Variability of Serum Concentration of Calcium, Phosphate and Parathyroid Hormone Depending on Time of Blood Draw for Patients on Nocturnal Home Hemodialysis

Background: Guidelines for patients treated with conventional hemodialysis patients have been written for target serum levels for calcium (Ca), phosphate (PO4) and intact parathyroid hormone (iPTH). No guidelines exist for nocturnal home hemodialysis (NHHD) patients for target values or timing of the blood sample draw. We undertook a prospective cohort study to examine the variability in pre, post and clinic (post-post) serum values for Ca, PO4, and iPTH in NHHD patients to determine if timing of blood draw could affect clinical decisions. Methods: Twenty prevalent NHHD patients collected blood pre and post their usual NHHD session with an additional blood sample drawn in clinic (post-post). Median and interquartile range of pre, post and clinic (post-post) values of iPTH, PO4 and Ca were calculated and compared with Freidman/Wilcoxon test. Serum concentrations were also categorized according to Canadian Society of Nephrology (CSN) guidelines target values for pre and clinic (post-post) samples. The proportion of patients that would be categorized differently by clinic (post-post) samples was determined. Results: There was a significant difference between pre-serum values compared to post and clinic (post-post) values. Overall, iPTH, PO4 and Ca values would be misclassified in 25%, 70% and 50% respectively if blood was drawn at the clinic visit (post-post) compared to pre-HD as per CSN guidelines. Conclusions: Although no specific guideline has been written for NHHD patients, to ensure consistency of management compared to in-centre HD patients, lab values should be drawn pre-HD until clinical evidence suggests that the recommendations should be different for NHHD.

MR Visualization of Spinal Dural Arterio-Venous Fistula Using T2-Weighted 3D SPACE—A Spin-Echo Technique

Spinal dural arteriovenous fistulas (SDAVF) account for about 70% of all spinal AV shunts. We re-port on a 56-year-old male patient with progressive neurological symptoms. Pre- and post-contrast MR imaging of the spine, including contrast-enhanced MR angiography was done on a 1.5 T MR scanner. In addition a special (pre-contrast) 3D turbo-spin-echo sequence with variable flip angle evolution (SPACE) was performed providing a slice thickness of 0.9 mm. T2-weighted images showed extensive widened and tortuous vascular structures with typical flow voids intradurally due to dilated pial veins. The origin of the spinal dural arteriovenous fistula was found to be the radiculomeningeal artery at level D10/11 on the left side, visualized by the T2-weighted 3D SPACE sequence and CE MRA. After imaging the patient was referred to an interventional neuroradiology department for therapy. With the detailed information from MR imaging concerning the fistula level the interventional radiologist was able to find the fistula immediately and consequently treated it successfully by endovascular therapy. Interpreting the MR images of this obvious case of a SDAVF, SPACE sequence after post processing with 3D MPR software directly led to the location of the AV shunt at D10/11 on the left side, better than 3D contrast-enhanced MR angiography did. Therefore, SPACE can be a useful adjunct in further evaluation of spinal dural arteriovenous fistula and especially in defining fistula level.

Cerebral state index versus Glasgow coma scale as a predictor for in-hospital mortality in brain-injured patients

Objective：To compare the value of Glasgow coma scale （GCS） and cerebral state index （CSI）on predicting hospital discharge status of acute braininjured ***：In 60 brain-injured patients who did not receive sedatives,GCS and CSI were measured daily during the first 10 days of *** outcome of prognostic cut-off points was calculated by GCS and CSI using receiver operating characteristic （ROC） curve regarding the time of admission and third day of ***,specificity and other predictive values for both indices were ***：Of the 60 assessed patients,14 patients had mild,13 patients had moderate and 33 patients had severe *** the course of the study,17 patients （28.3％） deteriorated in their situation and *** mean GCS and CSI in patients who deceased during hospitalization was significantly lower than those who were discharged from the ***＜4.5 and CSI＜64.5 at the time of admission was associated with higher mortality risk in traumatic brain injury patients and GCS was more sensitive than CSI to predict in-hospital death in these *** the first day of hospitalization,the area under ROC curve was 0.947 for GCS and 0.732 for ***：GCS score at ICU admission is a good predictor of in-hospital ***＜4.5 and CSI＜64.5 at the time of admission is associated with higher mortality risk in traumatic brain injury patients and GCS is more sensitive than CSI in predicting death in these patients.

