Impact of Maintenance steroids versus Rapid steroid Withdrawal in African-American Kidney Transplant Recipients: Comparison of Two Urban Centers

Background: Rapid steroid withdrawal (RsW) is used increasingly in kidney transplantation but long-term outcomes in African-American (AA) recipients are not well known. We compared 1 and 5 year transplant outcomes in a large cohort of AA patients who were maintained on continued steroid therapy (CsT) to those who underwent RsW. Methods: Post-transplant courses of A as receiving kidney allografts from 2003-2011 at two urban transplant centers in Chicago were followed. Prior to outcome analysis, we used Inverse Probability of Treatment Weights (IPTW) to match the two groups on a set of baseline risk factors. Graft and patient survival, GFR at 1 and 5 years, incidence and type of rejection, incidence of post-transplant diabetes mellitus (PTDM), delayed graft function, CMV and BK viremia were compared. Results: There were 150 AA recipients in the CsT analytic group and 157 in the RsW analytic group. Graft and patient survival was similar between the two groups. Rates of CMV viremia were higher in the RsW compared to the CsT analytic group at 1 year. Biopsy-proven acute rejection and PTDM were similar between the RsW and CsT groups. Conclusions: In AA recipients, RsW has similar long-term outcomes to CsT.

Mistaking statistics for Rigor in American Business school Research： should China Follow, or Lead in a New Direction？

The heavy emphasis on statistical rigor that business schools in American universities place on research is questionable in general and, for business schools in China, is not necessarily a good strategy by which either to produce useful research or to distinguish themselves from business schools in the United states. The reasons for this pertain to, first, what statistical analysis is good for in science; second, what statistical analysis is not good for in science; third, what statistical analysis is and is not good for in business research; fourth, how a large sample size is not generalizable but a single case is; fifth, how action research, design research, and Herbert simon＇s ＂sciences of the artificial＂ all demonstrate rigor without statistics; and finally, the historical context that explains the turn to statistics in American business schools but is irrelevant to China. The recommendation is for business schools in China not necessarily to reject statistical research, but rather, to also pursue other, equally scientific forms of research, including those recognized by Herbert simon.

Clinical outcomes with a new design in multifocal intraocular lens:a pilot study

Purpose:To evaluate the clinical and visual outcomes,quality of near vision and the influence of photic phenomena in patients bilaterally implanted with a new Precizon Presbyopic multifocal intraocular lens(IOL).Methods:In this prospective consecutive case series,20 eyes of 10 patients were included(mean age 63.80±12.55 years).Uncorrected and corrected visual acuity(far,intermediate and near),subjective refraction,binocular defocus curve,contrast sensitivity(CsV-1000)and quality of vision and satisfaction questionnaires were *** follow-up was 12 months after ***:At 12 months after surgery,uncorrected distance visual acuity(UDva)improved with surgery(p=0.001)with a value of 0.08±0.08 *** near visual acuity(UNva)was 0.22±0.12 logMAR and distance corrected near visual acuity(DCNva)was 0.16±0.13 *** distance visual acuity(UIva)was 0.22±0.10 *** sensitivity outcomes were similar to normal population in photopic conditions and slightly reduced in mesopic conditions of *** curve showed that this multifocal IOL was able to provide a visual acuity(va)equal or better to 0.16 logMAR between defocus levels of+1.00 to−2.50 *** patient satisfaction was obtained in quality of vision and satisfaction questionnaires ***:The Precizon Presbyopic Nva IOL(OPHTEC BV)provides good visual *** multifocal IOL provides a high percentage of spectacle independence due to good va at far,intermediate and near distances and satisfactory contrast *** patient satisfaction was observed in quality of vision and satisfaction questionnaires with a low percentage of patients manifesting photic phenomena.

On the electron-beam (EB) irradiation and grafting of acrylic monomers onto Eva copolymers

IN an ordinary way the grafting induced by electron and gamma preirradiations widely used for improving the compatibility of polymers in composites gives rise to an enhancement of their mechanical properties. However, very few have been reported in the flame retardancy of polymers using these sorts of techniques. Wilkie investigated the flame retardancy of photo-initiation grafted polymers. An attempt is made in this paper to try to use the EB preirradiation on Eva copolymers with different va contents and the subsequent grafting with saponification under atmosphere and to expedite the charring of polymers upon heating. Characterization of the flammability of the preirradiated Eva copolymers grafted with acrylic monomers indicates that this approach turns out to be a promising way and worthy of doing.

