Hepatocellular carcinoma: Where are we?

Hepatocellular carcinoma(HCC) is the second cause of death due to malignancy in the world, following lung cancer. The geographic distribution of this disease accompanies its principal risk factors: Chronic hepatitis B virus and hepatitis C virus infection, alcoholism, aflatoxin B1 intoxication, liver cirrhosis, and some genetic attributes. Recently, type Ⅱ diabetes has been shown to be a risk factor for HCC together with obesity and metabolic syndrome. Although the risk factors are quite well known and it is possible to diagnose HCC when the tumor is less than 1 cm diameter, it remains elusive at the beginning and treatment is often unsuccessful. Liver transplantation is thus far considered the best treatment for HCC as it cures HCC and the underlying liver disease. Using the Milan criteria, overall survival after liver transplantation for HCC is about 70% after 5 years. Many attempts have been made to go beyond the Milan Criteria and according to recent works reasonably good results have been achieved by using a histochemical marker such as cytokeratine 19 and the so-called "up to seven criteria" to divide patients into categories according to their risk of relapse. In addition to liver transplantation other therapies have been proposed such as resection, tumor ablation by different means, embolization and chemotherapy. An important step in the treatment of advanced HCC has been the introduction of sorafenib, the first oral, systemic drug that has provided significant improvement in survival. Treatment of HCC patients must be multidisciplinary and by using the different approaches discussed in this review it is possible to offer prolonged survival and quite good and sometimes even excellent quality of life to many patients.

Transpalpebral eyebrow lift: the mini eyebrow lift

Aim: This paper focus is to describe and evaluate the authors' personal technique for a simplified transpalpebral eyebrow lift. Methods: We performed surgery in 179 patients (95% females and 5% males) between years of 2008 and 2009. Initially, a thorough assessment of brow stability and symmetry was performed. To achieve desirable aesthetic we agreed upon the consensus that the eyebrow lateral third should be 0.3 to 1 cm above the supraorbital rim in women and just above or at supraorbital rim in men. Hence the surgery consisted of the removal of excess skin, treatment for bags if they were present and a transpalpebral lateral eyebrow lift. Results: There were only 5 cases in which patients reported episodes of pain on the temporal undermined area related with the suture with the evidence of pain subsiding over time and disappearing all together within 3 months. No patients required revision surgery and no patients presented laghophtalmos or any eyelid closure difficulties. Conclusion: This paper presents a simplified version of an eyebrow lift technique that is easy to reproduce and effective.

Innovation is managed through the development of processes and routines, considered as critical for business success and value creation. The identification of relevant routines as well as the understanding of which factors affect business innovation is the main objective of this research, based on a survey applied in Portuguese companies and their experience shared in best practices guides. Conclusions highlighted 10 key innovation routines, reinforcing the idea that innovation must be supported through a systematic and sustained management process. Results also indicate that SMEs face greater difficulties in benefiting from the potential of innovation practices and collaboration may present opportunities to overcome their limited resources. Therefore, regardless of size, business activity, science and technology intensity or other factors influencing firms dynamic capabilities, innovation should be considered as a management imperative as it can influence not only the present but also the future of business success.

Squamous cell carcinoma of the esophagus (SCCE) carries a poor prognosis due to late *** detection is highly desirable,since surgical and endoscopic resection offers the only possible cure for esophageal *** screening should be undertaken in high risk areas,and in low or moderate risk areas for people with risk factors (alcoholics,smokers,mate drinkers,history of head and neck cancer,achalasia and lye stricture of the esophagus).Esophageal balloon cytology is an easy and inexpensive sampling technique,but the current methods are insufficient for primary screening due to sampling *** endoscopy with biopsy remains the standard procedure for the identification of pre-malignant and early malignant changes in esophageal mucosa and endoscopic *** may be enhanced by several techniques such as dye and optic chromoendoscopy,magnifying endoscopy,and optical-based spectroscopic and imaging *** more than 80% of SCCE deaths occur in developing countries,where expensive techniques such as narrow band imaging (NBI) and autofluorescence imaging are unavailable,the most cost-effective tool for targeting biopsies may be Lugol dye chromoendoscopy,since it is easy,accurate,inexpensive and available *** ideal conditions,or in developed countries,is it reasonable to think that optimal detection will require a combination of techniques,such as the combination of Lugol’s chromoendoscopy and NBI to identify esophageal areas that require further characterization by a high resolution *** efficacy and cost-effectiveness will determine whether these modalities will become part of standard endoscopy practice.

Artificial intelligence and pituitary adenomas:A review

The aim of this review was to provide an overview of the main concepts in machine learning(ML)and to analyze the ML applications in the imaging of pituitary *** describing the clinical,pathological and imaging features of pituitary tumors,we defined the difference between ML and classical rule-based algorithms,we illustrated the fundamental ML techniques:supervised,unsupervised and reinforcement learning and explained the characteristic of deep learning,a ML approach employing networks inspired by brain’s *** assessment and neurosurgical outcome prediction were the potential ML applications using magnetic resonance *** pre-treatment assessment,ML methods were used to have information about tumor consistency,predict cavernous sinus invasion and high proliferative index,discriminate null cell adenomas,which respond to neo-adjuvant radiotherapy from other subtypes,predict somatostatin analogues response and visual pathway *** neurosurgical outcome prediction,the following applications were discussed:Gross total resection prediction,evaluation of Cushing disease recurrence after transsphenoidal surgery and prediction of cerebrospinal fluid fistula’s formation after *** clinical applicability requires more replicability,generalizability and validation,results are promising,and ML software can be a potential power to facilitate better clinical decision making in pituitary tumor patients.

