The ascorbic acid(AA)is a biomarker that can be used to detect the symptoms of severe disorders such as scurvy,Parkinson’s,Alzheimer’s,and cardiovascular *** this work,a simple and effective sensor model is developed to diagnose the presence of AA *** develop the sensor,a tapered single-mode optical fiber has been used with the well-known phenomenon of localized surface plasmon resonance(LSPR).For LSPR,the tapered region is immobilized with synthesized gold nanoparticles(AuNPs)and zinc oxide nanoparticles(ZnO-NPs)whose absorbance peak wavelengths appear at 519nm and 370nm,*** the basis of nanoparticles(NPs)configurations,two different biosensor probes are *** the first one,the sensing region is immobilized with AuNPs and named Probe *** the second probe,the immobilized layer of AuNPs is further coated with a layer of ZnO-NPs,and a resultant probe is termed as Probe *** characterizations of synthesized AuNPs and developed fiber probes are done by the ultraviolet-visible(UV-vis)spectrophotometer,high-resolution transmission electron microscope(HR-TEM),atomic force microscopy(AFM),and scanning electron microscope(SEM).To enhance the selectivity,a sensing region of probes is functionalized with ascorbate oxidase enzyme that oxidizes the AA in the presence of *** response of developed sensor probes is authenticated by sensing the samples of AA in the range from 500 nM to 1 mM,which covers the range of AA found in human bodies,i.e.,40μM-120μ*** performance analysis of the developed sensor probes has been done in terms of their stability,reproducibility,reusability,and *** observe the stability of AA,a pH-test has also been done that results in a better solubility of AA molecules in phosphate-buffered saline(PBS)solution.

Current Trends in Clinical Practice for the Minimal Invasive Medial Unicondylar Knee Arthroplasty

Unicondylar Knee Arthroplasty (UKA) is an early alternative surgical procedure for the unicondylar osteoarthritis or damaged knee joint with artificial prosthesis for the release of disabling painful condition and restoring the normal knee functions. Minimally UKA is one of the recent and the majority successful procedures in modern orthopedics for the osteoarthritis which is spreading throughout the worldwide. Recently, many orthopedic surgeons are expanding their abilities in this field. However, it needs good knowledge and well experience for the successful clinical outcomes. The minimal invasive approach is more efficient for short hospital stay and faster postoperative recovery with low morbidity of the patients after UKA. The aim of this article is to emphasize the steps in UKA based on modern facts: function of knee joint, diagnosis, less invasive approach for medial condylar replacement, radiographic evaluation, and earlier recovery, selecting the patient and implant survivorship with review of surgery.

A Case Report of Pediatric Cerebral Venous Thrombosis with Undiagnosed Complex Congenital Heart Disease: Tetralogy of Fallot with OS ASD: A Cataclysmic Ending

Cerebral Venous Sinus Thrombosis (CVST/CSVT) is occlusion of cerebral veins and venous sinuses of brain secondary to blood clot formation resulting in hindrance in the blood drainage system in brain, leading to disturbances the internal homeostasis of brain, raised intracranial pressure, cerebral edema, and 50% of cases will have venous infarction or venous hemorrhage (stroke). CVST although being a Rare disorder but may be more common in children than adults with greater risk in neonatal period i.e. first 28 days of life. Here we are discussing a case of Pediatric CVST in a 7-month-old baby boy who presented to Emergency Room (ER) with recurrent discrete episodes of vomiting, fever, seizures, drowsiness and respiratory distress. The fatal outcome in our child was attributed to delayed presentation in a tertiary care center, hence missed early diagnosis and treatment. In this child the CVST could be result of amalgamation of complex underlying ongoing multiple pathological processes: an acute systemic illness like sepsis, severe dehydration, undiagnosed and untreated complex congenital heart disease, tetralogy of fallot with osteum secondum atrial septal defect, worsening the coagulopathy. It takes this case even more unique. This discussion is to bring focus on the importance of knowledge about CVST amongst emergency physicians and primary care physicians, specially managing this rare disorder with flummox presentation mimicking other more common disorders, especially in pediatric and neonatal population where definitive history and chief complaints are often vague and difficult to obtain, making it more difficult to diagnose. We the authors hence reporting this case with intent to spread awareness of CVST, how to doubt it, detect it and then manage it, especially in places like Chhattisgarh, India, where CVST is not so uncommon. Surveillance of antibiotic sensitivity pattern in chronic suppurative otitis media of an Indian teaching hospital