Determination of water equivalent ratio for some dosimetric materials in proton therapy using MNCPX simulation tool

The water equivalent ratio(WER) was calculated for polypropylene(PP), paraffin, polyethylene(PE), polystyrene(PS), polymethyl methacrylate(PMMA), and polycarbonate materials with potential applications in dosimetry and medical physics. This was performed using the Monte Carlo simulation code, MCNPX, at different proton energies. The calculated WER values were compared with National Institute of Standards and Technology(NIST) data, available experimental and analytical results,as well as the FLUKA, SRIM, and SEICS codes. PP and PMMA were associated with the minimum and maximum WER values, respectively. Good agreement was observed between the MCNPX and NIST data. The biggest difference was 0.71% for PS at 150 MeV proton energy. In addition, a relatively large positive correlation between the WER values and the electron density of the dosimetric materials was observed. Finally, it was noted that PE presented the most analogous Depth Dose Characteristics to liquid water.

目的:免疫系统涉及炎症的很多病因学及病理学机制。药用植物中的很多成分被认为具有非特异性的免疫调节作用。基于此并且根据白细胞介素6在炎症和疼痛产生中的作用,本研究探讨桑托利纳蓍(Achilleasantolina)的提取物对弗氏佐剂致炎症大鼠的作用。方法:提取桑托利纳蓍地上部分的甲醇提取物和脱脂提取物用于本次研究。通过辐射热测量和器官充盈度测量器分别衡量弗氏佐剂致炎症大鼠不同实验天数后的痛觉敏感程度和足肿胀程度,并使用大鼠白细胞介素酶联免疫吸附试剂盒测量大鼠血清白细胞介素6的水平。结果:桑托利纳蓍的甲醇提取物减少炎症大鼠足肿胀、降低痛觉敏感程度和血清白细胞介素6水平呈量效关系。剂量为200 mg/kg的桑托利纳蓍甲醇提取物在预处理和短期治疗期间具有明显的抗痛觉过敏和抗炎作用,400和200 mg/kg剂量的治疗作用没有明显差别。桑托利纳蓍的脱脂提取物对于弗氏佐剂所导致的大鼠炎症反应在实验的各个阶段均未显示出明显的治疗作用(P>0.05)。短期治疗期间,200 mg/kg的桑托利纳蓍甲醇提取物在减少炎症大鼠足肿胀、降低痛觉敏感程度和血清白细胞介素6水平方面均优于阳性对照药消炎痛(P<0.01,P<0.01,P<0.05)。结论:桑托利纳蓍的甲醇提取物预处理用药和短期用药均有明显的抗炎和免疫调节作用。

Costs estimation of drilling for new wells is one of the main concerns of active companies in this *** some of features of drilling can not be expressed quantitatively and there are many qualitative features in data of the available *** a method should be applied to use these data to estimate the desired and ideal output of project *** case based reasoning （CBR） method covers the qualitative data with regard to its *** CBR method which is created based on the viewpoint of using presented solutions for previous solved problems in order to solve new similar problem save time and therefore speed of drilling is increased which it is very important,as regards available estimation methods spend the much time to estimate output which is cost of drilling wells in this case *** results show that obtained estimation accuracy is high.

