Effect of Periodontal Infection on Birth Weight of Infants: A Clinical and Microbiological Study

Background: Maternal periodontal infection has been proposed to influence pre-term delivery and low birth weight infants through mechanisms involving inflammatory mediators or direct bacterial assault on the amnion. Thus, the aim of the present study was to assess the presence of periodontal pathogens in maternal periodontal infection and their effect on the birth weight of infants. Materials and Methods: The case-control study included 30 mothers with a singleton gestation and gestational age of >37 weeks. Data regarding the periodontal status, pregnancy outcome variables, hemoglobin levels (Hb%) and other factors that may influence adverse pregnancy outcomes were collected. A qualitative analysis of the predominant anaerobic organisms was conducted. The data was analysed using one-way ANOvA test and Scheffe’s test. Results: The microbiological results showed that prevalent colonies of organisms isolated from the experimental groups were of Bacteroides spp. (n = 3), Fusobacterium spp. (n = 3) and Peptostreptococcus spp. (n = 9). In the control group, Peptostreptococcus spp. (n = 15) were predominant. In the experimental group, it was seen that Bacteroides spp. (p = 0.003) and Fusobacterium spp. (p = 0.050) were statistically significant with the birth weight of the infant. The one-way ANOvA test showed that the birth weight of the infant was inversely proportionate with increase in severity of the periodontal disease. Scheffe’s test showed that presence of a periodontal pocket showed most significance to the birth weight of an infant as compared to presence of local factors or bleeding on probing. Conclusion: The study revealed that the gram negative organisms, Bacteroides spp. and Fusobacterium spp. were significantly associated with the low birth weight (LBW) of the infants, although there was not a significant difference in the clinical periodontal status between the experimental groups and the controls. Aqueous Interleukin-6 (IL-6) Level Is a Marker for Treatment Resistance to Bevacizumab in Age-Related Macular Degeneration —Aqueous Cytokines after Bevacizumab

Background: To prospectively evaluate the effect of intravitreal bevacizumab on aqueous levels of interleukin-6 (IL-6) and vascular endothelial growth factor (vEGF) in patients with exudative age-related macular degeneration (AMD) and to correlate clinical outcomes of patients and aqueous cytokine levels before and after injection. Methods: The study group consisted of 30 eyes from 30 patients with exudative AMD who underwent intravitreal injection of bevacizumab three times at monthly intervals. The aqueous samples prior to the 1st injection (baseline) and 3rd injection were analyzed for vEGF and IL-6 levels, evaluating the effect of 2 doses of intravitreal bevacizumab. Study patients were sub-grouped based upon change in central subfield (CSF) macular thickness on SD-OCT, at 8 weeks. Group 1 included patients (n = 14) with a decrease in CSF thickness greater than 10% from the baseline and were categorized to have “improved”. Group 2 included patients (n = 16) who had a decrease in CSF thickness 10% or less and were considered “treatment-resistant”. Results: There was no statistically significant change in aqueous vEGF and IL-6 levels after intravitreal bevacizumab. In sub-group analysis, in both Groups 1 and 2 patients, aqueous IL-6 levels showed a better correlation with CSF thickness on SD-OCT (r = 0.72 and 0.71, respectively). Conclusions: Data from our study suggest that aqueous IL-6 may be an important marker of treatment response or resistance. Future therapeutic strategies may include targeted treatment against both vEGF and IL-6, in patients who do not respond to anti-vEGF treatment alone.

Corrosion Behavior of Cenosphere Reinforced Al7075 Metal Matrix Composite —An Experimental Approach

Aluminum-based metal matrix composites (MMCs) are considered in several technological applications owing to their enhanced mechanical properties when compared with monolithic metals. Research on the mechanical properties MMCs was done by many researchers;however in depth study on the oxidation behavior of cenosphere, reinforced MMCs are required, since the application of Aluminum-based MMCs is extensively used in applications like automobile, navigation and aviation, where the demand on light weight corrosion resistance material is very much required. In the present work varied compositions of Al7075 grade Aluminum-cenosphere composites use liquid metallurgy route adopting stir casting approach. The experimental study was aimed at experimental investigations of developed composites under different corrosive environments. The corrosion tests were carried out as per ASTM standards. Salt spray test using NaCl was carried out as per ASTM B117 and immersion tests using NaCl and NaOH as corrodents were carried out by following ASTM G31 standards. The results obtained from the tests revealed that as increase in weight % of reinforcement, corrosion resistance increases up to 7.5% reinforcement, and further the corrosion resistance decreases marginally. Solution heat treated samples exhibited higher resistance to oxidation than cast samples in all corrosive environments. The SEM images show the presence of micro cracks and occurrence of pitting corrosion on the corrosion tested specimens.

Photometric analysis of v30 of open cluster NGC 7789

We present the photometric solutions of the variable star v30＊ that is in the foreground of intermediate open cluster NGC 7789. The observations were done in the v passband using the 2 m telescope at the IUCAA-Girawali Observatory in India. The analysis is done using the Wilson-Devinney Code （2003） and the fitted light curve is presented. The photometric solutions reveal a W-subtype contact configuration. The photometric mass ratio is found to be 0.395 and the absolute masses and radii for the components are deduced as 1.25Ms and 0.97Rs for the primary and 0.49Ms and 0.93Rs for the secondary, respectively. No signature of third light is found in the system.

