to ulcers, diabetic patients who had aortocoronary bypass, deep venous thrombosis within six months, malignancy, and severe clinical depression. A modified IWGDF/IDSA guidelines definitions for DFI and DFU was considered. Statistical analysis was done using R programming. Statistical methods were employed as appropriate, and a significant P-value was considered for P Results: Most characteristics were well balanced between DFI and DFU, on imaging osteomyelitis and tissue swelling were significantly more in DFI. Endovascular radiological procedures showed angiograms to be considerably more in DFI, while angioplasty was more in DFU, in addition to smoking. Bacteremia was uncommon, and swab cultures were mostly polymicrobial in both ulcers;no clear association with blood bacteria was detected with the polymicrobial growth, though few were concordant. Antimicrobials prescribed for both ulcers were not statistically different except for carbapenems, which were more in DFI (P Conclusion: Attention should be paid to best practices while caring for diabetic ulcers. These include swab culture interpretations, the use of antimicrobials, and plan management according to DFI or DFU to utilize either local care or combination with antimicrobials.

Malnutrition is a liver cirrhosis complication affecting more than 20%-50%of *** the term can refer to either nutrient deficiency or excess,it usually relates to undernutrition in cirrhosis *** is defined as limited physical function due to muscle weakness,whereas sarcopenia is defined as muscle mass loss and an advanced malnutrition *** pathogenesis of malnutrition in liver cirrhosis is multifactorial,including decreased oral intake,maldigestion/malabsorption,physical inactivity,hyperammonemia,hypermetabolism,altered macronutrient metabolism and gut microbiome *** with chronic liver disease with a Body Mass Index of<18.5 kg/m2 and/or decompensated cirrhosis or Child-Pugh class C are at the highest risk of *** patients at risk of malnutrition,a detailed nutritional assessment is required,typically including a history and physical examination,laboratory testing,global assessment tools and body composition *** latter can be done using anthropometry,cross-sectional imaging including computed tomography or magnetic resonance,bioelectrical impedance analysis and dual-energy X-ray absorptiometry.A multidisciplinary team should screen for and treat malnutrition in patients with *** and sarcopenia are associated with an increased risk of complications and a poor prognosis in patients with liver cirrhosis;thus,it is critical to diagnose these conditions early and initiate the appropriate nutritional *** this review,we describe the prevalence and pathogenesis of malnutrition in liver cirrhosis patients and discuss the best diagnostic approach to nutritional assessment for them.

BACKGROUND Oral cancer is the sixth most prevalent cancer *** knowledge in oral cancer risk factors and survival is *** To come up with machine learning(ML)algorithms to predict the length of survival for individuals diagnosed with oral cancer,and to explore the most important factors that were responsible for shortening or lengthening oral cancer *** We used the Surveillance,Epidemiology,and End Results database from the years 1975 to 2016 that consisted of a total of 257880 cases and 94 *** ML techniques in the area of artificial intelligence were applied for model training and *** accuracy was evaluated using mean absolute error(MAE),mean squared error(MSE),root mean squared error(RMSE),R2 and adjusted *** The most important factors predictive of oral cancer survival time were age at diagnosis,primary cancer site,tumor size and year of *** of diagnosis referred to the year when the tumor was first diagnosed,implying that individuals with tumors that were diagnosed in the modern era tend to have longer survival than those diagnosed in the *** extreme gradient boosting ML algorithms showed the best performance,with the MAE equaled to 13.55,MSE 486.55 and RMSE *** Using artificial intelligence,we developed a tool that can be used for oral cancer survival prediction and for medical-decision *** finding relating to the year of diagnosis represented an important new discovery in the *** results of this study have implications for cancer prevention and education for the public.