Comparison of in vivo antiseptic and in vitro antimicrobial effects of Peganum harmala *** ethanolic extract with Betadine

Objective:To compare the antibacterial activity and antiseptic effect of ethanol extract of Peganum harmala L.(***)seeds and Betadine on eight reference strains of Streptococcus pneumoniae,Salmonella typhimurium,Escherichia coli,Corynebacterium pseudotuberculosis,Klebsiella pneumoniae,Staphylococcus aureus,Micrococcus luteus which are known to cause different types of skin infections,and Corynebacterium pseudotuberculosis isolated from an abscess on horse's ***:The antibacterial activity was assessed using a disc diffusion *** minimum inhibitory concentration was tested by serial dilution method and the percentage of bacterial growth inhibition by absorbance microplate reader *** minimum bactericidal concentration was then *** outcomes were obtained through washed up healing time of longitudinal and surface skin on the back of 16 rats with concentrations of 10,50,100,150 and 500 mg/mL of *** ***:The results of in vitro experiments showed that the lowest minimum inhibitory concentration(0.68-1.3 mg/mL)and minimum bactericidal concentration(1.3-5 mg/mL)values were observed in the ethanol extract of *** *** the results of in vivo experiment showed that wound healing in the concentration of 50 mg/mL of this plant extract was better and quicker than ***:This study confirmed that ethanol extract of *** has appropriate effect on the microorganisms and the healing of skin wounds in comparison with Betadine.

Efficacy of IMOD in the treatment of oral lichen planus—Efficacy of IMOD in oral lichen planus

Objectives: Oral lichen planus (OLP) is a chronic, immunologically mediated, mucocutaneous disorder. A wide range of topical and systemic therapies have been used in the treatment of OLP. The efficacy of IMOD (an Iranian new immunomodulator drug, con- taining selenium, carotene, and flavonoids) in the management of oral lichen planus was evaluated. Study design: In a before-after clinical trial study, thirty patients (21 women and 9 men;age range 35 - 66 years with 112 lesions) with lichen planus were enrolled. The study covered a three-month period of therapy by IMOD (400 mg/day) and a three-month follow-up period after drug cessation. Outcome mea- sures include soreness relief based on the “nu-meric scale”, and clinical improvement of lesion size and score. Saliva levels of TNF-α was analysed at the baseline and after treatment by ELISA. Statistical analysis of Wilcoxon and paired student’s t-test were used. Results: Approximately 85% of patients showed partial to complete improvement and re-mained sym- ptom free after drug cessation. There was no signifi-cant difference in mean saliva TNF-α level before and after the treatment. Conclusion: These results suggest that IMOD seems to be an effective alternative treat- ment for OLP and TNF-α may not be a good indica- tor for monitoring therapeutic response of OLP.

CT and MR Imaging Findings in the Joubert Syndrome,a“Ciliopathy”

A 7-year-old boy presented with cerebellar ataxia with reduced tonicity, deficits of the fine and gross motor coordination skills and vestibular stimulus processing, as well as significantly delayed language development. MR imaging showed the so-called “molar tooth sign”, which was highly pathognomonic for the Joubert-Syndrome—an inherited cerebellar ataxia with a variety of clinical symptoms—and related entities. It is caused by a complex malformation of the cerebellar vermis and the midbrain. The cerebellar vermis is hypoplastic or completely absent;at the same time, the superior cerebellar peduncles are thickened. There is a lack of normal decussation of the fiber tracts in mesencephalon, which follow an abnormal horizontal course, as well as a lack of the decussation of the corticospinal fiber tracts in the caudal medulla oblongata and deformity of the 4th ventricle. Clinically, the triad of cerebellar ataxia, developmental retardation, and abnormal eye movements is indicating a related syndrome of this spectrum. The appearance of the involved children is characterized by dysmorphic facial features with epicanthus, broad nose bridge, low set ears and typically triangularly shaped and opened mouth. The diagnosis is usually made by imaging and clinical findings. Recently, advantages were made in genetic research on the Joubert syndrome and interesting findings published about diffusion tensor imaging and tractography. However, standard MR imaging, applying an adequate imaging protocol including sequences with excellent T1 contrast and 3D imaging with isotropic spatial resolution allowing reconstructions in all orientations, remains an essential tool for making this diagnosis.
