New therapeutic strategy with extracorporeal membrane oxygenation for refractory hepatopulmonary syndrome after liver transplant: A case report

BACKGROUND Due to the lack of published literature about treatment of refractory hepatopulmonary syndrome(Hps)after liver transplant(LT),this case adds information and experience on this issue along with a treatment with positive *** is a complication of end-stage liver disease,with a 10%-30%incidence in cirrhotic *** can reverse the physiopathology of this process and restore normal ***,in some cases,refractory hypoxemia persists,and extracorporeal membrane oxygenation(ECMO)can be used as a rescue therapy with good *** sUMMARY A 59-year-old patient with alcohol-related liver cirrhosis and portal hypertension was included in the LT waiting list for *** had good liver function(Model for End-stage Liver Disease score 12,Child-pugh class B7).He had pulmonary fibrosis and a mild restrictive respiratory pattern with a basal oxygen saturation of 82%.The macroaggregated albumin test result was>*** demonstrated a forced expiratory volume in one second(FEV1)of 78%,forced vital capacity(FVC)of 74%,FEV1/FVC ratio of 81%,diffusion capacity for carbon monoxide of 42%,and carbon monoxide transfer coefficient of 57%.He required domiciliary oxygen at 2 L/min(16 h/d).The patient was admitted to the intensive care unit(ICU)and extubated in the first 24 h,needing high-flow therapy and non-invasive ventilation and inhaled nitric oxide *** was needed after 72 *** to the non-response to supportive therapies,installation of ECMO was decided with progressive recovery after 9 *** was possible on the tenth day,maintaining a high-flow nasal cannula and de-escalating to conventional oxygen therapy after 48 *** was discharged from ICU on postoperative day(pOD)20 with a 90%-92%oxygen *** recycling was needed twice for acute *** patient was discharged from hospital on pOD 27 with no symptoms,with an 89%-90%oxygen *** Due to the favorable results observed,ECMO could bec

This work explored possibilities of biodegradable magnesium alloy Mg-3Y preparation by two modern powder metallurgy techniques–spark plasma sintering(sps)and selective laser melting(sLM).The powder material was consolidated by both methods utilising optimised parameters,which led to very low porosity(∼0.3%)in the sLM material and unmeasurably low porosity in the sps *** main aim of the study was the thorough microstructure characterisation and interrelation between the microstructure and the functional properties,such as mechanical strength,deformability,and corrosion *** materials showed comparable strength of∼110 Mpa in tension and compression and relatively good deformability of∼9%and∼21%for the sLM and sps materials,*** corrosion resistance of the sps material in 0.1 M NaCl solution was superior to the sLM one and comparable to the conventional extruded *** digital image correlation during loading and the cross-section analysis of the corrosion layers revealed that the residual porosity and large strained grains have the dominant negative effect on the functional properties of the sLM *** the other hand,one of the primary outcomes of this study is that the sps consolidation method is very effective in the preparation of the W3 biodegradable alloy,resulting in material with convenient mechanical and degradation properties that might find practical applications.

Direct-to-indirect mapping for optimal low-thrust trajectories

Optimal,many-revolution spacecraft trajectories are challenging to solve.A connection is made for a class of models between optimal direct and indirect *** transfers that minimize thrust-acceleration-squared,primer vector theory maps direct,many-impulsive-maneuver trajectories to the indirect,continuous-thrust-acceleration *** mapping algorithm is independent of how the direct solution is obtained and requires only a solver for a boundary value problem and its partial derivatives.A Lambert solver is used for the two-body problem in this *** mapping is simple because the impulsive maneuvers and co-states share the same linear space around an optimal *** numerical results,the direct coast-impulse solutions are demonstrated to converge to the indirect continuous solutions as the number of impulses and segments *** two-body design space is explored with a set of three many-revolution,many-segment examples changing semimajor axis,eccentricity,and *** first two examples involve a small change to either semimajor axis or eccentricity,and the third example is a transfer to geosynchronous *** a single processor,the optimization runtime is seconds to minutes for revolution counts of 10 to 100,and on the order of one hour for examples with up to 500 *** of these thrust-acceleration-squared solutions are good candidates to start a homotopy to a higher-fidelity minimization problem with practical constraints.

Cross-method Analysis of Corotation Radii Data set for spiral Galaxies

A corotation radius is a key characteristic of disk galaxies that is essential to determine the angular speed of the spiral structure Ωp, and therefore understand its nature. In the literature, there are plenty of methods to estimate this value, but do these measurements have any consistency? In this work, we collected a data set of corotation radius measurements for 547 galaxies, 300 of which had at least two values. An initial analysis reveals that most objects have rather inconsistent corotation radius positions. Moreover, a significant fraction of galactic disks is distinguished by a large error coverage and almost uniform distribution of measurements. These findings do not have any relation to spiral type, Hubble classification, or presence of a bar. Among other reasons, obtained results could be explained by the transient nature of spirals in a considerable part of galaxies. We have made our collected data sample publicly available, and have demonstrated on one example how it could be useful for future research by investigating a winding time value for a sample of galaxies with possible multiple spiral arm patterns.

Non-participation of asymptomatic candidates in screening protocols reduces early diagnosis and worsens prognosis of colorectal cancer

The Agatsuma et al’s study shows that despite the evidence of the benefits of an early colorectal cancer(CRC)diagnosis,through screening in asymptomatic subjects,up to 50%of candidates reject this option and many of those affected are diagnosed later,in advanced *** efficacy of screening programs has been well-established for several years,which reduces the risk of CRC morbidity and mortality,without taking into account the test used for screening,or other ***,a significant proportion of patients remain unscreened,so understanding the factors involved,as well as the barriers of the population to adherence is the first step to possibly modify the participation *** barriers could include a full range of social and political aspects,especially the type of financial provision of each health *** Japan,health services are universal,and this advantageous situation makes it easier for citizens to access to these services,contributing to the detection of various diseases,including ***,the symptomatic CRC group had a lower early-stage diagnosis rate than the patients detected during follow-up for other comorbidities,and symptomatic and cancer screening groups showed similar early-stage diagnosis.
