Tuberculosis (TB) is a serious infectious disease and its control is considered a challenge, particularly among vulnerable populations such as prisoners. The prevalence of TB in prisons is an alarming public health problem in many countries. The aim of this study is to describe the epidemiology of tuberculosis and the strategies for controlling this disease in the Chadian prison population. During the course of our study, the prevalence of tuberculosis in prisons was 9.64%. The age group between 55 years and over (43.33%) was the most represented in this study, and all patients were men with a frequency of 62.66%. The cross-tabulation of Culture_7H9G and Culture_7H9P showed that out of our total positive sample, we found a total of 87 positive strains and 63 negative strains. Our study shows that it is essential to know the prevalence of tuberculosis in all the country’s prisons. Indeed, this can serve as an indication of the need for action in prisons to reduce TB rates, in particular by improving the structure of prison environments, diagnosing new cases quickly and accurately, identifying drug-resistant strains and implementing effective, direct treatment observed in people with tuberculosis.

全科医生作为居民健康“守门人”,在基层糖尿病等慢性疾病管理中发挥着重要作用。糖尿病管理可以延缓病情进展、减少糖尿病并发症和改善患者健康结局,这需要患者和医生之间的有效沟通并互相协作。全科医生具备良好的沟通能力有助于与糖尿病患者建立长期的照护关系、帮助患者建立有效的自我管理技能。本文总结了本研究团队在多种理论框架指导下的全科医生糖尿病沟通技能培训的设计和开发:基于系统综述发现的循证依据,通过定性研究进一步探索糖尿病患者与全科医生的沟通体验和想法,以及混合方法研究设计理念获取全科医学对于医患沟通培训内容的优先排序,旨在为高质量的基层糖尿病管理提供新思路,并为基于证据和医学教育框架设计全科医生相关培训项目提供借鉴。

Clinical epidemiology and Surgical Treatment of Spontaneous Perforations of the Terminal Ileum: A Multicentre Study in Cameroon

Background: Perforations of the terminal ileum are a frequent clinical situation and a therapeutic challenge. Surgical treatment is controversial. Several surgical modalities have been proposed and postoperative morbidity and mortality remain high. The aim of this study was to review our experience in the surgical management of perforations of the terminal ileum. Patients and methods: This is a descriptive cross-sectional study with retrospective data collection from January 1, 2017 to December 31, 2021, in five hospitals in Cameroon. Patients’ demographic, clinical presentation, surgical findings and 30-days postoperative outcomes data were collected. Results: We collected 34 files. The sex ratio was 1.4 and the average age was 20.28 years. The average consultation time was 9.1 days. Abdominal pain was present in all our patients. Peritoneal irritation was present in 46 cases (88.5%). The mean time to treatment after admission was 17.5 hours. The perforation was unique in 40 cases (76.9%) and located between 6 and 10 cm from the ileocecal valve in 29 cases (55.8%). Typhoid was the most suspected etiology intraoperatively in 38 cases (73.1%). A simple suture was performed in 18 patients (34.6%), resection with anastomosis in 11 patients (21.2%) and an ileostomy in 5 patients (9.6%). The average length of hospitalization was 21.74 days. Postoperative morbidity and mortality were 32.7% and 17.3% respectively. Conclusion: Perforations of the terminal ileum are common and affect young people. Surgical procedures are varied. Reducing consultation and care times and respecting treatment principles could improve postoperative morbidity and mortality, which remain high.

Introduction The distribution of congenital heart disease (CHD) in sub-Saharan Africa is highly imprecise and varies from one region to another due to the inequality of diagnostic facilities. The aim of this study was to determine the in-hospital prevalence of congenital heart disease in children at the Mother-Child hospital of Bingerville (HMe) by specifying the diagnostic, therapeutic and evolutionary aspects. Materials and methods We conducted a retrospective, descriptive, cross-sectional study at HMe of Bingerville from January 2021 to December 2022. All newborns with congenital heart disease confirmed by echocardiography were included in the study. Results Of 656 admissions to the neonatology department over the study period, congenital heart disease accounted for 14.9% (98/656) of cases. In our series, 76.7% were diagnosed before the 1st week of life, with a mean chronological age of 5.18 days and extremes of 0 and 46 days. There were as many male patients (50%) as female (50%), i.e. a sex ratio of 1. These newborns were premature in 60.2% of cases, with a mean and median gestational age of 34 weeks’ amenorrhea. Most were left-right shunts (90.8%). Persistent ductus arteriosus (PDA) (48.9%) predominated, followed by atrial septal defect (38.7%), ventricular septal defect (13.3%), common trunk artery (CTA) (3.1%) and open septal pulmonary atresia (OSPA) (1%) as the primary cyanogenic heart disease. Pulmonary arterial hypertension (PAH) (50%) was primary in 38.8% and secondary (61.2%). The mortality rate was 30.6%, and all CTA patients died (100%), with a significant statistical relationship (p = 0.027). Progression under treatment was marked by clinical stabilization (68/98) in 69.4% of cases. Conclusion: Congenital heart disease is relatively common at the Bingerville HMe. Access to echocardiography should be facilitated in neonatology departments for rapid diagnosis and optimal management of congenital heart disease in newborns.

