laparoscopic approach in gastrointestinal emergencies

This review focuses on the laparoscopic approach to gastrointestinal emergencies and its more recent indications. laparoscopic surgery has a specific place in elective procedures, but that does not apply in emergency situations. In specific emergencies, there is a huge range of indications and different techniques to apply, and not all of them are equally settle. We consider that the most controversial points in minimally invasive procedures are indications in emergency situations due to technical difficulties. Some pathologies, such as oesophageal emergencies, obstruction due to colon cancer, abdominal hernias or incarcerated postsurgical hernias, are nearly always resolved by conventional surgery, that is, an open approach due to limited intraabdominal cavity space or due to the vulnerability of the bowel. These technical problems have been solved in many diseases, such as for perforated peptic ulcer or acute appendectomy for which a laparoscopic approach has become a wellknown and globally supported procedure. On the other hand, endoscopic procedures have acquired further indications, relegating surgical solutions to a second place; this happens in cholangitis or pancreatic abscess drainage. This endoluminal approach avoids the need for laparoscopic development in these diseases. Nevertheless, new instruments and new technologies could extend the laparoscopic approach to a broader array of potentials procedures. There remains, however, a long way to go.

Preparation and Characterization of Holmium-Beta-Cyclodextrin Complex

The purpose of this study was to prepare and characterize of holmium-beta-cyclodextrin complex (Ho-β-CD) in order to increase the solubility and stability of Holmium. To achieve this goal, Ho-β-CD complex was prepared by evaporation method of holmium and beta cyclodextrin solutions in a proportion (1:1) and (1:3), respectively. Infrared (IR) and Raman spectroscopy, X-Ray Diffraction were performed to identify the complex. Morphology of the Ho, β-CD, and Ho-β-CD were studied using Scanning Electron Microscopy (SEM).

An Integrated ISFET pH Microsensor on a CMOS Standard Process

We present the design and integration of a nine-pH microsensor array on a single silicon substrate with its own signal readout circuit, integrated in a 0.6-μm commercial standard complementary metal oxide semiconductor (CMOS) process. An ion sensitive field effect transistor (ISFET) has been used as pH microsensor and an instrumentation amplifier as the read-out circuit. The ISFET structure is conformed by the channel length and ratio of MOS transistor, gate extended and the selective membrane, for which silicon nitride (Si3N4) is employed as an ion selective element. The complete design includes shielding around the pH microsensor and the readout circuit to avoid leakage of current to the substrate. The readout circuit is composed by three operational amplifiers and resistances that form the instrumentation amplifier, with a ±2.5 V bias has a 50 dB gain, power supply rejection ratio (PSSR) of 120 dB and common mode rejection ratio (CMRR) of 127 dB. The complete system is integrated in a 1.12 mm2 silicon area;it presents a 59 mV/pH linearity, within a concentration range of 2 to 12 of pH level, making it a good alternative for biological or medical applications.

In vitro Metabolomic Approaches to Investigating the Potential Biological Effects of Phenolic Compounds:An Update

Dietary phenolic compounds （PCs） have been receiving intercst lor their presumcd roles ill disease prevention. However, there is a lack of studies on the underlying molecular mechanisnls. In this regard, in vitro metabolomic approaches are suitable for the investigation o1 the molecular changes in response to PC exposure. Up to date, the biological effects of PCs have only been cxalnined for PCs from rosemary （Rosmarinus officinalis）, olive oil, and reSVCl-atrol using cell-based mctabolomic approach, although transcriptomic and/or proteomic studies have also been conducted in the same in vi[ro cell experiment in some cases. Our integral analysis of the reviewed studies suggest that PCs may be involved not only in basic cellular processes or macro-and micro-nutrient meta- bolism, but also in specific metabolic pathways that have been thoroughly investigated. These modulated pathways could have a clinical impact on neurodegenerativc diseases, type 2 diabetes, cancer, and cardiovascular diseases. In conclusion, the in vitro metabolomic approaches provide additional information of the molecular mechanisms involved in disease risk reduction of dietary PCs. In order to elucidate the mechanisms of action of PCs, more metabolomic cell-based studies are nccdcd and testing the physiological conjugated forms of PCs in these cell systems could be of special interest.

