Objective:Conservative approaches in muscle-invasive bladder cancer(MIBC)have been evolved to avoid aggressive surgery,but are limited to elderly,frail,and patients medically unfit for *** study aimed to assess the response rate of neoadjuvant chemotherapy(NACT)before radiotherapy(RT)in MIBC ***:Forty patients with urothelial carcinoma of stage T2-T4a,N0,M0 were enrolled between November 2013 and November 2015,and treated with three cycles of NACT with ***-NACT response was assessed using Response Evaluation Criteria in Solid Tumors(RECIST)*** who achieved complete response(CR)and partial response(PR)>50%were treated with radical RT,and those who had PR50%or CR,and were treated with ***(12.5%)patients who had PR<50%,SD,or PD underwent *** patients who received radiation showed CR after 6 *** follow-up was 43 months(range:10-66 months)and median overall survival(OS)was not ***-year OS,local control,and disease-free survival were 70.1%,60.9%,50.6%,respectively,and 50%of patients preserved their functioning ***-year OS rate was 88.9%in patients who achieved CR to NACT,73.1%in patients with PR≥50%and 40%in patients with PR<50%.Conclusion:NACT followed by RT provides a high probability of local response with bladder preservation in CR *** use of this treatment regimen in carefully selected patients may omit the need for morbid surgery.

The Ypresian Cambay Shale Formation at Vastan and Mangrol lignite mines in Gujarat, western India, has yielded a rich vertebrate fauna with numerous taxa of European affinities. Here we report a new, approximately contemporary vertebrate assemblage from two fossiliferous layers in the nearby mine of Tadkeshwar. These layers have yielded a similar mammal fauna with the co-occurrence of the perissodactyl-like cambaytheriid Cambaytherium thewissi, the adapoid primates Marcgodinotius indicus and cf. Asiadapis cambayensis, and the hyaenodontid lndohyaenodon raoi. The presence of these species in both Vastan and Tadkeshwar mines and at different levels suggests that the deposits between the two major lignite seams represent a single land mammal age. Apart from the aforementioned species there is a new, smaller species of Cambaytherium, and a new genus and species of esthonychid tillodont. This fauna also contains the first large early Eocene vertebrates from India, including an unidentified Coryphodon-like pantodont, a dyrosaurid crocodyliform and a new giant madtsoiid snake. Among the Tadkeshwar vertebrates several taxa are of Gondwana affinities, such as Pelomedusoides turtles, dyr- osaurids, and large madtsoiids, attesting that the early Eocene was a crucial period in India during which Laurasian taxa of European affinities co-existed with relict taxa from Gondwana before the India-Asia collision. Our results suggest that terrestrial faunas could have dispersed to or from Europe during episodes of contact between the Indian subcontinent and different island blocks along the northern margin of the Neotethys, such as the Kohistan Ladakh island-arc system. Gondwana taxa might represent remnants of ghost lineages shared with Madagascar, which reached the Indian subcontinent during the late Cretaceous; alternatively they might have come from North Africa and passed along the southern margin of the Neotethys to reach the Indian subcontinent. The Neoarchean Bundelkhand greenstone sequences at Mauranipur and Babina areas within the Bundelkhand Gneissic Complex preserve a variety of magmatic rocks such as komatiitic basalts, basalts,felsic volcanic rocks and high-Mg andesites belonging to the Baragaon, Raspahari and Koti *** intrusive and extrusive komatiitic basalts are characterized by low SiO_2(39-53 wt.%), high MgO(18-25 wt.%).moderately high Fe_2O_3(7.1-11.6 wt.%), Al_2O_3(4.5-12.0 wt.%), and TiO_2(0.4-1.23 wt.%)with super to subchondritic(Gd/Yb)N ratios indicating garnet control on the melts. The intrusive komatiitic suite of Ti-enriched and Al-depleted type possesses predominant negative Eu and positive Nb, Ti and Y anomalies. The chemical composition of basalts classifies them into three types with varying SiO_2, TiO_2, MgO, Fe_2O_3, Al_2O_3 and CaO. At similar SiO_2 content of type Ⅰ and Ⅲ basalts, the type II basalts show slightly high Al_2O_3 and Fe_2O_3 contents. Significant negative anomalies of Nb, Zr, Hf and Ti, slightly enriched LREE with relatively flat HREE and low ∑REE contents are observed in type Ⅰ and Ⅱ basalts. TypeⅢ basalts show high Zr/Nb ratios(9.8-10.4), TiO_2(1.97-2.04 wt.%), but possess strikingly flat Zr, Hf, Y and Yb and are uncontaminated. Andesites from Agar and Koti have high SiO_2(55-64 wt.%), moderate TiO_2(0.4-0.7 wt.%), slightly low Al_2O_3(7-11.9 wt.%), medium to high MgO(3-8 wt.%) and CaO contents(10-17 wt.%). Anomalously high Cr, Co and Ni contents are observed in the Koti rhyolites. Tholeiitic to calc alkaline affinity of mafic-felsic volcanic rocks and basalt-andesite dacite-rhyolite differentiation indicate a mature arc and thickened crust during the advanced stage of the evolution of Neoarchean Bundelkhand greenstone belt in a convergent tectonic setting where the melts were derived from partial melting of thick basaltic crust metamorphosed to amphibolite-eclogite facies. The ascorbic acid(AA)is a biomarker that can be used to detect the symptoms of severe disorders such as scurvy,Parkinson’s,Alzheimer’s,and cardiovascular *** this work,a simple and effective sensor model is developed to diagnose the presence of AA *** develop the sensor,a tapered single-mode optical fiber has been used with the well-known phenomenon of localized surface plasmon resonance(LSPR).For LSPR,the tapered region is immobilized with synthesized gold nanoparticles(AuNPs)and zinc oxide nanoparticles(ZnO-NPs)whose absorbance peak wavelengths appear at 519nm and 370nm,*** the basis of nanoparticles(NPs)configurations,two different biosensor probes are *** the first one,the sensing region is immobilized with AuNPs and named Probe *** the second probe,the immobilized layer of AuNPs is further coated with a layer of ZnO-NPs,and a resultant probe is termed as Probe *** characterizations of synthesized AuNPs and developed fiber probes are done by the ultraviolet-visible(UV-vis)spectrophotometer,high-resolution transmission electron microscope(HR-TEM),atomic force microscopy(AFM),and scanning electron microscope(SEM).To enhance the selectivity,a sensing region of probes is functionalized with ascorbate oxidase enzyme that oxidizes the AA in the presence of *** response of developed sensor probes is authenticated by sensing the samples of AA in the range from 500 nM to 1 mM,which covers the range of AA found in human bodies,i.e.,40μM-120μ*** performance analysis of the developed sensor probes has been done in terms of their stability,reproducibility,reusability,and *** observe the stability of AA,a pH-test has also been done that results in a better solubility of AA molecules in phosphate-buffered saline(PBS)solution.