Results of Surgical Management of Malignant Obstruction of the Common Bile Duct in Yaoundé

Introduction: In Cameroon, surgery remains the only approach in malignant obstructions of the common bile duct (MOCBD) even in palliative situations. The aim of this work was to describe the modalities of surgical treatment of MOCBD, evaluate the results and detect the factors associated with postoperative morbidity and mortality. Patients and Methods: We conducted an analytical observational study, with retrospective data collection from the files of patients operated upon for MOCBD. This was done in four referral hospitals in the city of Yaoundé for a 42-month period spanning from January 1, 2020 to June 30, 2023. Demographic data, clinical presentation, surgical data, and 30-day postoperative outcomes were collected. Results: We collected 71 files. The sex ratio was 1.4 and the mean age was 56 ± 11 years. Fifty-three (53) patients (74.6%) were overweight or obese and 10 patients (14.1%) were hypertensive. A clinical cholestasis syndrome was present in 69 patients (97.2%). Fifty-five (55) patients (77.5%) had a cancer of the head of the pancreas, 8 patients (11.3%) had an extra-hepatic cholangiocarcinoma and 8 patients (11.3%) had an ampullary adenocarcinoma. eight (8) resections (11.3%) with curative intent had been carried out and in 63 cases (88.7%), surgery was palliative. Postoperative morbidity was 55.7%, influenced by advanced WHO stage (p = 0.02). Postoperative mortality was 25.7%, associated with a high ASA score (p = 0.01). Conclusion: Pancreatic head cancer is the main etiology of malignant obstructions of the common bile duct in Cameroon. Surgical treatment is most often palliative. Postoperative morbidity and mortality are high, influenced by high WHO and ASA scores.

Background: Cancer continues to pose a significant threat to our society, representing one of the most pressing health concerns worldwide. This study aimed to evaluate the chemical composition and the antioxidant activity of aqueous and hydroethanolic extracts from Acacia nilotica (An), Bauhinia reticulate (Br), and Tamarindus indica (Ti) of Fabaceae family, traditionally used in Northern Cameroon for cancer treatment. Methods: The phytochemical screening of the three plants was conducted using conventional colorimetric methods, followed by the measurement of total phenol content, flavonoids, and tannins. The antiradical and antioxidant activities of both plant extracts were assessed through FRAP, ABTS, and DPPH methods. A principal components analysis was employed to correlate the quantities of the evaluated secondary metabolites with the activities. Results: Both types of extracts from the three plants contain alkaloids, saponins, flavonoids, phenolic compounds, tannins, glycosides, terpenoids, coumarins, anthocyanins, and anthraquinones. The aqueous extracts of Br and An are significantly richer (p Conclusion: The three Fabaceae plants from northern Cameroon, prepared in different solvents, can be utilized for their antiradical properties in cancer treatment.

expected adult lifespan in tropical trees: Long-term matrix demography in a large plot

Background: Documenting the entire lifetime of long-lived organisms requires splicing together short-term observations. Matrix demography provides a tool to calculate lifetime statistics, but large samples from juvenile to adult are needed, and few such studies have been done in tropical trees because high species diversity limits sample sizes. The 50-ha plot at Barro Colorado in Panama was designed to provide large samples, and with 30years of censuses, accurate population matrices can be ***: In 31 abundant species, I divided all individuals≥1 cm dbh into 4 or 5 size class in each of seven censuses. Movements of stems between size classes over two censuses are termed transitions, and I constructed complete transition matrices for each species. From the matrices, I derived analytic solutions for lifetime demographic statistics. expected adult lifespan from the sapling stage was the key ***: expected adult lifespan from the sapling stage varied 100-fold over the 31 species, from 0.5 to 50 years,and maturation time varied from 19 to nearly 200 years. Species with the highest growth rates also had high death rates, and theoretical calculations of reproductive lifespan show that the fast-growing pioneer species have short expected adult lifespans relative to the average slow-growing, shade-tolerant species. Within the slowgrowth category, however, there was high variation in expected adult lifespan, and several shade-tolerant species under-performed the pioneers in terms of adult ***: Analytical solutions from population matrices allow theoretical analyses that integrate short-term growth records into lifespans of tropical trees. The analyses suggest that pioneer species must reproduce more successfully than shade-tolerant species to persist in the Barro Colorado forest. My next goal is to incorporate seed production and germination into lifetime matrix demography to test this hypothesis.

Coverage and Factors Associated with Acceptance and Satisfaction of Seasonal Malaria Chemoprevention in Six Health Zones in Benin

Introduction: Seasonal malaria chemoprevention (SMC) was adopted in 2019 in two health zones in Benin where malaria transmission is very high. Positive results led to the extension of the intervention to other zones with additional financial support. Annual SMC campaigns from 2021 to 2023 were carried out in all six health zones in the Atacora and Alibori departments. In five years of implementation, various approaches have been developed on the basis of a communication plan facilitating buy-in and acceptance by all stakeholders. The aim of this study was to assess the effective coverage and acceptance of the SMC by their beneficiary populations in 2023. Methods: It was a cross-sectional study with an analytical focus. Data collection took place from November 30 to December 13, 2023. The study population consisted of children under 5 years of age residing in the departments of Atacora and Alibori in northern Benin. A total of 3573 children under 5 years of age were included in the study, and their parents or guardians were interviewed. Results: During the 2023 campaign, 87.7% of targets were reached by SMC administration and 100.00% of children had received at least one dose of SMC by the fourth visit. effective therapeutic coverage of SMC was 70.55%, with 99.60% in BNK, 69.40% in KGS, 16.20% in MK, 56.10% in 2KP, 92.40% in NBT and 89.60% in TMC. This coverage was statistically related to child and respondent ages (p Conclusion: SMC is a strategy accepted by the population, and the main reasons for non-participation in SMC were dominated by the absence of mothers or babysitters when the agents visited.
