Influence of irritable bowel syndrome on treatment outcome in gastroesophageal reflux disease

AIM: To investigate the influence of irritable bowel syndrome (IBS)-like symptoms on treatment outcomes with pantoprazole in gastroesophageal reflux disease (GERD) in a real life setting. METHODS: For this prospective, open-label, multinational, multicentre study, 1888 patients assessed by the investigators as suffering from GERD were recruited. The patients were additionally classified as with or without IBS-like symptoms at baseline. They were treated with pantoprazole 40 mg once daily and completed the Reflux Questionnaire (ReQuest) short version daily. Response rates and symptom scores were compared after 4 and 8 wk of treatment for subgroups defined by the subclasses of GERD [erosive(ERD) and non-erosive reflux disease (NERD)] and the presence of IBS-like symptoms. RESULTS: IBS-like symptoms were more prevalent in NERD than in ERD (18.3% vs 12.7%, P = 0.0015). Response rates after 4 and/or 8 wk of treatment were lower in patients with IBS-like symptoms than in patients without IBS-like symptoms in both ERD (Week 4: P com- plaints" symptom scores were highest in NERD patients with IBS-like symptoms. Additionally, these patients had the strongest symptom improvement after treatment compared with all other subgroups. cONcLUSION: IBS-like symptoms influence treatment outcome and symptom burden in GERD and should be considered in management. Proton pump inhibitors can improve IBS-like symptoms, particularly in NERD.

Reduced combustion time model for methane in gas turbine flow fields

computational fluid dynamics （cFD） modeling of the complex processes that occur within the burner of a gas turbine engine has become a critical step in the design process. However, due to computer limitations, it is very difficult to completely couple the fluid mechanics solver with the full combustion chemistry. Therefore, simplified chemistry models are required, and the topic of this research was to provide reduced chemistry models for cH4/O2 gas turbine flow fields to be integrated into cFD codes for the simulation of flow fields of natural gas-fueled burners. The reduction procedure for the cH4/O2 model utilized a response modeling technique wherein the full mechanism was solved over a range of temperatures, pressures, and mixture ratios to establish the response of a particular variable, namely the chemical reaction time. The conditions covered were between 1000 and 2500 K for temperature, 0.1 and 2 for equivalence ratio in air, and 0.1 and 50 atm for pressure. The kinetic time models in the form of ignition time correlations are given in Arrhenius-type formulas as functions of equivalence ratio, temperature, and pressure; or fuel-to-air ratio, temperature, and pressure. A single ignition time model was obtained for the entire range of conditions, and separate models for the low-temperature and high-temperature regions as well as for fuel-lean and rich cases were also derived. Predictions using the reduced model were verified using results from the full mechanism and empirical correlations from experiments. The models are intended for （but not limited to） use in cFD codes for flow field simulations of gas turbine combustors in which initial conditions and degree of mixedness of the fuel and air are key factors in achieving stable and robust combustion processes and acceptable emission levels. Evaluation of Spontaneous Fertility after Medical Treatment of Tubal Ectopic Pregnancy in Two Hospitals in the city of Yaounde

Background: Ectopic pregnancy is a major cause of maternal morbidity and mortality, estimated to occur in 1% - 2% of pregnancies worldwide. This condition also has an adverse effect on the fertility prospects of women who experience it. Objective: To determine the outcomes of subsequent spontaneous fertility after medical treatment of patients with methotrexate (MTX) in patients with ectopic pregnancy at two university teaching hospitals of Yaounde. Methodology: We carried out a cross-sectional study with retrospective data collection in two university teaching hospitals of Yaounde during a six years period from 1st January 2015 to 31st May 2021. Seventy records of patients who had medical treatment for ectopic pregnancy were included in this study. Statistical analysis was performed using SPSS. 23. The chi-2 statistical test was used to compare qualitative variables. Binary logistic regression method was performed to identify independent risk factors associated with infertility after medical treatment of tubal ectopic pregnancy (TEP). The significance level was set at 0.05. Results: The mean age in our study population was 27.8 ± 3.8 years. According to the past medical history, 52.9% had a pelvic inflammatory disease (PID) and the most frequently germ found was c. trachomatis (47.1%). Almost 15% of our study population had previous surgery for EP. The median Fernandez score was 11 with a minimum score of 4 and a maximum score of 13. The route of administration of methotrexate was intramuscular in all our patients, and the single-dose protocol was used most frequently (58.6%). After medical treatment of the EP, we found a spontaneous conception rate of 58.6%. After multivariate analysis, we were unable to confirm that there was an association between a history of sexually transmitted infections (STIs) and fertility prognosis. conclusion: The spontaneous fertility rate after medical management of EP was 58.6%, of which 73.2% were term pregnanci

