The Psycho-Social Impact of Urinary Incontinence on the Quality of life among Kuwaiti Women with Type 2 Diabetes mellitus

This study was conducted to determine the psycho-social impact of urinary incontinence (UI) on the quality of life of 250 Kuwaiti women with type 2 diabetic mellitus (Dm). A survey method, using a 33-item 5-point likert scale Arabic questionnaire adapted from the King’s Health Questionnaire (KHQ), was employed for data collection from February to may, 2014 while all the participants were receiving treatment for UI at a specialized urology center in Kuwait. Participants with UI and a co-morbidity of type 2 Dm and obesity were 20 to 65 years old. Results showed the following variables were statistically significant for frequency of urine leak: Age was (m>χm>2 = 36.877, df = 3, P ≤ 0.000). Parity showed nulliparous women reported less urine leak compared to parous women: Chi-square was (m>χm>2 = 24.83, df = 12, P ≤ 0.016). Type 2 Dm for more than 3 years duration had the highest incidence of several leaks per day. BmI of above 25 kg/m2 caused daily urine leak: Chi-square (m>χm>2 = 17.912, df = 9, P ≤ 0.036). Participants’ self reports of good general health were those who leaked urine either 2 - 3 times weekly or occasionally. Finally, the impact of incontinence on their lifestyle was reported as extreme by 128 (51.2%), and 6 (2.4%) reported little or no impact on their quality of life: Chi-square was (m>χm>2 = 52.392, df = 18, P ≤ 0.000). In conclusion, this study showed a clear correlation between UI and reduced quality of life. midwives are well positioned to correct the myth that UI is an inevitable byproduct of childbearing. midwives should explain to all pregnant women how childbirth can be a risk factor and provide anticipatory guidance by teaching preventive measures like pelvic floor exercises before and after delivery. Family members should assist sufferers in coping with their challenges by dispelling any form of stigmatization, joining them in practicing pelvic floor exercises, and encouraging,

Aim: The aim of the current study is to determine: (1) the prevalence of extended-spectrum β-lactamase-producing K. pneumoniae (ESBl-Kp) isolated from clinical samples and a hospital environment in Hassan II Hospital (Settat, morocco);(2) the associated risk factors of ESBl-Kp infections;(3) the link between clinical and environmental isolates. methods: During the study period (April 2010 to march 2011), all patients infected and hospital environment sites contaminated by K. pneumoniae were considered as the potential study population and environmental site. The clinical data were collected to identify risk factors for ESBl carriage of K. pneumoniae infection. Screening of ESBl-and carbapenemase-producing isolates was performed by using a double-disk synergy test and the modified Hodge test, respectively. ESBl-Kp isolates were tested for the presence of genes encoding β-lactamases and were investigated by PCR. The clonal relationship between ESBl-producing isolates was analysed by ERIC- and REP-PCR method. Results: The overall prevalence of ESBl-Kp among clinical and environmental K. pneumoniae isolates was 35.13% (13/37) and 4.04% (4/99), respectively. The main risk factors for carrying ESBl-Kp were renal disease (46.15%), recent surgery (53.84%), previous hospitalisation (76.92%), and the presence of many invasive devices (53.84%). All ESBl isolates were multidrug resistant. The blaCTX-m group1and blaSHV (70.58% for each) were the most prevalent followed by blaTEm (52.94%). Thirteen strains expressed at least two bla genes. One isolate was positive in the modified Hodge test and was a blaOXA-48 producer. ERIC and Rep-PCR methods revealed an epidemic clonal dissemination of these isolates. Conclusion: The emergence of OXA-48 carbapenemase, endemic clonal dissemination and multi-drug resistance of ESBl-Kp isolates in our institution is highly alarming.

Ulcerative colitis (UC) is a chronic inflammatory bowel disease (IBD), characterized by acute exacerbations and remissions. We report the clinical case of an infant, an 8 months female baby. The time between disease onset and diagnosis of the disease was 6 months. It was from an unknown etiology. The main clinical picture was weight stagnation, mucoid bloody diarrhea, and rectal bleeding. The macroscopic aspect in anorectoscopy was an erythematous mucosa with healthy beaches intervals. The mucosa was bleeding easily on contact. Pathological examination realized within the various lesions of the association was highly suggestive. The suggested treatment was made of symptomatic, corticosteroids and immunosuppressant’s therapy. Conclusion: Ulcerative colitis is rare in infants and difficult to manage in our tropical context.

Deadly Envenomation by massive Bee Stings in a 7-Year Old Child in Ouagadougou

Introduction: We are reporting a deadly case of massive bee stings observed in a 7-year-old child in Ouagadougou. Observation: The first symptoms, which appeared two hours after the stings of about 200 bees, motivated a consultation in a nearby dispensary in which the child received intravenous betamethasone before being referred to the University Hospital Center Yalgado Ouédraogo (CHUYO). Seven hours after the accident, a rapid deterioration in the general condition was noted with hemorrhage, respiratory distress, coma, and shock. A grade 3 anaphylaxis was diagnosed. The patient didn’t survive despite of the cardiopulmonary resuscitation using adrenaline in intensive care. Conclusion: The massive bee stings have caused a severe biphasic anaphylactic reaction which could not be reversed by the adrenaline administered in the second phase. The development of a protocol would contribute to a better management of the cases.

