BACKGROUND Cost analyses of patients undergoing esophagectomy is valuable for identifying modifiable expenditure drivers to target and curtail costs while improving the quality of *** aimed to define the cost-complication relationship after esophagectomy and delineate the incremental contributions to *** To assess the relationship between the hospital costs and potential cost drivers post esophagectomy and investigate the relationship between the cost-driving variables(predicting variables)and hospital costs(dependent variable).METHODs In this retrospective single center study,the severity of complications was graded using the Clavien-Dindo(CD)classification *** esophagectomy complications were categorized and defined according to consensus *** costing data included the in-hospital costs of the index admission and any unplanned admission within 30 postoperative *** used correlation analysis to assess the relationship between key clinical variables and hospital costs(in United states dollars)to identify cost drivers.A mediation model was used to investigate the relationship between these variables and hospital *** A total of 110 patients underwent primary esophageal *** median admission cost was $47822.7(interquartile range:35670.2-68214.0).The total effects on costs were $13593.9(95%CI:10187.1-17000.8,Pse in CD severity grade,$4781(95%CI:3772.7-5789.3,Pse in the number of complications,and $42552.2(95%CI:8309-76795.4,P=0.015)if a key esophagectomy complication *** esophagectomy complications drove the costs directly by $11415.7(95%CI:992.5-21838.9,P=0.032).CONCLUsION The severity and number of complications,and the development of key esophagectomy complications significantly contributed to total hospital *** institutional initiatives and strategies are needed to enhance patient outcomes and minimize costs.

Three-dimensional crystal plasticity and HR-EBsD analysis of the local stress-strain fields induced during twin propagation and thickening in magnesium alloys

Present work focuses on analysis of the stress and strain fields inside and around the individual {10–12} twin in magnesium alloy. The 3D crystal plasticity model represents twin as an ellipsoidal inclusion surrounded by the matrix. Five different twin thicknesses and three different lateral twin lengths are used for stress/strain analysis. The simulations are complemented with experimental observations using high-resolution electron backscattered diffraction. The simulations and experiments show a similar distribution of the shear stress and the spatial activity of individual slip systems(basal, prismatic, pyramidal). Plasticity induced inside the twin is dominantly caused by the prismatic dislocations slip and does not influence twin back stress which is identical to pure elastic twin. The twin with larger lateral dimension requires lower equilibrium stress which suggests anisotropic twin propagation and increased thickness of such twins. The lateral twin propagation is mostly influenced by prismatic and pyramidal slip in the twin vicinity. The twin thickness can reach a maximal level that is driven by the critical resolved shear stress values for dislocation slip with the significant influence of basal slip.

Coordinated investigations into the interactions between biologically mimicking(biomimetic)material constructs and stem cells advance the potential for the regeneration and possible direct replacement of diseased cells and *** clinically relevant therapies will require the development and optimization of methods that mass produce fully functional cells and *** advances in the design and synthesis of biomaterial scaffolds,one of the biggest obstacles facing tissue engineering is understanding how specific extracellular cues produced by biomaterial scaffolds influence the proliferation and differentiation of various cell *** elasticity is one such tailorable property of synthetic scaffolds that is known to differ between ***,we investigate the interactions between an elastically tailorable polyethylene glycol(PEG)-based hydrogel platform and human bone marrow-derived mesenchymal stem cells(hMsCs).For these studies,two different hydrogel compositions with elastic moduli in the ranges of 50-60 kPa and 8-10 kPa were *** findings demonstrate that the different elasticities in this platform can produce changes in hMsC morphology and proliferation,indicating that the platform can be implemented to produce changes in hMsC behavior and cell state for a broad range of tissue engineering and regenerative ***,we show that the platform’s different elasticities influence stem cell differentiation potential,particularly when promoting stem cell differentiation toward cell types from tissues with stiffer *** findings add to the evolving and expanding library of information on stem cell-biomaterial interactions and opens the door for continued exploration into PEG-based hydrogel scaffolds for tissue engineering and regenerative medicine applications.

Feasibility of breaking up sitting time in mainstream and special schools with a cognitively challenging motor task

Background: Children spend 70% of the school day sitting in class. Classroom-based active breaks can benefit children's physical health, but if the breaks are cognitively demanding(i.e., combine physical exertion and mental engagement), they may also improve focus and cognitive functions. Teachers and students play a crucial role in the successful implementation of active breaks, and their perspectives are critical to the feasibility of these strategies. The aim of this study was to assess the feasibility of implementing a cognitively challenging motor task as an active break in mainstream and special primary ***: A total of 5 teachers in 2 mainstream schools and 7 teachers in 1 special school(attended by children with neurodevelopmental disorders) attended a 20-min training on how to implement a 4-min cognitively challenging active break, before conducting a feasibility trial(twice a day for 1 week). To understand individual perceptions, one-on-one semistructured interviews were conducted before and after the trial with teachers, and focus group interviews were conducted with typically developing children after the trial. Questions were based on a predefined framework for feasibility studies. All interviews were audio recorded, transcribed and analyzed in NVivo 11 using a framework approach. A total of 12 teachers(11 females; 7 between 20 and 34 years old) and 34 children(16 girls; 9.3 § 1.7 years, mean § sD) participated in the ***: In mainstream schools, teachers viewed the cognitively challenging motor task as appropriate and potentially beneficial for children's health and focus. Children reported enjoying the active breaks. Teachers in special schools viewed the task as complex and potentially frustrating for children. The set of proteins required for mitotic division remains poorly characterized. Here, an extensive series of correlation analyses of human and mouse transcriptomics data were performed to identify genes strongly and reproducibly associated with cells undergoing s/G2-M phases of the cell cycle. In so doing, 701 cell cycle-associated genes were defined and while it was shown that many are only expressed during these phases, the expression of others is also driven by alternative promoters. Of this list, 496 genes have known cell cycle functions, whereas 205 were assigned as putative cell cycle genes, 53 of which are functionally uncharacterized. Among these, 27 were screened for subcellular localization revealing many to be nuclear localized and at least three to be novel centrosomal proteins. Furthermore, 10 others inhibited cell proliferation upon siRNA knockdown. This study presents the first comprehensive list of human cell cycle proteins, identifying many new candidate proteins.