Frozen shoulder-A prospective randomized clinical trial

AIM To compare the results of arthroscopic capsular release with intra-articular steroid injections in patients of frozen *** Fifty-six patients with frozen shoulder were randomised to one of two treatment groups: Group 1, complete 360 degree arthroscopic capsular release and group 2, intra-articular corticosteroid injection(40 mg methyl prednisolone acetate). Both groups were put on active and passive range of motion exercises following the intervention. The outcome parameters were visual analogue scale(vAS) score for pain, range of motion and Constant score which were measured at baseline, 4, 8, 12, 16 and 20 wk after *** All the parameters improved in both the groups. The mean vAS score improved significantly more in the group 1 as compared to group 2 at 8 wk. This greater improvement was maintained at 20 wk with P value of 0.007 at 8 wk, 0.006 at 12 wk, 0.006 at 16 wk and 0.019 at 20 wk. The Constant score showed a more significant improvement in group 1 compared to group 2 at 4 wk, which was again maintained at 20 wk with P value of 0.01 at 4, 8, 12 and 16 wk. The gain in abduction movement was statistically significantly more in arthroscopy group with P value of 0.001 at 4, 8, 12, 16 wk and 0.005 at 20 wk. The gain in external rotation was statistically significantly more in arthroscopy group with P value of 0.007 at 4 wk, 0.001 at 8, 12, and 16 wk and 0.003 at 20 wk. There was no statistically significant difference in extension and internal rotation between the two groups at any time. CONCLUSION Arthroscopic capsular release provides subjective and objective improvement earlier than intra-articular steroid injection.

Total knee arthroplasty in patients with Paget's disease of bone: A systematic review

AIM To determine the functional outcomes, complications and revision rates following total knee arthroplasty(TKA) in patients with Paget's disease of bone(PDB). METHODS A systematic review of the literature was performed. Four studies with a total of 54 TKAs were included for analysis. Functional outcomes, pain scores, complications and revision rates were assessed. The mean age was 72.0 years and the mean follow-up was 7.5 *** All studies reported significant improvement in knee function and pain scores following TKA. There were 2 cases of aseptic loosening, with one patient requiring revision of the femoral component 10 years after the index procedure. Malalignment, bone loss, soft tissue contractures were the most commonly reported intraoperative challenges. There were five cases(9%) that were complicated by intra-operative patellar tendon *** The findings support the use of TKA in patients with PDB. The post-operative functional outcomes are largely similar to other patients, however there are specific perioperative challenges that have been highlighted, in particular the high risk for patellar tendon avulsion.

Common Complication in an Uncommon Disease: Presence of Aortic Regurgitation in Patient with Quadricuspid Aortic valve

Background: Congenital heart defect exists from birth but is usually detected in a later stage of life, as in case of quadricuspid aortic valve. A great majority of quadricuspid aortic valves do not have a normal function and often necessitate surgical treatment. Aim: To report a case with quadricuspid aortic valve accompanied with complications. Case Presentation: We present a case of a male detected with quadricuspid aortic valve of four equal sized cusps accompanied by abnormalities, yet managed successfully under conservative treatment. Conclusion: Patients with a quadricuspid aortic valve require careful clinical evaluation and close follow up.

Long term outcomes of Cohen’s cross trigonal reimplantation for primary vesicoureteral reflux in poorly functioning kidney

BACKGROUND Open ureteric reimplantation by cross trigonal technique described by Cohen is considered a common surgical option for correction of vesicoureteral reflux(vUR). There is a lack of evidence in literature though for what happens to such kidneys, in the long run, particularly those which are poorly *** To assess the long-term outcomes of ureteric reimplantation in poorly functioning kidneys in children with unilateral primary *** Children with unilateral primary vUR and a relative renal function of less than 35% who underwent open or laparoscopic ureteric reimplantation between January 2005 and January 2017 were included in the study. Patients who had a follow up of less than five years were excluded. Preoperative evaluation consisted of a voiding cystourethrogram and Dimercaptosuccinic acid(DMSA) scan. In the follow-up period, patients underwent a diuretic scan at 6 weeks and 6 *** up ultrasound was done for change in grade of hydronephrosis and retrovesical ureteric diameter. Subsequent follow up was done at 6 monthly intervals with evaluation for proteinuria and hypertension and any recurrent urinary tract infection(UTI). For assessment of cortical function, DMSA was repeated annually for 5 years after surgery. A paired-samples t-test was used to test the mean difference of DMSA between pre-post *** During this period, 36 children underwent ureteric reimplantation for unilateral primary vUR. After excluding those with insufficient follow-up, 31 were included in the analysis. Most of the patients were males(n = 26/31, 83.8%). Patient’s age(mean ± SD, range) was 5.21 ± 3.71, 1-18 years. The grades of vUR were grade Ⅱ(1patient), grade Ⅲ(8 patients), grade Ⅳ(10 patients), and grade Ⅴ(12 patients). The pre and postoperative DMSA was 24.064 ± 12.02 and 24.06 ± 10.93, which was almost the same(statistically equal, paired-samples t-test: P = 0.873).