Long-term monitoring reveals the effect of precipitation and silviculture on Nothofagus regeneration in Northern Patagonia mixed forests

Background Tree regeneration is a key component of resilience because it promotes post-disturbance recovery of forests. Northwestern Patagonia from Argentina is occupied by Nothofagus alpina（Na）, N. obliqua（No）, and N. dombeyi（Nd） forests that grow along an intense precipitation gradient, managed throughout shelterwood silvicultural system by technicians of the Lanin Natural Reserve. The objective of this study was to evaluate the influence of seeding cuttings over the dynamics of Nothofagus mixed forests across landscape（precipitation gradient） based mainly on the composition and abundance of tree regeneration, permanent sampling plots and generalized linear mixed models. In particular, we analysed:（i） the structure of sexual and asexual regeneration during 20 years after harvest（the dynamics of managed forests）, and（ii） the structure of sexual regeneration in primary and managed forests after > 20 years of harvest（the effect of silviculture）.Results Nd was the most abundant species in the regeneration of managed forests during all periods in both sites despite its lower contribution to the adult cohort. During the 10–20 years period after harvest, the humid site exhibited higher regeneration density than the mesic site(120,000 and 6000 ind ha-1, respectively), and it decreased afterwards. The number of established regeneration（> 2 m height） was lower for Na in the mesic site and for No in the humid site(0 and 57 ind ha-1, respectively). However, in comparison to No, Na showed a higher number of sprouted stumps and sprouts per stump, and a higher sprout height in the mesic site. No exhibited higher sprout mortality in the humid site. Consequences of metabolic syndrome on postoperative outcomes after pancreaticoduodenectomy

AIM To analyze immediate postoperative outcomes after pancreaticoduodenectomy regarding metabolic *** In two academic centers, postoperative outcomes of patients undergoing pancreaticoduodenectomy from 2002 to 2014 were prospectively recorded. Patients presenting with metabolic syndrome [defined as at least three criteria among overweight(BMI ≥ 28 kg/m2), diabetes mellitus, arterial hypertension and dyslipidemia] were compared to patients without metabolic *** Among 270 consecutive patients, 29(11%) presented with metabolic syndrome. In univariable analysis, patients with metabolic syndrome were significantly older(69.4 years vs 62.5 years, P = 0.003) and presented more frequently with soft pancreas(72% vs 22%, P = 0.0001). In-hospital morbidity(83% vs 71%) and mortality(7% vs 6%) did not differ in the two groups so as pancreatic fistula rate(45% vs 30%, P = 0.079) and severity of pancreatic fistula(P = 0.257). In multivariable analysis, soft pancreas texture(P = 0.001), pancreatic duct diameter 30 kg/m2(P = 0.041) were identified as independent risk factors of pancreatic fistula after pancreaticoduodenectomy, but not metabolic *** In spite of logical reasoning and appropriate methodology, present series suggests that metabolic syndrome does not jeopardize postoperative outcomes after pancreaticoduodenectomy. Therefore, definition of metabolic syndrome seems to be inappropriate and fatty pancreas needs to be assessed with an international consensual histopathological classification.

Comparison between hepaticojejunostomy and hepaticoduodenostomy after excision of choledochal cyst in children:a cohort study

Objective Hepaticojejunostomy(HJ)and hepaticoduodenostomy(HD)are commonly used biliary reconstruction techniques after choledochal cyst *** has been suggested to be a more physiologic alternative during *** objective of this study is to compare operative time,hospital stay,morbidity(leak,cholangitis,ileus,and obstruction),and mortality between HJ and HD after cyst *** This is a 14-year retrospective cohort study of pediatric patients(≤18 years old)who underwent choledochal cyst excision and subsequent biliary reconstruction at the Philippine Children’s Medical *** were taken from inpatient charts,operative technique,OPD logbook,readmission,and OPD *** There were 122 patients:56%HD and 44%*** were female(72%),with 1:2.6 male to female *** average age was 36.1 months,with a mean follow-up of 32.8 months(range 6 months-14 years).The most common cyst was type I(87%).Operative time was longer for HJ compared with HD(321.3 vs 203.6 min;p=0.000).Hospital stay was longer with HJ compared with HD(7.7 vs 6.8 days;p=0.002).Mortality rate was low at 1.6%while morbidity was at 13.9%in both *** morbidity was higher among those who underwent HD,there was no significant difference between the two *** leak(4%)and cholangitis(7.4%)were observed in HD,and ileus(7.4%)was observed in the HJ *** In our series,HD provided less operative time and hospital stay than with *** did not observe bile gastritis after HD as compared with *** is suggested that longer follow-up is needed to confirm such findings.
