Cerebral Venous Sinus Thrombosis (CVST/CSVT) is occlusion of cerebral veins and venous sinuses of brain secondary to blood clot formation resulting in hindrance in the blood drainage system in brain, leading to disturbances the internal homeostasis of brain, raised intracranial pressure, cerebral edema, and 50% of cases will have venous infarction or venous hemorrhage (stroke). CVST although being a Rare disorder but may be more common in children than adults with greater risk in neonatal period i.e. first 28 days of life. Here we are discussing a case of Pediatric CVST in a 7-month-old baby boy who presented to Emergency Room (ER) with recurrent discrete episodes of vomiting, fever, seizures, drowsiness and respiratory distress. The fatal outcome in our child was attributed to delayed presentation in a tertiary care center, hence missed early diagnosis and treatment. In this child the CVST could be result of amalgamation of complex underlying ongoing multiple pathological processes: an acute systemic illness like sepsis, severe dehydration, undiagnosed and untreated complex congenital heart disease, tetralogy of fallot with osteum secondum atrial septal defect, worsening the coagulopathy. It takes this case even more unique. This discussion is to bring focus on the importance of knowledge about CVST amongst emergency physicians and primary care physicians, specially managing this rare disorder with flummox presentation mimicking other more common disorders, especially in pediatric and neonatal population where definitive history and chief complaints are often vague and difficult to obtain, making it more difficult to diagnose. We the authors hence reporting this case with intent to spread awareness of CVST, how to doubt it, detect it and then manage it, especially in places like Chhattisgarh, India, where CVST is not so uncommon. A series of 8-substituted alkyl xanthines were evaluated in vitro to test the cytotoxocity in cells. For this experiment, we utilized different mammalian cancer cell lines primarily representing prostrate and lung. One of the compounds synthesized, viz. 8-tertbutyl caffeine showed potent anticancer activity at low concentrations against DU145 when compared to adriamycin. Further experiments were carried out to check the cell cycle arrest in the DU145 cells treated with adriamycin, caffeine and 8-tert butyl caffeine. We observed that there was an arrest in G1 phase of cell cycle at 24 hours while at 48 hours of incubation, the cells were constantly distributed (59.71% -70.79%). We conclude that the effect of 8-tertbutyl caffeine is relatively comparable to caffeine whereas in adriamycin treated cells, we observed the cells underwent G2 arrest. We evaluate the studies on these effects by showing potent analogues which could be used as promising anticancer agents.

Introduction:Chronic suppurative otitis media (CSOM) is a common problem in worldwide and untreated CSOM leads to fatal complications like facial nerve paralysis,lateral sinus thrombosis,labyrinthitis,meningitis and brain abscess in developing country like ***:To isolate causative bacteria and antibiotic sensitivity pattern for CSOM and to know the prevalence of extended spectrum beta lactamases (ESBL) and Metallobetalactamases (MBL) in CSOM ***:A total of 500 ear swabs of clinical suspected CSOM patients were cultured on specific cultured medium and identified the bacteria with conventional *** all the identified bacteria were subjected with specific antibiotics by the Kirby-Bauer's method to know the resistance pattern of *** and MBL strains were detected by double disc diffusion ***:A total of 384 bacteria were isolated from 500 CSOM patients,among them 86 *** (22.40%),112 Staphylococcus aureus (29.17%),53 *** (13.80%),32 *** (18%),26 *** (6.77%),24 *** (6.25%),23 *** (5.99%),18 *** (4.69%) and 10 *** (2.60%) identified with conventional *** antibiotic disc diffusion methods 74.22% ESBL strains and 9.90%MBL strains were *** resistant strains of *** (86/384,22.40%) were more prevalent than those of *** (112/384,29.17%) and other bacteria in ear *** and vancomicin could control to gram negative bacteria and gram positive bacteria ***:Continuous and periodic evaluation of microbiological profile and antimicrobial sensitivity pattern of bacterial is essential for optimum management of CSOM patients.