Introduction:Chronic suppurative otitis media (CSOM) is a common problem in worldwide and untreated CSOM leads to fatal complications like facial nerve paralysis,lateral sinus thrombosis,labyrinthitis,meningitis and brain abscess in developing country like ***:To isolate causative bacteria and antibiotic sensitivity pattern for CSOM and to know the prevalence of extended spectrum beta lactamases (ESBL) and Metallobetalactamases (MBL) in CSOM ***:A total of 500 ear swabs of clinical suspected CSOM patients were cultured on specific cultured medium and identified the bacteria with conventional *** all the identified bacteria were subjected with specific antibiotics by the Kirby-Bauer's method to know the resistance pattern of *** and MBL strains were detected by double disc diffusion ***:A total of 384 bacteria were isolated from 500 CSOM patients,among them 86 *** (22.40%),112 Staphylococcus aureus (29.17%),53 *** (13.80%),32 *** (18%),26 *** (6.77%),24 *** (6.25%),23 *** (5.99%),18 *** (4.69%) and 10 *** (2.60%) identified with conventional *** antibiotic disc diffusion methods 74.22% ESBL strains and 9.90%MBL strains were *** resistant strains of *** (86/384,22.40%) were more prevalent than those of *** (112/384,29.17%) and other bacteria in ear *** and vancomicin could control to gram negative bacteria and gram positive bacteria ***:Continuous and periodic evaluation of microbiological profile and antimicrobial sensitivity pattern of bacterial is essential for optimum management of CSOM patients.

The pathogenesis of vestibular paroxysmia(VP)is the neurovascular cross-compression of cranial nerveⅧwith short episodes of vertigo as the common *** exact etiological and clinical profiles of the patients with VP are uncertainly reported in *** is general thought that loop of the anterior inferior cerebellar artery and posterior inferior cerebellar artery(less commonly),vertebral artery,or a vein is involved in compressing cranial nerveⅧ.Ephaptic neural discharges at the proximal end of cranial nerveⅧ,which is covered by oligodendrocytes,are thought to be the mechanism for *** with VP often suffer from brief vertiginous attacks of several episodes in a single day,and nystagmus is seen during the vertiginous *** magnetic resonance imaging could reveal the arterial compression of cranial nerveⅧ.The important differential diagnosis of VP includes vestibular migraine,Meniere's disease,benign paroxysmal positional vertigo,superior canal dehiscence syndrome,epileptic vertebral aura,paroxysmal brainstem attacks(in multiple sclerosis or after brainstem stroke),transient ischemic attacks and panic *** vertiginous attacks can be ceased by administration of a low dose of *** or oxcarbazepine,even in low doses,can be efficient to relieve VP,and they have the same efficiency both in adults and in children.

第一章：利用动脉自旋标记磁共振成像测量不同年龄组正常人脑学流量目的：评价动脉自旋标记磁共振成像测量不同年龄段正常脑灌注的作用。材料和方法：招募2014年1月至2016年1月80例没有神经疾病、非特异性头痛、药物使用、颅脑损伤的志愿者,其中男性37例,女性43例,年龄范围约20-79岁,平均年龄50.12±16.87岁。所有志愿者按年龄段分两组,20-49岁为一组,其中男性19例,女性21例,平均年龄35.2±7.45岁;50-79岁为一组,其中男性18例,女性22例,平均年龄65.05+8.00岁。所有志愿者排除动脉自旋标记磁共振成像扫描禁忌症。图像采集：使用3.0T磁共振成像系统进行传统磁共振图像采集,包括横断位、冠状位、矢状位T2加权成像、横断位T1加权成像、水压制成像、弥散加权成像。动脉自选标记成像使用3D伪连续动脉自旋标价方法。数据分析：在每个人的每个脑叶手工选取三个感兴趣区,获得这些感兴趣去的灰白质血流量,计算灰白质血流量比值。使用SPSS 16.0进行数据分析,使用ANOVA单因素方差分析双样本T检验后的两组数据,以P0.05),但组间有显著统计学差异(p=0.00)。使用Pearson's相关性分析发现年龄增长与四部分灰白质脑血流量具有显著负相关(成人组：额叶：r=-0.24,颞叶：r=-0.26,顶叶：F-0.25;枕叶：r=-0.29,p=00;老年组：额叶：r=-0.95;颞叶：r=-0.94;顶叶：r=-0.9;枕叶：r=-0.91,p=00).结论：动脉自旋标记磁共振成像能有效定量分析不同年龄段脑灌注特点,并且能可靠鉴别正常和异常脑血流量。第二章：动脉自旋标记磁共振成像鉴别诊断胶质瘤复发与放射性损伤目的：评价动脉自旋标记灌注磁共振在鉴别诊断胶质瘤复发与放射性坏死的作用。材料与方法：收集2012年3月至2014年3月57例胶质瘤术后患者行3.0T磁共振扫描,包含序列增强T1加权成像,其中有21例出现强化病灶。16例行50-60 Gy放射治疗,5例行50-60 Gy放射治疗伴替莫唑胺化学治疗。5例在调强放射治疗期间和28个治疗周期分别行行75 mg/m2和150 mg/m2剂量的替莫唑胺化学治疗。3个月后随访行T2加权成像、弥散加权成像、动态磁敏感增强成像、动脉自旋标记成像,选取三个感兴趣区,每个感兴趣区面积为0.5-2.0 cm2,计算三个感兴趣区的平均数并选取对侧脑白质数值。数据分析：所有数据使用SPSS 16.0软件进行统计学分析。将胶质瘤复发和放射性坏死脑血流量和相对脑容量值行配对样本T检验,以P<0.05作为统计学差异。结果：胶质瘤复发患者的标准脑血流量比值(4.45+2.72)明显高于放射性损伤患者(1.22±0.61)(P<0.01),胶质瘤复发患者的标准脑容量比值(3.38±2.08)均明显高于放射性损伤患者(1.09±0.55)(P<0.05)。动态磁敏感增强和动脉自旋标记磁共振灌注成像在鉴别诊断胶质瘤复发和放射性损伤具有一致性(R=0.83;P=0.005)。结论：动脉自旋标记能够鉴别诊断胶质瘤复发与放射性坏死,并且动脉自旋标记较动态磁敏感增强更准确。