Suppression of innate antiviral response by severe acute respiratory syndrome coronavirus m protein is mediated through the first transmembrane domain

Coronaviruses have developed various measures to evade innate immunity, We have previously shown that severe acute respiratory syndrome （SARS） coronavirus m protein suppresses type I interferon （IFN） production by impeding the formation of functional TRAF3-containing complex. In this study, we demonstrate that the IFN-antagonizing activity is specific to SARS coronavirus m protein and is mediated through its first transmembrane domain （Tm 1） located at the N terminus. m protein from human coronavirus HKU 1 does not inhibit IFN production. Whereas N-linked glycosylation of SARS coronavirus m protein has no influence on IFN antagonism, Tm1 is indispensable for the suppression of IFN production. Tm 1 targets SARS coronavirus m protein and heterologous proteins to the Golgi apparatus, yet Golgi localization is required but not sufficient for IFN antagonism. mechanistically, Tm 1 is capable of binding with RIG-I, TRAF3, TBK1 and IKK~, and preventing the interaction of TRAF3 with its downstream effectors. Our work defines the molecular architecture of SARS coronavirus m orotein reouired for suooression of innate antiviral re^nnn^e.

Impact of the Thicknesses of the p and p+ Regions on the Electrical Parameters of a Bifacial PV Cell

The present paper is about a contribution to the bifacial PV cell performances improvement. The PV cell efficiency is weak compared to the strong energy demand. In this study, the base thickness impacts and the p+ zone size influence are evaluated on the rear face of the polycrystalline back surface field bifacial silicon PV cell. The photocurrent density and photovoltage behaviors versus thickness of these regions are studied. From a three-dimensional grain of the polycrystalline bifacial PV cell, the magneto-transport and continuity equations of excess minority carriers are solved to find the expression of the density of excess minority carriers and the related electrical parameters, such as the photocurrent density, the photovoltage and the electric power for simultaneous illumination on both sides. The photocurrent density, the photovoltage and electric power versus junction dynamic velocity decrease for different thicknesses of base and the p+ region increases for simultaneous illumination on both sides. It is found that the thickness of the p+ region at 0.1 μm and the base size at 100 μm allow reaching the best bifacial PV cell performances. Consequently, it is imperative to consider the reduction in the thickness of the bifacial PV cell for exhibition of better performance. This reduced the costs and increase production speed while increasing conversion efficiency.

Types of Urinary Incontinence Prevalent among Kuwaiti Women with Type Two Diabetes mellitus

The aim of this study was to determine the types of urinary incontinence prevalent among Kuwaiti women with Type two Diabetes mellitus attending the outpatient clinic at the Urology center and suggest appropriate nursing interventions. Urinary incontinence is not a life threatening condition but it can be emotionally devastating and therefore affects the sufferer’s quality of life. This cross-sectional descriptive survey consisted of 250 Kuwaiti women with type two diabetes mellitus receiving treatment at the outpatient department at Sabah Al-Ahmad Urology Center, Kuwait. The questionnaire comprised of a total of 33 questions, twelve of which explored participants’ socio demographic characteristics, risk factors and symptoms of their urinary incontinence. The remaining 21 items were adapted from King’s Health Questionnaire (KHQ) formulated in 1997 at King’s college, london, for the assessment of quality of life in women with urinary incontinence. Data collection was from February to may 2014. The result revealed that mixed urinary incontinence was the most prevalent type of urinary incontinence affecting Kuwaiti women with type two diabetes mellitus. Symptoms of both stress incontinence and urge incontinence (Over active bladder) were not mutually exclusive as they were present in 247 (98.8%) and 240 (96%) of the participants respectively. Key risk factors for developing incontinence were over three years duration of diabetes mellitus in 115 (56%) and delivery of one or more children reported by 206 (82%) of the participants. Analysis and result of King Health Questionnaire on quality of life will be reported in separate paper on the psychosocial impact of urinary incontinence on diabetic Kuwaiti women. In conclusion, urinary incontinence had a devastating effect on the lives of sufferers and therefore should be prevented at all cost by nurses providing anticipatory guidance to all pregnant and diabetic women and routinely teaching pelvic floor exercises to all post

Objectives: At the beginning of 2012, northern mali was occupied by several jihadist movements with the aim of creating a Khalifa that required the intervention of a UN peacekeeping mission after a war that slowed down their development. This mission is the deadliest in a complex attack and UN attack. This experience feedback in an international context of the resurgence of asymmetrical wars makes it possible to share in order to better anticipate and improve strategic approaches to assessments and therapeutic techniques. Description: After the presentation of the context with these particularities and the various adaptations, we made formulations of the missions such as lived and the details on the medico-psychological techniques used. The place of the brief physical examination should not be overlooked. The debriefings, even if they have proven themselves in the evaluation and prevention of psychotraumatic pathologies, must be adapted on a case-by-case basis in a certain programmed improvisation. maintaining the site of the attack survivors without real military activities by reinforcing them with a new functional team was a decisive element in the mourning process according to their own statement. Conclusion: Forward psychiatry is a necessity in asymmetric warfare to maintain the operationality of the troops.

Additive manufacturing (Am), while enabling the production of parts with complex geometries, presents new challenges. In particular, the achievement of the basic mechanical properties of the alloy must be ensured. In general, the strength-ductility properties of metals depend strongly on their microstructure, and controlling these properties requires paying attention to the alloy composition, processing technique and heat treatments. Austenite 316l stainless steel parts produced by Am demonstrate good ductility and high yield strength—higher than that obtained with annealed 316l. Some preferred orientation of the mechanical properties was found as a function of the laser path, namely, the Young’s modulus varied with respect to the angle between the build direction and the normal to the build direction. In the present study, samples of Am 316l in three orientations relative to the print direction (0˚, 45˚and 90˚) are compared to a forged sample. mechanical properties, scanning electron microscopy-SEm fractography, energy dispersive X-ray spectroscope-EDS analysis of the fracture and optical cross section images of the samples along the stress tension after the failure are presented.