The pathogenesis of vestibular paroxysmia(VP)is the neurovascular cross-compression of cranial nerveⅧwith short episodes of vertigo as the common *** exact etiological and clinical profiles of the patients with VP are uncertainly reported in *** is general thought that loop of the anterior inferior cerebellar artery and posterior inferior cerebellar artery(less commonly),vertebral artery,or a vein is involved in compressing cranial nerveⅧ.Ephaptic neural discharges at the proximal end of cranial nerveⅧ,which is covered by oligodendrocytes,are thought to be the mechanism for *** with VP often suffer from brief vertiginous attacks of several episodes in a single day,and nystagmus is seen during the vertiginous *** magnetic resonance imaging could reveal the arterial compression of cranial nerveⅧ.The important differential diagnosis of VP includes vestibular migraine,Meniere's disease,benign paroxysmal positional vertigo,superior canal dehiscence syndrome,epileptic vertebral aura,paroxysmal brainstem attacks(in multiple sclerosis or after brainstem stroke),transient ischemic attacks and panic *** vertiginous attacks can be ceased by administration of a low dose of *** or oxcarbazepine,even in low doses,can be efficient to relieve VP,and they have the same efficiency both in adults and in children.

摘要：信息和通信技术(ICT)的发展使得利用大规模的分布式数据成为可能.而各种信息资源管理之间的异构性问题也日益严峻.语义网(即数据的网络)作为超文本网络的扩展,能够在网络上交换和集成数据,以便在在世界范围内实现一种新的人机的合作方式.本体作为语义网的支柱在语义网中起着至关重要的作用.本体映射是本体查询和集成的一个关键部分。本体数量的增殖在查询多个异构、独立本体时带来了问题,本体规模的增长加大了对系统效率和管理的挑战?随着本体数量的不断增加,本体的重用和共享成为迫切需要解决的问题.不同的本体之间的映射是解决本体之间通信和互操作问题的关键。目前,已有许多能够处理本体语义信息的映射策略,大部分实用的本体映射系统都使用多策略以获得更佳结果。本体映射是解决网络异构性的关键。本文重点研究本体映射算法。首先,简要介绍本文的研究背景,接着阐述最新的本体映射技术以及映射的发展形势。此外,针对当前映射算法时间效率低的问题,本文提出一种基于子树映射的本体映射方法。它根据本体的结构特征将本体分割成不同粒度的子树,然后使用子树映射算法映射。之后,对树分割算法和子树映射算法的时间复杂度进行分析。算法的时间复杂度和实验结果表明,与当前的映射算法比较,本文方法在不影响映射查全率和查准率的情况下,改善了映射的效率。为了改善本体映射系统的性能,多策略组合往往是必要的。每个策略都有其自身的优点和缺点。前面组合策略给予不同的策略不同的权重,然后计算加权平均。本文提出基于树分割的本体映射,该算法有效地提高了精度,同时保持了好的查全率。最后,为了发现现有复杂映射算法中最简单的复杂映射关系,本文提出了一个基于包含和等价关系的复杂映射算法。其首先利用已有的映射提供的信息(包括基于包含关系的实例和属性等)发现复杂映射,然后,基于等价关系的特征：等价节点和模式,从中发现新的映射关系。实验结果表明,本文提出的算法能够在现有算法的基础上,在一般的本体映射中有显著的改善。

Objective:To evaluate the clinical profiles,diagnosis,and treatment of patients with acute epiglottitis at a tertiary care teaching hospital of eastern ***:This is a retrospective study in which 28 patients of acute epiglottitis *** study was conducted between January 2016 and October *** diagnosis of acute epiglottitis was based on the clinical presentations and fiberoptic laryngoscopic examinations of the larynx and *** of age,gender,personal history,clinical symptoms,treatment,and hospital stay were ***:Out of the 28 patients with acute epiglottitis,17(60.71%)were male and 11(39.29%)were female with age ranged from 4 years to 62 years and a mean age of 23.45 *** median duration from the onset to resorting to physicians is 3 *** the 28 patients,4 patients(14.29%)were under the treatment for diabetes *** cases(14.29%)required airway management with intubation and ***:Acute epiglottitis is an acute infectious condition of the supraglottis and may lead to airway *** presentations alone are insufficient for the diagnosis of acute *** nasopharyngolaryngoscopic examination should be performed as soon as possible for confirmation of the disease.

