Weathering Processes on Martian Craters: Implications on Recurring Slope Lineae and the Location of Liquid Water

Recent attention has been put into recurring slope lineae (RSL), after the discovery that water is present in them. It is assumed that RSL are due to flowing water. However, even though that might be the case, the general characteristics of RSL as well as their seasonal and spatial distribution in Mars, and their occurrence within craters, suggest that RSL correspond to the weathering of frozen aquifers, which coincides with slope stability processes occurring in impact craters and scree slopes from Earth. In this study, we associated RSL with similar weathering processes occurring on impact craters and hydrogeological processes occurring on Earth (including ice, water, and wind erosion and natural aquifer recharge processes). We were able to create a conceptual model on how RSL develop, why are they found mostly in mid latitudes around craters, why are they present in more frequency in one side of crates in high latitudes, and why are there more RSL in the Martian southern hemisphere. Considering the whole hydrogeological processes occurring in craters that experience RSL, we were able to predict where large quantities of liquid water are most likely to be present in the red planet.

Post-transplant plasma cell hepatitis in a liver transplant patient treated with pegylated interferon plus ribavirin

The recurrence of hepatitis C after a liver transplant remains an important cause of graft loss and retransplant. Antiviral therapy with peginterferon plus ribavirin (PEG-INF/RBV) can achieve a sustained viral response and histological improvement in a high percentage of cases. However, this treatment is not exempt from important side effects or from the possibility of precipitating rejection, with the resulting graft loss. We report the case of a liver transplant patient who received treatment with PEG-INF/RBV and developed plasma cell hepatitis as the presenting form of rejection.

Using fuzzy-AHP and parametric technique to assess soil fertility status in Northeast of Iran

A good understanding of the levels and distribution patterns of soil properties and/or quality indicators is a prerequisite for developing sustainable agricultural land management programs. Traditional assessments of these parameters of soil fertility status are somewhat costly, in both economics and time aspects. Different modelling techniques have been proposed as a useful tool for determination of soil quality indicators and development of soil fertility maps, but to what extent these results are reliable remains under-quantified in many regions worldwide. To address this uncertainty, Fuzzy-Analytical Hierarchy Process(Fuzzy-AHP) and Parametric analyses were conducted to ascertain the soil fertility status of a semiarid region in the Northeast of Iran for some selected crops: alfalfa, corn silage, potato, sugar beet, tomato and wheat. The Fuzzy-AHP and Parametric techniques using soil suitability indices were estimated for each crop and each soil delineation was achieved by Ordinary Kriging. The Technique for Order of Preference by Similarity to Ideal Solution(TOPSIS) was used as a compensatory method to allow tradeoffs among the selected criteria. Our results demonstrated that from the Fuzzy-AHP analysis, the soil fertility indices ranged from moderate to high for production of alfalfa;from low to high for production of corn silage and sugar beet;moderate to high for production of potato and tomato;and from low to moderate for production of wheat. However, the parametric analysis showed soil fertility classes ranging from very low to high for production of all the selected crops. High correlations were also observed between soil fertility indices predicted by both models. Similarly, the capacities of both models to predict soil fertility status for production of the selected crops were also highly correlated. The preference for the cultivation of the selected crops based on the Fuzzy-AHP analysis was sugar beet > corn silage > wheat > alfalfa > tomato > potato.

Background: Starr-Edwards prosthetic valves were used for valve replacement, but due to their high thrombogenic risk, they were withdrawn from market. Nevertheless, there are some cases of Starr-Edwards prosthetic valve carriers that have shown long-term survival reaching up to 50 years. The objective of this study was to determine survival in 12 patients with mechanical Starr-Edwards?prosthetic valve and risk factors for predicting valve dysfunction. Methods: Cross-sectional study of patients who had valve replacement with a Starr-Edwards prosthetic valve in a single center from 1968 to 1990. Socio-demographic data, valvular dysfunction variables and mortality were recorded. Logistic regression models to determine valvular dysfunction were constructed. Survival was analyzed with Cox regression and Kaplan-Meier survival curves. Results: A total of 12 patients were analyzed. The median age was 59 years (48.5 - 64). Eleven patients had normal right and left ventricular function. The most common cause of valve replacement was rheumatic valve disease (75%) and it was more frequently in mitral position (50%). Valvular dysfunction was detected in 3 patients (25%). Atrial fibrillation had the highest association with valvular dysfunction (P = 0.005). Stroke was seen in 25% of the population and the overall mortality was 33.3%. Conclusions: The survival of patients with Starr-Edwards prosthetic valve was 66.66% in the 50-year follow-up. Atrial fibrillation had the highest association with prosthetic valvular dysfunction.