Objective:To identify genotypes of Oesophagostmum venulosum(***) prevailing in West Bengal,India by comparing variation of nucleotide sequences among 28S ***: PCR amplification of partial segment of 28 S rRNA sequence and analysis of sequence amplified product by single strand conformation polymorphism(SSCP).Results:Two distinct conformers among male and female parasites were identified by PCR-SSCP *** analysis among conformers revealed the presence of five single nucleotide polymorphisms(SNP) in codon 64,66,86,125 and *** RNA prediction structure showed that out of 5 SNPs,4 occurred at interior loop of RNA which confirmed evolutionary changes among isolates prevailing in this ***:SNPs occured in different isolates of *** might influence critical changes in rRNA folding pattern which influence evolutionary changes among isolates.

YFa and analogs:Investigation of opioid receptors in smooth muscle contraction

AIM:To study the pharmacological profile and inhibition of smooth muscle contraction by YFa and its analogs in conjunction with their receptor selectivity. METHODS:The effects of YFa and its analogs (D-Ala2) YFa, Y (D-Ala2) GFMKKKFMRF amide and Des-Phe-YGGFMKKKFMR amide in guinea pig ileum (GPI) and mouse vas deferens (MVD) motility were studied using an isolated tissue organ bath system, and morphine and DynA (1-13) served as controls. Acetylcholine was used for muscle stimulation. The observations were validated by specific antagonist pretreatment experiments using naloxonazine, naltrindole and norbinaltor-phimine norBNI. RESULTS:YFa did not demonstrate significant inhibition of GPI muscle contraction as compared with mor-phine (15% vs 62%, P = 0.0002), but moderate inhibition of MVD muscle contraction, indicating the role of κ opioid receptors in the contraction. A moderate inhibition of GPI muscles by (Des-Phe) YFa revealed the role of anti-opiate receptors in the smooth muscle contraction. (D-Ala-2) YFa showed significant inhibition of smooth muscle contraction, indicating the involvement of mainly δ receptors in MVD contraction. These results were supported by specific antagonist pretreatment assays. CONCLUSION:YFa revealed its side-effect-free analgesic properties with regard to arrest of gastroin-testinal transit. The study provides evidences for the involvement of κ and anti-opioid receptors in smooth muscle contraction.

