Is myopia a protective factor against central serous chorioretinopathy?

AIM： To evaluate if any association exists between central serous chorioretinopathy （CSCR） and the refractive status of the eye. METHODS： This retrospective, institutional, case control study included 499 patients, wherein 262 patients diagnosed as acute CSCR, were compared with an age and gender matched control group of 237 patients. All patients were evaluated with a detailed systemic and ocular history, obj.ctive and subj.ctive refractions for both eyes and complete ocular examination by a retina specialist, at all visits. Optical coherence tomography confirmed the diagnosis of CSCR. ~ RESULTS： The mean age was found to be 40.7y in the study group （Group 1） compared to 38 10y in the control group （Group 2）. Most common refractive status in the study group, was emmetropia seen in 191 patients （72.9%）, followed by hypermetropia seen in 47 patients （17.9%） and astigmatism seen in 21 patients （8.0%）. Only 3 subj.cts （1.1%） had myopia, which was less than or equal to 1.0 D, compared to 70 subj.cts （29.5%） in the control group, suggesting a statistically significant lower incidence of CSCR among the myopic patients （P〈 0.0001）. With respect to the systemic factors, 26 （9.9%） patients were using systemic steroids in the study group （Group 1） compared to none in the control group （Group 2） suggesting a statistically significant association of CSCR with systemic steroid use （P〈0.05）. No other significant systemic risk factors were noted. CONCLUSION： Though CSCR is a multifactorial disease, myopia serves as a protective factor for CSCR. Thus, myopic eyes are less likely to develop CSCR. Since both retinal pigment epithelium （RPE） and choriocapillaris are postulated in the pathogenesis of CSCR, chorio-retinal thinning and atrophy seen in myopic eyes are less likely to cause CSCR.

Cytogenetic analysis of chromosomal abnormalities in Sri Lankan children

Background: Cytogenetic analysis is a valuable investigation in the diagnostic work up of children with suspected chromosomal disorders. The obj.ctive of this study was to describe the prevalence of various types of chromosomal abnormalities in Sri Lankan children undergoing cytogenetic analysis. Methods: Cytogenetic reports of 1554 consecutive children with suspected chromosomal disorders who underwent karyotyping in two genetic centers in Sri Lanka from j.nuary 2006 to December 2011 were reviewed retrospectively. Results: A total of 1548 children were successfully karyotyped. Abnormal karyotypes were found in 783 (50.6%) children. Numerical and structural abnormalities accounted for 90.8% and 9.2%, respectively. Down syndrome was the commonest aneuploidy identifi ed. Other various autosomal and sex chromosomal aneuploidies as well as micro-deletion syndromes were also detected. Conclusions: The prevalence of chromosomal abnormalities in Sri Lankan children undergoing cytogenetic analysis for suspected chromosomal disorders was relatively higher than that in Caucasian and other Asian populations.

Prospect of Fuelwood Plantations for Marginal Small Tea Farmers: A Case Study in Matara and Badulla Districts, Sri Lanka

This study demonstrates the importance of conversion of tea farms in marginal land to fuelwood plantation by analyzing the current biophysical, economic and institutional pros and cons of each land uses for marginal small tea plantation holders (MSTH). The study is based on household survey and field investigation conducted in Matara and Badulla Districts of Sri Lanka. Both qualitative and quantitative data on important biophysical, economic and institutional factors was collected from a total of 81 MSTH, 50 from Matara and 31 from Badulla, and fuelwood consuming industries within 20 km of the study area. The result showed that MSTH are facing biophysical and economic problems that are forcing them to leave portion of their tea planation land uncultivated. With ongoing demand increase for fuelwood, presence of tree species already adaptable to the area and favorable property right condition, conversion of the existing marginal land to fuelwood plantation is a viable way to sustainably manage the MSTH farmland. This will also contribute to tackle environmental disasters the area is frequently facing due to abandoning of the marginal tea plantation land.

