Background: Upper gastrointestinal bleeding (UGIB) is a medical emergency. Timely and appropriate treatment can be lifesaving. Where medical equipment and supplies are inadequate, management of upper gastrointestinal bleeding is challenging. Methods: A retrospective review of charts of patients who were admitted during the year 2010, with a diagnosis of Upper Gastrointestinal Bleeding (UGIB), was done at Kamuzu Central Hospital, Lilongwe, Malawi. A Rockall score was applied to determine mortality risk. Results: A total number of 187 records (119 men and 68 women, mean age of 40.7 ± 15.3 years) were reviewed. The mortality rate was 23.5%, with a non-significant gender difference. Bleeding oesophageal varices were the most common clinical cause of UGIB (42.8%), with more males (63.1%) than females affected. About 40% of patients had no cause of UGIB indicated in their records. 14 (7.5%) patients had a normal oesophagogastroduodenoscopy (OGD). Oesophageal tumor was present in 2.7% of the subjects as a cause of UGIB. Access to endoscopy, for diagnosis and therapeutic intervention, and surgery (Hassab procedure) was available to less than 50% of the patients. sixteen patients (9.5%) had surgery after endoscopy due to lack of variceal banding materials.?Conclusion:?Upper gastrointestinal bleeding is an important and common clinical problem at Kamuzu Central Hospital. Oesophageal varices seem to be the commonest cause of UGIB. Inadequacy of resuscitation materials and perhaps timely diagnostic and therapeutic endoscopic and surgical interventions are important limiting factors to favourable patient outcome. Work towards regular provision and supply of interventional resources regarding UGIB management may improve patient outcome.

species＇ partitioning of resources remains one of the most integral components for understanding community assem- bly. Analysis of stable carbon and nitrogen isotopes in animal tissues has the potential to help resolve patterns of partitioning be- cause these proxies represent the individual＇s diet and trophic niche, respectively. Using free-ranging rodents in a southern Afri- can savanna as a model community, we find that syntopic species within habitats occupy distinct isotope niches. Moreover, spe- cies with strongly overlapping isotope niches did not overlap in their spatial distribution patterns, suggesting an underlying effect of competitive exclusion. Niche conservatism appears to characterize the behaviour of most species in our sample - with little or no observed changes across habitats - with the exception of one species, Mastomys coucha. This species displayed a generalist distribution, being found in similar abundances across a variety of habitats. This spatial pattern was coupled with a generalist isotope niche that shifted across habitats, likely in response to changes in species composition over the same spatial gradient. The case for M. coucha supports contentions that past competition effects played a significant evolutionary role in shaping community structures of today, including the absence of strong interspecific niche overlaps within particular habitats. Our study highlights the value of stable isotope approaches to help resolve key questions in community ecology, and moreover introduces novel ana- lytical approaches to quantifying isotope niche breadths and niche overlaps that are easily comparable with traditional metrices

路表纹理包络轮廓计算及其对抗滑性能的影响

为了分析轮胎与路表的接触特性及其对路面抗滑性能的影响,采用路面纹理测量车(road surface tester)获取路表纹理二维轮廓离散数据,运用印痕法模型,利用MATLAB软件计算路表纹理包络轮廓,进一步计算了包络轮廓对应的平均断面深度(mean profile depth,MPD)、宏观纹理表面构造水平指数LMa(macro texture indices calculated with texture level)、粗大纹理表面构造水平指数LMe(mega texture indices calculated with texture level)等纹理指标,并与初始轮廓纹理指标进行对比;最后计算包络轮廓、初始轮廓纹理参数MPD、LMa与路面摩擦因数(longitudinal frictional coefficient,LFC)的相关性系数,分析了路表纹理包络轮廓对路面抗滑性能的影响。研究结果表明:包络轮廓纹理参数与初始轮廓纹理参数相关性良好(判定系数R2>0.85);包络轮廓MPD值较初始轮廓MPD值有明显下降,且初始轮廓MPD值越大,包络轮廓MPD值下降幅度也越大;包络轮廓表面构造水平指数较初始轮廓表面构造水平指数下降,LMa的下降幅度比LMe的下降幅度高10%左右;印痕法参数s取值对包络轮廓纹理参数有较大影响,s值越小,其对应的包络轮廓纹理参数值较初始轮廓纹理参数值下降的幅度越大;此外,包络轮廓纹理参数与摩擦因数的判定系数较初始轮廓纹理参数与摩擦因数的判定系数有较大提升,其中MPD与摩擦因数的判定系数提高了约35%,LMa与摩擦因数的判定系数提高了约23%;印痕法参数s值越小,包络纹理参数与摩擦因数的判定系数越高。该结果可为研究胎-路接触特性对路用性能的影响提供参考。

背景以往的观点认为因左侧双腔管不易发生错位所以比右侧双腔管更安全。如果这种观点正确，那么胸科手术中，右侧双腔管的低氧血症、高碳酸血症、高气道压的发生率和严重程度直比左侧双腔管高。方法我们采用自动化麻醉信息管理系统回顾性分析了2003年4月15日到2004年12月31日的胸外科手术麻醉记录？该系统每隔30秒自动记录脉搏氧饱和度、呼气末二氧化碳浓度、气道峰压，也记录手术的部位和双腔管的位置。我们比较胸科麻醉医师使用左右侧双腔管的频率。继而，测定肺脏和胸壁手术患者的低氧血症（spO2〈90％）、高碳酸血症（RtCO2〉45mmHg）和高气道压（气道峰压〉35cmH2O）的发生率、持续时间和严重程度，采用标准统计学方法比较各组的例数及数据。结果右侧双腔管（n=241）和左侧双腔管（n=450）广泛应用于对侧手术。在低氧血症、高碳酸血症或高气道压的发生率或持续时间上，两种方式没有显著差别。在左肺通气的患者中，EtCO2轻厦升高但差异有显著统计学意义。结论在手术期间，当低氧血症、高碳酸血症以及高气道压的发生率和严重程度作为安全标准时，比较来自同一厂家的两种双腔管的数据，本研究结果否定了左侧双腔管比右侧双腔管安全的推测。
