With rapid advancement in surgical techniques and improvement in implant materials, rate of internal fixation for pubic symphyseal disruption in rotationally and vertically unstable pelvic ring injuries has increased. Among various modes of implant failure, screw/ plate breakage and loosening are common complications following unstable fixation. Migration of loose screws into the urinary bladder has been reported as an extremely uncommon complication of pubic symphyseal plating. Here we present a case report of a 52-year-old female who pre- sented with asymptomatic passage of screws in her urine following migration into the bladder, 2 years after symphy- seal plating for pubic diastasis in an anteroposterior compression pelvic ring injury.

Purpose:Comminuted intraarticular distal radial fractures are difficult to treat conservatively and require operative *** study compared the functional outcomes between variable angle volar plating and external fixator with K-wire augmentation in open reduction and internal ***:A total of 62 adult patients with comminuted intraarticular distal radius fracture were randomized into 2 groups:volar plate group and external fixator *** patients aged between 18 and 60 years had unilateral fractures,and agreed to be included in the *** with a history of fracture,bilateral fracture,associated other injuries,delayed injury for more than 2 weeks,open fracture,pre-existing arthrosis or disability,psychiatric illness and pathological fracture were *** were followed up at 6 weeks,3 months,6 months and 1 *** assessment of pain,functional activity,range of motion and grip strength was done at each stage of *** pain and functional activities were assessed by patient rated wrist evaluation(PRWE)score and disabilities of the arm,shoulder and hand(DASH)***:Patients in volar plate group had superior PRWE score and DASH score at each stage of *** 1 year follow-up,the mean PRWE score were 7.48 for volar plate group and 7.35 for external fixator group;while the mean DASH score was 4.65 for volar plate group and 5.61 for external fixator *** had better flexion and extension range of *** also had better pronation and supination range of motion at initial follow-up,however the difference get attenuated by 1 *** plate group had significantly better grip strength than external fixator *** rates were higher in external fixation ***:Fixation with variable angle volar plate results in early wrist mobilization,better range of movement,less pain and disability and early return of function.

The intention behind the present work was to develop a microsponge based novel dosage form for sustained delivery of ***-emulsion solvent diffusion method was employed using Eudragit RS-100 with various drug-polymer ratios for the preparation of *** optimization purposes,several factors which affect microparticles’physical properties were *** techniques followed for the formed microsponges were DSC,FTIR,SEM,XRD and particle size analysis,along with morphology,drug loading and in vitro drug *** was found that there were no chemical interactions between drugs and polymers used as per DSC and FTIR *** drug-polymer ratio showed remarkable impact on drug content,encapsulation efficiency and particle *** micrographs revealed that microsponges were spherical in shape with porous surface,and had 104±0.22μm mean particle *** microsponges were then loaded in capsules followed by in vitro drug release study;which depicted that microsponges with drug-polymer ratio of 1:2 were more proficient to give extended drug release of 76.38%at the end of 8 h,superior in contrast to conventional marketed formulation Domstal®,which got exhausted incredibly earlier by releasing 82.57%drug at the end of 1⁄2 h ***,the developed microsponge based formulation of domperidone would be an expectant,promising substitute to conventional therapy of gastroparesis,emesis and alike gastric ailments.

BACKGROUND Duodenal variceal bleeding is a rare cause of gastrointestinal bleeding. The most common site is the duodenal bulb. It is usually detected endoscopically but it can be very challenging to diagnose if it is located distal to the second part od duodenum. The pre- transjugular intrahepatic portosystemic shunt (TIPS) presence of spontaneous portosystemic shunt (SPSS) was found to be associated with an increased risk of early morbidity and mortality after TIPS placement. CASE SUMMARY A 43-year-old cirrhotic male presented with melena for three days. Upper endoscopy was performed and showed active blood oozing from the distal duodenum concerning for ectopic duodenal varix. A computed tomography (CT) angiogram was performed and showed an enlarged cluster of venous collaterals around the distal duodenum. He underwent TIPS placement. He had another episode of melena three days later. Push enteroscopy with injection sclerotherapy into the duodenal varices was performed with no success. A repeat CT angiogram showed occluded TIPS shunt. Therefore, a TIPS revision was performed and there was an extensive portal venous thrombosis with a large shunt between the inferior mesenteric vein and left renal vein via the left gonadal vein. Thrombectomy and TIPS shunt balloon angioplasty was performed, followed by embolization of the portosystemic. The melena was resolved, and patient was discharged with arranged hepatology follow up. CONCLUSION It importance to look and embolize the SPSS shunts in patients with early TIPS dysfunction and recurrent duodenal variceal bleeding.