Mixture models detect large effect QTL better than GBLUP and result in more accurate and persistent predictions

Background： Accurate evaluation of SNP effects is important for genome wide association studies and for genomic prediction. The genetic architecture of quantitative traits differs widely, with some traits exhibiting few if any quantitative trait loci （QTL） with large effects, while other traits have one or several easily detectable QTL with large effects. Methods： Body weight in broilers and egg weight in layers are two examples of traits that have QTL of large effect A commonly used method for genome wide association studies is to fit a mixture model such as BayesB that assumes some known proportion of SNP effects are zero. In contrast, the most commonly used method for genomic prediction is known as GBLUP, which involves fitting an animal model to phenotypic data with the variance-covariance or genomic relationship matrix among the animals being determined by genome wide SNP genotypes. Genotypes at each SNP are typically weighted equally in determining the genomic relationship matrix for GBLUP. We used the equivalent marker effects model formulation of GBLUP for this study. We compare these two classes of models using egg weight data collected over 8 generations from 2,324 animals genotyped with a 42 K SNP ***： Using data from the first 7 generations, both BayesB and GBLUP found the largest QTL in a similar well-recognized QTL region, but this QTL was estimated to account for 24 % of genetic variation with BayesB and less than 1% with GBLUP. When predicting phenotypes in generation 8 BayesB accounted for 36 % of the phenotypic variation and GBLUP for 25 %. When using only data from any one generation, the same QTL was identified with BayesB in all but one generation but never with GBLUP. Predictions of phenotypes in generations 2 to 7 based on only 295 animals from generation 1 accounted for 10 % phenotypic variation with BayesB but only 6 % with GBLUP. Degradation of naturally occurring and engineered antimicrobial peptides by proteases

We hypothesized that current antimicrobial peptides should be susceptible to proteolytic digestion. The antimicrobial peptides: Griffithinsin, RC-101, LL-37, LSA-5, PSC-RANTES and Dj.07 were degraded by commercially available proteases. Two different species of anaerobic vaginal flora, Prevotella bivia and Porphyromonas asaccharolytica also degraded the materials. Griffithsin was resistant to digestion by 8 of the 9 proteases and the bacteria while LL-37 was the most sensitive to protease digestion. These data suggests most of the molecules may not survive for very long in the proteolytic rich environments in which they are intended to function.

In vivo and ex vivo confocal endomicroscopy of pancreatic cystic lesions: A prospective study

To investigate the reproducibility of the in vivo endoscopic ultrasound (EUS) - guided needle based confocal endomicroscopy (nCLE) image patterns in an ex vivo setting and compare these to surgical histopathology for characterizing pancreatic cystic lesions (PCLs).METHODSIn a prospective study evaluating EUS-nCLE for evaluation of PCLs, 10 subj.cts underwent an in vivo nCLE (AQ-Flex nCLE miniprobe; Cellvizio, MaunaKea, Paris, France) during EUS and ex vivo probe based CLE (pCLE) of the PCL (Gastroflex ultrahigh definition probe, Cellvizio) after surgical resection. Biopsies were obtained from ex vivo CLE-imaged areas for comparative histopathology. All subj.cts received intravenous fluorescein prior to EUS and pancreatic surgery for in vivo and ex vivo CLE imaging *** total of 10 subj.cts (mean age 53 ± 12 years; 5 female) with a mean PCL size of 34.8 ± 14.3 mm were enrolled. Surgical histopathology confirmed 2 intraductal papillary mucinous neoplasms (IPMNs), 3 mucinous cystic neoplasms (MCNs), 2 cystic neuroendocrine tumors (cystic-NETs), 1 serous cystadenoma (SCA), and 2 squamous lined PCLs. Characteristic in vivo nCLE image patterns included papillary proj.ctions for IPMNs, horizon-type epithelial bands for MCNs, nests and trabeculae of cells for cystic-NETs, and a “fern pattern” of vascularity for SCA. Identical image patterns were observed during ex vivo pCLE imaging of the surgically resected PCLs. Both in vivo and ex vivo CLE imaging findings correlated with surgical *** vivo nCLE patterns are reproducible in ex vivo pCLE for all maj.r neoplastic PCLs. These findings add further support the application of EUS-nCLE as an imaging biomarker in the diagnosis of PCLs.

