目的探讨在中国将远程医疗模式应用于阻塞性睡眠呼吸暂停低通气综合征(OSAHS)诊治的可行性。方法纳入2015年10月至2016年9月就诊于北京大学人民医院睡眠中心的24例疑似OSAHS患者,以电子问卷评估和家庭睡眠呼吸暂停监测(HSAt)进行确诊,对确诊患者进行远程自动气道正压通气(APAP)治疗。治疗满1周、1个月和3个月时分别对患者进行视频随访,通过独立建立的数据管理平台由患者填写随访问卷,通过APAP内置的数码卡获取APAP治疗数据和依从性数据。采用线性回归模型探索影响患者依从性的相关因素,采用单因素重复测量方差分析比较不同治疗时点患者APAP使用时间和呼吸暂停低通气指数(AHI)等指标的变化,采用配对t检验比较治疗前后爱泼沃斯嗜睡量表(ESS)评分。结果确诊为OSAHS的患者共22例,其中男性20例(90.9%),年龄为(45.6±10.2)岁,治疗前患者AHI为(46.9±20.4)次/h。20例OSAHS患者接受了APAP治疗,治疗满1周、1个月和3个月时依从性良好患者占比分别为15/19、10/19和8/18。患者治疗前嗜睡严重程度影响依从性,ESS评分每增加1分,依从性(95%CI)增加6.16%(3.01%,9.31%),年龄、体质指数及治疗前AHI对依从性影响均无统计学意义(均P>0.05)。治疗满1周、1个月和3个月患者AHI分别为(2.5±2.1)、(2.2±1.6)和(1.9±1.0)次/h(P=0.195);治疗满3个月患者ESS评分为(7.0±3.3)分,低于治疗前(10.6±3.1)分(P=0.079)。结论远程医疗模式诊治OSAHS的患者依从性和治疗效果好,实际可行。

Academic productivity correlates with industry earnings in foot and ankle fellowship programs in the United States:A retrospective analysis

BACKGROUND the study investigates the connection between academic productivity and industry earnings in foot and ankle orthopedic surgery *** metrics like the H-index and Open Payments Database(OPD)data,it addresses a gap in understanding the relationship between scholarly achievements and financial outcomes,providing a basis for further exploration in this specialized medical *** to elucidate the trends between academic productivity and industry earnings across foot and ankle orthopedic surgery fellowship programs in the United *** this study is a retrospective analysis of the relationship between academic productivity and industry earnings of foot and ankle orthopedic surgery fellowships at an individual faculty and fellowship *** productivity was defined via H-index and recorded from the Scopus *** earnings were recorded from the *** Forty-eight foot and ankle orthopedic surgery fellowships(100%of fellowships)in the United States with a combined total of 165 physicians(95.9%of physicians)were *** individual physician(n=165)total life-time earnings reported on the OPD website was United States Dollar(USD)451430.30±1851084.89(range:USD 25.16-21269249.85;median:USD 27839.80).Mean physician(n=165)H-index as reported on Scopus is 14.24±12.39(range:0-63;median:11).there was a significant but weak correlation between individual physician H-index and individual physician total life-time earnings(Pt and moderate positive correlation between combined fellowship H-index and total life-time earnings per fellowship(P=0.004,Spearman’s rho=0.409).CONCLUSION there is a significant and positive correlation between academic productivity and industry earnings at foot and ankle orthopedic surgery fellowships in the United *** observation is true on an individual physician level as well as on a fellowship level.

We study the acoustomagnetoelectric (AME) effect in two-dimensional graphene with an energy bandgap using the semiclassical Boltzmann transport equation within the hypersound regime, (where represents the acoustic wavenumber and is the mean free path of the electron). the Boltzmann transport equation and other relevant equations were solved analytically to obtain an expression for the AME current density, consisting of longitudinal and Hall components. Our numerical results indicate that both components of the AME current densities display oscillatory behaviour. Furthermore, geometric resonances and Weiss oscillations were each defined using the relationship between the current density and Surface Acoustic Wave (SAW) frequency and the inverse of the applied magnetic field, respectively. Our results show that the AME current density of bandgap graphene, which can be controlled to suit a particular electronic device application, is smaller than that of (gapless) graphene and is therefore, more suited for nanophotonic device applications.

Chemical insecticides have been considered as a means to combat crop pests. Although their effectiveness is evident, their impact on the environment is increasingly being discussed. the aim of this study is to determine the agro-ecological potential of a biological insecticide (C25H32O12) based on Aloe barbadensis in a Sahelian context. For this purpose, a completely randomized block experimental design with 3 replications and 4 treatments was set up to experiment with Aloe barbadensis as a bioinsecticide against pests of Abelmoschus esculentus. However, data were collected using an observation and parameter monitoring grid. this includes the cultivation of Abelmoschus esculentus, soil preparation, seeding and watering, plot labeling, preparation of the bioinsecticide (selection and preparation of raw materials, grinding of Aloe barbadensis miller and extraction of the crude bioinsecticide, quantification of treatment doses and dilution, and obtaining the formulated bioinsecticide), plant watering, plant treatment, and finally parameter monitoring. the results obtained reveal that the level of damage is significantly high in the control treatment t0 (63%) compared to the other treatments, with 29% for treatment t1, 7% for t2, and 1% for t3, implying a strong action capability of this insecticide against pests of Abelmoschus esculentus. therefore, it can be concluded that for a normal growing season of Abelmoschus esculentus, this biological insecticide should be sprayed 12 times. Furthermore, this biological insecticide is unique in that it does not inflict any gastric toxicity on the pests, which gives it the characteristic of being a repellent. It is a biological insecticide whose efficacy period has been tested, with a minimum duration of 21 days. In conclusion, this formulated bioinsecticide based on Aloe barbadensis demonstrates significant efficacy against pests of Abelmoschus esculentus. In the future, we will consider experim