摘要：信息和通信技术(ICT)的发展使得利用大规模的分布式数据成为可能.而各种信息资源管理之间的异构性问题也日益严峻.语义网(即数据的网络)作为超文本网络的扩展,能够在网络上交换和集成数据,以便在在世界范围内实现一种新的人机的合作方式.本体作为语义网的支柱在语义网中起着至关重要的作用.本体映射是本体查询和集成的一个关键部分。本体数量的增殖在查询多个异构、独立本体时带来了问题,本体规模的增长加大了对系统效率和管理的挑战?随着本体数量的不断增加,本体的重用和共享成为迫切需要解决的问题.不同的本体之间的映射是解决本体之间通信和互操作问题的关键。目前,已有许多能够处理本体语义信息的映射策略,大部分实用的本体映射系统都使用多策略以获得更佳结果。本体映射是解决网络异构性的关键。本文重点研究本体映射算法。首先,简要介绍本文的研究背景,接着阐述最新的本体映射技术以及映射的发展形势。此外,针对当前映射算法时间效率低的问题,本文提出一种基于子树映射的本体映射方法。它根据本体的结构特征将本体分割成不同粒度的子树,然后使用子树映射算法映射。之后,对树分割算法和子树映射算法的时间复杂度进行分析。算法的时间复杂度和实验结果表明,与当前的映射算法比较,本文方法在不影响映射查全率和查准率的情况下,改善了映射的效率。为了改善本体映射系统的性能,多策略组合往往是必要的。每个策略都有其自身的优点和缺点。前面组合策略给予不同的策略不同的权重,然后计算加权平均。本文提出基于树分割的本体映射,该算法有效地提高了精度,同时保持了好的查全率。最后,为了发现现有复杂映射算法中最简单的复杂映射关系,本文提出了一个基于包含和等价关系的复杂映射算法。其首先利用已有的映射提供的信息(包括基于包含关系的实例和属性等)发现复杂映射,然后,基于等价关系的特征：等价节点和模式,从中发现新的映射关系。实验结果表明,本文提出的算法能够在现有算法的基础上,在一般的本体映射中有显著的改善。

Cardioprotective activity of alcoholic extract of Tinospora cordifolia (Willd.) Miers in calcium chloride-induced cardiac arrhythmia in rats

The present study investigated the antiarrhythmic activity of alcoholic extract of Tinospora cordifolia （T. cordifolia） in CaCl2 induced arrhythmia. CaCl2 （25 mg/kg） was administered by intravenous infusion （iv） to produce arrhythmia in rats. The animals were then treated with T. cordifolia extract （150, 250, and 450 mg/kg） and verapamil （5 mg/kg,iv）. Lead II electrocardiogram was monitored. Plasma calcium, sodium and potassium levels were measured. In CaCl2 induced arrhythmia, heart rate was decreased by 41.10%, T. cordifolia at 150, 300, and 450 mg/kg decreased the heart rate by 26.30%, 29.16%, and 38.29%, respectively, and verapamil reduced the heart rate by 9.70% compared to the normal group. The PQRST waves were normalized and atrial and ventricular fibrillation was controlled in rats treated with verapamil and T. cordifolia. CaCl2 increased calcium and sodium levels and decreased potassium levels in blood. T. cordifolia dose-dependently decreased calcium and sodium levels and increased potassium levels. Hence, T. cordifolia can be used in antiarrhythmic clinical settings and beneficial in atrial and ventricular fibrillation and flutter and may be indicated in ventricular tachyarrhythmia.

Clinical characteristics and treatment of acute epiglottitis: A retrospective study of 28 cases

Objective:To evaluate the clinical profiles,diagnosis,and treatment of patients with acute epiglottitis at a tertiary care teaching hospital of eastern ***:This is a retrospective study in which 28 patients of acute epiglottitis *** study was conducted between January 2016 and October *** diagnosis of acute epiglottitis was based on the clinical presentations and fiberoptic laryngoscopic examinations of the larynx and *** of age,gender,personal history,clinical symptoms,treatment,and hospital stay were ***:Out of the 28 patients with acute epiglottitis,17(60.71%)were male and 11(39.29%)were female with age ranged from 4 years to 62 years and a mean age of 23.45 *** median duration from the onset to resorting to physicians is 3 *** the 28 patients,4 patients(14.29%)were under the treatment for diabetes *** cases(14.29%)required airway management with intubation and ***:Acute epiglottitis is an acute infectious condition of the supraglottis and may lead to airway *** presentations alone are insufficient for the diagnosis of acute *** nasopharyngolaryngoscopic examination should be performed as soon as possible for confirmation of the disease.
