Endoscopic retrograde cholangiography for pediatric choledocholithiasis: Assessing the need for endoscopic intervention

AIM:To assess pediatric patients for *** applied current adult guidelines to identify predictivefactors in ***:A single-center retrospective analysis was performed at a tertiary children's *** evaluated 44 consecutive pediatric patients who underwent endoscopic retrograde cholangiography(ERcP) for suspected *** were stratified into those with common bile duct stones(cBDs) at ERcP vs those that did not using the American society of Gastrointestinal Endoscopy(AsGE) guidelines(Very strong and strong criteria) for suspected ***:cBDs were identified in 84% at the time of *** ultrasound identified cBDs in 36% of *** bilirubin ≥ 0.5 mg/d L was an independent risk factor for cBDs(P = 0.003).The Very strong(59.5%) and strong(48.6%) AsGE criteria identified the majority of patients(P = 0.0001).A modified score using conjugated bilirubin had a higher sensitivity(81.2% vs 59.5%) and more likely to identify a stone than the standard criteria,odds ratio of 25.7 compared to *** aminotransferase and gamma-glutamyl transferase values identified significant differences in a subset of patients with odds ratio of 4.1 and 3.25,***:current adult guidelines identified the majority of pediatric patients with cBDs,but specific pediatric guidelines may improve detection,thus decreasing risks and unnecessary procedures.

AIM To assess magnetic resonance imaging(MRI) and faecal calprotectin to detect endoscopic postoperative recurrence in patients with crohn's disease(cD).METHODs from two tertiary centers, all patients with cD who underwent ileocolonic resection were consecutively and prospectively included. All the patients underwent MRI and endoscopy within the first year after surgery or after the restoration of intestinal continuity [median = 6 mo(5.0-9.3)]. The stools were collected the day before the colonoscopy to evaluate faecal calprotectin level. Endoscopic postoperative recurrence(POR) was defined as Rutgeerts' index ≥ i2b. The MRI was analyzed independently by two radiologists blinded from clinical *** Apparent diffusion coefficient(ADc) was lower in patients with endoscopic POR compared to those with no recurrence(2.03 ± 0.32 vs 2.27 ± 0.38 × 10^(-3) mm^2/s, P = 0.032). clermont score(10.4 ± 5.8 vs 7.4 ± 4.5, P = 0.038) and relative contrast enhancement(RcE)(129.4% ± 62.8% vs 76.4% ± 32.6%, P = 0.007) were significantly associated with endoscopic POR contrary to the magnetic resonance index of activity(Ma ria)(7.3 ± 4.5 vs 4.8 ± 3.7; P = 0.15) and MR scoring system(P = 0.056). ADc s [sensitivity = 0.85, specificity = 0.65, positive predictive value(PPV) = 0.85, negative predictive value(NPV) = 0.65] and RcE > 100%(sensitivity = 0.75, specificity = 0.81, PPV = 0.75, NPV = 0.81) were the best cutoff values to identify endoscopic POR. clermont score > 6.4(sensitivity = 0.61, specificity = 0.82, PPV = 0.73, NPV = 0.74), Ma ria > 3.76(sensitivity = 0.61, specificity = 0.82, PPV = 0.73, NPV = 0.74) and a MR scoring system ≥ MR1(sensitivity = 0.54, specificity = 0.82, PPV = 0.70, and NPV = 0.70) demonstrated interesting performances to detect endoscopic POR. faecal calprotectin values were significantly higher in patients with endoscopic POR(114 ± 54.5 μg/g vs 354.8 ± 432.5 μg/g; P = 0.0075). faecal calprotectin > 100 μg/g demonstrated high perfor

The Meconial Aspiration syndrome (MAs) is a complication caused by the presence of meconium amniotic fluid with consequent aspiration, being responsible for important rates of perinatal morbidity and mortality. This study aims to analyze the bibliographical production on newborn care with meconium elimination in the delivery room. It was a literature integrative review electronically based. Most of the studies referred to tracheal intubation and exogenous surfactant ministration as main care methods. The Meconium Aspiration syndrome is presented as main theme. It is understood that health care professionals must develop knowledge, skills and actions, showing conduct based on scientific, critical and reflexive knowledge with purposeful interventions directed to the patient’s needs.

strong>Objectivestrong>: To analyze the evidence in the evolution of scientific production on the factors associated with curative practices of the umbilical stump. Materials and Methods: The study data collection took place in April 2017. The electronic databases used to search the articles were: PubMed (National Library of Medicine and National Institutes of Health) and VHL (Virtual Health Library). We used the keywords “perinatal care”, “umbilical cord”, “patient care team”, “neonatology”, “newborn”, in the Portuguese, English and Spanish languages, accompanied by the expression AND and selected through DeCS (Descriptors in Health Sciences). Results: A final sample of 9 articles was obtained from the PubMed and VHL databases, which met the inclusions criteria. It has been found that traditional umbilical stump care practices are still present in many communities, however, have been replaced by appropriate practices, the most recommended being: chlorhexidine 4% sanitation, soap and water and alcohol antisepsis 70%. Conclusions: There is a need for multi-professional involvement, approaching these women since pregnancy, with detailed information and accessible language about proper care with the newborn and the umbilical stump.span>

The Anonnaceae family has 2,500 species and 135 genera, which has about 14 representatives of species in Brazil,including Pseudoxandra cuspidata. The objective of this research was to study the aqueous-acid extract of P. cuspidata, evaluatingits antioxidant, larvicidal, antimicrobial and cytotoxic potential. In the preliminary phytochemical analysis, there were positivity toorganicacids and alkaloids. Inhibitory Activity of EAA was 65% against DPPH. The AAE presented to A. salina nauplii at Lc50%of 600.79 μg/mL for a periodof 24 hours. In relation to larvicidal activity to Aedes aegypti intermediates, AAE presented the Lc50%in 24 Hours of 475.91 ppm and 290.73 ppm in 48 Hours. Regarding the antimicrobial action, the EAA presented 50 mg/mL MIc forP. Aeruginosa and MBc at 100 mg/mL, and for s. aureus it presented MIc of 50 mg/mL and did not present MBc. This researchcharacterized the presence of alkaloids and organicacids present in the aqueousacid extract of P. cuspidata. The extract presentedlowtoxicity in relation to the microcrustacean A. salina. It also had a relevant antioxidant potential (65% of Inhibition),larvicidalaction considered effective, antimicrobialaction for both P. Aeruginosa and s. Aureus even did not present bactericidalaction.