Isolation of Microorganisms According to Burned Body Surface: Experience in a Reference Center in Mexico City

Background: Burns is one of the most common and severe forms of trauma, as they cause an alteration in the protective barrier exerted by the skin. The objective of this work is to describe the bacterial pathogens that cause infections in the different body segments as well as to know the susceptibility in patients with burns during a period of two years. Methods: This study was conducted over a period of 2 years, January 2017 to January 2019. The analysis of the isolated microbiological agents was obtained from culture samples in different anatomic areas. The variables were measured in the general population and also by groups, considering: age, sex, TBSA with Wallace's rule of nine, anatomic area affected, type of burn, depth of burn, microorganism reported in cultures, antibiotics empirically administered and mortality rate. Results: Regardless of the anatomical segment studied, the most frequently isolated microorganism was Pseudomonas aeruginosa. Conclusions: Identification of the microorganisms that cause infection reduces the hospital stay, as well as the admission to the ICU. A fundamental research guideline is to pay more attention to the post-burn consequences, and the sequelae and functional limitations that it entails, since few since few studies focus on this aspect.

Time to Progression of AFP(TPA)as a Predictor of Survival in Hepatocellular Carcinoma Treated with Sorafenib(SOR)

Background: The standard therapy in advanced hepatocellular carcinoma (HCC) is sorafenib (SOR), which has the inconvenience of toxicity and discontinuation. Patient selection and the use of early markers are critical for optimizing the potential benefit of SOR. Alpha-fetoprotein (AFP) has an established role in HCC prognosis. The objective was to evaluate whether AFP variation during SOR treatment reflects the lack of progression to SOR and can be used as a prognostic factor. Methods: AFP levels were prospectively analyzed in 114 patients to determine whether the time to progression of AFP (TPA) at 3 months had a prognostic value for survival. Results: Between July 2007 and October 2012, 114 patients were included (mean age 64 years, 97 male, 96 with cirrhosis). Etiology was alcohol 47 (41%) and hepatitis C virus (HCV) 31 (27%). According to the Barcelona Clinic Liver Cancer (BCLC) staging system: A (one case), B (24 cases) and C (89 cases). The Child-Pugh was Class A in 89 cases. The general condition of the patient according to ECOG-PS was 0 in 73 cases. The median duration of treatment was 5 months (3.47 - 6.53, 95% CI). The median overall survival (OS) was 9.23 months. The standard dose was maintained in 26 patients (22.8%). Sixty-seven percent of patients experienced at least one adverse event grade 3-4. The time to progression of AFP lower or higher than 3 months was an independent prognostic factor of OS (univariate and multivariate analysis): 8.10 vs. 18.85 months, P 3 months had longer OS, and TPA was an independent prognostic factor.

Role of double-balloon enteroscopy in malignant small bowel tumors

AIM: To assess the double-balloon enteroscopy(DBE) role in malignant small bowel tumors(MSBT).METHODS: This is a retrospective descriptive study performed in a single center. All consecutive patients who underwent a DBE with final diagnosis of a malignant neoplasm from 2004 to 2014 in our referral center were included. Patient demographic and clinical pathological characteristics were recorded and reviewed. MSBT diagnosis was achieved either by DBE directed biopsy with multiple tissue sampling, endoscopic findings or histological analysis of surgical specimen. We have analyzed double-balloon enteroscopy impact in outcome and clinical course of these patients. RESULTS: Of 627 patients, 28(4.5%)(mean age = 60 ± 17.3 years) underwent 30 procedures(25 anterograde, 5 retrograde) and were diagnosed of a malignant tumor. Patients presented with obscure gastrointestinal bleeding(n = 19, 67.9%), occlusion syndrome(n = 7, 25%) and diarrhea(n = 1, 3.6%). They were diagnosed by DBE biopsy(n = 18, 64.3%), histological analysis of surgical specimen(n = 7, 25%) and unequivocal endoscopic findings(n = 2, 7.1%). Gastrointestinal stromal tumor(n = 8, 28.6%), adenocarcinoma(n = 7, 25%), lymphoma(n = 4, 14.3%), neuroendocrine tumor(n = 4, 14.3%), metastatic(n = 3, 10.7%) and Kaposi sarcoma(n = 1, 3.6%) were identified. DBE modified outcome in 7 cases(25%), delaying or avoiding emergency surgery(n = 3), modifying surgery approach(n = 2) and indicating emergency SB partial resection instead of elective approach(n = 2).CONCLUSION: DBE may be critical in the management of MSBT providing additional information that may be decisive in the clinical course of these patients.

