Aims: As ar.hythmias in the long QT syndr.me(LQTS) ar. tr.gger.d by hear. r.te deceler.tion or.acceler.tion, we speculated that the sudden br.dycar.ia and subsequent tachycar.ia that follow adenosine injection would unr.vel QT changes of diagnostic value in patients with LQTS. Methods and r.sults: Patients(18 LQTS and 20 contr.ls) r.ceived intr.venous adenosine dur.ng sinus r.ythm. Adenosine was injected at incr.mental doses until atr.oventr.cular.block or.sinus pauses lasting 3 s occur.ed. The QT dur.tion and mor.hology wer. studied at baseline and at the time of maximal br.dycar.ia and subsequent tachycar.ia. Despite simitar.degr.e of adenosine- induced br.dycar.ia(longest r. r.1.7± 0.7 vs. 2.2± 1.3 s for.LQTS and contr.ls, P=NS), the QT inter.al of LQT patients incr.ased by 15.8± 13.1% , wher.as the QT of contr.ls incr.ased by only 1.5± 6.7% (Pr.y, despite similar.r.flex tachycar.ia(shor.est r. r.0.58± 0.07 vs. 0.55± 0.07s for.LQT patients and contr.ls, P=NS), LQTS patients developed gr.ater.QT pr.longation(QTc=569± 53 vs. 458± 58 ms for.LQT patients and contr.ls, Pr.minator.was the QTc dur.ng maximal br.dycar.ia. Notched T- waves wer. obser.ed in 72% of LQT patients but in only 5% of contr.ls dur.ng adenosine- induced br.dycar.ia(Pr.voking tr.nsient br.dycar.ia followed by sinus tachycar.ia, this adenosine challenge test tr.gger. QT changes that appear.to be useful in distinguishing patients with LQTS fr.m healthy contr.ls.

Backgr.und: Anatomical var.ations in the liver.ar.er.al supply ar. quite common and can affect the sur.ical str.tegy when per.or.ing a minimally invasive pancr.aticoduodenectomy(MIPD). Their.pr.sence must be pr.emptively detected to avoid postoper.tive liver.and biliar. complications. Data sour.es: Following the Pr.SMA guidelines and the Cochr.ne pr.tocol we conducted a systematic r.view on the management of an accessor. or.r.placed r.ght hepatic ar.er.(r.A) ar.sing fr.m the super.or.mesenter.c ar.er. when per.or.ing an MIPD. r.sults: Five studies involving 118 patients wer. included. The most common r.por.ed management of the aber.ant r.A was conser.ative(97.0%);however. patients under.oing aber.ant r.A division without r.constr.ction did not develop liver.or.biliar. complications. No differ.nces in postoper.tive mor.idity or.long-ter. oncological r.lated over.ll sur.ival wer. r.por.ed in all the included studies when compar.ng MIPD in patients with standar. anatomy to those with aber.ant r.A. Conclusions: MIPD in patients with aber.ant r.A is feasible without incr.ase in mor.idity and mor.ality. As pr.oper.tive str.tegy is cr.cial, we suggested planning an MIPD with an anomalous r.A focusing on pr.oper.tive vascular.aber.ancy assessment and differ.nt str.tegies to r.duce the r.sk of liver.ischemia.

r.cent evidence suggests that ather.scler.sis is an inflammator. disor.er.in which cytokines appear.to play an impor.ant r.le. Special attention center.d over.the possible contr.bution of cytokines to the destabilization of the plaque. IL-18 is a pr.inflammator. cytokine of the IL-1 family, r.cognized for.its ability to pr.mote IFN-γsecr.tion. It has r.cently been detected in human plaques and its administr.tion was associated with incr.ased ather.s-cler.sis in apolipopr.tein E(apoE) mice concomitant with an incr.ase in plaque infiltr.ting inflammator. cells. In our.study, we investigated whether.patients with established ather.scler.sis, with either.stable or.unstable angina, possessed high levels of IL-18. Patients with stable angina(n=48) wer. fr.m the outpatient clinic wher.as patients with unstable angina(n=73) wer. r.cr.ited upon admission and pr.or.to per.or.ance of cor.nar. angiogr.phy. Contr.l patients(n=19) wer. healthy subjects with no evidence of cor.nar. ar.er. disease. Ser.m levels of IL-18 wer. assayed by ELISA. Patients with stable and unstable angina exhibited higher.ser.m levels of IL-18(77.1±7.2 and 61.5±5.1 pg/ml, r.spectively) in compar.son to contr.l subjects(p=0.002 and p=0.02, r.spectively). However. levels of IL-18 did not differ.significantly between patients with stable and unstable angina. No differ.nces wer. evident in the ser.m concentr.tions of IL-18 in patients with unstable angina(n=17) upon admission and 1-3 months later.when the angina was alr.ady contr.lled. Although IL-18 ser.m levels appear.elevated in the pr.sence of cor.nar. ather.scler.sis, ther. is no evidence to associate this pr.gr.ssion towar.s plaque instability.

r.pidly solidified Al100-x-Cux alloys （x = 5, 10, 15, 25, 35 wt%） wer. pr.par.d and analyzed. High cooling r.te incr.ased the Cu solubility in α-AI matr.x. The influence of the cooling r.te on Cu solubility extension in AI was exper.mentally simulated. Thus the pour.ng was per.or.ed in metallic die and by melt spinning-low pr.ssur. （MS- LP） technique. Melt pr.cessing by liquid quenching was per.or.ed using a self-designed melt spinning set-up which combined the cooling technology of a melt jet on the spinning disc with the pr.nciple of the mold feeding fr.m low pr.ssur. casting technology. The thickness of the melt-spun r.bbons was in the r.nge of 30-70 μm. The cooling r.te pr.vided by MS-LP was within 105-106 K/s after.the device calibr.tion. The obtained alloys wer. char.cter.zed fr.m str.ctur.l, ther.al and mechanical point of view. Optical micr.scopy and scanning electr.n micr.scopy wer. employed for.the micr.str.ctur.l char.cter.zation which was followed by X-r.y analysis. The ther.al pr.per.ies wer. evaluated by dilatometr.c and differ.ntial scanning calor.metr.c measur.ments. Vicker. micr.har.ness measur.ments wer. per.or.ed in the study. In the case of the hyper.utectic alloy with 35 wt% Cu obtained by MS-LP method, the micr.har.ness value incr.ased by 45% compar.d to the same alloy obtained by gr.vity casting method. This was due to the extended solubility of the alloying element in the α-AI solid solution.
