Low-pressure pneumoperitoneum with abdominal wall lift in laparoscopic total mesorectal excision for rectal cancer:initial experience

AIM To evaluate the safety and feasibility of a new technology combining low-pressure pneumoperitoneum(LPP) and abdominal wall lift(AWL) in laparoscopic total mesorectal excision(TME) for rectal *** From November 2015 to July 2017,26 patients underwent laparoscopic TME for rectal cancer using LPP(6-8 mm Hg) with subcutaneous AWL in Qilu Hospital of Shandong University,Jinan,*** data regarding patients' demographics,intraoperative monitoring indices,operation-related indices andpathological outcomes were prospectively *** Laparoscopic TME was performed in 26 cases(14 anterior resection and 12 abdominoperineal resection) successfully,without conversion to open or laparoscopic surgery with standard-pressure *** monitoring showed stable heart rate,blood pressure and paw airway *** mean operative time was 194.29 ± 41.27 min(range:125-270 min) and 200.41 ± 20.56 min(range:170-230 min) for anterior resection and abdominoperineal resection,*** mean number of lymph nodes harvested was 16.71 ± 5.06(range:7-27).There was no positive circumferential or distal resection *** local recurrence was observed during a median follow-up period of 11.96 ± 5.55 mo(range:5-23 mo).CONCLUSION LPP combined with AWL is safe and feasible for laparoscopic *** technique can provide satisfactory exposure of the operative field and stable operative monitoring indices.

Khubchandani's procedure combined with stapled posterior rectal wall resection for rectocele

BACKGROUND Obstructed defecation syndrome(ODS) is a widespread disease in the *** is the most common cause of ODS in females. Multiple procedures have been performed to treat rectocele and no procedure has been accepted as the gold-standard procedure. Stapled transanal rectal resection(STARR) has been widely used. However, there are still some disadvantages in this procedure and its effectiveness in anterior wall repair is doubtful. Therefore, new procedures are expected to further improve the treatment of *** To evaluate the efficacy and safety of a novel rectocele repair combining Khubchandani's procedure with stapled posterior rectal wall *** A cohort of 93 patients were recruited in our randomized clinical trial and were divided into two different groups in a randomized manner. Forty-two patients(group A) underwent Khubchandani's procedure with stapled posterior rectal wall resection and 51 patients(group B) underwent the STARR ***-up was performed at 1, 3, 6, and 12 mo after the operation. Preoperative and postoperative ODS scores and depth of rectocele, postoperative complications, blood loss, and hospital stay of each patient were documented. All data were analyzed statistically to evaluate the efficiency and safety of our *** In group A, 42 patients underwent Khubchandani's procedure with stapled posterior rectal wall resection and 34 were followed until the final analysis. In group B, 51 patients underwent the STARR procedure and 37 were followed until the final analysis. Mean operative duration was 41.47 ± 6.43 min(group A) vs39.24 ± 6.53 min(group B). Effects of sleeve gastrectomy plus trunk vagotomy compared with sleeve gastrectomy on glucose metabolism in diabetic rats

AIM To investigate the effects of sleeve gastrectomy plus trunk vagotomy(SGTV) compared with sleeve gastrectomy(SG) in a diabetic rat *** SGTV, SG, TV and Sham operations were performed on rats with diabetes induced by high-fat diet and streptozotocin. Body weight, food intake, oral glucose tolerance test, homeostasis model assessment of insulin resistance(HOMA-IR), hepatic insulin signaling(IR, IRS1, IRS2, PI3 K and AKT), oral glucose stimulatedinsulin secretion, GLP-1 and ghrelin were compared at various postoperative *** Both SG and SGTV resulted in better glucose tolerance, lower HOMA-IR, up-regulated hepatic insulin signaling, higher levels of oral glucose-stimulated insulin secretion, higher postprandial GLP-1 and lower fasting ghrelin levels than the TV and Sham groups. No significant differences were observed between the SG and SGTV groups. In addition, no significant differences were found between the TV and Sham groups in terms of glucose tolerance, HOMA-IR, hepatic insulin signaling, oral glucose-stimulated insulin secretion, postprandial GLP-1 and fasting ghrelin levels. No differences in body weight and food intake were noted between the four *** SGTV is feasible for diabetes control and is independent of weight loss. However, SGTV did not result in a better improvement in diabetes than SG alone.