Increased susceptibility of aging gastric mucosa to injury:The mechanisms and clinical implications

This review updates the current views on aging gastric mucosa and the mechanisms of its increased susceptibility to injury. Experimental and clinical studies indicate that gastric mucosa of aging individuals-“aging gastropathy”-has prominent structural and functional abnormalities vs young gastric mucosa. some of these abnormalities include a partial atrophy of gastric glands, impaired mucosal defense (reduced bicarbonate and prostaglandin generation, decreased sensory innervation), increased susceptibility to injury by a variety of damaging agents such as ethanol, aspirin and other non-steroidal anti-inflammatory drugs (NsAIDs), impaired healing of injury and reduced therapeutic efficacy of ulcer-healing drugs. Detailed analysis of the above changes indicates that the following events occur in aging gastric mucosa: reduced mucosal blood flow and impaired oxygen delivery cause hypoxia, which leads to activation of the early growth response-1 (egr-1) transcription factor. Activation of egr-1, in turn, upregulates the dual specificity phosphatase, phosphatase and tensin homologue deleted on chromosome ten (PTEN) resulting in activation of pro-apoptotic caspase-3 and caspase-9 and reduced expression of the anti-apoptosis protein, survivin. The imbalance between pro- and anti-apoptosis mediators results in increased apoptosis and increased susceptibility to injury. This paradigm has human relevance since increased expression of PTEN and reduced expression of survivin were demonstrated in gastric mucosa of aging individuals. Other potential mechanisms operating in aging gastric mucosa include reduced telomerase activity, increase in replicative cellular senescence, and reduced expression of vascular endothelial growth factor and importin-α-a nuclear transport protein essential for transport of transcription factors to nucleus. Long-term differences in urinary, bowel and sexual function among men treated with surgery versus radiation for prostate cancer

surgery and radiation have both been shown to increase the longterm diseasespecific survival rate for men with din icaUy localized prostate cancer. Although both modalities have demonstrated favor able effects on cancer control, questions regarding quality of life （QoL） and func tional outcomes remain incompletely answered. To date, no randomized prospective trials have been performed comparing the two treatment modalities and so indirect compar isons of longterm functional outcomes have served as a substitute to aid in patient coun seling and decisionmaking. As there is a pau city of longterm data comparing functional outcomes after radical prostatectomy and external beam radiation therapy, a recent art icle by Resnick et al.,1 has attempted to pro vide additional information about this topic in terms of continence, erectile function and bowel function. Utilizing the Prostate Cancer Outcomes study cohort, a populationbased cohort of men diagnosed with prostate cancer in the pro statespecific antigen （PsA） era, the authors compared rates of urinary incontinence, erect ile dysfunction, and bowel urgency at 2, 5 and 15 years after primary therapy. They showed that men undergoing prostatectomy had higher rates of incontinence and erectile dys function at 2 and 5 years, but these rates were similar to those in the radiotherapy group at 15 years. More specifically, men were approxi mately five times more likely to have urinary continence issues if they underwent prostatec tomv versus radiotherapy and almost three anda half times more likely to develop erectile dys function in the short to intermediateterm following primary treatment. As expected, rates of bowel urgency were higher in the radio therapy group at 2 and 5 years, but not sig nificantly different from the surgery group at 15 years. Quantification of intermuscular and intramuscular adipose tissue using magnetic resonance imaging after neurodegenerative disorders

Ectopic adiposity has gained considerable attention because of its tight association with metabolic and cardiovascular health in persons with spinal cord injury（sCI）. Ectopic adiposity is characterized by the storage of adipose tissue in non-subcutaneous sites. Magnetic resonance imaging（MRI） has proven to be an effective tool in quantifying ectopic adiposity and provides the opportunity to measure different adipose depots including intermuscular adipose tissue（IMAT） and intramuscular adipose tissue（Intra MAT） or intramuscular fat（IMF）. It is highly important to distinguish and clearly define these compartments, because controversy still exists on how to accurately quantify these adipose depots. Investigators have relied on separating muscle from fat pixels based on their characteristic signal intensities. A common technique is plotting a threshold histogram that clearly separates between muscle and fat peaks. The cut-offs to separate between muscle and fat peaks are still not clearly defined and different cut-offs have been identified. This review will outline and compare the Midpoint and Otsu techniques, two methods used to determine the threshold between muscle and fat pixels on T1 weighted MRI. The process of water/fat segmentation using the Dixon method will also be outlined. We are hopeful that this review will trigger more research towards accurately quantifying ectopic adiposity due to its high relevance to cardiometabolic health after sCI.

The current perspective is aimed to highlight the significance of adding low-dose testosterone replacement therapy(TRT) to electrically evoked resistance training(RT).Evoked RT is primarily designed to load the paralyzed lower extremity muscles to elicit muscle hypertrophy in persons with spinal cord injury(sCI)(Ryan et al.,2013;Gorgey et al.,2019).The rationale was based on previous work that indicated that highdose TRT is likely to enhance muscle quality。