Erosion Behavior of AISI 6061-T6

In this work, erosion tests were performed to study the behavior of an aluminium alloy known as AISI 6061-T6 against the impact action of aluminium oxide abrasive particles. This material was selected because of its high ductility and tenacity. An erosion rig based on that in ASTM G76-95 was used to conduct the tests. The alumina particles had a particle size between 300 and 400 μm. Four incident angles, 30°, 45°, 60° and 90° were used to conduct the erosion tests. The particle velocity and the abrasive flow rate were 24 ± 2 m/s and 63 ± 0.5 g/min. The room temperature was between 35°C and 40°C. Chemical analysis of the material and abrasive particles were obtained using energy dispersive X-ray analysis (EDS). SEM images were employed to identify the wear mechanisms, which were characterized by high plastic deformation. The erosion rates were obtained and the results indicated that the maximum erosion damage was achieved at 30° reducing progressively at normal incidence.

Breast Reconstruction: Does It Worth to Start If the Patient Is Not Sure to Take All the Process Along?

Introduction: Breast cancer is the most common cancer in women worldwide, representing a major public health problem. There is still little information comparing the satisfaction of the patients who finished their process against the ones who start it but did not finish it. The aim of our retrospective study was to analyze the results in terms of satisfaction after one year of undergoing to complete breast reconstruction (CBR) vs incomplete breast reconstruction (IBR). Materials and Methods: Retrospective study of patients that underwent breast reconstruction (BR) surgery after mastectomy for breast cancer treatment. Performed at Hospital Central Sur de Alta Especialidad PEMEX in Mexico City, including patients from January 1, 2015 to January 01, 2020. Demographic baseline variables were included. BREAST-Q satisfaction questionnaires one year after the last reconstructive procedure were analyzed. Results: A total of 44 patients fulfilled the inclusion criteria. Of these 44 patients, 11 were included in the IBR group, and 33 patients in the CBR group. There were no statistically significant differences between the two groups in terms of age (IBR 56.09 vs CBR 53.06 years, p = 0.321);BMI (IBR 27.94 vs CBR 26.40, p = 0.253), time from mastectomy to first reconstructive procedure (IBR 22.8 vs CBR 31 months, p = 0.957), history of chemotherapy (IBR 27.3% vs CBR 33.3%, p = 0.709) and radiotherapy (IBR 54.5% vs CBR 42.4%, p = 0.484), additionally type of reconstruction, affected side or complication rate were not significantly different. Regarding postoperative satisfaction, only the second module of satisfaction with breasts displayed statistically significant differences, with a higher score in the CBR group (46.27 vs 52.27, p = 0.019). Background: In clinical practice, acute and chronic wounds continue to be a challenge. Unfortunately, wound care is often inadequate or inappropriate. The implementation of wound clinics involving an interdisciplinary team for wound care and treatment yields favorable clinical outcomes as well as a reduction in economic expenses due to a higher rate of healing, a decrease in hospital stays, and a reduction in the number of readmissions. The establishment of wound clinics enhances the utilization of diagnostic, preventive, and therapeutic resources, improving the quality of life of patients and facilitating their reintegration into their normal lives. The objective is solely to describe the importance of wound clinics in the medical profession. Methods: A retrospective literature search was conducted to include articles no more than 10 years old that reported measures or results of the study in order to perform a literature review and describe the importance of wound clinics in medical practice. Results: A total of 7 articles were obtained with the inclusion criteria, the most recent being from the year 2023 and the oldest from 2013. Conclusion: An important area for improvement in the healthcare industry is the management and treatment of complex wounds. The establishment of wound clinics offers comprehensive and specialized care for both acute and chronic complex wounds with the goal of enhancing secondary illness patients’ prevention, treatment, and rehabilitation.
