Right hepatectomy for giant cavernous hemangioma with diffuse hemangiomatosis around Glisson's capsule

Diffuse liver hemangiomatosis with giant cavernous hemangioma in adult is extremely rare. A 35 year-old woman presented to hospital with main complaint of epigastric pain and abdominal fullness. An enhanced computed tomography scan revealed a massive liver tumor in right lobe about 150 mm in size. There was contrast enhancement at the periphery of the mass consistent with a cavernous hemangioma. she underwent right hepatectomy. Histologically, it was diagnosed as a cavernous hemangioma. And also, hemangiomatous lesions were scattered around the Glisson’s capsule on the back ground liver. These hemangiomatous lesions were not recognized preoperatively. Even if we couldn’t diagnose hemangiomatosis around the main giant hemangioma preoperatively, we need to take enough surgical margins because the giant hemangioma has the potential to have small hemangiomatous lesions around the tumor. We reported right hepatectomy for giant cavernous hemangioma with diffuse hepatic hemangiomatosis without an extrahepatic lesion in an adult.

To the Editor:An 82-year-old man with symptomatic severe calcific aortic valve stenosis(peak velocity:5.19 m/s;mean gradient:78.0 mmHg;aortic valve area:0.48 cm2)underwent transfemoral transcatheter aortic valve *** the procedure,a 0.035-inch straight-tip glide wire(Terumo,somerset,NJ,UsA)supported by a 6 Fr Judkins Right 4(JR4)coronary catheter(Medtronic,Minneapolis,MN,UsA)were used to cross the severely stenotic aortic ***,the JR4 catheter could not pass the aortic valve because the wire crossed the aortic valve in the left/right coronary commissure creating an unfavorable angle and limited pushability for the *** of switching to a differently shaped catheter(eg,an Amplatz left 1 catheter),we advanced the glide wire and therefore the more supportive portion of the wire further through the aortic ***,both wire and JR4 catheter dropped into the non/left coronary commissure,allowing the JR4 catheter to cross the valve ***,a 34 mm self-expanding Evolut R valve(Medtronic)was implanted,and the patient was discharged uneventfully(Figure 1A-E and supplementary Video 1,http://***/CM9/A353["Commissural drop"wiring technique.]).

This paper proposes a novel standing assistance robot,which realizes voluntary movements of the patient within a safety motion *** the previous studies,conventional assistive robots did not require patients to use their own physical strength to stand,which led to decreased strength in the *** general assistive robots helped patients by using a fixed motion reference pathway in spite of their original intention,and as a result,these robots failed to use the physical strength of the ***,this study proposes a novel method for assisting the patient to stand up safely while using their physical abilities,by determining the range of movements that can be safely performed from the patient’s physical condition and allowing the patient to move freely within this *** standing motion is a set of different movements:inclining the trunk forward,lifting the trunk,and extending the *** this study,the range of movements in which the patient can safely stand in each movement is determined in terms of body stability and the muscle output that the patient can ***,this study proposes a robot control method that allows movements based on the patient’s free will if the patient’s posture is within the safety tolerance,and it corrects the movements when the patient’s posture is estimated to fall outside of *** proposed idea is implemented in our new prototype,and its effectiveness is verified by experimental results with elderly subjects who live in the nursing care house.

Characteristics and prognosis of synchronous multiple early gastric cancer

AIM:To assess the clinicopathologic characteristics,risk factors,and prognosis for synchronous multiple early gastric cancer(sMGC).METHODs:A total of 146 patients with sMGC and1194 patients with single gastric cancer who had undergone gastrectomy between 1989 and 2008 were retrospectively analyzed to determine their clinicopathologic characteristics and postoperative *** were classified into groups on the basis of location and *** habits were evaluated using the Brinkman *** and pathological factors were compared using either Fisher’s exact test or Pearson’sχ*** regression analysis was performed to identify independent risk *** rate was calculated using the Kaplan-Meier ***:sMGCs accounted for 10.9%of gastric cancer cases and occurred predominantly in elderly male patients with a family history of gastric cancer who were both smokers and *** tumors were typically macroscopically elevated and histologically *** were no significant differences between sMGC and single gastric cancer patients with respect to tumor location,tumor size,lymph node metastasis,the number of metastatic lymph nodes,venous invasion,or tumor stage(P=0.052,P=0.347,P=0.595,P=0.805,P=0.559,and P=0.408,respectively).Further,there was no significant difference in postoperative survival between the patient groups(P=0.200).Of the 146sMGC patients,a single patient had remnant ***:A careful preoperative endoscopy is necessary for patients who are at high risk of sMGC,and minimally invasive treatment may be indicated in some cases.

Objective: To investigate the morphological chan ges of the roof of the subacromial bursa (sAB) and its involvement extent after rotator cuff tear. Methods: In the experimental group,the roof of sAB was obtaine d from 30 cases of rotator cuff tear both at the tear site and a site 2.5 - 3.0 cm distal to the tear site during rotator cuff repair. In the control gr oup,the roof of sAB was obtained from the exposed site of recurrently dislocate d shoulder or fractured humeral shaft of 8 cases. The specimens were stained wit h hematoxylin and eosin and observed under a transmission electron microscope. T he cell number was quantitated through counting the blue-stained nucleus in sAB with a computer image analysis ***: The number of cells increased significantly in the roo f of sAB in the experimental group compared with that of the control group. Howe ver,no difference of the bursal reaction was found among the type of rotator cu ff tear,the bursa thickness and the presence of fluid in the bursa. The great m ajority of cells were type B cells observed under the transmission electron micr ***: The increase in cell number in the roof of sAB in the experimental group is a reactive increase rather than an inflammatory proces s and the involvement of sAB is not limited in extent. The change of the roof of sAB is a secondary reaction to the rotator cuff tear.

Esophageal diverticula are rare conditions that cause esophageal symptoms, such as dysphagia, regurgitation, and chest pain. They are classified according to their location and characteristic pathophysiology into three types: epiphrenic diverticulum, Zenker's diverticulum, and Rokitansky diverticulum. The former two disorders take the form of protrusions, and symptomatic cases require interventional treatment. However, the esophageal anatomy presents distinct challenges to surgical resection of the diverticulum, particularly when it is located closer to the oral orifice. since the condition itself is not malignant,minimally invasive endoscopic approaches have been developed with a focus on alleviation of symptoms. several types of endoscopic devices and techniques are currently employed, including peroral endoscopic myotomy(POEM). However,the use of minimally invasive endoscopic approaches, like POEM, has allowed the development of new disorder called iatrogenic esophageal diverticula. In this paper, we review the pathophysiology of each type of diverticulum and the current state-of-the-art treatment based on our experience.
