Major comorbid disease processes associated with increased incidence of acute kidney injury

Acute kidney injury （AKI） is commonly seen amongst critically ill and hospitalized patients. Individuals with certain co-morbid diseases have an increased risk of developing AKI. Thus, recognizing the co-morbidities that predispose patients to AKI is important in AKI prevention and treatment. Some of the most common co-morbid disease processes that increase the risk of AKI are diabetes, cancer, cardiac surgery and human immunodefciency virus （HIV） acquired immune defciency syndrome （AIDS）. This review article identifies the increased risk of acquiring AKI with given co-morbid diseases. Furthermore, the pathophysiological mechanisms underlying AKI in relation to co-morbid diseases are discussed to understand how the risk of acquiring AKI is increased. This paper reviews the effects of various co-morbid diseases including： Diabetes, cancer, cardiovascular disease and HIV AIDS, which all exhibit a significant increased risk of developing AKI. Amongst these co-morbid diseases, inflammation, the use of nephrotoxic agents, and hypoperfusion to the kidneys have been shown to be major pathological processes that predisposes individuals to AKI. The pathogenesis of kidney injury is complex, however, effective treatment of the co-morbid disease processes may reduce its risk. Therefore, improved management of co-morbid diseases may prevent some of the underlying pathology that contributes to the increased risk of developing AKI.

The cancellable biometric transformations are designed to be computationally difficult to obtain the original biometric *** paper presents a cancellable multi-biometric identification scheme that includes four stages:biometric data collection and processing,Arnold’s Cat Map encryption,decimation process to reduce the size,and finalmerging of the four biometrics in a single generated ***,a 2D matrix of size 128×128 is created based on Arnold’s Cat Map(ACM).The purpose of this rearrangement is to break the correlation between pixels to hide the biometric patterns and merge these patterns together for more *** decimation is performed to keep the dimensions of the overall cancellable template similar to those of a single template to avoid data ***,some sort of aliasing occurs due to decimation,contributing to the intended distortion of biometric *** hybrid structure that comprises encryption,decimation,andmerging generates encrypted and distorted cancellable *** simulation results obtained for performance evaluation show that the system is safe,reliable,and feasible as it achieves high security in the presence of noise.

Spectrofluorimetric method for determination of some angiotensin II receptor antagonists

A simple, rapid, accurate and highly sensitive spectrofluorimetric method has been developed for determination of some angiotensin II receptor antagonists (AIIRA’s), namely Losartan potassium (Los-K), Irbesartan (Irb), Valsartan (Val) and Candesartan cilexetil (Cand) in pure forms as well as in their pharmaceutical dosage forms. All the variables affecting the relative fluorescence intensity (RFI) were studied and optimized. Under the optimum conditions, linear relationships with good correlation coefflcients (0.9982–0.9991) were obtained over the concentration range from 0.006 mg/mL to 1.7 mg/mL. Good accuracy and precision were successfully obtained for the analysis of tablets containing each drug alone or combined with hydrochlorothiazide (HCTZ) without interferences from the co-formulated HCTZ or the additives commonly present in tablets.

Cytokine and apoptosis gene polymorphisms influence the outcome of hepatitis C virus infection

BACKGROUND:Hepatitis C virus (HCV) infection is thought to be chronic and the factors leading to viral clearance or persistence are poorly *** study was undertaken to investigate the possibility of a significant relationship between the spontaneous clearance or the persistence of hepatitis C virus (HCV) infection and cytokine and apoptosis gene polymorphisms in Tunisian patients on ***:Polymorphisms of the genes IL-1 (-889 IL-1α,-511 and +3954 IL-1β,IL-1Ra),IL-18 (-137 and-607),IL-12 (-1188) and Apo1/Fas (-670) were determined by PCR-RFLP,PCR-SSP and PCR-VNTR in 100 healthy blood donors and 100 patients infected with HCV and undergoing *** patients were classified into two groups:G1 consisted of 76 active chronic hepatitis patients (positive for HCV RNA) and G2 consisted of 24 hemodialysed patients who spontaneously eliminated the virus (negative for HCV RNA).RESULTS:The frequency of genotype association [-137GC/-607CA] IL-18 was higher in G2 (41.7%) than in G1 (15.8%) (P=0.008;OR=0.26;95% CI,0.10-0.73).We also found a higher frequency of the AA genotype of the Apo1/Fas gene in G2 (41.6%) than in G1 (17.5%) (P=0.026;OR=3.49;95% CI,1.13-10.69).Adjustment for known covariate factors (age,gender and genotype) confirmed these univariate findings and revealed that the genotype association GC-CA of the (-137 and-607) IL-18 gene and the AA genotype of the Apo1/Fas gene were associated with the clearance of HCV (P=0.041 and 0.017,respectively).CONCLUSION:The two genotypes GC-CA of the (-137 and-607) IL-18 polymorphism and the AA genotype of the Apo1/Fas gene influence the outcome of HCV infection in Tunisian patients on hemodialysis.