背景和目的:研究下肢冠状位力线对内侧UKA患者临床功能的影响。分析微创内侧UKA术后的临床和影像学结果。评估接受内侧UKA手术的患者术前和术后下肢冠状位力线,并探讨影响患者满意度和对短期功能结果影响的因素。方法:选择2018年11月至2020年9月接受内侧UKA治疗的109例患者。记录患者的人口统计信息,如年龄、性别、BMI,以及患者的手术时间、失血量、术前术后冠状面力线等。所有患者均接受临床和放射学评估,包括标准化X线片（下肢全长前后位X光片）评估。临床结果评分采疼痛数值评定量表（numerical rating scale,NRS）、美国特种外科医院膝关节评分（the hospital for special surgery,HSS）、美国西部Ontario和Mc Master大学骨关节炎指数评分（the western Ontario and Mc Master universities osteoarthritis index,WOMAC）。用均值±标准差表示的正态分布定量资料,用中位数和四分位数表述非正态分布变量资料。对正态分布的连续变量采用t检验比较两组间的差异,对非正态分布的连续变量采用Mann-Whitney U检验。多因素logistic回归分析用于确定与临床结果相关的影响因素,包括患者满意度。P值＜0.05,认为具有统计学意义。结果:122例患者中,4例因排除标准被排除。随访期间有9例患者失访。因此,共有109名患者纳入研究,其中男性31名（28.4%）,女性78名（71.6%）。年龄54-91岁,平均（67.95±7.31）岁。BMI为18.00-40.60,平均（26.61±3.87）,体重45kg-100kg,平均（69.66±11.60）kg。左膝手术为47例（43.1%）,右膝为60例（55%）,双侧膝为2例（1.9%）。术前下肢冠状面力线为中性（0-3度）46例（42%）和内翻（＞3度）63例（58%）。平均手术时间为（111.98±26.67）分钟。止血带的平均时间是（79.24±16.73）分钟。平均引流管失血量为（195.60±152.83）毫升。平均住院费用为（52714.86±7696.4）元人民币,平均住院时间为（8.72±3.07）天。术前NRS得分中位数为5.49±0.83,手术3个月后降至2.19±0.67,6个月后最终降至0.29±0.53（t=-9.218,P＜0.001）。HSS评分从术前60.07±6.15分显着增加至术后3个月的80.03±2.03分和6个月的91.21±3.55分（t=-9.07,P＜.001）。然而,不同时间跨度的HSS中位数得分有显着增加。术前平均WOMAC评分从73.94±3.71分下降至术后3个月47.69±4.30分、术后6个月22.37±1.97分。WOMAC评分在术前与术后差异具有统计学意义（P＜0.001）。此外,术前中位MAA从5.01±4.03度到术后2.61±1.91度（P＜0.001）,术前中位JLCA从4.65±1.08度到术后2.95±1.01度（P＜0.001）,均有统计学意义。术前和术后LDFA（P=0.350）和m MPTA（P=0.062）角度没有显著性差异。109例患者的满意度为非常满意71例（65.1%）,满意31例（28.4%）,一般4例（3.7%）,差3例（2.8%）。结论:微创内侧UKA治疗内侧骨关节炎创伤小、恢复快,术后明显改善患者的疼痛及膝关节功能,短期随访结果满意度高。

Objective:Andrographis paniculata is a well-known medicinal plant in Southeast Asia,India and *** plant contains andrographolide(AN),a very important phytochemical used in various health ***,AN is low in oral absorption bioavailability of AN due to the rapid clearance and high protein binding ***:The present study was aimed to develop a nano-phytovesicular formulation of semi-purified AN extracts from a naturally occurring phospholipid(soya phosphatidylcholine)in order to increase the oral absorption and antihyperglycemic activity in ***:The nano-phyto vesicle of semi-purified AN extracts equivalent to 25 mg/kg AN significantly protected the hyperglycemic condition of *** in vitro and in vivo experiments results proved that the nano-phytovesicular system of plant extracts containing AN produced better oral absorption,bioavailability and improved antihyperglycemic activity compared with that of free AN at dose of 50 mg/***:Hence,the prepared semi-purified extract nano-phytovesicular system is helpful in solving the problem of rapid clearance of AN.