XGBoost model for electrocaloric temperature change prediction in ceramics

An eXtreme Gradient Boosting(XGBoost)machine learning model is built to predict the electrocaloric(EC)temperature change of a ceramic based on its composition(encoded by Magpie elemental properties),dielectric constant,Curie temperature,and characterization conditions.A dataset of 97 EC ceramics is assembled from the experimental *** sampling data from clusters in the feature space,the model can achieve a coefficient of determination of 0.77 and a root mean square error of 0.38 K for the test *** analysis shows that the model captures known physics for effective EC *** Magpie features help the model to distinguish between materials,with the elemental electronegativities and ionic charges identified as key *** model is applied to 66 ferroelectrics whose EC performance has not been ***-free candidates with a predicted EC temperature change above 2 K at room temperature and 100 kV/cm are identified.

Paracentesis in cirrhotics is associated with increased risk of 30-day readmission

AIM To determine the readmission rate, its reasons, predictors, and cost of 30-d readmission in patients with cirrhosis and *** A retrospective analysis of the nationwide readmission database(NRD) was performed during the calendar year 2013. All adults cirrhotics with a diagnosis of ascites,spontaneous bacterial peritonitis, or hepatic encephalopathy were identified by ICD-9 codes. Multivariate analysis was performed to assess predictors of 30-d readmission and cost of *** Of the 59597 patients included in this study, 18319(31%) were readmitted within 30 d. Maj.rity(58%) of readmissions were for liver related reasons. Paracentesis was performed in 29832(50%) patients on index admission. Independent predictors of 30-d readmission included age 3 Elixhauser comorbidity(OR: 1.13; CI: 1.05-1.22), nonalcoholic cirrhosis(OR: 1.16; CI: 1.10-1.23), paracentesis on index admission(OR: 1.28; CI: 1.21-1.36) and having hepatocellular carcinoma(OR: 1.21; CI: 1.05; 1.39). Cost of index admission was similar in patients readmitted and not readmitted(P-value: 0.34); however cost of care was significantly more on 30 d readmission($30959 ± 762) as compared to index admission($12403 ± 378), P-value: < *** Cirrhotic patients with ascites have a 33% chance of readmission within 30-d. Younger patients, with public insurance, nonalcoholic cirrhosis and increased comorbidity who underwent paracentesis are at increased risk of readmission. Risk factors for unplanned readmission should be targeted given these patients have higher healthcare utilization.

Dyes are an integral part of the dying industry and have significantly resulted in environmental pollution by altering the standardwater quality after their discharge into the water *** culprits behind the altered water quality are the pretreatment chemicals used during dying manufacturing *** advanced treatmentmethods using conventional and advanced treatment options including solar energy have been put forth by researchers for the treatment of the dying effluents but,these methods have not proved significantly ***,the present study intends to check the efficiency of solar parabolic trough collector for treating the dying effluents in terms of color and ***,other wastewater parameters(BOD,nitrate and phosphate)have also been considered for this experimental *** standard flow rates(0.5,1.3,1.95 and 2.6 L/M)were maintained during the experiment for six hours at solar intensity(849.3±21.2W/m^(2))and the optimized flow rate(1.95 L/M)was detected and considered for further study during the present *** color concentration showed a significant reduction(≥76.4%)by treatment with the designed solar *** significant results were also noticed in terms of COD(79%).In this context,the current experimental study provides ideas on the decolorization and COD reduction efficiency with optimal flow rate in terms of cost-effectiveness with designed experimental reactor which could be further used and implemented for advanced scientific purposes.