Knowledge, attitude and practice of patients with type 2 diabetes mellitus towards complementary and alternative medicine

OBJECTIVE： The use of complementary and alternative medicine（CAM） is relatively common among patients with type 2 diabetes mellitus（T2DM） around the world. Like many other countries, Saudi Arabia has a high incidence of T2DM, and incorporates the use of CAM in its treatment. This study explores the knowledge, attitude and practice（KAP） of CAM therapies among patients with T2DM and explores the relationships between sociodemographics and the KAP of CAM modalities. METHODS： This is a hospital-based, observational, cross-sectional study that selected 302 outpatients with T2DM, from Diabetic Center of King Salman bin Abdul-Aziz Hospital, in Riyadh city, in May 2014. Patients were interviewed using a predesigned questionnaire. RESULTS： The mean age of patients was（51.6 ± 10.6） years and 43.4% of them were males. The prevalence of CAM practices was 30.5%; 30.39% of them used herbs, 20.58% used wet cupping and 17.64% used nutritional supplements and other therapies. The factors found to predict the use of CAM therapies among patients with T2DM were： age above 51 years, unemployment and the participants’ knowledge about the effectiveness of CAM products. CONCLUSION： This preliminary study estimated the prevalence of CAM use, revealed positive attitude towards CAM and found three significant predictors of CAM use among patients with T2DM, which is consistent with regional data. A community-based research with a larger sample that targets T2DM population is needed in Saudi Arabia.

Multiresolution Video Watermarking Algorithm Exploiting the Block-Based Motion Estimation

This paper presents a novel technique for embedding a digital watermark into video frames based on motion vectors and discrete wavelet transform (DWT). In the proposed scheme, the binary image watermark is divided into blocks and each watermark block is embedded several times in each selected video frame at different locations. The block-based motion estimation algorithm is used to select the video frame blocks having the greatest motion vectors magnitude. The DWT is applied to the selected frame blocks, and then, the watermark block is hidden into these blocks by modifying the coefficients of the Horizontal sub-bands (HL). Adding the watermark at different locations in the same video frame makes the scheme more robust against different types of attacks. The method was tested on different types of videos. The average peak signal to noise ratio (PSNR) and the normalized correlation (NC) are used to measure the performance of the proposed method. Experimental results show that the proposed algorithm does not affect the visual quality of video frames and the scheme is robust against a variety of attacks.

The Effect of Simulation-Based Training on Nursing Students’ Communication Skill, Self-Efficacy and Clinical Competence for Nursing Practice

Background: Simulation-based training is a new strategy in teaching that gives the students good opportunities to learn and apply what they learn in nursing care safely. Aim: This study conducted to evaluate the effects of simulation-based training on nursing students’ communication skill, self-efficacy and clinical competence in practice. Subjects and Methods: Quiz-experimental design was used in this study (pre-posttest intervention), it was carried out on 100 nursing students first semester in 2019 using low and high-fidelity simulators. This study was carried out at College of Applied Medical Sciences-Bshia University. Data Collection: demographic data, communication skill, self-efficacy and clinical competence questionnaires. Analysis is done by SPSS version 20 software. Results: Participants who received the simulation-based training, showed statistical significant improvement in communication skill, self-efficacy, and clinical competence scores after participation in the simulation program (t = −32.64, p = 0.001;t = −19.9, p = 0.001;16.4, p = 0.001). Also, there are significant relation between gender and clinical competency (t = 2.768, p Conclusion: Simulation-based training in medical courses is effective in enhancing communication skill, self-efficacy and clinical competence. Multiple-patient simulations as a teaching-learning strategy in the nursing curriculum are highly recommended.

A Descriptive Cross-Sectional Study to Assess the Perception and Knowledge of Staff Nurses regarding Physical Restrains

Purpose: The purpose of this study was to examine the perceptions and knowledge regarding physical restraint used among registered nurses (RNs) and nursing assistants (NAs). Method and Sample: A descriptive cross-sectional design was used. A convenient sample was recruited from nursing staff from Palestinian and Saudi Hospitals. Perceptions of Restraint Use Questionnaire (PRUQ) were used [1]. Results: A total of 144 nursing staffs were included. Physical restraint use was perceived as more important in some circumstances than others. In the critical care unit/ICU, the most important perceived reason for physical restraint use was protecting from falling out of bed and preventing removing dressing. There were significant differences of overall score for the PRUQ among all nursing staff according to hospital, t (142) = 8.74, P = 0.001. The mean of Saudi Arabia hospitals group (M = 4.56) was higher than the mean of Palestinian hospital group (M = 3.67). At the same time, there were significant differences of overall score for the PRUQ among all nursing staff according to specialized education in geriatrics, t (40) = 3.60, P = 0.001. The mean of no specialized education in geriatrics group (M = 4.13) was higher than the mean of yes specialized education in geriatrics group (M = 3.69). Conclusion and Implications: Nursing staff showed positive attitudes towards restrains application with difference between Palestinian and Saudi Arabian nurses. Proper implementation and good awareness of the complications are prerequisite for application of physical restrains for patients.