AIM： To study the influence of CXCR4/stromal cell- derived factor-1 （SDF-1） axis on E-cadherin/β-catenin complex expression in HT29 colon cancer ceils and its underlying mechanisms. METHODS： Effect of SDF-1 on E-cadherin/β-catenin expression was detected by immunocytochemistry. E-cadherin and/3-catenin mRNA expression levels were measured by reverse transcriptase-polymerase chain reaction. SDF-l-induced phosphorylation of phosphati- dylinositol 3-kinase （PI3K）/AKT and β-catenin was detected by Western blotting. RESULTS： The E-cadherin and β-catenin mRNA ex-pression levels in HT29 cells were lower 48 h after incubated with SDF-1 at the concentrations of 20 and 40 ng/mL （P 〈 0.05）. SDF-l-induced significant phosphorylation of PI3K/AKT and β-catenin. AMD3100 and LY294002 inhibited the phosphorylation of PI3K/AKT and β-catenin. CONCLUSION： SDF-1 down-regulates the E-cadherin/ β-catenin complex expression in HT29 cells by decreasing mRNA synthesis and increasing β-catenin phosphorylation.

Application of Laparoscopic Extralevator Abdominoperineal Excision in Locally Advanced Low Rectal Cancer

Background：When compared with conventional abdominoperineal resection （APR）,extralevator abdominoperineal excision （ELAPE） has been demonstrated to reduce the risk of local recurrence for the treatment of locally advanced low rectal *** with the laparoscopic technique,laparoscopic ELAPE （LELAPE） has the potential to reduce invasion and hasten postoperative *** this study,we aim to investigate the advantages of LELAPE in comparison with conventional ***：From October 2010 to February 2013,23 patients with low rectal cancer （T3-4N0-2M0） underwent LELAPE;while during the same period,25 patients were treated with conventional *** patient characteristics,intraoperative data,postoperative complications,and follow-up results were retrospectively compared and ***：The basic patient characteristics were similar;but the total operative time for the LELAPE was longer than that of the conventional APR group （P =0.014）.However,the operative time for the perineal portion was comparable between the two groups （P =0.328）.The LELAPE group had less intraoperative blood loss （P =0.022）,a lower bowel perforation rate （P =0.023）,and a positive circumferential margin （P =0.028）.Moreover,the patients,who received the LELAPE,had a lower postoperative Visual Analog Scale,quicker recovery of bowel function （P =0.001）,and a shorter hospital stay （P =0.047）.However,patients in the LELAPE group suffered more chronic perineal pain （P =0.002）,which may be related to the coccygectomy （P =0.033）.Although the metastasis rate and mortality rate were similar between the two groups,the local recurrence rate of the LELAPE group was statistically improved （P =0.047）.Conclusions：When compared with conventional APR,LELAPE has the potential to reduce the risk of local recurrence,and decreases operative invasion for the treatment of locally advanced low rectal cancer.

A regression approach for seismic first-break picking

The picking efficiency of seismic first breaks(FBs)has been greatly accelerated by deep learning(DL)***,the picking accuracy and efficiency of DL methods still face huge challenges in low signal-to-noise ratio(SNR)*** address this issue,we propose a regression approach to pick FBs based on bidirectional long short-term memory(Bi LSTM)neural network by learning the implicit Eikonal equation of 3D inhomogeneous media with rugged topography in the target *** employ a regressive model that represents the relationships among the elevation of shots,offset and the elevation of receivers with their seismic traveltime to predict the unknown FBs,from common-shot gathers with sparsely distributed *** from image segmentation methods which automatically extract image features and classify FBs from seismic data,the proposed method can learn the inner relationship between field geometry and *** addition,the predicted results by the regressive model are continuous values of FBs rather than the discrete ones of the binary *** picking results of synthetic data shows that the proposed method has low dependence on label data,and can obtain reliable and similar predicted results using two types of label data with large *** picking results of9380 shots for 3D seismic data generated by vibroseis indicate that the proposed method can still accurately predict FBs in low SNR *** subsequent stacked profiles further illustrate the reliability and effectiveness of the proposed *** results of model data and field seismic data demonstrate that the proposed regression method is a robust first-break picker with high potential for field application.

Trial application of the envelope method to the potential ambiguity problem

The optical potential ambiguity is a long-standing problem in the analysis of elastic scattering *** a specific collid-ing system,ambiguous potential families can lead to different behaviors in the nearside and farside scattering *** contrast,the envelope method can decompose the experimental data into two components with negative and positive deflection angles,***,a question arises as to whether the comparison between the calculated nearside(or farside)component and the derived positive-deflection-angle(or negative-deflection-angle)component can help analyze the potential ambiguity *** this study,we conducted a trial application of the envelope method to the potential ambiguity *** envelope method was improved by including uncertainties in the experimental *** colliding systems of 16O+28Si at 215.2 MeV and 12C+12C at 1016 MeV were considered in the *** each colliding system,the angular distribution experimental data were described nearly equally well by two potential sets,one of which is“surface transpar-ent”and the other is *** calculated angular distributions were decomposed into nearside and farside scattering *** the improved envelope method,the experimental data were decomposed into the positive-deflection-angle and negative-deflection-angle components,which were then compared with the calculated nearside and farside *** capability of the envelope method to analyze the potential ambiguities was also discussed.