Unicondylar Knee Arthroplasty (UKA) is an early alternative surgical procedure for the unicondylar osteoarthritis or damaged knee joint with artificial prosthesis for the release of disabling painful condition and restoring the normal knee functions. Minimally UKA is one of the recent and the majority successful procedures in modern orthopedics for the osteoarthritis which is spreading throughout the worldwide. Recently, many orthopedic surgeons are expanding their abilities in this field. However, it needs good knowledge and well experience for the successful clinical outcomes. The minimal invasive approach is more efficient for short hospital stay and faster postoperative recovery with low morbidity of the patients after UKA. The aim of this article is to emphasize the steps in UKA based on modern facts: function of knee joint, diagnosis, less invasive approach for medial condylar replacement, radiographic evaluation, and earlier recovery, selecting the patient and implant survivorship with review of surgery.

Cerebral Venous Sinus Thrombosis (CVST/CSVT) is occlusion of cerebral veins and venous sinuses of brain secondary to blood clot formation resulting in hindrance in the blood drainage system in brain, leading to disturbances the internal homeostasis of brain, raised intracranial pressure, cerebral edema, and 50% of cases will have venous infarction or venous hemorrhage (stroke). CVST although being a Rare disorder but may be more common in children than adults with greater risk in neonatal period i.e. first 28 days of life. Here we are discussing a case of Pediatric CVST in a 7-month-old baby boy who presented to Emergency Room (ER) with recurrent discrete episodes of vomiting, fever, seizures, drowsiness and respiratory distress. The fatal outcome in our child was attributed to delayed presentation in a tertiary care center, hence missed early diagnosis and treatment. In this child the CVST could be result of amalgamation of complex underlying ongoing multiple pathological processes: an acute systemic illness like sepsis, severe dehydration, undiagnosed and untreated complex congenital heart disease, tetralogy of fallot with osteum secondum atrial septal defect, worsening the coagulopathy. It takes this case even more unique. This discussion is to bring focus on the importance of knowledge about CVST amongst emergency physicians and primary care physicians, specially managing this rare disorder with flummox presentation mimicking other more common disorders, especially in pediatric and neonatal population where definitive history and chief complaints are often vague and difficult to obtain, making it more difficult to diagnose. We the authors hence reporting this case with intent to spread awareness of CVST, how to doubt it, detect it and then manage it, especially in places like Chhattisgarh, India, where CVST is not so uncommon. Introduction:Chronic suppurative otitis media (CSOM) is a common problem in worldwide and untreated CSOM leads to fatal complications like facial nerve paralysis,lateral sinus thrombosis,labyrinthitis,meningitis and brain abscess in developing country like ***:To isolate causative bacteria and antibiotic sensitivity pattern for CSOM and to know the prevalence of extended spectrum beta lactamases (ESBL) and Metallobetalactamases (MBL) in CSOM ***:A total of 500 ear swabs of clinical suspected CSOM patients were cultured on specific cultured medium and identified the bacteria with conventional *** all the identified bacteria were subjected with specific antibiotics by the Kirby-Bauer's method to know the resistance pattern of *** and MBL strains were detected by double disc diffusion ***:A total of 384 bacteria were isolated from 500 CSOM patients,among them 86 *** (22.40%),112 Staphylococcus aureus (29.17%),53 *** (13.80%),32 *** (18%),26 *** (6.77%),24 *** (6.25%),23 *** (5.99%),18 *** (4.69%) and 10 *** (2.60%) identified with conventional *** antibiotic disc diffusion methods 74.22% ESBL strains and 9.90%MBL strains were *** resistant strains of *** (86/384,22.40%) were more prevalent than those of *** (112/384,29.17%) and other bacteria in ear *** and vancomicin could control to gram negative bacteria and gram positive bacteria ***:Continuous and periodic evaluation of microbiological profile and antimicrobial sensitivity pattern of bacterial is essential for optimum management of CSOM patients.