Analysis of hydrodynamic effects on biofilm thickness in fluidized-bed tapered bioreactors

Fluidized beds in tapered geometries provide advantages such as a broad range of optimum operating ***,the dynamics of the flow inside these bioreactors is significantly more complex,as they promote non-uniform bioparticles *** this end,the CFD technique can give a detailed description of the flow in the ***,the aim of this work is to assess the influence of the hydrodynamic on the steady-state biofilm thickness through numerical simulations of liquid–solid fluidized *** geometries of tapered bioreactors in the operating conditions used to develop *** were run to solve the two-phase RANS equations using the open-source software OpenFOAM in *** SST k−ω model was used to estimate the turbulent features of the *** and bioparticles distribution inside the reactor were analyzed for different inlet velocities,while bioparticles collision frequency and shear stresses were evaluated for various operating *** comparison between the turbulent kinetic energy and the granular temperature indicated that the fluid flow is more turbulent than the bioparticles *** hydrodynamics influence on the steady-state biofilm thickness was assessed,and results led to the conclusion that biofilm is more sensitive to hydrodynamic shear stress than bioparticles collisions.

Irritable bowel syndrome:A clinical review

Irritable bowel syndrome (IBS) remains a clinical challenge in the 21st century. It’s the most commonly diagnosed gastrointestinal condition and also the most common reason for referral to gastroenterology clinics. Its can affect up to one in five people at some point in their lives, and has a significantly impact of life quality and health care utilization. The prevalence varies according to country and criteria used to define IBS. Various mechanisms and theories have been proposed about its etiology, but the biopsychosocial model is the most currently accepted for IBS. The complex of symptoms would be the result of the interaction between psychological, behavioral, psychosocial and environmental factors. The diagnosis of IBS is not confirmed by a specific test or structural abnormality. It is made using criteria based on clinical symptoms such as Rome criteria, unless the symptoms are thought to be atypical. Today the Rome Criteria III is the current gold-standard for the diagnoses of IBS. Secure positive evidence of IBS by means of specific disease marker is currently not possible and cannot be currently recommended for routine diagnosis. There is still no clinical evidence to recommend the use of biomarkers in blood to diagnose IBS. However, a number of different changes in IBS patients were demonstrated in recent years, some of which can be used in the future as a diagnostic support. IBS has no definitive treatment but could be controlled by non-pharmacologic management eliminating of some exacerbating factors such certain drugs, stressor conditions and changes in dietary *** traditional pharmacologic management of IBS has been symptom based and several drugs have been used. However, the cornerstone of its therapy is a solid patient physician relationship. This review will provide a summary of pathophysiology, diagnostic criteria and current and emerging therapies for IBS.

Targeting neuronal nitric oxide synthase as a valuable strategy for the therapy of neurological disorders

The management of neurological disorders have huge and increasing human and economic costs. Despite this, there is a scarcity of effective therapeutics, and there is an extreme urgency for new and real treatments. In this short review we analyze some promising advancements in the search of new bioactive molecules targeting neuronal nitric oxide synthase （nNOS）, an enzyme deputed to the biosynthesis of nitric oxide （NO）. In different conditions of neuronal damages, this molecule is overproduced, contributing to the pathogenesis and progression of neuronal diseases. Two main approaches to modulate nNOS are discussed： a first one consisting in the direct inhibition of the enzyme by means of small organic molecules, which can be also active against other different targets involved in such diseases. A second section is dedicated to molecules able to prevent the formation of the ternary complex N-methyl-D-aspartate （NMDA）type glutamate receptors, postsynaptic density-95 （PSD95） protein-nNOS, which is necessary to activate the latter for the biosynthesis of NO.

Necrotizing Pneumonia and Conservative Treatment: A Case Report and Review of the literature

We present a case of necrotizing pneumonia in an 87-year-old man without severe respiratory instability. Clinical suspicion arose due to the need for supplementary oxygen and persistent fever during treatment for community-acquired pneumonia. The diagnosis was confirmed by chest computed tomography. Although necrotizing pneumonia typically requires major surgical intervention upon diagnosis, we chose conservative management with antimicrobials and chest drainage alone. The patient experienced significant improvement and resolution of pneumonia with conservative management.

Conformity to Traditional Gender Norms in a Feminized Occupation:The Influence on Health Behaviors

The object of this study is to examine the relationships between conformity to traditional gender norms and health in a special group of the Spanish population, male/female nurses. Nursing is a particularly good example of feminized occupation. Spanish male (n = 98) and female (n = 98) nurses matched in diverse sociodemographic variables complete measures of gender norms and health behavior variables. Measures of conformity to gender norms in female and male nurses are compared with those of the general population. The analysis of the inter-gender relationships and health highlights a clearly masculine pattern for men and a clearly feminine one for women. A detailed analysis of the scores in each of the scales reveals a pattern of illness in male nurses that is still masculine despite their lower conformity, as a group, to masculine norms, and a clearly feminine illness pattern in female nurses. The consideration of gender from a multidimensional perspective (the different factors that make up Masculinity and Femininity, as assessed in this work) has allowed us to study the relations between gender norms and health in more detail. limitations and implications are discussed.