Following an elective transphenoidal resection of a pituitary tumor, a 66-year-old Hispanic male acutely developed fulminant hepatic failure and severe coagulopathy. He received parenteral corticosteroids the day prior to surgery, and was first noted to have significant coagulopathy intraoperatively. Despite aggressive workup and treatment for fulminant hepatic failure, the patient developed multiorgan failure by post-operative day 2, and expired on post-operative day 3. On post-mortem examination, hemorrhagic necrosis of the liver was noted and microscopic examination revealed Cowdry type A inclusions, consistent with Herpes Simplex Virus (HSV) infection. This diagnosis was subsequently confirmed by immunehistochemistry. Fulminant hepatic failure due to HSV is a rare but highly fatal disease if untreated. Most case reports include only immunocompromised or pregnant patients. However, the lack of clinical suspicion in this previously healthy patient may have delayed prompt treatment with antiviral agents. It is important to raise the awareness of this rare and life-threatening, but potentially treatable, etiology when clinicians are faced with acute idiopathic fulminant hepatic failure.

AIM:To summarize the experience of response to COVID-19 outbreak at a tertiary eye care institute and its network of health facilities in ***:Our responses are based on the principles of social distancing,hand hygiene,respiratory etiquettes,surface disinfection protocol,and rational use of appropriate personal protective equipment(PPE).We describe our response in terms of administrative controls,clinical protocols,staff protection,environmental controls,and social distancing *** also discuss our communication strategies and monitoring systems,to ensure compliance to ***:Administrative control is mainly related to formation of task force and its *** protocols are related to patient triaging methods and clinical examination guidelines in Outpatient,Inpatient and Operating *** protection is focused on training staff on the protocols to be followed in hospital as well as at home,and use of *** protocol is focused on cleaning and disinfectant methods to be used in the *** addition,there are systems for communication as well as monitoring compliance to ***:We hope that these protocols and our experience would help the ophthalmic community globally and serve as a guide to protect ophthalmologists and ophthalmic care personnel,and their patients across the world.

The heavy fermion ferromagnet CeRh6Ge4is the first example of a clean stoichiometric system where the ferromagnetic transition can be continuously suppressed by hydrostatic pressure to a quantum critical *** order to reveal the outcome when the magnetic lattice of CeRh6Ge4is diluted with non-magnetic atoms,this study reports comprehensive measurements of the physical properties of both single crystal and polycrystalline samples of *** increasing x,the Curie temperature decreases,and no transition is observed for x>0.25,while the system evolves from exhibiting coherent Kondo lattice behaviors at low x to the Kondo impurity scenario at large ***,non-Fermi liquid behavior is observed over a wide doping range,which agrees well with the disordered Kondo model for 0.52≤x≤0.66,while strange metal behavior is revealed in the vicinity of xc=0.26.

Halo fixation devices are often employed for critically ill or trauma patients with unstable cervical pathologies. These include fractures, spinal decompression and reconstruction procedures. However, the critical care literature has surprisingly little information in regard to associated complications. Perry and Nickel pioneered the initial halo device in 1959 and soon afterward recognized complications associated with its use [1]. They developed a detailed regimen to prevent abnormal pin placement and infections. The details include pin placement in “safe” zones, specific degrees of torque, and techniques to minimize infection risk. Despite a low death rate, a cerebral brain abscess often leads to prolonged neurological morbidity [2]. Seizures and pneumocranium have also been ascribed to intracranial penetration of halo pins [3,4]. The following describes a patient with cerebral abscess secondary to halo pin penetration. He then developed several other associated complications during hospitalization.