Envita’s Precision cancer care: 35-Fold Improvement in Response Rates

New clinical approaches are imperative beyond the widely adopted National comprehensive cancer Network (NccN) guidelines, utilized by prominent cancer institutions. cancer is the leading cause of death among individuals younger than 85 years within the United States. Despite significant technological advances, including the expenditure of hundreds of billions, treatment outcomes and overall survival have not notably improved for most types of advanced cancer over the last several decades. Over the past 24 years, Envita Medical centers has pioneered a unique form of personalized treatment approach for late-stage and refractory cancer patients, introducing groundbreaking innovations in the field. Our integrated algorithm utilizes advanced genomics, transcriptomics, and highly tailored immunotherapy, resulting in remarkable outcome improvements. This study presents Envita’s innovative personalized treatment algorithms and examines the response outcomes of 199 late-stage cancer patients treated at Envita Medical centers over a two-year period. compared to standard of care and palliative chemotherapy, Envita’s treatment demonstrated a remarkable 35-fold improvement in overall response rates (Figure 1). Moreover, 88% of the patients, the majority presenting with Stage 3 or 4 cancer, experienced a 43-fold improvement in quality of life with minimal side effects, as compared to standard of care chemotherapy and palliative care. This revolutionary success is attributed to Envita’s personalized therapeutic algorithms, which incorporate customized immunotherapy. Envita’s precision care approach has also achieved a 100% better response rate compared to over 65 global chemotherapy clinical trials with more than 2700 patients. The results from this study suggest that a wider utilization of Envita’s personalized approach can significantly benefit patients with late-stage and refractory cancer.

Evaluation of maize inbred lines and topcross progeny for resistance to pre-harvest aflatoxin contamination

Pre-harvest aflatoxin contamination occurs in maize following kernel colonization by Aspergillus flavus. Aflatoxin contamination resistance is a highly desired trait in maize breeding *** identification of novel sources of resistance to pre-harvest aflatoxin contamination is a major focus in germplasm screening efforts. Here, we performed a field evaluation of 64 inbred lines over two years for pre-harvest aflatoxin contamination. Topcrosses were also performed with two testers, B73 and Mo17, to generate 128 F1 hybrids which were also evaluated over two years. Hybrid performance was used to calculate both general combining ability(GcA) of the inbreds, and observed heterosis for aflatoxin resistance. Over both years of the study, aflatoxin concentrations ranged from 80 ± 47 to 17,617 ± 8816 μg kg^(-1) for inbreds, and from 58 ± 39 to 2771 ± 780 μg kg^(-1) for hybrids with significant variation between years and lines. The inbred lines cML52, cML69, cML247, GT-603, GEMS-0005, Hi63, Hp301, and M37 W showed ccumulation in both years of the study and less than the resistant check, Mp313 E, in at least one season. Among these, cML52, GT-603, and Hi63 also showed significant GcA with the testers in hybrid progeny. cML52, GT-603, and M37 W also showed heterotic effects of-13.64%,-12.47%, and-24.50%, respectively, with B73 resulting in reduced aflatoxin contamination. GT-603 also showed a similar heterotic effect for aflatoxin contamination,-13.11%, with Mo17 indicating that this line may serve as a versatile source of aflatoxin contamination resistance in breeding programs.

characterization of energy and performance of swine fed a novel corn-soybean extruded product

Background： A novel extruded product was characterized with a metabolism and lactation trial to establish the product＇s energy content, and its effects on lactating sow performance. The product was composed of a 60：40 corn-soybean blend, which was then extruded. This product containing the co-extruded 60：40 corn-soybean blend was commercially developed and is used extensively in swine diets in southwest Minnesota. GE of dietary treatments were determined by isoperibol bomb calorimetry. Twelve barrows （59.9 ± 1.4 kg）, were used to determine the digestible and metabolizable energy of the extruded product. DE of treatments was determined by subtracting fecal energy from GE provided to barrows by each respective treatment. ME was determined by subtracting urinary energy from calculated digestible energy. Sixty-three sows were used for the lactation trial. Three dietary treatments were utilized： cONTROL （an industry standard diet）; PRODUcT （contained the product, vitamins and minerals）; OIL （matched the lysine：ME ratio of PRODUcT by addition of soy oil）. Sow weight, backfat thickness at the right and left last ribs, body condition score, number of piglets, and litter weights were recorded on the date of farrowing （d 0）, （d 9）, and at weaning. Blood and milk samples were obtained at weaning, and blood was analyzed for plasma urea nitrogen （PUN）, milk was analyzed for total protein and fat content. Results： On a dry-matter basis, the test diet provided 3,908 kcal/kg DE and 3,833 kcal/kg ME, which was significantly greater than the basal diet, which provided 3,633 kcal/kg DE and 3,567 kcal/kg ME （P 〈 0.0001）. These data were used to establish the DE and ME of the product, which were 3,882 kcal/kg and 3,798 kcal/kg, respectively, on an as-fed basis. No effect of diet was observed for changes in sow backfat （RBF P = 0.24; LBF P = 0.07） or body condition score （P = 0.12） during lactation. Beyond Li-ion: electrode materials for sodium- and magnesium-ion batteries

The need for economical and sustainable energy storage drives battery research today. While Li-ion batteries are the most mature technology, scalable electrochemical energy storage applications benefit from reductions in cost and improved ***- and magnesium-ion batteries are two technologies that may prove to be viable alternatives. Both metals are cheaper and more abundant than Li, and have better safety characteristics,while divalent magnesium has the added bonus of passing twice as much charge per atom. On the other hand, both are still emerging fields of research with challenges to overcome. For example, electrodes incorporating Na+ are often pulverized under the repeated strain of shuttling the relatively large ion, while insertion and transport of Mg2+ is often kinetically slow, which stems from larger electrostatic forces. This review provides an overview of cathode and anode materials for sodium-ion batteries, and a comprehensive summary of research on cathodes for magnesium-ion batteries. In addition, several common experimental discrepancies in the literature are addressed, noting the additional constraints placed on magnesium electrochemistry. Lastly, promising strategies for future study are highlighted.
